BACKGROUND Cardiac tumors are rare and complex entities.Surgery represents the cornerstone of therapy,while the role of adjuvant treatment remains unclear and,in case of relapse or metastatic disease,the prognosis is very poor.Lack of prospective,randomized clinical trials hinders the generation of high level evidence for the optimal diagnostic workup and multimodal treatment of cardiac sarcomas.Herein,we describe the multidisciplinary clinical management and molecular characterization of a rare case of cardiac myxofibrosarcoma in an elderly woman.CASE SUMMARY A 73-year-old woman presented signs and symptoms of acute left-sided heart failure.Imaging examination revealed a large,left atrial mass.With suspicion of a myxoma,she underwent surgery,and symptoms were promptly relieved.Histology showed a cardiac myxofibrosarcoma,a rare histotype of cardiac sarcoma.Eight months later,disease unfortunately relapsed,and after a multidisciplinary discussion,a chemotherapy with doxorubicin and then gemcitabine was started,achieving partial radiologic and complete metabolic response,which was maintained up to 2 years and is still present.This report is focused on the entire clinical path of our patient from diagnosis to follow-up,through surgery and strategies adopted at relapse.Moreover,due to their rarity,very little is known about the molecular landscape of myxofibrosarcomas.Thus,we also performed and described preliminary genome analysis of the tumor tissue to get further insight on mechanisms involved in tumor growth,and to possibly unveil new clinically actionable targets.CONCLUSION We report a case of cardiac myxofibrosarcoma that achieved a very good prognosis due to an integrated surgical,cardiac and oncologic treatment strategy.

Exclusive breastfeeding greater than 50%, success of education in a university hospital in Bogotá: Case-control study

BACKGROUND Maintenance rates of exclusive breastfeeding(EBF)worldwide are low,thus,one of the objectives of the summary of policies on breastfeeding(BF)in world nutrition goals for 2025 are that at least 50%of infants under six months of age receive EBF that year.The Objective of this study is to document the rates of EBF in children born in San Ignacio University Hospital(HUSI)and identify factors associated with maintenance.AIM To document the percentages of EBF in those that were born at HUSI and identify factors associated to their maintenance.METHODS This is a study of cases and controls in an analytic,retrospective cohort that took children born alive between January 2016 and January 2019 at HUSI located in the city of Bogotá,Colombia.RESULTS Receiving information about BF at HUSI was able to maintain EBF up until 4 mo(OR=1.65;95%CI:1.02-2.66).The presence of gynecologic and obstetric comorbidities(OR=0.32;95%CI:0.12-0.83),having mastitis(OR=0.56;95%CI:0.33-0.94),and receiving information from mass media(OR=0.52;95%CI:0.31-0.84)are factors associated with not maintaining EBF.CONCLUSION Receiving education at a Women-and Child-Friendly Institution was the only significant factor to achieve EBF until 4 mo,with a frequency greater than the one reported in the country,which matches multiple studies where counseling and individualized support on BF achieve this purpose.Knowledge about BF and early detection of obstetric/gynecologic complications must be strengthened among the healthcare staff in charge of mothers during post-partum.Additionally,strategies must be promoted to continue BF such as creating milk banks with the objective of increasing BF rates even when mothers return to work.

Evaluation of Serum Anti-Müllerian Hormone (AMH) Values for 28,016 Bulgarian Women: Prognostic Statistical Model of Age Specific AMH Declining

The present study aims to establish a relationship between serum AMH levels and age in a large group of women living in Bulgaria, as well as to establish reference age-specific AMH levels in women that would serve as an initial estimate of ovarian age. A total of 28,016 women on the territory of the Republic of Bulgaria were tested for serum AMH levels with a median age of 37.0 years (interquartile range 32.0 to 41.0). For women aged 20 - 29 years, the Bulgarian population has relatively high median levels of AMH, similar to women of Asian origin. For women aged 30 - 34 years, our results are comparable to those of women living in Western Europe. For women aged 35 - 39 years, our results are comparable to those of women living in the territory of India and Kenya. For women aged 40 - 44 years, our results were lower than those for women from the Western European and Chinese populations, close to the Indian and higher than Korean and Kenya populations, respectively. Our results for women of Bulgarian origin are also comparable to US Latina women at age 30, 35 and 40 ages. On the base on constructed a statistical model to predicting the decline in AMH levels at different ages, we found non-linear structure of AMH decline for the low AMH 3.5) the dependence of the decline of AMH on age was confirmed as linear. In conclusion, we evaluated the serum level of AMH in Bulgarian women and established age-specific AMH percentile reference values based on a large representative sample. We have developed a prognostic statistical model that can facilitate the application of AMH in clinical practice and the prediction of reproductive capacity and population health.

Anti-inflammatory properties of the short-chain fatty acids acetate and propionate:A study with relevance to inflammatory bowel disease

AIM: To compare the anti-inflammatory properties of butyrate with two other SCFAs,namely acetate and propionate,which have less well-documented effects on inflammation. METHODS: The effect of SCFAs on cytokine release from human neutrophils was studied with ELISA. SCFA-dependent modulation of NF-κB reporter activity was assessed in the human colon adenocarcinoma cell line,Colo320DM. Finally,the effect of SCFAs on gene expression and cytokine release,measured with RT-PCR and ELISA,respectively,was studied in mouse colon organ cultures established from colitic mice. RESULTS: Acetate,propionate and butyrate at 30 mmol/L decreased LPS-stimulated TNFα release from neutrophils,without affecting IL-8 protein release. All SCFAs dose dependently inhibited NF-κB reporter activity in Colo320DM cells. Propionate dose-dependently suppressed IL-6 mRNA and protein release from colon organ cultures and comparative studies revealed that propionate and butyrate at 30 mmol/L caused a strong inhibition of immune-related gene expression,whereas acetate was less effective. A similar inhibition was achieved with the proteasome inhibitor MG-132,but not the p38 MAPK inhibitor SB203580. All SCFAs decreased IL-6 protein release from organ cultures. CONCLUSION: In the present study propionate and butyrate were equipotent,whereas acetate was less effective,at suppressing NF-κB reporter activity,immune-related gene expression and cytokine release in vitro. Our findings suggest that propionate and acetate,in addition to butyrate,could be useful in the treatment of inflammatory disorders,including IBD.

Background The clinical presentations of frontotemporal dementia(FTD)are diverse and overlap with other neurological disorders.There are,as of today,no biomarkers in clinical practice for diagnosing the disorders.Here,we aimed to find protein markers in cerebrospinal fluid(CSF)from patients with FTD,presymptomatic mutation carriers and non-carriers.Methods Antibody suspension bead arrays were used to analyse 328 proteins in CSF from patients with behavioural variant FTD(bvFTD,n=16)and progressive primary aphasia(PPA,n=13),as well as presymptomatic mutation carriers(PMC,n=16)and non-carriers(NC,n=8).A total of 492 antibodies were used to measure protein levels by direct labelling of the CSF samples.The findings were further examined in an independent cohort including 13 FTD patients,79 patients with Alzheimer’s disease and 18 healthy controls.Results We found significantly altered protein levels in CSF from FTD patients compared to unaffected individuals(PMC and NC)for 26 proteins.The analysis show patterns of separation between unaffected individuals and FTD patients,especially for those with a clinical diagnosis of bvFTD.The most statistically significant differences in protein levels were found for VGF,TN-R,NPTXR,TMEM132D,PDYN and NF-M.Patients with FTD were found to have higher levels of TN-R and NF-M,and lower levels of VGF,NPTXR,TMEM132D and PDYN,compared to unaffected individuals.The main findings were reproduced in the independent cohort.Conclusion In this pilot study,we show a separation of FTD patients from unaffected individuals based on protein levels in CSF.Further investigation is required to explore the CSF profiles in larger cohorts,but the results presented here has the potential to enable future clinical utilization of these potential biomarkers within FTD.

Microalgae Improve the Photosynthetic Performance of Rice Seedlings(Oryza sativa L.)under Physiological Conditions and Cadmium Stress

The aim of this study was to assess the impact of the microalgae Chlorella vulgaris on the rice seedlings at physiological conditions and under cadmium(Cd)stress.We examined the effects of C.vulgaris in the nutrient solution on rice seedlings grown hydroponically in the presence and the absence of 150μM CdCl2,using the low(77 K)temperature and pulse amplitude modulated(PAM)chlorophyll fluorescence,P700 photooxidation measurements,photochemical activities of both photosystems,kinetic parameters of oxygen evolution,oxidative stress markers(MDA,H_(2)O_(2) and proline),pigment content,growth parameters and Cd accumulation.Data revealed that the application C.vulgaris not only stimulates growth and improves the functions of photosynthetic apparatus under physiological conditions,but also reduces the toxic effect of Cd on rice seedlings.Furthermore,the presence of the green microalgae in the nutrient solution of the rice seedlings during Cd exposure,signifi-cantly improved the growth,photochemical activities of both photosystems,the kinetic parameters of the oxygen-evolving reactions,pigment content and decreased lipid peroxidation,H_(2)O_(2) and proline content.Data showed that the alleviation of Cd-induced effects in rice seedlings is a result of the Cd sorption by microalgae,as well as the reduced Cd accumulation in the roots and its translocation from the roots to the shoots.

T cell responses to hepatitis B surface antigen are detectable in non-vaccinated individuals

AIM: To evaluate, whether humoral hepatitis-B-vaccine non-responders also fail to mount a T cell response and to compare these results to normal vaccinees. METHODS: Fourty-seven health care employees were enrolled in this study including all available non- responders (n = 13) with an anti-HBsAg titer 1000 kU/L as controls. PBMC from all subjects were analyzed by IFN-γ and IL-4 ELISPOT assays for the presence of hepatitis B surface antigen (HBsAg) reactive T cells. RESULTS: Non-responders and low-responders had no or only very limited T cell responses, respectively. Indi- viduals responding to vaccination with the induction of a high anti-HBsAg titer showed a strong T cell response after the third vaccination. Surprisingly, these individuals showed response even before the first vaccination. T cell response to control antigens and mitogens was similar in all groups. CONCLUSION: Our data suggest that there is no gen- eral immune deficiency in non-/low-responders. Thus, we hypothesize that the induction of anti-HBsAg re- sponses by vaccination is significantly dependent on the pre-existing T cell repertoire against the specific antigen rather than the presence of a general T cell defect.

Biophotonics is an emerging multidisciplinary research area, embracing all light-based technologies applied to the life sciences and medicine [1-6]. The expression itself is the combination of the Greek syllables ＂bios＂ standing for life and ＂phos＂ standing for light. Photonics is the technical term for all methodologies and technologies utilizing photons over the whole spectrum from X-ray over the ultraviolet, visible and the infrared to the terahertz region, and its interaction with any matter. Beyond this definition, biophotonics is a scientific discipline of remarkable societal importance. As a part of photonics, it has proved to be an important enabling technology for accelerated progress in medicine and biotechnology. It can do so, because it originated at the interface of the most innovative academic disciplines of the last century, i.e., photonics, biotechnology and nanotechnologies. This field of research brings together scientists from different professions, such as physicists, biologists, pharmacists, medical doctors, etc. To work on common projects, they need to understand the ideas and the techniques of their counterparts.

Fungi are eukaryotes that play essential roles in ecosystems.Among fungi,Basidiomycota is one of the major phyla with more than 40,000 described species.We review species diversity of Basidiomycota from five groups with different lifestyles or habitats:saprobic in grass/forest litter,wood-decaying,yeast-like,ectomycorrhizal,and plant parasitic.Case studies of Agaricus,Cantharellus,Ganoderma,Gyroporus,Russula,Tricholoma,and groups of lichenicolous yeast-like fungi,rust fungi,and smut fungi are used to determine trends in discovery of biodiversity.In each case study,the number of new species published during 2009–2020 is analysed to determine the rate of discovery.Publication rates differ between taxa and reflect different states of progress for species discovery in different genera.The results showed that lichenicolous yeast-like taxa had the highest publication rate for new species in the past two decades,and it is likely this trend will continue in the next decade.The species discovery rate of plant parasitic basidiomycetes was low in the past ten years,and remained constant in the past 50 years.We also found that the establishment of comprehensive and robust taxonomic systems based on a joint global initiative by mycologists could promote and standardize the recognition of taxa.We estimated that more than 54,000 species of Basidiomycota will be discovered by 2030,and estimate a total of 1.4–4.2 million species of Basidiomycota glob-ally.These numbers illustrate a huge gap between the described and yet unknown diversity in Basidiomycota.

A New Protocol for Controlled Ovarian Stimulation Monitoring by Self-Determination of Estrone-3-Glucuronide and Single Ultrasound (COSSESU)

Background: Answering the question of what is the optimal protocol for monitoring controlled ovarian stimulation (COS) still remains a challenge. The rapid introduction of new diagnostic methods and various components of telemedicine makes it possible to reduce the number of patient visits during ovarian stimulation, which will reduce the loss of time, costs, and risk for the patient from COVID-like situations. Methods: The different COS monitoring protocols are examined, thus proposing a new approach consisting of two successive phases. Results: In the first phase, E3G in urine samples is being examined, which is performed by the patient themselves with a small portable analyzer. Based on the results, the specialist prescribes the doses for stimulation. The second phase involves one single determination of the size and number of follicles at the end of stimulation, using TVUS, as well as the dynamics of serum levels of P4 and E2. This proves to be in many cases sufficient.
