Aim： t. st.dy t.e relat.onship bet.een circulat.ng androgens （t.t.l t.st.st.rone [t.], free t.st.st.rone [It. and dihydrot.st.st.rone [DHt.） and high-densit. lipoprot.in cholest.rol （HDL-C） in men wit. and wit.out.cardiovascular disease （CVD）. Met.ods： Cross-sect.onal analyses included 1 661 baseline samples from t.e Massachuset.s Male Aging St.dy （MMAS）, a populat.on-based cohort.of men ages 40-70 years. Serum hormones were measured by radioimmunoassay and HDL-C was det.rmined following precipit.t.on of t.e lower densit. lipoprot.ins. CVD was det.rmined by self-report. Analyses were performed using mult.ple linear regression. Result.： t. and HDL-C were posit.vely correlat.d in t.e ent.re sample （r = 0.11, P = 0.0001）. Aft.r adjust.ng for confounders, we found t.is relat.onship was most.y limit.d t. t.e 209 men wit. CVD. Among men wit. CVD, t. （P = 0.0004）, it.（P = 0.0172） and DHt.（P = 0.0128） were all posit.vely correlat.d wit. HDL-C, whereas in men wit.out.CVD only t. correlat.d wit. HDL-C （P = 0.0099）. Conclusion： Our result. suggest.t.at.if androgens cont.ibut. t. BACKGROUND Due t. development.of an immune-dysregulat.d phenot.pe,advanced liver disease in all forms predisposes pat.ent. t. sepsis acquisit.on,including by opport.nist.c pat.ogens such as *** dat. exist. on fungal infect.on wit.in a medical int.nsive liver unit.MILU),part.cularly in relat.on t. acut. on chronic liver *** t. invest.gat. t.e impact.of fungal infect.ons among crit.cally ill pat.ent. wit. advanced liver disease,and compare out.omes t. t.ose of pat.ent. wit. bact.rial *** From our prospect.ve regist.y of MILU pat.ent. from 2018-2022,we included 27 pat.ent. wit. cult.re-posit.ve fungal infect.ons and 183 wit. bact.rial *** compared out.omes bet.een pat.ent. admit.ed t. t.e MILU wit. fungal infect.ons t. bact.rial *** was ext.act.d t.rough chart.*** All fungal infect.ons were due t. Candida species,and were most.frequent.y blood *** among pat.ent. wit. fungal infect.ons was significant.y worse relat.ve t. t.e bact.rial cohort.93%vs 52%,Pt.e majorit. of t.e fungal cohort.developed grade 2 or 3 acut. on chronic liver failure(ACLF)(90%vs 64%,P=0.02).Pat.ent. in t.e fungal cohort.had increased use of vasopressors(96%vs 70%,P=0.04),mechanical vent.lat.on(96%vs 65%,Pt. acut. kidney injury(78%vs 52%,P=0.014).On MILU admission,t.e fungal cohort.had significant.y higher Acut. Physiology and Chronic Healt. Evaluat.on(108 vs 91,P=0.003),Acut. A growing body of lit.rat.re has est.blished t.e anabolic benefit. of t.st.st.rone （t. t.erapy in hypogonadal men. However, t.ere remains a paucit. of dat. regarding t.e risks of exogenous androgen use in older men and t.e pot.nt.al for adverse effect. on t.e prost.t. gland. Whet.er t.t.erapy in older, hypogonadal men might.worsen lower urinary t.act.sympt.ms or exacerbat., unmask, or even incit. prost.t. cancer development.has t.mpered ent.usiasm for t.t.erapy, while known prost.t.c disease has served as a relat.ve cont.aindicat.on t. t.t.erapy. Androgens are necessary for t.e development.and maint.nance of t.e prost.t. gland. However, epidemiologic st.dies do not.consist.nt.y find a posit.ve relat.onship bet.een endogenous serum androgen concent.at.ons and t.e risk of prost.t. disease. Recent.dat. demonst.at. t.at.5α-reduct.se inhibit.rs decrease t.e risk of low-grade prost.t. cancer, suggest.ng t.at.modifying androgen met.bolism may have beneficial effect. on prost.t. healt., yet.similar reduct.ons in high-grade disease have not.been observed, t.ereby quest.oning t.e t.ue clinical benefit. of t.ese agent. for chemoprevent.on. Knowing how t. best.invest.gat. t.e relat.onship bet.een androgens and t.e development.of prost.t. disease given t.e lack of large, randomized t.ials is difficult. Accumulat.ng dat. challenges t.e assumpt.on t.at.alt.rat.ons in serum androgens have parallel effect. wit.in t.e prost.t. hormonal environment.or change androgen-regulat.d processes wit.in t.e gland. Long-t.rm int.rvent.on st.dies are needed t. t.uly ascert.in t.e effect. of androgen manipulat.on on prost.t. t.ssue and disease risk.
