Morbidity Profile and Causes of Mortality in Type 2 Diabetes Patients: Data from a Tertiary Teaching Hospital from Eastern India

Background: Data regarding the prevalence of morbidity and mortality in patients of Type 2 Diabetes Mellitus (T2DM) is scanty in India. Objectives: To determine the prevalence of micro and macro vascular complications, acute metabolic complications, infections, Non Alcoholic Fatty Liver Disease (NAFLD) and cause of mortality in T2DM patients admitted to a tertiary care teaching hospital in Eastern India. Material and Methods: This was a hospital-based prospective study evaluating 150 T2DM patients admitted to a tertiary care institution in Eastern India. Diagnosis of micro and macro vascular complications, infections and NAFLD was made using standard protocols. In case of death, the most probable cause was noted. Results: Out of 150 patients, 14.7% of patients were newly diagnosed T2DM and out of them 41% of patients had vascular complications and 54.5% had infections. Of the total patients, 56% had nephropathy, 20% neuropathy, 17.3% retinopathy, 31.3% CVD, 11.3% CAD, 4.6% acute metabolic complications, 44% infections and 16.6% had NAFLD respectively. Macrovascular events occured earlier than microvascular complications. Multiple logistic regression analysis showed strong association of age, duration of diabetes, serum cholesterol, triglyceride, LDL-C with retinopathy (Regression coefficient β: -0.1086807, 0.4127152, -0.0513393, 0.0146429, 0.0587475;p < 0.05, < 0.001, < 0.05, < 0.05, < 0.05 respectively), while only duration of diabetes was strongly associated with nephropathy and neuropathy (Regression coefficient β: 0.2538751, 0.2261636;p < 0.001 for each). Increasing age was associated with CAD (Regression coefficient β: 0.055392;p β: 0.0055014;p 18.6% patients died due to diabetes related complications. Cardiovascular (CV)-related deaths (CVD+CAD) were most common cause (51.5%: CVD 36.4%, CAD 15.1%) to be followed by infections (27.3%) and then chronic kidney disease (12.1%). We conducted a comprehensive study of the neutron capture cross section of 12C within the relevant astrophysical energy *** rigorous R-matrix analysis,we determined the Maxwellian-averaged cross section to be 11.98±0.25μb at kT=30 *** result is approximately four times higher than the thermal neutron capture cross section estimated in earlier studies assuming the 1/v *** implications of our findings extend to the region of inhomogeneous Big-Bang models in nuclear astrophysics,where understanding the behaviour of neutron capture cross sections plays a crucial role in elucidating the intricate processes that shaped the early universe.

SARS-CoV2 Nsp3 protein triggers cell death and exacerbates amyloid β42-mediated neurodegeneration

Infection caused by the severe acute respiratory syndrome coronavirus 2(SARS-CoV2) virus, responsible for the coronavirus disease 2019(COVID-19) pandemic, induces symptoms including increased inflammatory response, severe acute respiratory syndrome(SARS), cognitive dysfunction like brain fog, and cardiovascular defects. Long-term effects of SARS-CoV2 COVID-19 syndrome referred to as post-COVID-19 syndrome on age-related progressive neurodegenerative disorders such as Alzheimer’s disease remain understudied. Using the targeted misexpression of individual SARS-CoV2 proteins in the retinal neurons of the Drosophila melanogaster eye, we found that misexpression of nonstructural protein 3(Nsp3), a papain-like protease, ablates the eye and generates dark necrotic spots. Targeted misexpression of Nsp3 in the eye triggers reactive oxygen species production and leads to apoptosis as shown by cell death reporters, terminal deoxynucleotidyl transferase(TdT) dUTP Nick-end labeling(TUNEL) assay, and dihydroethidium staining. Furthermore, Nsp3 misexpression activates both apoptosis and autophagy mechanism(s) to regulate tissue homeostasis. Transient expression of SARS-CoV2 Nsp3 in murine neuroblastoma, Neuro-2a cells, significantly reduced the metabolic activity of these cells and triggers cell death. Misexpression of SARS-CoV2 Nsp3 in an Alzheimer’s disease transgenic fly eye model(glass multiple repeats [GMR]>amyloid β42) further enhances the neurodegenerative rough eye phenotype due to increased cell death. These findings suggest that SARS-CoV2 utilizes Nsp3 protein to potentiate cell death response in a neurodegenerative disease background that has high pre-existing levels of neuroinflammation and cell death.

Impact of Cumulative Exposure to Circulating Ovarian Sex Hormones on Increasing the Risk of Hormone Receptor-Positive Breast Cancer

There were 2.3 million new cases of breast cancer registered in 2020, establishing it as the world’s leading cancer. The regular exposure of breast cells to circulating ovarian sex hormones may contribute to cancer growth at all stages. A literature review was performed by downloading research articles from PubMed Central, with keywords such as breast cancer, hormone exposure in breast cancer, estrogen/progesterone exposure in breast cancer, and breast cancer risk factors. Since breast cancer is a hormone-related disease, several factors that influence hormonal conditions or are indicators of alterations in hormonal conditions have been linked with the risk for breast cancer. Likewise, an elevated risk for breast cancer has been related to reproductive history and exogenous estrogen exposure because they may amplify lifetime estrogen exposure. All these factors address the total cumulative exposure to circulating ovarian sex hormones that a female is vulnerable to in her lifetime. A healthy lifestyle, a more extended breastfeeding period, and regular physical activity may help lower one’s risk. This review summarizes the factors that increase the risk of estrogen/progesterone (hormone receptor) receptor-positive breast cancer due to overall cumulative estrogen exposure, with an understanding of estrogen biosynthesis mechanisms and estrogen receptor signaling.

Dietary supplementation of zinc oxide modulates intestinal functionality during the post-weaning period in clinically healthy piglets

Design of a Robust Optimal Decentralized PI Controller Based on Nonlinear Constraint Optimization For Level Regulation: An Experimental Study

This paper presents the development of a new robust optimal decentralized PI controller based on nonlinear optimization for liquid level control in a coupled tank system. The proposed controller maximizes the closed-loop bandwidth for specified gain and phase margins, with constraints on the overshoot ratio to achieve both closed-loop performance and robustness. In the proposed work, a frequency response fitting model reduction technique is initially employed to obtain a first order plus dead time(FOPDT) model of each higher order subsystem. Furthermore, based on the reduced order model, a proposed controller is designed. The stability and performance of the proposed controller are verified by considering multiplicative input and output uncertainties. The performance of the proposed optimal robust decentralized control scheme has been compared with that of a decentralized PI controller. The proposed controller is implemented in real-time on a coupled tank system. From the obtained results,it is shown that the proposed optimal decentralized PI controller exhibits superior control performance to maintain the desired level, for both the nominal as well as the perturbed case as compared to a decentralized PI controller.
