Bone loss associated with type 1 diabetes mellitus(T1DM)begins at the onset of the disease,already in childhood,determining a lower bone mass peak and hence a greater risk of osteoporosis and fractures later in *** mechanisms underlying diabetic bone fragility are not yet completely *** and insulin deficiency can affect the bone cells functions,as well as the bone marrow fat,thus impairing the bone strength,geometry,and *** factors,like insulin and growth hormone/insulin-like growth factor 1,can control bone marrow mesenchymal stem cell commitment,and the receptor activator of nuclear factor-κB ligand/osteoprotegerin and Wnt-b catenin pathways can impair bone *** myokines may have a key role in regulating metabolic control and improving bone mass in T1DM *** aim of this review is to provide an overview of the current knowledge of the mechanisms underlying altered bone remodeling in children affected by T1DM.

Colorectal cancer(CRC)is a significant health problem,with around 1 million new cases and 500000 deaths every year *** the last two decades,the use of novel therapies and more complex treatment strategies have contributed to progressively increase the median survival of patients with unresectable advanced CRC up to approximately 30 *** availability of additional therapeutic options,however,has created new challenges and generated more complicated treatment ***,several clinically important points are still in debate in first-line,such as the optimal treatment intensity,the most appropriate maintenance strategy,the preferred biologic to be used upfront in patients with KRAS wild-type CRC,and the need for more detailed information on tumor *** this moving landscape,this review analyses why the firstline treatment decision is crucial and how the choicemay impact on further treatment *** addition,it focuses on results of major phaseⅢrandomized trials.

Phosphorus(P)deficiency is one of the main constraints on crop production in Arenosols(acid sandy soil).The high cost of P fertilizers may represent an insurmountable obstacle in many poor countries,leaving the exploitation of their own calcareous and phosphate rocks as the only low-cost and long-term *** is suggested to have positive effects on soil properties;however,there is no published research on the synergistic effects of biochar and rocky materials in modifying soil *** aim of this study was to investigate the chemical and biochemical responses of an acid Arenosol treated with phosphate rock(PR),calcareous rock(CR),and biochar(BC),and the implications for corn yield.A soil from Marracuene District,Mozambique was used,where corn was grown for 90 d with the soil treated with:no addition(control),water-soluble zinc phosphite fertilizer(WSP),PR,WSP+CR,WSP+BC,WSP+CR+BC,PR+BC,and PR+CR+*** was produced by pyrolysis of babycorn peels for 4 h at 450?C and applied at 11 g *** soil pHH2 Oincreased from about 4.54 in the control to 7.38 in the PR+CR+BC *** oxidizable organic carbon,cation exchange capacity,and available P were higher in the treatments containing BC than in the *** treatments containing CR and/or BC led to the highest activities of alkaline phosphomonoesterase,phosphodiesterase,andα-glucosidase,which increased P availability and gave the greatest biomass and *** suggest that biochar provides additional soluble P and supplies adsorption sites for phosphate,preventing its evolution to unavailable ***,PR applied together with BC contributed to an 840%yield increase compared to the *** treatments containing WSP and BC facilitated phosphite oxidation to phosphate and increased crop yield.

The growing use of nanomaterials in commercial goods and novel technologies is generating increasing questions about possible risks for human health and environment, due to the lack of an in-depth assessment of their potential toxicity. In this context, we investigated the effects of citrate-capped gold nanoparticles （AuNPs） on the model system Drosophila melanogaster upon ingestion. We observed a significant in vivo toxicity of AuNPs, which elicited clear adverse effects in treated organisms, such as a strong reduction of their life span and fertility, presence of DNA fragmentation, as well as a significant overexpression of the stress proteins. Transmission electron microscopy demonstrated the localization of the nanoparticles in tissues of Drosophila. The experimental evidence of high in vivo toxicity of a nanoscale material, which is widely considered to be safe and biocompatible in its bulk form, opens up important questions in many fields, including nanomedicine, material science, health, drug delivery and risk assessment.

Endoscopic ultrasonography(EUS) is a technique with an established role in the diagnosis and staging of gastro-intestinal tumors. In recent years, the spread of new devices dedicated to tissue sampling has improved the diagnostic accuracy of EUS fine-needle aspiration. The development of EUS-guided drainage of the biliopancreatic region and abdominal fluid collections has allowed EUS to evolve into an interventional tool that can replace more invasive procedures. Emerging techniques applying EUS in pancreatic cancer treatment and in celiac neurolysis have been described. Recently, confocal laser endomicroscopy has been applied to EUS as a promising technique for the in vivo histological diagnosis of gastro-intestinal, bilio-pancreatic and lymph node lesions. In this state-of-the-art review, we report the most recent data from the literature regarding EUS devices, interventional EUS, EUS-guided confocal laser endomicroscopy and EUS pancreatic cancer treatment, and we also provide an overview of their principles, clinical applications and limitations.

An Evaluation of Burnout among Foundation Doctors in Malta—A Cross-Sectional Observational Study

Background: Postgraduate medical trainees are at an increased risk of burnout. Burnout has been defined as “a psychological condition of emotional exhaustion, depersonalization and reduced personal accomplishment occurring in people persistently exposed to emotional and interpersonal stressors at work”. It can arise either from within the individual, from the medical profession itself or from healthcare organisations. Burnout is associated with mental health issues in trainees, impaired work performance and suboptimal patient outcomes. Objective: To evaluate the level of burnout among trainees of the Malta Foundation Programme. Methods: A cross-sectional observational study of three cohorts of the Malta Foundation Programme was conducted using the Copenhagen Burnout Inventory (CBI) to evaluate incidence of personal-, work-related and client related burnout. The cohorts were at 3 months, 1 year and 2 years of training respectively. The CBI was administered via Google Forms after ethical clearance. Descriptive statistics and non-parametric comparative statistics were used to analyse the results through SPSS v25.0. Results: A global response rate of 26.5% was achieved. CBI and its subscales showed good internal validity. High scores for burnout in all three scales were identified except for client-related burnout in First Year doctors. Burnout peaked midway through the 2-year period. High numbers reported “feeling “tired” and “physically exhausted”. Males reported more burnout than females. Conclusion: The high level of burnout so early in the career of Maltese postgraduate medical trainees is of concern. More studies are needed on the causality of this burnout and what factors are associated with burnout in this population.
