Neuromodulation by ultrasound has recently received attention due to its noninvasive stimulation capability for treating brain *** there have been several studies related to ultrasonic neuromodulation,these studies have suffered from poor spatial resolution of the ultrasound and low repeatability with a fixed condition caused by conventional and commercialized ultrasound *** addition,the underlying physics and mechanisms of ultrasonic neuromodulation are still *** determine these mechanisms and accurately modulate neural circuits,researchers must have a precisely controllable ultrasound transducer to conduct experiments at the cellular ***,we introduce a new MEMS ultrasound stimulation system for modulating neurons or brain slices with high spatial *** piezoelectric micromachined ultrasonic transducers(pMUTs)with small membranes(submm membranes)generate enough power to stimulate neurons and enable precise modulation of neural *** designed the ultrasound transducer as an array structure to enable localized modulation in the target *** addition,we integrated a cell culture chamber with the system to make it compatible with conventional cell-based experiments,such as in vitro cell cultures and brain *** this work,we successfully demonstrated the functionality of the system by showing that the number of responding cells is proportional to the acoustic intensity of the applied *** also demonstrated localized stimulation capability with high spatial resolution by conducting experiments in which cocultured cells responded only around a working transducer.

Laparoscopic cholecystectomy has become a standard treatment of symptomatic gallstone disease. Although spilled gallstones are considered harmless, unretrieved gallstones can result in intra-abdominal abscess. We report a case of abscess formation due to spilled gallstones after laparoscopic cholecystectomy mimicking a retroperitoneal sarcoma on radiologic imaging. A 59-year-old male with a surgical history of a laparoscopic cholecystectomy complicated by gallstones spillage presented with a 1 mo history of constant right-sided abdominal pain and tenderness. Computed tomography and magnetic resonance imaging demonstrated a retroperitoneal sarcoma at the sub-hepatic space. On open exploration a 5 cm × 5 cm retroperitoneal mass was excised. The mass contained purulent material and gallstones. Final pathology revealed abscess formation and foreign body granuloma. Vigilance concerning the possibility of lost gallstones during laparoscopic cholecystectomy is important. If possible, every spilled gallstone during surgery should be retrieved to prevent this rare complication.

Effect of robot-assisted gait training on the biomechanical properties of burn scars:a single-blind,randomized controlled trial

Background:Robot-assisted gait training(RAGT)is more effective in the range of motion(ROM)and isometric strength in patients with burns than conventional ***,concerns have been raised about whether RAGT might negatively affect the scars of patients with ***,we investigated the effects of RAGT-induced mechanical load on the biomechanical properties of burn ***:This was a single-blind,randomized clinical trial conducted on inpatients admitted to the Department of Rehabilitation Medicine between September 2020 and August *** was conducted for 30 min per day,five days a week for 12 weeks and the control group received conventional gait training for 12 *** pre-training ROM of lower extremity joints was evaluated and the levels of melanin,erythema,trans-epidermal water loss,scar distensibility and elasticity were assessed before training and at 4 and 12 weeks after ***,19 patients in the gait assistance robot(GAR)group and 20 patients in the control group completed the 12-week trial and all ***:There were no significant differences in the epidemiologic characteristics,pre-training ROM of joints and pre-training biomechanical properties of the burn scar between the groups(p>0.05 for all).None of the patients experienced skin abrasion around the burn scar where the fastening belts were applied or musculoskeletal or cardiovascular adverse events during the *** thickness significantly increased in both groups(p=0.037 and p=0.019)and scar distensibility significantly decreased in the control group(p=0.011)during the *** was significantly decreased in the GAR group during the training(p=0.038).The GAR and control groups showed significant difference in the change in the values of hysteresis between pre-training and 12 weeks after training(p=0.441 and p=0.049).Conclusions:RAGT significantly decreased hysteresis in hypertrophic burn scars and did not cause a significant decrease

Primary intestinal lymphangiectasia(PIL) is a rare protein-losing enteropathy with lymphatic leakage into the small intestine. Dilated lymphatics in the small intestinal wall and mesentery are observed in this disease. Laboratory tests of PIL patients revealed hypoalbuminemia, lymphocytopenia, hypogammaglobulinemia and increased stool α-1 antitrypsin clearance. Cell-mediated immunodeficiency is also present in PIL patients because of loss of lymphocytes. As a result, the patients are vulnerable to chronic viral infection and lymphoma. However, cases of PIL with chronic viral infection, such as human papilloma virus-induced warts, are rarely reported. We report a rare case of PIL with generalized warts in a 36-year-old male patient. PIL was diagnosed by capsule endoscopy and colonoscopic biopsy with histological tissue confirmation. Generalized warts were observed on the head, chest, abdomen, back, anus, and upper and lower extremities, including the hands and feet of the patient.

AIM： To investigate the relationship between postendoscopic resection （ER） scars on magnifying endoscopy （ME） and the pathological diagnosis in order to validate the clinical significance of ME. METHODS： From January, 2007 to June, 2008, 124 patients with 129 post-ER scar lesions were enrolled. Mucosal pit patterns on ME were compared with conventional endoscopy （CE） findings and histological results obtained from targeted biopsies. RESULTS： CE findings showed nodular scars （53/129）, erythematous scars （85/129）, and ulcerative scars （4/129）. The post-ER scars were classified into four pit patterns of sulci and ridges on ME：（ Ⅰ ） 47 round; （Ⅱ） 54 short rod or tubular; （Ⅲ） 19 branched or gyrus-like;and （Ⅳ） 9 destroyed pits. Sensitivity and specificity were 88.9% and 62.5%, respectively, by the presence of nodularity on CE. Erythematous lesions were high sensitivity （100%）, but specificity was as low as 36.7%. The range of the positive predictive va ue （PPV） on CE was as low as 10.6%-25%. Nine type N pit patterns were diagnosed as tumor lesions, and 120 cases of type Ⅰ-Ⅲ pit patterns revealed non-neoplastic lesions. Thus, the sensitivity, specificity, and the PPV of ME were 100%. CONCLUSION： ME findings can detect the presence of tumor in post-ER scar lesions, and make evident the biopsy target site in short-term follow-up. Further large-scale and long-term studies are needed to determine whether ME can replace endoscopic biopsy.

Chemical ablation of the gallbladder is effective in patients at high risk of complications after surgery. Percutaneous gallbladder drainage is an effective treatment for cholecystitis; however, when the drain tube cannot be removed because of recurrent symptoms, retaining it can cause problems. An 82-year-old woman presented with cholecystitis and cholangitis caused by biliary stent occlusion and suspected tumor invasion of the cystic duct. We present successful chemical ablation of the gallbladder using pure alcohol, through a percutaneous gallbladder drainage tube, in a patient who developed intractable cholecystitis with obstruction of the cystic duct after receiving a biliary stent. Our results suggest that chemical ablation therapy is an effective alternative to surgical therapy for intractable cholecystitis.

Duodenal perforations caused by biliary prostheses are not uncommon, and they are potentially life threatening and require immediate treatment. We describe an unusual case of aortic aneurysm and rupture which occurred after retroperitoneal aortoduodenal fistula formation as a rare complication caused by biliary metallic stent-related duodenal perforation. To our knowledge, this is the first report describing a lethal complication of a bleeding, aortoduodenal fistula and caused by biliary metallic stent-induced perforation.

AIM: To determine the genomic changes in hepatitis B virus(HBV) and evaluate their role in the development of hepatocellular carcinoma(HCC) in patients chronically infected with genotype C ***: Two hundred and forty chronic hepatitis B(CHB) patients were subjected and followed for a median of 105 mo. HCC was diagnosed in accordance with AASLD guidelines. The whole X, S, basal core promoter(BCP), and precore regions of HBV were sequenced using the direct sequencing ***: All of the subjects were infected with genotype C HBV. Out of 240 CHB patients, 25(10%) had C1653 T and 33(14%) had T1753 V mutation in X region; 157(65%) had A1762T/G1764 A mutations in BCP region, 50(21%) had G1896 A mutation in precore region and 67(28%) had pre-S deletions. HCC occurred in 6 patients(3%). The prevalence of T1753 V mutation was significantly higher in patients who developed HCC than in those without HCC. The cumulative occurrence rates of HCC were 5% and 19% at 10 and 15 years, respectively, in patients with T1753 V mutant, which were significantly higher than 1% and 1% in those with wild type HBV(P < 0.001).CONCLUSION: The presence of T1753 V mutation in HBV X-gene significantly increases the risk of HCC development in patients chronically infected with genotype C HBV.

Flexible copper clad laminates(FCCLs) were fabricated using the electro-plating process and the combined effect of the current density and plating time on their surface morphology,texture,hardness,electrical resistivity and folding behavior was *** achieve Cu layers with similar thicknesses,the current density was varied in the range of 0.2-3 A/dm2 and the plating time was controlled in the range of 0.5-7.5 h to compensate for the variation of the current *** surface morphology,hardness,and folding behavior were characterized by atomic force microscopy,nanoindentation technique and Massachusett Institute of Technology folding endurance test,*** X-ray diffraction patterns indicated that the Cu phase was formed without any secondary phases;however,the preferred orientation changed from(220) to(111) as the current density increased over 1 A/*** addition,it was observed that the root-mean-square and hardness values decreased when the current density increased and the plating time decreased *** electrical resistivity was as low as approximately 21 nΩ·m and the number of cycles without failure in the folding test was over 15 000,which were comparable to those of commercial FCCLs.
