Cardiovascular Disorders during HIV-AIDS in the Point G University, Hospital Internal Medical Service

The number of HIV-infected patients in the world is estimated at 34 million, including 30.1 million adults with 2.7 million new infections and 1.8 million deaths from HIV according to the 2011 UN AIDS report [1]. The prevalence of HIV infection in the general population of Mali is estimated at 1.1% according to DHSM-V [2]. Cardiovascular manifestations are increasingly encountered during our consultations with patients living with HIV. Thus we initiated this work with the aim of studying the cardiovascular lesions during the HIV-AIDS in the department of internal medicine of the point G. Methods: This retrospective descriptive study was conducted in the Internal Medicine department of the University Hospital Center (CHU) of the Point G from January 2008 to December 2012. Having included all HIV + patients hospitalized in the service with cardiovascular manifestations and who had performed a chest X-ray, cardiac and vascular ultrasound, electrocardiogram and/or cerebral computed tomography. Results: At the end of this work, 20 patients with cardiovascular lesions were identified in 273 hospitalized patients during the study period, i.e. a frequency of 7.3%. The 40 to 50 age group accounted for 45.0% with an average age of 42.4 ± 8.73 years. The most common cardiovascular risk factors were hypertension and active smoking with 31.58% each. Pericarditis accounted for 32% of cases, followed by cardiomyopathies with 23%. Conclusion: In our study, cardiovascular lesions are common in PHAs and etiologies are dominated by pericarditis.

在过去几十年里,家长参与一直是研究的焦点,显示出对学生学业成就至关重要。许多研究人员已经注意到家长在提高学生学业成绩方面的作用,但很少有研究调查了公立高中(Lycée)学生家长参与与学业成绩之间的实际关系。本研究的主要目标是确定马里家长在子女教育中的参与情况,家长参与子女学业成就的影响因素,以及这些因素对学生学业成绩的影响。具体目标包括:(1)评估家长参与的情况,(2)深入探讨影响家长参与的因素,(3)研究这些因素对子女学业成绩的影响。本研究的参与者来自马里首都巴马科第四区的一所公立高中。采用混合研究方法,招募了来自四个班级的184名十年级和十一年级学生的家长。志愿学生向其家长进行了调查。采用了封闭式结构化问卷和半结构化访谈来收集数据。通过描述性分析对马里公立高中家长参与情况进行了探索。采用主题分析来探讨家长参与对子女学业成绩的影响因素;采用多元回归分析来研究这些因素对学生学业成绩的影响。结果显示:(a)高中学生家长对家长参与有着显著的认知;(b)家长自身经验、家长的愿望和期望、家长的社会经济地位、家长的教育水平、家庭规模、家庭结构和家长职业是主要影响因素;(c)家长参与对子女学业成绩产生了积极影响。具体来说,家长在家庭和学校的参与是影响学业成绩的最重要预测因素。根据研究结果,提出以下建议和启示,以增进家长参与并促进学业成绩:鼓励改变,改善家长在子女生活各个方面的参与,进而提高学生学业成绩,例如协助学校人员更加开放和易于家长参与;制定计划,让家长更加意识到他们对子女成绩的影响。

Management of a Severe Form of Verneuil’s Disease in Its Genitoperineal Localisation in Bobo-Dioulasso

The authors report a severe form of genitoperineal localisation of Verneuil’s disease managed in a surgical environment, having evolved favorably with improvement of the patient’s quality of life, but persistence of unsightly skin scars. They insist on the unexceptional nature of this disease whose prognosis is reserved, sometimes leaving severe functional sequelae. The long evolution of the disease with prolonged hospitalization in a professionally active young patient and the extension of suppurations to the perineal, scrotal and inguinal regions was the features of this observation. After wide excision of the lesions with antibiotic associated with local care, evolution had taken to complete wound healing with improved quality of life and a recovery of his professional activity.

Postoperative Occlusion of the Small Bowel with Flanges and/or Adhesions in General Surgery of kati BSS CHU

Objectives were to determine the frequency, describe the epidemiological and clinical aspects, therapeutic and analyze the postoperative course. Methodology: This was a retrospective study that covered 08 years (January 2009-December 2017). Inclusion criteria: all patients operated for obstruction of the small bowel by hail and/or flanging. exclusion criteria: other types of occlusion and non-operated patients. Result: We recorded a total of 162 cases of hail obstruction by adhesions and/or flanges at 2.87%. The average age was 32.04, the sex ratio was 1.2. The average consultation time was 4 days. Abdominal pain associated with stopping of material and gas was present in all our patients. X-ray of the abdomen without preparation carried out in all the patients made it possible to objectify in 150 patients (92.6%) of the hydro-hail levels. Inoperative occlusion of hail on flange was present in 80 patients (49.4%). Occlusion of the small bowel on flange and adhesion was present in 69 patients. Adhesion obstruction of hail accounted for 6.8% (11 cases). The most commonly used surgical technique was flange resection in 91 patients (56.2%). The follow-up was simple in 151 patients (93.2%). Mortality was 1.2% of cases, i.e. 2 deaths. The average duration of hospitalization was 6 days. Conclusion: Occlusion of the small bowel by flanging and/or adherence is a surgical emergency whose prognosis depends on early management.

Drug Prescription Analysis at Hospital Discharge for Heart Failure Patients at the Institute of Cardiology of Abidjan

Introduction: Understanding improvement of pathophysiology of heart failure has allowed therapeutic progress over the past two decades in the pathology management. Our patients should benefit from these new drugs that improve survival. Objective: To analyze the treatment of hospital discharge according to eSC (european Society of Cardiology) Guidelines. Methods: We carried out a retrospective and descriptive study which included completed survey of patients hospitalized for heart failure in Medicine Department of cardiology Institut of Abidjan between January 1st 2011 to December 31st 2012. We analyzed the drugs prescription during hospital discharge by using the register of hospitalization. Results: 92.9% of the 532 files retained were included. Patients had a mean age of 54.4 ± 16.4 years old. 36.3% of the cases had a heart failure history with an average of 5.7 ± 3.2 days of hospital stay. At the hospital discharge, patients had for prescription: a diuretic specially Furosemide (100%), angiotensin converting enzyme inhibitor (ACeI) or angiotensin receptor blockers (ARB) (63.7%), a beta-blocker (17.9%) and a mineralocorticoids receptor antagonist (MRA) (51.2%). Diuretic, IeC or ARB and the MRA were prescribed systematically. Beta-blockers were lower prescribed to patients who showed no more signs of congestion. Conclusion: Our prescribing practices were adapted to the guidelines for heart failure management. However, the gaps will be corrected through sensitization and training.

Acute Intestinal Occlusions at the Cs Ref of Commune I of Bamako

Acute intestinal obstructions are defined as a complete and persistent cessation of materials and gases in a segment of the digestive tract. They constitute a medical-surgical emergency. Our work aimed to study acute intestinal obstructions, to determine the hospital frequency, to describe the aspects (epidemiological, clinical and therapeutic), to analyze the surgical consequences and to evaluate the cost of the management of obstructions. acute intestinal infections in the general surgery department of the reference health center of commune I of Bamako in Mali. Our retrospective, longitudinal and descriptive study took place from January 1, 2015 to December 31, 2019 in the general surgery department of the reference health center in commune I of Bamako. The average age was 47.72 years with extremes of 15 and 78 years and a standard deviation of 16.07. Our sex ratio (56 men/15 women) was 3.38. The clinical signs were dominated by abdominal pain (100%), vomiting (52.9%), cessation of materials and gases (25.4%) and meteorism (35.3%). The main etiologies found intraoperatively were strangulated hernia (54.9%), bands and/or adhesions (21.1%), sigmoid volvulus (12.7%), colorectal tumor (7%), small bowel volvulus (2.8%) and acute intestinal intussusception (1.4%). Hernia repair was the most performed surgical procedure, i.e. 54.9%. The overall mortality rate was 1.4%.

The issue of wound care has always played an important role in the practice of medicine. This is evidenced by Ambroise Paré’s decision to publish his first work on this subject, “La manière de traiter les Plaies” (The Way of Treating Wounds) in 1545. Objective: To evaluate the therapeutic methods we practice and to describe the impact of the pathology on socio-economic and professional development. Patients and Method: This was a retrospective study carried out at the Fousseyni Daou Hospital in kayes from January 1, 2018, to December 31, 2020. We included all patients with an ulcero-necrotic wound on immunocompetent terrain hospitalized in the department. Patients with ulcerative necrotic wounds of diabetes, HIV, or cancerous origin were not included. The parameters studied were: etiologies, local care, sequelae, and socio-economic and professional aspects. Results: We collected 57 patients of whom 43 were men and 14 were women, i.e. a sex ratio of 3. The mean age was 40.7 years with a standard deviation of 8.4 with extremes (7 years and 80 years). The average consultation time was 25.1 days. The most represented socio-professional stratum was agropastoralism in 37 cases (65%). The predominant etiological factor was neglected traumatic wounds in 34 cases (59.6%). The site was the lower limb in 39 cases (68.5%). The germ found was Staphylococcus aureus in 21 cases (36.8%). The particularity during local care was the use of table sugar in 9 cases (15.7%) and maggot therapy in 2 cases (3.5%). Hyperthermia was the clinical sign of aggravation in 22 cases (38.6%) and we diagnosed 2 cases (3.5%) of tetanus. The mortality rate was 15.7% (9 cases) and 30 cases (52.7%) of sequelae after recovery. The average length of hospitalization was 38 days. Hospital care was provided by the social welfare service in 35 cases (61.4%). Nineteen (19) patients (39.5%) were unable to resume their socio-professional activity. Despite the efforts made to fight the tuberculosis, Mycobacterium tuberculosis still remains a public health problem, particularly for low-income countries. According to the World Health Organization data, our country, Mali has detected only half of the 10,385 cases of tuberculosis expected for 2014 for a population of 17,309,000 inhabitants. The objective of this present work was to describe the different clinical aspects and the epidemiology of tuberculosis at Hospital Sominé Dolo in Mopti. We performed a retro-prospective and descriptive of tuberculosis cases diagnosed in our department of medicine between May 2016 and August 2018. A total of 96 tuberculosis cases were recorded, i.e. 4.6% and 1.0% for hospitalizations and consultations patterns, respectively. The median of age was 41 with extremes from 5 to 80 years. The age group [31 - 40 years] was the most affected with 20.8%. Men and women were affected in identical proportions, i.e. 50%. Pulmonary locations were the most frequent with 55.2%.
