Clinical meaning of BRAF mutation in Korean patients with advanced colorectal cancer

AIM:To evaluate the clinicopathological features ofcolorectal cancer(CRC)with a v-Raf murine sarcomaviral oncogene homolog B1(BRAF)mutation and itsmolecular interaction with microsatellite instability(MSI)and v-Ki-ras2 Kirsten rat sarcoma viral oncogene homolog(KRAS)in patients with advanced CRCs.METHODS:From October 2009 to December 2011,141 patients with stageⅢ(n=51)orⅣ(n=90)CRCs who were tested for the BRAF mutation at Severance Hospital were included.Among 141 patients,fivewere excluded due to follow-up loss.Therefore,136patients were included in the study.The clinicopathological data,MSI status,and KRAS/BRAF mutation status were reviewed retrospectively.In addition,to evaluating the value of BRAF mutation status,progressionfree survival and overall survival in all patients werecollected and compared between the BRAF wild-typegroup and BRAF mutation group.RESULTS:Of 136 patients,80(58.8%)were male and the mean age was 59 years.BRAF and KRAS mutations were detected in 9.6%and 35.3%of patients,respectively.Only 4.3%of patients had MSIhigh tumors and there were no MSI-high in tumors with a BRAF mutation.BRAF mutations tended to be more frequent in stageⅣthan in stageⅢ(11.76%vs 5.88%,P=0.370).Patients with a BRAF mutation had a lower incidence of KRAS mutation than those without(7.69%vs 38.21%,P=0.033).Overall survival was significantly shorter in the BRAF mutation group than in the BRAF wild-type group both by univariate analysis(P=0.041)and multivariate analysis(HR=2.195;95%CI:1.039-4.640;P=0.039),while progression-free survival was not different according to BRAF mutation status.CONCLUSION:CRCs with a BRAF mutation have distinct molecular features and resulted in a poor prognosis in Korean patients with advanced CRC.

The below-cloud aerosol scavenging process by precipitation is one of the most important mechanisms to remove aerosols from the atmosphere.Due to its complexity and dependence on both aerosol and raindrop sizes,wet scavenging process has been poorly treated,especially during the removal of fine particles.This makes the numerical simulation of below-cloud scavenging in large-scale aerosolmodels unrealistic.To consider the slip effects of submicron particles,a simplified expression for the diffusion scavenging was developed by approximating the Cunningham slip correction factor.The derived analytic solution was parameterized as a simple power function of rain intensity under the assumption of the lognormal size distribution of particles.The resultant approximated expression was compared to the observed data and the results of previous studies including a 3D atmospheric chemical transport model simulation.Compared with the default GEOS-Chem coefficient of 0.00106R0.61 and the observation-based coefficient of 0.0144R0.9268,the coefficient of a and b in∧m=aRb spread in the range of 0.0002-0.1959 for a and 0.3261-0.525 for b over a size distribution of GSD of 1.3–2.5 and a geometric mean diameter of 0.01-2.5μm.Overall,this study showed that the scavenging coefficient varies widely by orders of magnitude according to the size distribution of particles and rain intensity.This study also demonstrated that the obtained simplified expression could consider the theoretical approach of aerosol polydispersity.Our proposed analytic approach showed that results can be effectively applied for reduced computational burden in atmospheric modeling.

Studies on three-dimensional structured carbon templates have focused on how to guide homogeneous lithium metal nucleation and growth for lithium metal anodes(LMAs).However,there is still insufficient evidence for a key factor to achieve their high electrochemical performance.Here,the effects of nanopores and sulfur doping on carbon-based nanoporous host(CNH)electrode materials for LMAs were investigated using natural polymer-derived CNHs.Homogeneous pore-filling behaviors of lithium metal in the nanopores of the CNH electrode materials were first observed by ex situ scanning electron microscopy analysis,where the protective lithium metal nucleation and growth process led to significantly high Coulombic efficiency(CE)of~99.4%and stable 600 cycles.In addition,a comparison study of CNH and sulfurdoped CNH(S-CNH)electrodes,which differ only in the presence or absence of sulfur,revealed that sulfur doping can cause lower electrochemical series resistance,higher CE value,and better cycling stability in a wide range of current densities and number of cycles.Moreover,S-CNH-based LMAs showed high electrochemical performance in full-cell Li-S battery tests using a sulfur copolymer cathode,where a high energy density of 1370Wh kgelectrode−1 and an excellent power density of 4120Wkgelectrode−1 were obtained.

BACKGROUND Noninvasive ventilation(NIV)reduces intubation rates,mortalities,and lengths of hospital and intensive care unit stays in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Helmet-based NIV is better tolerated than oronasal mask-based ventilation,and thus,allows NIV to be conducted for prolonged periods at higher pressures with minimal air leaks.CASE SUMMARY A 73-year-old man with a previous diagnosis of COPD stage 4 was admitted to our medical intensive care unit with chief complaints of cough,sputum,and dyspnea of several days’duration.For 10 mo,he had been on oxygen at home by day and had used an oronasal mask-based NIV at night.At intensive care unit admission,he breathed using respiratory accessory muscles.Hypercapnia and signs of infection were detected,and infiltration was observed in the right lower lung field by chest radiography.Thus,we diagnosed AECOPD by communityacquired pneumonia.After admission,respiratory distress steadily deteriorated and invasive mechanical ventilation became necessary.However,the patient refused this option,and thus,we selected helmet-based NIV as a salvage treatment.After 3 d of helmet-based NIV,his consciousness level and hypercapnia recovered to his pre-hospitalization level.CONCLUSION Helmet-based NIV could be considered as a salvage treatment when AECOPD patients refuse invasive mechanical ventilation and oronasal mask-based NIV is ineffective.

To the Editor： Spontaneous ureteral rupture is rare, and urolithiasis is the most common cause. The underlying mechanism may be the pressure around the ureteral wall due to stone impaction or a tear during the stone passage. Smaller distal stones exert higher intraureteral pressures than do larger proximal stones because of the greater length of obstructed ureter and the smaller diameter of the distal ureter.

A strategy for fabricating microcrystalline cellulose–Ti_(3)C_(2)T_(x)(MCC–MXene)nanocomposite films with high relative permittivity,high thermal conductivity,and excellent mechanical properties was developed.The MCC–MXene nanocomposite film was fabricated by casting a solution containing N,N-dimethylacetamide/lithium chloride(DMAc/LiCl)-soluble MCC and DMAcdispersible MXene nanosheets,followed by humidity control drying.The MXene nanosheets greatly enhanced the permittivity of the nanocomposite films owing to interfacial polarization.Thus,the nanocomposite film with 20 wt.%MXene content achieved a desirable permittivity of 71.4 at 102 Hz(a 770%improvement against that of neat cellulose),while the dielectric loss only increased by 1.8 times(from 0.39 to 0.70).The obtained nanocomposite films with 20 wt.%and 30 wt.%MXene exhibited remarkable in-plane thermal conductivities of 8.523 and 9.668 W∙m^(−1)∙K^(−1),respectively,owing to the uniform dispersion and selfalignment of the MXene layered structure.Additionally,the uniformly dispersed MXene nanosheets in the MCC network with interfacial interaction(hydrogen bonding)and mechanical entanglement endowed the nanocomposite films with excellent mechanical properties and flexibility.Furthermore,the thermal stability,water resistance,and antibacterial properties of the nanocomposite films were effectively improved with the introduction of MXene.Moreover,using DMAc/LiCl as the solvent system not only improves the compatibility between MCC and MXene but also avoids the problem of easy oxidation of MXene in aqueous systems.With the high stability of the MCC–MXene solution and enhanced properties of the MCC–MXene films,the proposed strategy manifests great potential for fabricating natural biomass-based dielectric materials.

We aimed to evaluate the current nationwide trend,efficacy,safety,and quality of life(QoL)profiles of hormone treatment in real-world practice settings for prostate cancer(PCa)patients in Korea.A total of 292 men with any biopsy-proven PCa(TanyNanyMany)from 12 institutions in Korea were included in this multi-institutional,observational study of prospectively collected data.All luteinizing hormone-releasing hormone(LHRH)agonists were allowed to be investigational drugs.Efficacy was defined as(1)the rate of castration(serum testosterone≤50 ng dl^-1)at 4-week visit and(2)breakthrough(serum testosterone>50 ng dl^-1 after castration).Safety assessments included routine examinations for potential adverse events,laboratory tests,blood pressure,body weight,and bone min era I density(BMD,at baseli ne and at the last follow-up visit).QoL was assessed using the Expa nded Prostate Cancer Index Composite-26(EPIC-26).The most comm on initial therapeutic regimen was LHRH agonist with an ti-androge n(78.0%),and the most commonly used LHRH agonist for combination and mono therapy was leuprolide(64.0%for combi natio n and 58.0%for monotherapy).The castration and breakthrough rates were 78.4%and 6.6%,respectively.The laboratory results related to dyslipidemia worsened after 4 weeks of hormone treatment.In addition,the mean BMD T-score was significantly lower at the last follow-up(mean:-1.950)compared to baseline(mean:-0.195).The mean total EPIC-26 score decreased from 84.8(standard deviation[s.d.]:12.2)to 78.3(s.d.:8.1),with significant deterioration only in the urinary domain(mean:23.5 at baseli ne and 21.9 at the 4-week visit).These findi ngs dem on strate the n ationwide trend of current practice setti ngs in hormone treatment for PCa in Korea.

Sorafenib,a potent multikinase inhibitor,lead to a significant improvement in progression free survival and overall survival in patients with advanced hepatocellular carcinoma(HCC).Though sorafenib has proven its efficacy in advanced stage HCC,there are limitedreports on the role of sorafenib allowing for curative treatment by down-staging.We herein report a case of advanced HCC with vascular invasion,which showed treatment response by sorafenib therapy as to allow for radiofrequency ablation as curative treatment.The patient was followed-up for 6 mo without recurrence with continued sorafenib therapy.
