Background:Newly diagnosed breast cancer patients experience symptoms that may affect their quality of life,treatment outcomes,and *** and managing breast cancer-related symptoms soon after diagnosis is *** purpose of this study was to investigate the associations between health-related fitness(hRF)and patient-reported symptoms in newly diagnosed breast cancer ***:This study utilized baseline data from the Alberta Moving Beyond Breast cancer cohort Study that were collected within 90 days of *** measures included peak cardiopulmonary fitness(peak volume of oxygen consumption(VO_(2peak))),maximal muscular strength and endurance,flexibility,and body *** measures included depression,sleep quality,and *** multivariable logistic regression was performed for ***:Of 1458 participants,51.5%reported poor sleep quality,26.5%reported significant fatigue,and 10.4%reported moderate *** multivariable-adjusted models,lower relative VO_(2peak)was independently associated with a greater likelihood of all symptom measures,including moderate depression(pcant fatigue(p=0.008),any symptom(phreshold associations with all symptom measures such that all 3 lower quartiles exhibited similar elevated risk compared to the highest *** strength of the threshold associations varied by the symptom measure with odds ratios ranging from-1.5 for poor sleep quality to-3.0 for moderate depression and multiple ***,lower relative upper body muscular endurance was also independently associated with fatigue in a dose-response manner(p=0.001),and higher body weight was independently associated with poor sleep quality in an inverted U pattern(p=0.021).conclusion:Relative VO_(2peak)appears to be a critical hRF component associated with multiple patient-reported symptoms in newly diag

背景变应原特异性IgG4 (sIgG4)抗体通常与耐受性相关,目前已有报道称在成人嗜酸性粒细胞性食管炎(EoE)患者中,sIgG4抗体与某些食物有关,但目前尚缺乏儿科患者针对某些食物的sIgG4抗体阳性率及滴度的研究。目的调查EoE患儿和未经选择的对照幼儿中抗某些食物的血清sIgG4水平,并探讨性别、年龄和牛奶摄入量对sIgG4水平的影响。方法对71例EoE患儿和210名青春期儿童(源自未经选择的出生队列,此队列来自Viva项目)的血清进行检测,测定抗牛乳(cow's milk,cM)中主要蛋白(α乳清蛋白、β乳球蛋白、酪蛋白)、小麦、大豆、鸡蛋、花生蛋白的sIgG4和特异性IgE (specific IgE,sIgE)水平。结果与对照组相比,EoE患儿血清中抗牛乳蛋白sIgG4滴度(≥10μg/ml)普遍升高,两组的比值比为5. 5～8. 4,而EoE患儿抗牛乳蛋白sIgE水平多≤4 U/ml,因此sIgG4/sIgE的比值通常≥10 000。当对年龄和牛奶摄入量进行调整后发现,高滴度的抗牛乳蛋白sIgG4水平与EoE相关;男性儿童中3种抗牛乳蛋白sIgG4水平在病例组与对照组的比值比均>20。结论与对照组相比,EoE患儿的抗牛乳蛋白sIgG4抗体的阳性率及滴度均增加。虽然尚不清楚这种抗体是否为EoE的致病因素,但sIgG4抗体的增加是引起EoE局部免疫应答的重要特征。

Zeolite-13X-supported Fe（Fe/zeolite-13X） catalysts with various Fe contents were prepared by the wet impregnation *** catalysts were characterized by N2 adsorption-desorption isotherms to estimate the Brunauer-Emmett-Teller surface areas and barrett-Joyner-hanlenda pore size distributions.X-ray diffraction,scanning electron microscopy,temperature-programmed reduction,and temperature-programmed desorption of Nh3 were used to investigate the textural properties of the Fe/zeolite-13 X *** catalytic activities were determined for the complete oxidation of1,4-dioxane using air as the oxidant in a fixed-bed flow reactor in the temperature range 100-400℃.The influences of various process parameters,such as reaction temperature,metal loading,and gas hourly space velocity（GhSV）,on the dioxane removal efficiency by catalytic oxidation were *** stability of the catalyst was tested at 400℃ by performing time-on-stream analysis for 50 *** Fe/zeolite-13 X catalyst with 6 wt%Fe exhibited the best catalytic activity among the Fe/zeolite-13 X catalysts at 400℃ and a GhSV of 24000 h^（-1）,with 97%dioxane conversion and95%selectivity for the formation of carbon oxides（cO and cO2）.Trace amounts（ 3%） of acetaldehyde,ethylene glycol monoformate,ethylene glycol diformate,1,4-dioxane-2-ol,1,4-dioxane-2-one,and 2-methoxy-1,3-dioxalane were also formed as degradation products.A plausible degradation mechanism is proposed based on the products identified by Gc-MS analysis.

Mortality is a well-established patient-important outcome in critical care *** contrast,morbidity is less uniformly reported(given the myriad of critical care illnesses and complications of each)but may have a common end-impact on a patient’s functional capacity and health-related quality-of-life(hRQoL).Survival with a poor quality-of-life may not be acceptable depending on individual patient values and ***,as mortality decreases within critical care,it becomes increasingly important to measure intensive care unit(IcU)survivor *** measurements with a preference-based scoring algorithm can be converted into health utilities on a scale anchored at 0(representing death)and 1(representing full health).They can be combined with survival to calculate quality-adjusted life-years(QALY),which are one of the most widely used methods of combining morbidity and mortality into a composite *** QALYs have been use for health-technology assessment decision-making,an emerging and novel role would be to inform clinical decision-making for patients,families and healthcare providers about what expected hRQoL may be during and after IcU *** care randomized control trials(RcTs)have not routinely measured or reported hRQoL(until more recently),likely due to incapacity of some patients to participate in patient-reported outcome *** differences in hRQoL measurement tools can lead to non-comparable *** this end,we propose the validation of a gold-standard hRQoL tool in critical care,specifically the *** combined health-utility and mortality(disaggregated)and QALYs(aggregated)can be reported,with disaggregation allowing for determination of which components are the main drivers of the QALY *** use of hRQoL,health-utility,and QALYs in critical care RcTs has the potential to:(1)Increase the likelihood of finding important effects if they exist;(2)improve research efficiency;and(3)help inform optimal m

AIM:To demonstrate the feasibility of optical coherence tomography(OcT)imaging in differentiating cervical inlet patch(cIP)from normal esophagus,barrett'sesophagus(BE),normal stomach and duodenum. METhODS:This study was conducted at the Veterans Affairs Boston healthcare System(VABhS).Patients undergoing standard esophagogastroduodenoscopy at VABhS,including one patient with cIP,one representative patient with BE and three representative normal subjects were *** light video endoscopy was performed and endoscopic 3D-OcT images were obtained in each patient using a prototype OcT *** OcT imaging probe passes through the working channel of the endoscope to enable simultaneous video endoscopy and 3D-OcT examination of the human gastrointestinal(GI)*** hematoxylin and eosin(h and E)histology was performed on biopsy or endoscopic mucosal resection specimens in order to compare and validate the 3D-OcT data. RESULTS:cIP was observed from a 68-year old male with gastroesophageal reflux *** cIP region appeared as a pink circular lesion in the upper esophagus under white light *** imaging over the cIP region showed columnar epithelium structure,which clearly contrasted the squamous epithelium structure from adjacent normal esophagus.3D-OcT images obtained from other representative patients demonstrated distinctive patterns of the normal esophagus,BE,normal stomach,and normal duodenum ***,such as squamous epithelium,lamina propria, muscularis mucosa,muscularis propria,esophageal glands,barrett's glands,gastric mucosa,gastric glands, and intestinal mucosal villi were clearly observed with OcT and matched with h and E *** results demonstrated the feasibility of using OcT to evaluate GI tissue morphology in situ and in real-time. cONcLUSION:We demonstrate in situ evaluation of cIP microstructures using 3D-OcT,which may be a useful tool for future diagnosis and follow-up of patients with cIP.

Background: Assessment of clinical factors associated with barrett’s esophagus (BE) length remained within the realm of anecdotal reports or one center’s experience. The aim of this multicenter study was to determine which clinical factors are highly correlated with the length of BE. Methods: Patients diagnosed with BE were recruited into the study from 5 academic centers in the United States. All patients had an upper endoscopy that documented BE by the presence of intestinal metaplasia in biopsy specimens. All patients were evaluated by a validated demographic questionnaire and the GERD Symptom checklist. Results: A total of 263 patients with BE were recruited into the study. Mean BE length for the entire sample was 4 ±3.3 cm. A longer hiatal hernia (r = 0.22, p h2 receptor antagonist (h2-RA) consumption (t = 1.98, p correlated with a longer segment of BE. Proton pump inhibitors (PPI) (t=1.96, p correlated with a shorter segment of BE. conclusions: PPIs were correlated with shorter lengths of BE. In contrast, a longer hiatal hernia, any dysplasia, nonsmoking, or use of h2-RAs were correlated with a longer BE segment.

Surgical esophagectomy, intensive endoscopic surveillan- ce,and mucosal ablative techniques, particularly photodynamic therapy (PDT), h ave been proposed as possible management strategies for barrett’s high-grade d ysplasia (hGD). Each option has advantages and disadvantages, and no firm consen sus exists for the preferred strategy at this time. The purpose of this pilot st udy was to gain insight into patient preferences in barrett’s hGD management. T wenty patients with barrett’s esophagus were enrolled in a questionnaire study. The three possible management options for barrett’s hGD including each option ’s potential benefits and harms were presented to the subject in a formalized p resentation that was designed to be easily comprehendible by patients. The subje cts rated each strategy using a health-related quality of life instrument and c hose one of the management strategies assuming they were found to have hGD. The average feeling thermometer rating scale values for the management strategies we re as follows: endoscopic surveillance,79; esophagectomy, 46; and PDT, 60. When asked to choose a strategy, 14 (70%) chose endoscopic surveillance,3 (15%) cho se esophagectomy, and 3 (15%) chose PDT. These findings were statistically sign ificant (P = 0.0024). The patients who chose endoscopic su- rveillance felt “comfortable" with endoscopy,while the most common concern ab out esophagectomy,and PDT was the risk of death and the unknown risk of recurren ce, respectively. In summary, when patients with barrett’s esophagus were prese nted with three options to manage hGD, the majority chose endoscopic surveillanc e. Familiarity with endoscopic surveillance was the predominant reason for the c hoice.

Background: Numerous ablative modalities have demonstrated varying degrees of efficacy in the eradication of barrett‘s esophagus (BE). however, they are associated with either high cost, patient discomfort, complications, tedious applica tion, residual underlying BE, or some combination of the above. The aim of this study was to evaluate the safety and the efficacy of a new cryogenic device by u sing liquid nitrogen sprayed through an open tipped cryogenic catheter placed th rough the accessory channel of an upper endoscope. Methods: A prospective, singl e center studywas performed on patients with a long-standing history of BE foll owed in a barrett‘s registry. Patients with degrees of dysplasia ranging from n one to multifocal high-grade dysplasia were treated. All patients were treated with 40 mg rabeprazole 3 times per day during the treatment period. Elimination of acid reflux was confirmed via 24-hour esophageal ph studies. cryoablation wa s applied hemicircumferentially to 4-cm-long segments at monthly intervals, un til the entire segment of BE was eliminated. Results: Eleven patients were treat ed. There was reversal of BE in all patients. In 9 of 11 (78%) patients who com pleted the protocol, there was complete endoscopic and histologic reversal of BE . There was no subsquamous specialized intestinal metaplasia at the 6-month fol low-up. No complications occurred. conclusions: Based on preliminary results, l ow-pressure spray cryoablation of BE under direct endoscopic visualization is s afe and easy to perform. Its relative lack of patient discomfort and its simplic ity and demonstrated efficacy make it a modality that should be further explored in the ablation of GI mucosal lesions such as BE and perhaps early esophageal c ancer.

Background and aims: When saliva, with its high nitrite content derived from the enterosalivary recirculation of dietary nitrate, meets acidic gastric juice, the nitrite is converted to nitrous acid, nitrosative species, and nitric oxide. In healthy volunteers this potentially mutagenic chemistry is focused at the gastric cardia. We have studied the location of this luminal chemistry in barrett’s patients during acid reflux. Methods: Ten barrett’s patients were studied before and after administration of 2 mmol nitrate. Using microdialysis probes we measured nitrite, ascorbic acid, total vitamin c, and thiocyanate concentrations and ph simultaneously in the proximal oesophagus, barrett’s segment, hiatal sac, proximal stomach, and distal stomach. In a subgroup, real time nitric oxide concentrations were also measured. Results: During acid reflux, barrett’s segment was the anatomical site with maximal potential for acid catalysed nitrosation, with its median concentration of nitrite exceeding that of ascorbic acid in two of 10 subjects before nitrate and in four of nine after nitrate. Thiocyanate, which catalyses acid nitrosation, was abundant at all anatomical sites. On entering the acidic barrett’s segment, there was a substantial fall in nitrite and the lowest ascorbic acid to total vitamin c ratio, indicative of reduction of salivary nitrite to nitric oxide at this anatomical site. Episodes of acid reflux were observed to generate nitric oxide concentrations of up to 60 μM within the barrett’s segment. conclusion: The interaction between acidic gastric refluxate and nitrite rich saliva activates potentially mutagenic luminal nitrosative chemistry within barrett’s oesophagus.
