To describe the longitudinal course of acquisition of healthcare transition skills among adolescents and young adults with inflammatory bowel *** recruited adolescents and young adults (AYA) with inflammatory bowel diseases (IBD), from the pediatric IBD clinic at the University of North Carolina. Participants completed the TRxANSITION Scale™ at least once during the study period (2006-2015). We used the electronic medical record to extract participants’ clinical and demographic data. We used ordinary least square regressions with robust standard error clustered at patient level to explore the variations in the levels and growths of healthcare transition *** sample (n = 144) ranged in age from 14-22 years. Age was significantly and positively associated with both the level and growth of TRxANSITION Scale™ scores (P < 0.01). Many healthcare transition (HCT) skills were acquired between ages 12 and 14 years, but others were not mastered until after age 18, including self-management *** is one of the first studies to describe the longitudinal course of HCT skill acquisition among AYA with IBD, providing benchmarks for evaluating transition interventions.

A review of self-assembled monolayers as potential terahertz frequency tunnel diodes

In this review, we describe the principles of the tunnel junction, self-assembled monolayer （SAM） application techniques, experimental testbed fabrication, and characterization of the films and devices. In addition, techniques for directed application, removal, and functionalization of the monolayers are discussed. Bottom-up fabrication techniques have seen increased attention because of their versatility and ease of use. These films see mechanical uses as surface modifiers and micro-scale lubricants. Advances in nanowatt electronics and ultra-low power sensors have opened up an energy harvesting niche for solutions which would have proven ineffective just some years ago. The focus of this study is the two- terminal junction which has potential applications in THz rectification for energy harvesting, medical imaging, and defense sensing. The quantum theory of operation behind these devices is touched on briefly---describing tunneling through the organic monolayers. Commentary on trends in research and potential future work are presented as well.

Non-surgical periodontal therapy:An update on current evidence

Periodontal disease is an inflammatory condition that involves a complex interaction between pathogenic bacteria,environmental and acquired factors and host related *** recently periodontal treatment was directed primarily towards reduction of bacterial load by subgingival debridement of root surfaces and modification of environmental risk *** current paradigm of periodontal disease stresses greater role of hostmediated inflammatory response in tissue destruction characteristic of periodontal *** therapeutic modalities have been developed adjuvant to mechanical periodontal *** use of laser and photodynamic therapy show great promise but their effectiveness has still not been conclusively *** agents,either systemic and local antimicrobials or host modulating drugs,played pivotal role in better and more predictable management of periodontal *** present review focuses on the best available evidence,for the current management of the chronic periodontal patients,gathered from systematic reviews and metaanalysis of mechanical non surgical periodontal therapy(NSPT)(subgingival debridement,laser therapy and photodynamic therapy)and the adjunctive chemothera-peutic approaches such as systematic and local antibiotics and antiseptics,subgingival pocket irrigation and host modulation *** review also attempts to briefly introduce future developments in some of these *** the end,the review summarizes the analysis of the current evidence that suggests that thorough subgingival debridement remains the mainstay of NSPT and that adjunct use of chemotherapeutic agents may offer better management of clinical parameters in periodontitis patients.

Autologous bone marrow derived stem cell therapy in patients with type 2 diabetes mellitus-defining adequate administration methods

AIM To carry out randomized trial for evaluating effects of autologous bone marrow derived stem cell therapy(ABMSCT) through different *** Bone marrow aspirate was taken from the iliac crest of patients. Bone marrow mononuclear cells were separatedand purified using centrifugation. These cells were then infused in a total of 21 patients comprising three groups of 7 patients each. Cells were infused into the superior pancreaticoduodenal artery(Group Ⅰ), splenic artery(Group Ⅱ) and through the peripheral intravenous route(Group Ⅲ). Another group of 7 patients acted as controls and a sham procedure was carried out on them(Group Ⅳ). The cells were labelled with the PET tracer F18-FDG to see their homing and in vivo distribution. Data for clinical outcome was expressed as mean ± SE. All other data was expressed as mean ± SD. Baseline and post treatment data was compared at the end of six months, using paired t-test. Cases and controls data were analyzed using independent t-test. A probability(P) value of < 0.05 was regarded as statistically significant. Measures of clinical outcome were taken as the change or improvement in the following parameters:(1) C-peptide assay;(2) HOMA-IR and HOMA-B;(3) reduction in Insulin dose; subjects who showed reduction of insulin requirement of more than 50% from baseline requirement were regarded as responders; and(4) reduction in HbA 1c. RESULTS All the patients, after being advised for healthy lifestyle changes, were evaluated at periodical intervals and at the end of 6 mo. The changes in body weight, body mass index, waist circumference and percentage of body fat in all groups were not significantly different at the end of this period. The results of intra-group comparison before and after ABMSCT at the end of six months duration was as follows:(1) the area under C-peptide response curve was increased at the end of 6 mo however the difference remained statistically non-significant(P values for fasting C-peptide were 0.973, 0

We read with great interest the article, ＂Non-alcoholic fatty liver disease may not be a severe disease at presentation among Asian Indians＂ by Madan et at in the recent issue of WJG. Twenty-eight （55%） out of 51 patients with nonalcoholic fatty liver disease （NAFLD） who presented with abnormal transaminases had histological evidence of nonalcoholic steatohepatitis （NASH）. The majority of patients had grade 1 [32 （63%）] or grade 2 [16 （31%）] inflammation and either had no [23 （45%）] fibrosis or stage I [19 （37%）] fibrosis. None of the patients had cirrhosisf11. We agree with Madan et al that Asian Indians with NAFLD who present with unexplained increase in transaminases may have mild disease at presentation on the basis of similar observations made by us. NAFLD has a spectrum which includes patients with only steatosis and NASH that can progress to cirrhotic and hepatocellular carcinoma.

Ochratoxin-A[7-(L-β-phenylalanylcarbonyl)-carboxyl-5-chloro-8-hydroxy-3,4-dihydro-3R-methyl-isocumarin, OTA] is a common food contaminant mycotoxin that enters the human body through the consumption of improperly stored food products. Upon ingestion, it leads to immuno-suppression and immuno-toxicity. OTA has been known to produce nephrotoxic, teratogenic, and carcinogenic activity (via oxidative DNA damage) in several species. This review introduces potentials of electrochemical biosensor to provide breakthroughs in OTA detection through improved selectivity and sensitivity and also the current approaches for detecting OTA in food products.

Twin-T Oscillator Containing Polymer Coated Parallel Plate Capacitor for Sea Water Salinity Sensing

This paper presents the development of a Twin-T oscillator comprising polymer coated parallel plates as a sensor for ocean water salinity *** sensor employs a parallel plate capacitor design, with sea water serving as the medium between plates. Novalac resin and a proprietary commercial polymer (AccufloTW) were investigated as corrosion protective coatings for the copper electrodes of the capacitor. Electrochemical Impedance Spectroscopy (EIS) was employed to evaluate corrosion inhibition of polymer coatingin sea water. A detection circuit was designed and simulated using P-spice and then implemented in Printed Circuit Board (PCB). EIS results indicate that Accuflo exhibits better corrosion inhibition in ocean water than Novolac. Further, the use of Twin-T oscillator based detection circuit resulted in enhanced sensitivity and better detection limit. Experiments performed using ocean water samples resulted in oscillator frequency shift of 410 Hertz/power supply unit (Hz/PSU). Oscillator frequency drift was reduced using frequency-to-voltage converters and sensitivity of 10 mV/PSU was achieved.

A 32-year-old lady presented to us at 6 months of gestation with acute pancreatitis due to parathyroid hormone-dependent hypercalcemia and with insulin-dependent hypoglycemia,owing to parathyroid adenoma and possibly insulinoma,*** parathyroid adenoma was localized on the magnetic resonance imaging of the neck;however,imaging for the insulinoma could not be done due to the gravid *** to the simultaneous occurrence of tumours in two endocrine glands,namely,parathyroid gland and pancreatic islet cells,a diagnosis of MEN-1(multiple endocrine neoplasia)was considered,which is very rarely seen in *** hypercalcemia was effectively managed with cinacalcet and alcohol ablation of the parathyroid adenoma while her hypoglycemic episodes were managed with short and long-acting octreotide injections during the antenatal *** had a full-term cesarean delivery,with no maternal or neonatal complications,except for transient neonatal *** patient was followed up for 1 year after her delivery with no further episodes of hypercalcemia and hypoglycemia,on medical ***99m sestamibi scan was done after delivery which confirmed the presence of a left inferior parathyroid ***-1 with pregnancy thus poses a diagnostic and therapeutic challenge and our case highlights the role of multimodal medical therapy for successful management.

从桃胚的子叶中诱导出不定体细胞胚,并从体细胞胚中分化出了细胞悬浮培养物。在补充了2,4—D、BAP、KN和肌醇的改良MS培养基上诱导出胚性愈伤组织。对悬浮培养中胚的发育进行了生长模式鉴定。并用醋酸洋红染剂进行组织学和细胞学鉴定。使用2,4—D(5μM)诱导出的胚性愈伤组织比使用NAA诱导出的多。逐渐减少培养基中2,4—D的浓度,可以加快球状胚的复原。在含有Ca(NO_3)_2(1000mgl^(-1))和6%蔗糖的高氮盐培养基上诱导出心状胚。体细胞胚在白光下变成绿色,有时变成红色。每14天继代培养一次,这样,胚的活力最强。用亚历山大染荆对正在分化的胚和细胞的活力进行显微观察,有活力的胚被染成粉红一红色,没有活力的胚被染成绿一兰色。2星期后,用2,4—D,BAP和KN浓度比较低的培养基有规律地继代培养,可以使胚性细胞悬浮培养物的活力维持10个月之久。
