Long-term follow-up of kidney transplant recipients admitted to a tertiary care transplant center with SARS-CoV-2

BACKGROUND Kidney transplant recipients(KTR)are at risk of severe coronavirus disease 2019(COVID-19)disease and mortality after severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)*** predicted that hospitalization for COVID-19 and subsequent admission to the intensive care unit(ICU)would yield worse outcomes in *** was stratified by ICU *** of interest included risk factors for ICU admission and mortality,length of stay(LOS),respiratory symptoms at admission,all-cause graft failure at the last follow-up,and death related to *** 96 KTRs were hospitalized for SARS-COV-2 infection.21(22%)required ICU *** ICU group had longer hospital LOS(21.8 vs 8.6 days,P<0.001)and were more likely to experience graft failure(81%vs 31%,P<0.001).Of those admitted to the ICU,76%had death at last-follow up,and 71%had death related to *** factors for ICU admission included male sex(aHR:3.11,95%CI:1.04-9.34;P=0.04).Risk factors for all-cause mortality and COVID-19-related mortality included ICU admission and advanced age at SARS-CoV-2 *** was highest within a month of COVID-19 diagnosis,with the ICU group having increased risk of all-cause(aHR:11.2,95%CI:5.11-24.5;P<0.001)and COVID-19-related mortality(aHR:27.2,95%CI:8.69-84.9;P<0.001).CONCLUSION ICU admission conferred an increased risk of mortality,graft failure,and longer ***-fifth of those hospit-alized died of COVID-19,reflecting the impact of COVID-19-related morbidity and mortality among KTRs.

评估极端干旱对优势植物物种光合速率(Pn)的影响对于我们理解极端干旱影响生态系统功能的机制至关重要。极端干旱可能由降雨量减少或降雨频率减少造成。这两种不同模式的极端干旱可能造成不同的影响。此外,不同草原类型可能对这两种极端干旱模式表现出不同的敏感性。然而,目前尚不清楚这两种极端干旱模式对草原优势植物物种Pn的影响有何差异,以及在不同草原类型这两种不同模式的极端干旱的效应是否改变。为此,我们于2014年在内蒙古草原两个不同类型的草原(荒漠草原vs.典型草原)站点各进行了一项多年的极端干旱(CHR:将生长季期间每次降雨事件的降雨量减少66%***:完全排除生长季内一段较短时间的降水)模拟实验。于2017年的7月和8月,我们在这两个草原站点各测量了两种优势物种的Pn。结果发现,CHR和INT均显著降低了优势物种的Pn。无论草原类型如何,INT对Pn造成的负面影响都更大。在荒漠草原,极端干旱对Pn的负面影响普遍大于典型草原,尤其是对于CHR处理下的羊草(Leymus chinensis)。这些结果表明,与降雨量减少相比,降雨频率减少所引起的极端干旱对Pn的负面影响更大。草地类型可能会改变极端干旱模式效应的程度,但不会改变其方向。这些发现强调了应对未来极端干旱的草原生态系统管理应考虑极端干旱模式和草地类型的影响。

Phenotype analysis and rescue on female FVB.129-Fmr1 knockout mice

Fragile X syndrome （FXS） is the most common monogenic cause of intellectual disability and a cause for autism. FXS females report milder phenotypes and a lower rate of cognitive problems compared to males. This is most likely because most females are heterozygous, while males are hemizygous for the disease. Thus, most preclinical studies have been completed in males. As there is major interest in testing experimental drugs for FXS, it is imperative to determine whether females in animal models used for research, present behavioral alterations that might translate to humans in order to confirm that experimental drugs have an effect on both genders. In our study we describe behavioral phenotypes in homozygous FXS female mice developed on the FVB.129 background. We focused on detection of hippocampal-mediated cognitive abilities and other behaviors described for FXS. Our research shows that, while female FVB.129-Fmrl knockout mice present normal learning, they have impaired memory, as well as susceptibility to audiogenic seizures. In agreement with previous reports in rodents and humans, significant levels of the small GTPase Racl were found in FXS female mice. Because Racl is involved in neuronal development, plasticity and behavior, we additionally aimed to pharmacologically inhibit Racl and determine whether observed phenotypes are rescued. Treatment of female FVB.129-Fmrl knockout with a Racl inhibitor abolished behavioral deficits, bringing phenotypes to control levels. Our results suggest that female FVB.129-Fmrl knockout mice display behavioral impairments that resemble FXS in humans. Moreover, those behavioral shortfalls might be associated with alteration of plasticity involving excessive Racl function, since pharmacological reduction of Racl normalizes previously altered phenotypes to control levels.

Split-dose bowel preparation improves adequacy of bowel preparation and gastroenterologists' adherence to National Colorectal Cancer Screening and Surveillance Guidelines

AIM To quantify the impact of split-dose regimen on endoscopists' compliance with guideline recommendations for timing of repeat colonoscopy in patients with normal colonoscopy or 1-2 small polyps( 50 years old with a normal screening colonoscopy and 1-2 small polyps. Data were abstracted from two time periods, pre and post-split-dose bowel preparation institution. Main outcome measurements were recommendation for timing of repeat colonoscopy and bowel preparation quality. Bivariate analysis by χ~2 tests and Student's t-tests were performed to assess differences between the two cohorts. Multivariable logistic regression was used with guideline consistent recommendations as the dependent variables and an indicator for 2011 cohort as the primary predictor. RESULTS Four thousand two hundred and twenty-five patients were included in the study; 47.0%(1987) prior to the institution of split dose bowel preparation, and 53.0%(2238) after the institution of split dose bowel preparation. Overall, 82.2%(n = 3472) of the colonoscopies were compliant with guideline recommendations, with a small but significantly increased compliance rate in year 2011(83.7%) compared to year 2009(80.4%, P = 0.005), corresponding to an unadjusted odds ratio of 1.25(95%CI: 1.07-1.47; P = 0.005). Colonoscopies with either "Adequate" or "Excellent" had increased from 30.6% in year 2009 to 39.6% in year 2011(P < 0.001). However, there was no significant difference in poor/inadequate category of bowel preparation as there was a mild increase from 4.6% in year 2009 to 5.1% in year 2011(P = 0.50). CONCLUSION Split-dose bowel regimen increases endoscopists' compliance to guidelines in average-risk patients with normal colonoscopy or 1-2 small polyps.

Contradiction between strong lensing statistics and a feedback solution to the cusp/core problem

Standard cosmology has many successes on large scales, but faces some fundamental difficulties on small, galactic scales. One such difficulty is the cusp/core problem. High resolution observations of the rotation curves for dark matter domi- nated low surface brightness （LSB） galaxies imply that galactic dark matter halos have a density profile with a flat central core, whereas N-body structure formation simula- tions predict a divergent （cuspy） density profile at the center. It has been proposed that this problem can be resolved by stellar feedback driving turbulent gas motion that erases the initial cusp. However, strong gravitational lensing prefers a cuspy density profile for galactic halos. In this paper, we use the most recent high resolution observations of the rotation curves of LSB galaxies to fit the core size as a function of halo mass, and compare the resultant lensing probability to the observational results for the well defined combined sample of the Cosmic Lens All-Sky Survey （CLASS） and Jodrell Bank/Very Large Array Astrometric Survey （JVAS）. The lensing probabilities based on such density profiles are too low to match the observed lensing in CLASS/JVAS. High baryon densities in the galaxies that dominate the lensing statis- tics can reconcile this discrepancy, but only if they steepen the mass profile rather than making it more shallow. This places contradictory demands upon the effects of baryons on the central mass profiles of galaxies.

Chondroitinase ABC combined with Schwann cell transplantation enhances restoration of neural connection and functional recovery following acute and chronic spinal cord injury

Schwann cell transplantation is considered one of the most promising cell-based therapy to repair injured spinal cord due to its unique growth-promoting and myelin-forming properties.A the Food and Drug Administration-approved Phase I clinical trial has been conducted to evaluate the safety of transplanted human autologous Schwann cells to treat patients with spinal cord injury.A major challenge for Schwann cell transplantation is that grafted Schwann cells are confined within the lesion cavity,and they do not migrate into the host environment due to the inhibitory barrier formed by injury-induced glial scar,thus limiting axonal reentry into the host spinal *** we introduce a combinatorial strategy by suppressing the inhibitory extracellular environment with injection of lentivirus-mediated transfection of chondroitinase ABC gene at the rostral and caudal borders of the lesion site and simultaneously leveraging the repair capacity of transplanted Schwann cells in adult rats following a mid-thoracic contusive spinal cord *** report that when the glial scar was degraded by chondroitinase ABC at the rostral and caudal lesion borders,Schwann cells migrated for considerable distances in both rostral and caudal *** Schwann cell migration led to enhanced axonal regrowth,including the serotonergic and dopaminergic axons originating from supraspinal regions,and promoted recovery of locomotor and urinary bladder ***,the Schwann cell survival and axonal regrowth persisted up to 6 months after the injury,even when treatment was delayed for 3 months to mimic chronic spinal cord *** findings collectively show promising evidence for a combinatorial strategy with chondroitinase ABC and Schwann cells in promoting remodeling and recovery of function following spinal cord injury.
