Asymptomatic Screening of Clients on Opioid Substitution Therapy for Tuberculosis: An Experience from India

Clients on Opioid Substitution Therapy (OST) may be at risk of TB. We screened for tuberculosis in asymptomatic OST clients using Chest-X-ray (CXR) and sputum from those with CXR suggestive of TB were tested using Xpert MTB/Rif. Among 472 who underwent CXR, 0.1% had CXR suggestive of TB. The number needed to screen was 118 clients for one CXR suggestive of TB. All four clients with CXR suggestive of TB underwent MTB/Rif assay and one was diagnosed with TB. The TB patient was asymptomatic and could have been missed through the passive approach. We recommend further studies to explore ACF among OST clients.

The abscisic acid （ABA） signaling pathway is regulated by clade A type 2C protein phosphatases （PP2CAs） in plants. In the presence of ABA, PP2Cs release stress/ABA-activated protein kinases by binding to ABA-bound receptors （PYL/RCARs） for activation. Although the wedging tryptophan in PP2Cs is critical in the interaction with PYL/RCARs in Arabidopsis and rice, it remains elusive as to how other interface regions are involved in the interaction. Here, we report the identification of a conserved region on PP2Cs, termed the VxG4ФL motif, which modulates the interaction with PYL/RCARs through its second and fourth residues. The effects of the second and fourth residues on the interaction of OsPP2CS0 with several OsPYIJRCAR proteins were investigated by systematic mutagenesis. One OsPP2C50 mutant, VFGML （＂FM＂） mutant, lowered the affinity to OsPYL/RCAR3 by Ф15-fold in comparison with the wUd-type. Compar- ison of the crystal structures of wild-type OsPP2C50：ABA：OsPYI./RCAR3 with those composed of FM mutant revealed local conformational changes near the VxGФL motif, further supported by hydrogen-deute- rium exchange mass spectrometry. In rice protoplasts, ABA signaling was altered by mutations in the VxGФL motif. Transgenic Arabidopsis plants overexpressing OsPP2C50 and OsPP2C5OFM showed altered ABA sensitivity. Taken together, the VxGФL motif of PP2Cs appears to modulate the affinity of PP2Cs with PYL/RCARs and thus likely to alter the ABA signaling, leading to the differential sensitivity to ABA in planta.

Endometritis and toxic shock syndrome associated with Clostridium sordellii have previously been reported after childbirth and, in one case, after medical abortion. We describe four deaths due to endometritis and toxic shock syndrome associated with C. sordellii that occurred within one week after medically induced abortions. Clinical findings included tachycardia, hypotension, edema, hemoconcentration, profound leukocytosis, and absence of fever. These cases indicate the need for physician awareness of this syndrome and for further study of its association with medical abortion.

Purpose: Ranitidine hydrochloride (HCl) remains an important medication for treating acid-peptic ailments such as Gastroesophageal reflux disease (GERD). The main objective of this Post Marketing Surveillance (PMS) clinical study was to test the efficacy and safety of Ranitidine HCl in Indian patients suffering from GERD. Patients and Methods: Data of 2446 patients (1307 males;1121 females) from 21 centers across India were analyzed. Patients received either of the three treatments: Ranitidine HCl 150 mg twice a day (BID) (ARM-A), Ranitidine HCl 300 mg once daily (OD) or BID (ARM-B), and Ranitidine HCl 300 mg OD (ARM-C). Gastroesophageal Reflux Disease Symptom Assessment Scale (GSAS) score and Heartburn Severity score were used to assess the drug’s efficacy. The adverse events reported by patients or investigators were analyzed to assess the safety profile of Ranitidine. Results: Of the 2446 subjects screened, 2428 were enrolled. There was a significant reduction in GSAS scores from baseline to the end of the study visit in all three ARMs. The GSAS scores reduced from 2.02 to 0.23 in ARM-A, 2.01 to 0.24 in ARM-B, and 2.07 to 0.26 in ARM-C patients. In ARM A, 72.82% had 24 hours heartburn-free days, and 66.89% had 7 consecutive heartburn-free days, which was more significant than the other two ARMs. 128 (5.27%) patients reported ADRs due to Ranitidine HCl at different doses. The most frequently reported ADR was constipation (17.18%), followed by oliguria (14.06%), cold (13.28%), and dysuria (12.5%). Of 128 ADRs, 113 (88.28%) were mild, and only 11 (8.59%) ADRs were related to the study drug. No severe ADRs were reported during the study. Conclusion: Ranitidine HCl 150/300 mg tablet was found to be an effective and safe H2-receptor antagonist for treating GERD in Indian Patients.

In this paper, we have deposited microstructure (straight wire like shape some time name as fiber) of tin oxide on silicon substrate by simplest known technique thermal evaporation. It is observed that these fibers have same property like tin oxide material (nanowire) and size were approximate in micron. Diamond peak found in X-ray diffraction patter in toward higher glancing angle. Observed tin oxide fibers were characterized by using scanning electron microscopy, X-ray diffraction and optical property have been measure by ultra violet visible diffuse reflection spectroscopy.

In this article, the effect of synthesis parameters on structural properties of SnO2 nanowire by thermal evaporation process, have been studied. The nanostructures are characterized by scanning electron microscopy, Transmission electron microscopy and X-ray spectroscopy. By X-ray diffraction pattern, synthesized SnO2 nanowire were found to be crystalline rutile SnO2. We studied the effect of synthesis parameters such as the deposition temperature, distance variation of the substrate from precursor on the structure of our synthesized nanowires. It was found that their growth was governed by vapor-liquid-solid mechanism (VLS) because of using Sn self catalyst. In this mechanism, the Sn droplet deposited serve as preferred sites for the growth of SnO2 nanowire.

H2S Gas Sensor Based on Cu Doped SnO2 Nanostructure

Sensor based on Cu doped SnO 2 thin films is found most suitable sensor for H 2 S gas detection. As the nanostructures have high surface to volume ratio, they show relatively high selectivity and sensitivity for different gases. In the present work, the nanostructures of 5 wt% and 10 wt% of Cu doped SnO 2 were prepared by thermal evaporation technique. The precursor powder of Cu doped SnO 2 was prepared by sol gel solution method. The structural and surface morphology of nanostructures grown on gold coated silicon substrates were characterized by X-ray diffraction, SEM and EDX technique. The gas response of the sensors at different temperature and at different concentration of H2S and NO2 gases is investigated. The results show high response at room temperature.
