Limb Salvage Using Human Placental Allografts: Adding to the Reconstructive Ladder Paradigm

Human placental allografts are the latest treatment modalities for diabetic ulcers, chronic wounds, limbs, and tendons for healing cascade and promoting granulation tissue coverage. Purion® processed dehydrated human amnion/chorion membrane allografts (dHACM), decellularized human collagen matrix (dHCM), dehydrated umbilical cord (dHUC) and micronized dehydrated human amnion/chorion membrane allografts (mdHACM) have been the newest modality used to salvage injured human extremities with tendon and bone exposure. A 53-year-old male was assaulted and sustained second and third degree burns to both legs. The left extremity had a 9.5% total body surface area (%TBSA) burned. The right extremity had a 5.5% TBSA and three exposed tendons (Achilles/flexor digitorum longus/tibialis anterior), muscles, talar and tibial bones. Bilateral extremity pedal pulses were present, and all toes had less than two second capillary refill. Sensation, motor, and strength were normal. During the 48-day-hospital stay, the patient had eight operations: tangential excisional debridements of necrotic tissue with weekly wound dressing applications of dHACM and dHCM, on both legs. In addition, dHUC was applied over the exposed tendons on weeks 2 - 5. The exposed tendons were injected with mdHACM on weeks 4 and 5. Negative pressure wound therapy was applied at 125 mmHg for

Methoprene, a juvenile hormone （JH） analog, is a widely.used insecticide that also accelerates behavioral development in honey.bees （Apis mellifera）. JH regulates the transition from nursing to foraging in adult worker bees, and treatment with JH or methoprene have both been shown to induce precocious foraging. To determine how methoprene changes honey.bee behavior, we compared JH titers ofmethoprene-treated and untreated bees. Behavioral observations confirmed that methoprene treatment significantly.increased the number of precocious foragers in 3 out of 4 colonies. In only.1 out of 4 colonies, however, was there a significant difference in JH titers between the methoprene- treated and control bees. Further, in all 4 colonies, there was no significant differences in JH titers between precocious and normal-aged foragers. These results suggest that methoprene did not directly.affect the endogenous JH secreted by.corpora allata. Because methoprene caused early.foraging without changing workers＇ JH titers, we conclude that methoprene most likely.acts directly.on the JH receptors as a substitute for JH.

Methods of vascular control technique during liver resection:a comprehensive review

BACKGROUND: Significant hemorrhage together with blood transfusion increases postoperative morbidity.and mortality.of hepatic resection. Hepatic vascular occlusion is effective in minimizing bleeding during hepatic parenchy.al transection. This article aimed to review the current role and status of various techniques of hepatic vascular occlusion during hepatic resection. DATA SOURCES: The relevant manuscripts were identified by.searching MEDLINE, and PubMed for articles published between January.1980 and April 2010 using the key.ords 'vascular control', 'vascular clamping', 'vascular exclusion' and 'hepatectomy.. Additional papers were identified by.a manual search of the references from the key.articles. RESULTS: One randomized controlled trial (RCT) and 5 RCTs showed intermittent Pringle maneuver and ischemic preconditioning followed by.continuous Pringle maneuver were superior to continuous Pringle maneuver alone, respectively. Two RCTs compared the outcomes of hepatectomy.with and without intermittent Pringle maneuver. One showed Pringle maneuver to be beneficial, while the other failed to show any.benefit. One RCT showed that ischemic preconditioning had significantly.less blood loss than using intermittent Pringle maneuver. Four RCTs evaluated the use of hemihepatic vascular occlusion. One RCT showed it had significantly.less blood loss than Pringle maneuver, while the other 3 showed no significant difference. Only.1 RCT showed it had significantly.less liver ischemic injury. No RCT had been carried out to assess segmental vascular occlusion. Two RCTs compared the outcomes of total hepatic vascular exclusion (THVE) and Pringle maneuver. One RCT showed THVE resulted in similar blood loss, but a higher postoperative complication. The other RCT showed less blood loss using THVE but the postoperative complication rate was similar. The current role of neoadjuvant/adjuvant/chemoprevention therapy.in partial hepatectomy.for hepatocellular carcinoma:a sy.tematic review

BACKGROUND: Following curative treatment for hepatocellular carcinoma (HCC), 50%-90% of postoperative death is due to recurrent disease. Intra-hepatic recurrence is frequently.the only.site of recurrence. Thus, any.neoadjuvant or adjuvant therapy. which can decrease or delay.the incidence of intra-hepatic recurrence, or any.cancer chemoprevention which can prevent a new HCC from developing in the liver remnant, will improve the results of liver resection. This article sy.tematically.reviewed the current evidence of neoadjuvant, adjuvant, and chemoprevention in partial hepatectomy.of HCC. DATA SOURCES: Studies were identified by.searching MEDLINE and PubMed databases for articles from January.1990 to November 2008 using the key.ords 'hepatocellular carcinoma', 'hepatectomy., 'adjuvant therapy., 'neoadjuvant therapy., and 'regional therapy.. Additional papers and book chapters were identified by.a manual search of the references from the key.articles. RESULTS: Neoadjuvant transarterial chemoembolization or adjuvant regional transarterial chemotherapy.embolization+sy.temic chemotherapy.did not add benefit. Both adjuvant transarterial radioembolization with (131)I-lipiodol and adjuvant sy.temic interferon showed promising results. However, there were only.a limited number of such studies. CONCLUSIONS: Further randomized controlled studies need to be carried out. Currently. there is no consensus on a standard neoadjuvant/adjuvant/chemoprevention therapy.in partial hepatectomy.for HCC.

Hemoperitoneum is a rare and potentially.life-threatening complication of GIST. We reported a 54- y.ar-old man who developed disseminated intra-abdominal recurrence from a previously.resected gastrointestinal stromal tumour （GIST） of the small bowel, and the patient presented with hemoperitoneum. Emergent debulking surgery.was performed. A high dose imatinib was prescribed. Despite the presence of residual disease, the patient was well clinically.8 months after the operation. Even though, there is no evidence to support the routine use of debulking surgery.in the management of GIST. In our patient, disease progression after second line targeted therapy.and the absence of alternative treatment options for spontaneous rupture and hemoperitoneum prompted us to treat the patient aggressively. Resection of the ruptured GIST was carried out for control of bleeding and to prevent recurrent bleeding in this patient with good surgical risks. During the treatment decision-making, the patient＇s general condition, the risk of surgery.and the extent of dissemination were taken into consideration. In this patient who presented with spontaneous rupture of a small intestinal GIST, the novel use of targeted therapy.and aggressive surgical treatment produced reasonably.good survival outcome.

Objectives Safety.restraints reduce injuries from motor vehicle collisions(MVCs)but are often improperly.applied or not *** Childhood Opportunity.Index(COI)reflects social determinants of health and its study.in pediatric trauma is *** hy.othesized that MVC patients from low-opportunity.neighborhoods are less likely.to be appropriately.*** A retrospective cross-sectional study.was performed on children/adolescents≤18 y.ars old in MVCs between January.1,2011 and December 31,*** were identified from the Children's Hospital Los Angeles trauma *** outcome was safety.restraint use(appropriately.restrained,not appropriately.restrained).COI levels by.home zip codes were stratified as very.low,low,moderate,high,and very.*** regression controlling for age identified factors associated with safety.restraint *** Of 337 patients,73.9%were appropriately.restrained and 26.1%were not appropriately.*** with appropriately.restrained patients,more not appropriately.restrained patients were from low-COI(26.1%vs 20.9%),high-COI(14.8%vs 10.8%)and very.high-COI(10.2%vs 3.6%)*** analy.is demonstrated no significant associations in appropriate restraint use and *** was a non-significant trend that children/adolescents from moderate-COI neighborhoods were more likely.than those from very.low-COI neighborhoods to be appropriately.restrained(OR=1.82,95%CI 0.78,4.28).Conclusion Injury.prevention initiatives focused on safety.restraints should target families of children from all neighborhood ty.es.

Geochemistry.of fine-grained sediments of the upper Cretaceous to Paleogene Gosau Group(Austria,Slovakia):Implications for paleoenvironmental and provenance studies

Bulk rock geochemistry.of 169 fine-grained sediment samples of the upper Cretaceous to Paleogene Gosau Group （Northern Calcareous Alps, Austria and Slovakia） fiom borehole and outcrop localities was performed to separate non-marine and marine deposits. Geochemical characteristics of different Gosau depositional sy.tems, basins and sediment provenance using major-, trace-, and rare earth elements were also investigated. Geochemical proxies such as boron concentrations were tested for seeking the possibilities of paleosalinity.indicators. Due to the [act that several pelagic sections are represented by.extremely.low boron contents, B/AI＊ ratios are recognized as more robust and differentiate reliably.between marine （mean： 160 ± 34） and non-marine （mean： 133 ± 33） samples. Using statistical factor analy.is, hemipelagic to pelagic samples flom the Gieghi_ibl Sy.cline and Slovakian equivalents can be differentiated from marginal-marine to non-marine samples from the Grtinbach and Glinzendorf Sy.cline related to terrigenous （SiO2, AI203, K20, Th, Rb, Zr and others） and pelagic indicative elements （CaO, St, TOT/C and B/AI＊）. A clear indication for ophiolitic provenance is traced by.high amounts of chromium and nickel. Only.non-marine successions of the Glinzendorf Sy.cline show higher Cr and Ni concentrations （up to 250 and 400 ppm, respectively. and enriched Cr/V and y.Ni ratios trending to an ultramafic source.
