Introduction: Despite progress in universal health coverage, maternal mortality remains a concern for developing countries, including Mali. The problem is not due only to a lack of health services but to a combination of factors that need to be identified to give hope to women of childbearing age. Objective: Determine the factors associated with maternal mortality in the Timbuktu region of Mali. Methodology: Retrospective, descriptive, cross-sectional and analytical study from January 1, 2018 to December 31, 2022 using a questionnaire and an interview guide to determine the factors associated with maternal mortality in the Timbuktu region of Mali. Results: A total of 112 maternal deaths were recorded between 2018 and 2022. The average age of the women who died was 26 years (standard deviation 7.6 years). The 14 – 23-year-old age group is the most affected (43.8%). 66.1% of women came from rural areas. Illiteracy (77.7%), lack of ANC (54.5%), distance of households from health facilities (76%) and insecurity (52.7%) have been identified as factors contributing to maternal mortality. Conclusion: This study reported a high prevalence of maternal mortality rates that may be associated with women’s socioeconomic conditions. Actions must be developed to integrate CPN into an advanced strategy for women living in rural areas in Mali.

Vibration-Based Analysis for Detecting Turbocharger Blade Defect on High-Power Diesel Generator

This study is concerned with 12-MW capacity turbochargers on diesel power generators. These are generators equipped with 18 cylinders. Our previous studies [1] showed that the processing of vibration signature collected from a power generator is very complex, insomuch the dominant vibration remains the one originating from explosion frequency in the diesel generator cylinders, with a fairly high number of cylinders. This vibration drowns out all other possible vibrations, which can expose defects. The study at hand is focused on turbochargers with 20,940 rpm, while the rotation speed in the diesel engine is 600 rpm only. With the turbocharger rotating at very high speed, it results in severe mechanical constraints on the rotor shaft in the turbocharger and its related organs (turbine blades). The wear of turbine blades can result in breakdowns in the turbocharger. This article is an attempt to early detect defaults in turbine blades based on vibration signature that can be experimentally determined. We noted in our investigations that a diesel engine and its turbochargers do not bear the same mechanic loads. While the diesel engine is the seat of violent shocks brought about by explosions in cylinders, the turbochargers are driven by the action of exhaust gas from explosions, without being affected by explosion shocks. The study found that explosion frequency in the diesel engine cylinders, which disrupted the vibration signals in the diesel engine and alternator, did not impact on the vibration signals in the turbocharger. We experimentally determined, following several campaigns of measurements, a vibration signature on the turbochargers under study, which corresponds to a defect in turbine blades.

Prevalence and Factors Associated with the Anxio-Depressive Symptoms of the Families of Patients in Intensive Care Unit at Gabriel Toure University Hospital, Bamako, Mali

Introduction: Hospitalization in intensive care is a source of stress and anxiety for close to the patients. Anxio-depressive symptoms appear to be common in intensive care and their prevalence is poorly evaluated. The objective of this study was to evaluate the prevalence of anxio-depressive symptoms in families. Materials and method: Observational prospective study in families of patients hospitalized in the intensive care unit for a period of 5 months. Symptoms of anxiety and depression were measured using Hospital Anxiety and Depression Scale (HADS) during hospitalization. Anxiety and depression were defined by a score greater than 10. Factors associated with the onset of anxious-depressive symptoms were sought. The prevalence of post-traumatic stress disorder in close relatives was measured by the Impact Event Scale-Revised scale (IES-R). Results: A total of 107 patients were admitted to the intensive care unit, of which 49 families agreed to participate in our study. Overall mortality was 32.2% during this period. Fifty-eight (58) patients were not included for the following reasons: death or hospitalization of less than 48 hours, refusal of families, institution, and lack of parents speaking French. Forty-nine (49) relatives completed the HADS questionnaire. Forty-three families completed the IES-R questionnaire, a return rate of 87.7%. The prevalence of anxiety was 61.2% among parents in early hospitalization. The level of anxiety was significantly associated with male parents (p = 0.035) and those with lower education (p = 0.046). The prevalence of depressive symptoms in parents was evaluated 53% at D3. Education level (p = 0.048) and male parents (p = 0.048) appeared to be a significant depression factor. The prevalence of post-traumatic stress disorder was 55.1% among the relatives of the patients. Primary Intramedullary Ewing’s Sarcoma: A Case Report and Review of the Literature

Ewing’s sarcoma, a group of primary neuroectodermal tumor (PNET), is an uncommon rare highly malignant tumor mostly affecting the bones of male patients, and accounts for approximately 10% of the primary malignant bone tumors, and occupies the second place after osteosarcoma in children’s less than 20 years. Primary intramedullary Ewing’s sarcoma is an extremely rare condition. Here, we present a case of a 16-year-old girl with progressive weakness of lower extremities, inability to walk, progressive low back pain, bilateral leg pain, paresthesia and urinary retention. Spinal MRI showed intramedullary abnormal signal focus from T11 to L3 vertebrae with moderate heterogeneous enhancement on T1 weighted sequences. After surgery, histological examination found a lot of small round-cell tumors with a high nuclear-cytoplasmic ratio, frequent mitoses and apoptotic nuclei suggesting a diagnosis of Ewing’s sarcoma and confirmed by CD99 reactivity. She had undergone adjuvant chemotherapy and irradiation and was free of symptoms since 02 years. Within this single pure case of intramedullary Ewing’s sarcoma, perhaps the 1st in the English literature, the authors described this extremely rare uncommon localization with a brief review of the literature.

In Mali, chest injuries remain a real public health problem and are associated with heavy morbidity and mortality. Faced with a resurgence of urban civil violence and the explosion of road accidents, we decided to conduct this study in order to describe the epidemiological, clinical and therapeutic aspects of thoracic trauma in the emergency department. Method and Material: This is a descriptive prospective study over a period of one year in the emergency department of the CHU Gabriel Touré. Including all patients admitted for thoracic trauma. Analysis and Entry: Data were entered and analyzed on SPSS software version 20.0. The test was significant for a p value Results: We recorded 21,090 appeals in our structure among which 1284 patients were suspected of thoracic trauma. The diagnosis of thoracic trauma was retained in 119 (0.56%) patients. All patients were transported to the emergency room without prehospital medicalization. Clinical presentation was dominated by dyspnea in 54.6% of patients, however pain was the almost constant symptom in conscious victims. Various traumatic mechanisms had caused these lesions of the thorax, of which road traffic accidents represented half of the causes, followed by urban civil violence in 28.6% of patients. Landslides and falls from a great height were responsible for 19.5% of chest injuries. Open chest trauma was the type of lesion found in a third of the cases. This type of injury was exclusively due to blows and injuries during the brawls. Standard chest radiography was performed as the first intention in 60% of patients compared to 10.9% for the pleuropulmonary ultrasound. Thirteen patients required ventilatory assistance after orotracheal intubation. The average length of stay was 65.23 hours. Pregnancy, which is responsible for the decline in immunity, and the immediate postpartum period can lead to reactivation or worsening of tuberculosis. We report a case of a patient who consulted for neurological disorders in the context of a deterioration in general condition. The CT scan revealed a brainstem lesion which was successfully treated like a tuberculosis. However, reactivation has been observed in the postpartum period of a pregnancy contracted during anti-tuberculosis treatment. Further clinical improvement has been achieved with anti-tuberculosis treatment. Pregnancy and the immediate postpartum had led to a transient decline in immunity in part by decreasing in the inflammatory activity of type 1 helper T cells so that the fetus, which is a foreign body, was accepted by the maternal body. This decline in immunity during pregnancy and the immediate postpartum period due to immune reconstitution had been responsible for a high degree of vulnerability, usually characterized by a significant exacerbation of tuberculosis symptoms and an unfavorable course of disease.

Bilateral Chronic Subdural Hematoma after Endoscopic Third Ventriculostomy in a Child: A Case Report and Review of the Literature

Hydrocephalus had been managed by ventriculoperitoneal shunt (VPS) or endoscopic third ventriculostomy (ETV) since several years. But these two different technics had some complications that must be managed promptly to avoid eventual fatal evolution. Chronic subdural hematomas after ETV is among these complications and is a very rarely situation observed in our department. This rare event associated with malaria in a child is considered to have a high mortality. Here we report a rare case of bilateral chronic subdural hematoma occurring in a 4-month-old boy after ETV and we discuss the likely pathogenesis and the difficulties of management.

Infracallosal Aneurysm Rupture Revealed by Chronic Subdural Hematoma: A Case Report

Chronic subdural hematoma (CSDH) is one of the most common entities seen in neurosurgery. We report a case of infracallosal aneurysm revealed by CSDH in a 52 years old woman. She presented 3 days alteration of consciousness in 2019 and repeated in September 2020 a severe headache and dizziness. There was no history of others symptoms, nor head trauma. Routine blood examination and coagulation profile were normal. Emergency computed tomography (TC) showed a left temporoparietal-occipital CSDH. Cerebral Angio CT found a bilobated of A2 aneurysm. The precallosal interhemispheric approach was made and clipping the aneurysm after evacuation of CSDH without complication. CSDH is one of the revealed features of intracerebral aneurysm rupture.
