Optimal postoperative surveillance strategies for stage III colorectal cancer

BACKGROUND Optimal surveillance strategies for stage III colorectal cancer(CRC)are lacking,and intensive surveillance has not conferred a significant survival *** To examine the association between surveillance intensity and recurrence and survival rates in patients with stage III *** Data from patients with pathologic stage III CRC who underwent radical surgery between january 2005 and December 2012 at Asan Medical Center,Seoul,Korea were retrospectively *** consisted of abdominopelvic computed tomography(CT)every 6 mo and chest CT annually during the 5 year follow-up period,resulting in an average of three imaging studies per *** who underwent more than the average number of imaging studies annually were categorized as high intensity(HI),and those with less than the average were categorized as low intensity(LI).RESULTS Among 1888 patients,864(45.8%)were in HI ***,sex,and location were not different between *** group had more advanced T and N stage(P=0.002,0.010,each).Perineural invasion(PNI)was more identified in the HI group(21.4%vs 30.3%,P<0.001).The mean overall survival(OS)and recurrence-free interval(RFI)was longer in the LI group(P<0.001,each).Multivariate analysis indicated that surveillance intensity[odds ratio(OR)=1.999;95%confidence interval(CI):1.680–2.377;P<0.001],pathologic T stage(OR=1.596;95%CI:1.197–2.127;P=0.001),PNI(OR=1.431;95%CI:1.192–1.719;P<0.001),and circumferential resection margin(OR=1.565;95%CI:1.083–2.262;P=0.017)in rectal cancer were significantly associated with *** mean post-recurrence survival(PRS)was longer in patients who received curative resection(P<0.001).Curative resection rate of recurrence was not different between HI(29.3%)and LI(23.8%)groups(P=0.160).PRS did not differ according to surveillance intensity(P=0.802).CONCLUSION Frequent surveillance with CT scan do not improve OS in stage III CRC *** need to evaluate role of other surveillance method rather tha

Congenital absence of the splenic artery is a very rare *** the best of our knowledge, congenital absence of the splenic artery accompanied with absence of the splenic vein has not been *** report a case of the absence of the splenic artery and vein in a 61-year-old woman who presented with postprandial epigastric discomfort. Upper gastrointestinal endoscopy showed a dilated, pulsatile vessel in the fundus and duodenal stenosis. An abdominal computed tomography(CT)scan revealed absence of the splenic vein with a tortuously engorged gastroepiploic ***-dimensional CT demonstrated the tortuously dilated left gastric artery and the left gastroepiploic artery with non-visualization of the splenic *** administration of a proton pump inhibitor,abdominal symptoms resolved without any recurrence of symptoms during 6 mo of follow-up.

AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method,and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,***: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall,cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis,low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Electric field enhancement of coupled plasmonic nanostructures for optical amplification

Plasmonic effects that enhance electric fields and amplify optical signals are crucial for improving the resolution of optical imaging systems. In this paper, a metal-based plasmonic nanostructure (MPN) is designed to increase the resolution of an optical imaging system by amplifying a specific signal while producing a plasmonic effect via a dipole nanoantenna (DN) and grating nanostructure (GN), which couple the electric field to be focused at the center of the unit cell. We confirmed that the MPN enhances electric fields 15 times more than the DN and GN, enabling the acquisition of finely resolved optical signals. The experiments confirmed that compared with the initial laser intensity, the MPN, which was fabricated by nanoimprint lithography, enhanced the optical signal of the laser by 2.24 times. Moreover, when the MPN was applied in two optical imaging systems, an indistinguishable signal that was similar to noise in original was distinguished by amplifying the optical signal as 106 times in functional near-infrared spectroscopy(fNIRS), and a specific wavelength was enhanced in fluorescence image. Thus, the incorporation of this nanostructure increased the utility of the collected data and could enhance optical signals in optics, bioimaging, and biology applications.

Bawu Decoction(八物汤) Ameliorates Benign Prostatic Hyperplasia in Rats
