Background:Patients with bilateral chronic otitis media-mucosal type have been conventionally treated with sequential *** simultaneous tympanoplasty is usually not preferred because of the theoretical risk of iatrogenic sensorineural hearing *** the advent of modern surgical instruments and surgical techniques,the risk is expected to be *** study compares the clinical outcomes in typeⅠtympanoplasty performed simultaneously and ***&methods:This randomized prospective study was carried out in a tertiary care hospital between August 2015 and July 2017.A total of 30 patients were divided into two groups of 15 *** study analyzed the graft uptake,pure tone audiogram findings pre-and post-operatively,duration of surgery and number of hospital visit for each patient and the outco mes were co mpared between both the ***:Patients undergoing bilateral simultaneous tympanoplasty had significantly lesser mean duration of surgery and number of hospital visits than the patients undergoing sequential *** uptake and postoperative wound infections were similar in both the *** hearing improvement was significantly better in the bilateral simultaneous tympanoplasty ***,further studies are needed to authenticate this *** of the patients had a postoperative deterioration of hearing or sensorineural hearing ***:Bilateral simultaneous tympanoplasty is not only feasible but also better than sequential tympanoplasty,especially in terms of operating time,follow-up and overall financial implications on the patient.

Diagnostic role of Xpert-MTB RIF assay in osteoarticular tuberculosis:A retrospective study

BACKGROUND Osteoarticular tuberculosis(OATB) is a severe form of extrapulmonary TB, which causes notable morbidity and warrants a high index of suspicion for prompt management. The diagnosis of OATB poses a challenge, because of the difficulty of collecting the samples and, secondarily, the paucibacillary nature of lesion, which gives poor sensitivity and reproducibility, with long turnaround time of routine/conventional laboratory tests and the requirement for invasive procedures and expertise. The Xpert MTB/RIF assay has been approved by the World Health Organization as a rapid diagnostic tool for diagnosing pulmonary and extrapulmonary *** To emphasize the diagnostic efficiency of gene Xpert for OATB in suspected patients in a tertiary care hospital of Eastern *** This retrospective study was conducted in the Department of Microbiology and Orthopaedics by analyzing the data of the gene Xpert assay over a 3-year duration from January 2018 to February 2021. Demographic and clinical data were recorded. The diagnostic efficiency of gene Xpert was evaluated against the composite reference standard(CRS).RESULTS A total of 37 cases fell into positive, probable, and possible categories of osteoarticular TB out of 112 patients included in the study by CRS;gene Xpert result was positive in 35 out of the 37 different CRS categorized cases. Of the 112 cases, culture was put in 40 cases, and, of these cultures, 5 cases showed the growth of MTB. Of these, 4 cases were included in the 35 cases diagnosed by gene *** microscopy was positive in 6 out of 37 CRS categorized cases. When compared with CRS, the sensitivity of gene Xpert assay, culture, and smear was found to be 94.6%, 13.5%, and 16.2%, respectively, while specificity in all the three types of tests was 100%. When kappa statistics were applied, the percentage of agreement gene Xpert, culture, and microscopy with CRS was found to be 95%, 20%, and 22.6%, respectively. Follow-up of the gene Xpert posit

Statistical modeling and optimization of a multistage gas-solid fluidized bed for removing pollutants from flue gases

The present paper describes the statistical modeling and optimization of a multistage gas-solid fluidized bed reactor for the control of hazardous pollutants in flue gas. In this work, we study the hydrodynamics of the pressure drop and minimum fluidization velocity. The hydrodynamics of a three-stage fluidized bed are then compared with those for a single-stage unit. It is observed that the total pressure drop over all stages of the three-stage fluidized bed is less than that of an identical single-stage system. However, the minimum fluidization velocity is higher in the single-stage unit. Under identical conditions, the minimum fluidization velocity is highest in the top bed, and lowest in the bottom bed. This signifies that the behavior of solids changes from a well-mixed flow to a plug-flow, with intermediate behavior in the middle bed.

Evaluating role of bone marrow-derived stem cells in dry age-related macular degeneration using multifocal electroretinogram and fundus autofluorescence imaging

AIM：To evaluate the role of bone marrow-derived stem cells in the treatment of advanced dry age-related macular degeneration（AMD）using multifocal electroretinogram（mf-ERG）and fundus autofluorescence ***：Thirty patients（60 eyes）with bilateral central geographic atrophy（GA）were *** eye of each patient received autologous bone marrow-derived hematopoietic stem cells（BM-HSCs）（group 1）and the fellow eye with better visual acuity served as control（group2）.The effect of stem cell therapy was determined in terms of visual acuity,amplitude and implicit time in mf-ERG and size of GA on fundus autofluorescence *** tests were performed at presentation and first,third and sixth month follow *** events（if any）were also ***：At 6mo follow-up there was no statistically significant improvement in median log MAR best corrected visual acuity（BCVA）in either ***-ERG revealed significant improvement in amplitude and implicit time in the intervention group.A significant decrease was also noted in greatest linear dimension（GLD）of GA in the eyes receiving stem cells[6.78±2.60 mm at baseline to 6.56±2.59 mm at 6mo（P=0.021）].However,no such improvement was noted in the control ***：Electrophysiological and anatomical improvement in the intervention group sheds light on the therapeutic role of *** studies are required to determine the stage of disease at which the maximal benefit can be achieved and to standardize the dose andfrequency of stem cell injection.

Life cycle greenhouse gas emissions from five contrasting rice production systems in the tropics

Carbon footprint (CF) quantification of major rice production systems (RPSs) is a prerequisite for developing strategies for climate change mitigation in agriculture. Total life cycle greenhouse gas emissions (LC-GHGs) from rice production to consumption might provide precise CFs for RPSs. Therefore, we assessed three segments (pre-farm, on-farm, and post-farm) of LC-GHGs under five major contrasting RPSs, i.e., aerobic rice (AR), shallow lowland rice(SLR), system of rice intensification (SRI), deep water rice (DWR), and zero-tilled direct-seeded rice (ZTR), in India to determine the corresponding *** footprint was the lowest for ZTR, while LC-GHGs were the lowest for AR. Therefore, AR is an adequate option for short-term reduction of GHG emissions. However, ZTR might be promoted by incentives as a long-term strategy. Among segmental LC-GHGs, on-farm GHG emissions contributed less than the other two segmental GHG emissions. The post-farm (i.e., farm gate to consumption) segment contributed the largest proportion (54%–69%) of total LC-GHGs, followed by pre-farm (i.e., cradle to farm) segment (21%–27%) and on-farm operation (11%–23%). These findings suggest that post-farm components that contribute to maximum GHG emissions must be scientifically tackled with proactive policy initiatives. However, the data of this segment are limited and scattered. Therefore, real-time assessment of GHG emissions during post-farm operation and input transportation from cradle to farm requires more precise quantification. Although CF in SRI was higher, this system had the potential to achieve higher yields and better soil carbon storage. Therefore,SRI may be encouraged from the perspectives of food security and long-term sustainability by reducing GHG emissions by three to four times.

This study investigated the hot corrosion performance of a dissimilar weldment of Ni-based superalloy and stainless steel joined by CO2-laser welding and improved by high-velocity oxy-fuel(HVOF)coating in a Na2SO4-60wt%V2O5 environment at 900℃.A dissimilar butt joint of AISI 321 and alloy 825 was fabricated by CO2-laser welding with low heat input after obtaining the optimum welding parameters by bead-on-plate *** metallurgical and mechanical properties of the laser weldment were *** tensile test results indicated the occurrence of fracture in the base metal AISI 321 *** HVOF process was employed to coat Ni-20wt%Cr on the *** evaluate the surface morphology of the corrosion products formed on the uncoated and Ni-20wt%Cr-coated weldments,scanning electron microscopy(SEM)analysis was ***-dispersive spectroscopy(EDS)was used to determine the different elements present on the surface *** existence of oxide phases on the weldments was determined by X-ray diffraction(XRD).The cross sections of the weldments were characterized by SEM with EDS line mapping *** results indicated that the Ni-20wt%Cr-coated weldment exhibited superior hot corrosion resistance due to the development of Cr2O3 and NiCr2O4 protective oxide scales.
