Prospective evaluation of laparoscopic Roux en Y gastric bypass in patients with clinically severe obesity

AIM: To evaluate and present our experience with laparoscopic Roux en Y gastric bypass (RYGB) in a selected patient population. METHODS: A cohort of 130 patients with a body mass index (BMI) between 35 and 50 kg/m2 were evaluated in relation to postoperative morbidity,weight loss and resolution of co-morbidities for a period of 4 years following laparoscopic RYGB. RESULTS: Early morbidity was 10.0%,including 1 patient with peritonitis who was admitted to Intensive Care Unit (ICU) and 1 conversion to open RYGB early in the series. There was no early or late mortality. Maximum weight loss was achieved at 12 mo postoperatively,with mean BMI 30 kg/m2,mean percentage of excess weight loss (EWL%) 66.4% and mean percentage of initial weight loss (IWL%) 34.3% throughout the follow-up period. The majority of preexisting comorbidities were resolved after weight loss and no major metabolic disturbances or nutritional deficiencies were observed. CONCLUSION: Laparoscopic RYGB appears to be a safe and effective procedure for patients with BMI 35-50 kg/m2 with results that are comparable to previously published data mostly from the USA but from Europe as well.

Machine learning(ML)is a type of artificial intelligence that assists computers in the acquisition of knowledge through data analysis,thus creating machines that can complete tasks otherwise requiring human *** its various applications,it has proven groundbreaking in healthcare as well,both in clinical practice and *** this editorial,we succinctly introduce ML applications and present a study,featured in the latest issue of the World Journal of Clinical *** authors of this study conducted an analysis using both multiple linear regression(MLR)and ML methods to investigate the significant factors that may impact the estimated glomerular filtration rate in healthy women with and without non-alcoholic fatty liver disease(NAFLD).Their results implicated age as the most important determining factor in both groups,followed by lactic dehydrogenase,uric acid,forced expiratory volume in one second,and *** addition,for the NAFLD-group,the 5th and 6th most important impact factors were thyroid-stimulating hormone and systolic blood pressure,as compared to plasma calcium and body fat for the NAFLD+***,the study's distinctive contribution lies in its adoption of ML methodologies,showcasing their superiority over traditional statistical approaches(herein MLR),thereby highlighting the potential of ML to represent an invaluable advanced adjunct tool in clinical practice and research.

BACKGROUND:Lumbar hernia is a rare congenital or acquired defect of the posterior abdominal wall. The acquired type is more common and occurs mainly as an incisional defect after flank surgery. Incarceration or strangulation of hernia contents is uncommon. METHOD:Segmental liver incarceration through a recurrent incisional lumbar defect was diagnosed in a 58 years old woman by magnetic resonance imaging. RESULTS:The patient underwent an open repair of the com-plicated hernia. An expanded polytetraflouoroethylene(e-PTFE) mesh was fashioned as a sublay prosthesis. She had an uncomplicated postoperative course. Follow-up examinations revealed no evidence of recurrence. CONCLUSIONS:Although lumbar hernia rarely results in incarceration or strangulation,early repair is necessary because of the risks of complications and the increasing difficulty in repairment as it enlarges. Surgical repair is often difficult and challenging.

Magnetic resonance （MR） imaging of the scrotum represents an important supplemental diagnostic tool in the evaluation of scrotal diseases. Diffusion-weighted （DW） MR imaging is a developing technique, proved to improve tissue characterization. We evaluated the feasibility and diagnostic performance of DW MR imaging in the detection and characterization of scrotal lesions. We retrospectively evaluated 31 scrotal lesions （23 intratesticular and 8 extratesticular） in 26 men. All MR examinations were performed on a 1.5-T unit, using a pelvic-phased array coil. DW sequences were obtained using a single shot, multislice spin echo planar diffusion pulse sequence and a b factor of 0 and 900 s mm-2. The DW MR characteristics and the apparent diffusion coefficient （ADC） values of normal scrotal contents and scrotal diseases were evaluated. Comparison between the ADC values of normal scrotum, benign lesions and scrotal malignancies was performed. The accuracy of conventional sequences, DW images alone and DW imaging combined with conventional images in differentiating benign from malignant scmtal lesions was calculated. The ADC values of testicular malignancies were different from those of normal testis and benign intratesticular lesions, and the ADC values of benign extratesticular lesions from those of normal epididymis （P〈0.05）. The overall accuracy of conventional imaging, DW imaging alone and DW MR combined with conventional sequences in the characterization of intratesticular lesions was 91%, 87% and 100%, respectively. Our findings suggest that DW MR imaging and ADC values may provide valuable information in the diagnosis and characterization of scrotal diseases.

Spontaneous coronary artery dissection(SCAD)is a rare non-atherosclerotic cause of acute coronary syndromes defined as non-iatrogenic,non-traumatic separation of the coronary artery *** most common profile is a middle-aged woman between 44 and 53 years with few cardiovascular risk *** is frequently linked with predisposing factors,such as postpartum,fibromuscular dysplasia or other vasculopathies,connective tissue disease and hormonal therapy,and it is often triggered by intense physical or emotional stress,sympathomimetic drugs,childbirth and activities increasing shear stress of the coronary artery *** with SCAD usually present at the emergency department with chest discomfort,chest pain,and rapid heartbeat or *** the last decades,the most common problem of SCAD was the lack of awareness about this condition which has led to significant underdiagnosis and ***,modern imaging techniques such as optical coherence tomography,intravascular ultrasound,coronary angiography or magnetic resonance imaging have contributed to the early diagnosis of the *** of SCAD remains controversial,especially during the last years,where invasive techniques are being used more often and in more emergent cardiac *** conservative treatment combining aspirin and betablocker remains the recommended strategy in most cases,revascularization could also be suggested as a method of treatment in specific indications,but with a higher risk of *** prognosis of SCAD is usually good and long-term mortality seems to be low in these ***-up should be performed on a regular basis.

We analyze the performance of a quantum Stirling heat engine(QSHE), using a two-level system and a harmonic oscillator as the working medium, that is in contact with a squeezed thermal reservoir and a cold reservoir. First, we derive closed-form expressions for the produced work and efficiency, which strongly depend on the squeezing parameter rh. Then, we prove that the effect of squeezing heats the working medium to a higher effective temperature, which leads to better overall performance. In particular, the efficiency increases with the degree of squeezing, surpassing the standard Carnot limit when the ratio of the temperatures of the hot and cold reservoirs is small. Furthermore, we derive the analytical expressions for the efficiency at maximum work and the maximum produced work in the high and low temperature regimes,and we find that at extreme temperatures the squeezing parameter rhdoes not affect the performance of the QSHE. Finally,the performance of the QSHE depends on the nature of the working medium.

Nowadays,the evaluation of coal deposits becomes crucial,due to many uncontrollable factors,which affect the energy sector.A comparative evaluation of coal deposits is essential for their hierarchical classification regarding their sustainable exploitation,when compared to other coal deposits or competitive fuels,which may be used as alternative solutions for electricity *** this paper,a method for spatial analysis and evaluation of a lignite deposit is proposed,by creating four spatial key indicators via GIS analysis,which are then aggregated by applying a weighted linear *** analytical hierarchy process is applied to estimate the relative weights of the indicators,in order to perform a weighted cartographic *** the synthesis of the indicators,an overall,total spatial quality indicator is *** weighted analysis was shown to be more effective compared to the unweighted one,because it can provide more reliable results regarding the exploitation of the examined lignite *** implementation of GIS-based analytical hierarchy process in spatial analysis and evaluation of lignite deposits,in terms of sustainable exploitation,demonstrates that this method can be extensively applied for evaluating the economic potential of mineral deposits.

We contimle the work initiated in [1] （Second order nonlinear evolution inclusions I： Existence and relaxation results. Acta Mathematics Science, English Series, 21（5）, 977-996 （2005）） and study the structural properties of the solution set of second order evolution inclusions which are defined in the analytic framework of the evolution triple. For the convex problem we show that the solution set is compact Rs, while for the nonconvex problem we show that it is path connected, Also we show that the solution set is closed only if the multivalued nonlinearity is convex valued. Finally we illustrate the results by considering a nonlinear hyperbolic problem with discontinuities.
