AIM: To identify risk factors for nonalcoholic steatohepatitis following pancreaticoduodenectomy, with a focus on factors related to pancreatic secretions. METHODS: The medical records of 228 patients who had a pancreaticoduodenectomy over a 16-mo period were reviewed retrospectively. The 193 patients who did not have fatty liver disease preoperatively were included in the final analysis. Hepatic steatosis was diagnosed using the differences between splenic and hepatic attenuation and liver-to-spleen attenuation as measured by non-enhanced computed tomography. REsuLTS: Fifteen patients (7.8%) who showed postoperative hepatic fatty changes were assigned to Group A, and the remaining patients were assigned to Group B. Patient demographics, preoperative laboratory findings (including levels of C-peptide, glucagon, insulin and glucose tolerance test results), operation types, and final pathological findings did not differ significantly between the two groups; however, the frequency of pancreatic fistula (P = 0.020) and the method of pancreatic duct stenting (P = 0.005) showed significant differences between the groups. A multivari- ate analysis identified pancreatic fistula (HR = 3.332, P = 0.037) and external pancreatic duct stenting (HR = 4.530, P = 0.017) as independent risk factors for the development of postoperative steatohepatitis. CONCLUSION: Pancreatic fistula and external pancreatic duct stenting were identified as independent risk factors for the development of steatohepatitis following pancreaticoduodenectomy.

Wetting phenomena between MgO C and CaO SiO2 slags were investigated by varying carbon content.A sessile drop technique was adopted to study the wetting phenomena in conjunction with a high speed camera for the observation of intrinsic wetting *** results show that the high content of SiO2 and the presence of Al2O3 in slags enhance the diffusion of Mg2+,leading to the promotion of reactive *** carbon in MgO C refractory impedes the penetration of slags by repelling the slag and slowing the diffusion of Mg2+.This accounts for the non-wetting behavior of the slag on MgO C refractory with 17% (mass fraction) carbon similar to that of graphite.

This study investigates the characteristics of a heavy snowfall event over the southwestern part of the Korean Peninsula on 4 December 2005. The snowstorm was a type of mesoscale maritime cyclone which resulted from barotropic instability, and diabatic heating from the warm ocean in continental polar air masses. Based on surface observations, radiosonde soundings, MTSAT-1R satellite data and the 10-km grid RDAPS （Regional Assimilation and Prediction System based on the Psu/NCAR MM5） data, the evolution of the mesocyclone is explained by the following dynamics; （1） In the initial stage, the primary role in the cyclogenesis process of the mesocyclone is a barotropic instability in the horizontal shear zone. （2） In the developing stage, the mesocyclone moves and deepens into a baroclinic zone corresponding to the surface heating and moistening. （3） In the mature stage, it is found that the mesocyclone is intensified by the destabilization caused by enhanced low-level heating and condensation, the moisture flux convergence, and the interaction between upper and lower-level potential vorticity anomalies. We suggest that a checklist with stepwise indicators responsible for development be prepared for the forecasting of heavy snowfall over the southwestern part of the Korean Peninsula.

AIM:To compare survival between bile duct segmental resection(BDSR)and pancreaticoduodenectomy(PD) for treating distal bile duct cancers. METHODS:Retrospective analysis was conducted for 45 patients in a BDSR group and for 149 patients in a PD group. REsuLTS:The T-stage(Psurvival rates for the BDSR group and PD group were 51.7%and 36.6%,respectively and 46.0%and 38.1%,respectively(P=0.099).The BDSR group and PD group did not show any significant difference in survival when this was adjusted for the TNM *** 3-and 5-year survival rates were: stageⅠa[BDSR(100.0%and 100.0%)vs PD(76.9% and 68.4%)(P=0.226)];stageⅠb[BDSR(55.8% and 32.6%)vs PD(59.3%and 59.3%)(P=0.942)]; stageⅡb[BDSR(19.2%and 19.2%)vs PD(31.9%and 14.2%)(P=0.669)]. CONCLUSION:BDSR can be justified as an alternative radical operation for patients with middle bile duct inselected patients with no adjacent organ invasion and resection margin is negative.

BACKGROUND The role of multidetector computed tomography(MDCT)in patients with acute infectious colitis is still *** To examine the usefulness of MDCT in distinguishing the etiology of acute infectious *** Overall,244 patients who met the criteria for acute infectious colitis and visited the Hospital from February 2015 to 2018 were retrospectively enrolled and divided into two groups(bacterial:204,viral:40)according to causes of acute colitis,based on stool *** MDCT parameters,including wall thickening,submucosal edema,mucosal enhancement,serosa involvement,empty colon sign,small bowel involvement,comb sign,continuous distribution,accordion sign,mucosal thickening,and lymph node enlargement,were constructed in a blinded *** MDCT parameters of wall thickening(OR:13.60;95%CI:5.80–31.88;Psubmucosal edema(OR:36.08;95%CI:13.54–96.13;Psubmucosaledema, mucosal enhancement, continuous distribution, mucosal thickening) in summer showed ahigh probability of bacterial colitis (sensitivity, 41.67;specificity, 92.50;OR: 24.95).CONCLUSIONMDCT provides many clues that can be useful in suggesting a specific etiology of acute infectiouscolitis.
