Dose requirement and complications of diluted and undiluted propofol for deep sedation in endoscopic retrograde cholangiopancreatography

BACKGROUND:In general,the dose requirement and complications of propofol are lower when used in the diluted form than in the undiluted *** aim of this study was to determine the dose requirement and complications of diluted and undiluted propofol for deep sedation in endoscopic retrograde ***:Eighty-six patients were randomly assigned to either group D (diluted propofol) or U (undiluted propofol).All patients were sedated with 0.02-0.03 mg/kg midazolam (total dose ≤2 mg for age <70 years and 1 mg for age ≥70) and 0.5-1 μg/kg fentanyl (total dose ≤75 μg for age <70 and ≤50 μg for age ≥70).Patients in group U (42) were sedated with standard undiluted propofol (10 mg/mL).Patients in group D (44) were sedated with diluted propofol (5 mg/mL).All patients in both groups were monitored for the depth of sedation using the Narcotrend *** primary outcome variable was the total dose of propofol used during the *** secondary outcome variables were complications during and immediately after the procedure,and recovery ***:All endoscopies were completed *** propofol doses per body weight and per body weight per hour in groups D and U were 3.0 mg/kg,6.2 mg/kg per hour and 4.7 mg/kg,8.0 mg/kg per hour,*** mean dose of propofol,expressed as total dose,dose/kg or dose/kg per hour and the recovery time were not significantly different between the two ***-related adverse events during and immediately after the procedure were higher in group U (42.9%) than in group D (18.2%) (P=0.013).CONCLUSIONS:Propofol requirement and recovery time in the diluted and undiluted propofol groups were ***,the sedation-related hypotension was significantly lower in the diluted group than the undiluted group.

AIM:To study the anesthetic management of patients undergoing small bowel enteroscopy in the World Gastroenterology Organization(WGO) Endoscopy Training Center in ***:Patients who underwent small bowel enteroscopy during the period of March 2005 to March 2011 in Siriraj Gastrointestinal Endoscopy Center were retrospectively *** patients' characteristics,pre-anesthetic problems,anesthetic techniques,anesthetic agents,anesthetic time,type and route of procedure and anesthesia-related complications were ***:One hundred and forty-four patients underwent this procedure during the study *** mean age of the patients was 57.6 ± 17.2 years,andmost were American Society of Anesthesiologists(ASA) class Ⅱ(53.2%).Indications for this procedure were gastrointestinal bleeding(59.7%),chronic diarrhea(14.3%),protein losing enteropathy(2.6%) and others(23.4%).Hematologic disease,hypertension,heart disease and electrolyte imbalance were the most common pre-anesthetic *** anesthesia with endotracheal tube was the anesthetic technique mainly employed(50.6%).The main anesthetic agents administered were fentanyl,propofol and *** mean anesthetic time was 94.0 ± 50.5 *** balloon and oral(antegrade) intubation was the most common type and route of *** anesthesia-related complication rate was relatively *** overall and cardiovascular-related complication rates including hypotension in the older patient group(aged ≥ 60 years old) were significantly higher than those in the younger ***:During anesthetic management for small bowel enteroscopy,special techniques and drugs are not routinely ***,for safety reasons anesthetic personnel need to optimize the patient's condition.

A cation-dipole-reinforced elastic polymer electrolyte enabling long-cycling quasi-solid-state lithium metal batteries

The application of ionic liquids(IL)in polymer electrolytes represents a safer alternative to the currently used organic solvents in lithium batteries due to their nonflammability and thermal ***,as a plasticizer,it is generally agreed that the introduction of ionic liquid usually leads to a trade-off between ion transport and mechanical properties of polymer *** we report the synthesis of an IL-embedded polymer electrolyte with both high ionic conductivity(2.77×10^(-4)S cm^(-1)at room temperature)and excellent mechanical properties(high tensile strength up to 11.4 MPa and excellent stretchability of 387%elongation at break)achieved by strong ion–dipole interactions between polymer electrolyte components,which was unveiled by the DFT ***,this polymer electrolyte also exhibits nonflammability,good thermal stability and the ability to recover reversibly from applied stress,i.e.,excellent *** highly viscoelastic polymer electrolyte enables tight interfacial contact and good adaptability with electrodes for stable lithium stripping/plating for 2000 h under a current density of 0.1 mA cm^(-2).By coupling with this polymer electrolyte,the LiFePO_(4)/Li cells exhibit outstanding cycling stability at room temperature as well as the reliability under extreme environmental temperature or being abused.

Five years of post-validation surveillance of lymphatic filariasis in Thailand

Background The World Health Organization (WHO) validated Thailand in 2017 as having eliminated lymphatic filariasis (LF) as a public health problem with recommendations for continued surveillance. This article describes measures and progress made in Thailand with post-validation surveillance (PVS) of LF from 2018 until *** The implementation unit (IU) is a sub-village in 11 former LF endemic provinces. Human blood surveys are targeted in 10% of IUs each year. InWuchereria bancrofti areas, filaria antigen test strips (FTS) are used, and inBrugia malayi areas, antibody test kits (Filaria DIAG RAPID) are used. Positive cases are confirmed by thick blood film (TBF) and polymerase chain reaction (PCR). Vector surveys for mosquito species identification and dissection for microfilaria (Mf)/filarial larvae are done in 1% of IUs where human blood surveys are conducted. Human blood surveys using FTS are conducted among migrants in five provinces. Surveillance of cats is done in areas that previously recorded > 1.0% Mf rate among cats. Morbidity management and disability prevention (MMDP) are done every 2 years in LF-endemic areas where chronic disease patients *** From 2018 to 2022, in a total of 357 IUs in 11 provinces, human blood surveys were conducted in 145 IUs (41%) with an average population coverage of 81%. A total of 22,468 FTS and 27,741 FilariaDIAG RAPID were performed. 27 cases were detected: 3 cases ofW. bancrofti in Kanchanaburi province and 24 cases ofB. malayi in Narathiwat province. 4 cases ofW. bancrofti were detected in two provinces through routine public health surveillance. Vector surveys in 47 IUs detectedB. malayi Mf filarial larvae only in Narathiwat province. Chronic LF patients reduced from 114 in 2017 to 76 in 2022. Surveys among 7633 unregistered migrants yielded 12 cases ofW. bancrofti. Mf rate among cats in Narathiwat province declined from 1.9% in 2018 to 0.7% in 2022. The tissue engineering triad comprises the combination of cells,scaffolds and biological ***,we prepared cell-and drug-loaded hydrogels using in situ silk fibroin(SF)hydrogels induced by dimyristoyl glycerophosphoglycerol(DMPG).DMPG is reported to induce rapid hydrogel formation by SF,facilitating cell encapsulation in the hydrogel matrix while maintaining high cell viability and proliferative *** addition,DMPG can be used for liposome formulations in entrapping drug ***(Dex)was loaded into the DMPG-induced SF hydrogels together with human osteoblast-like SaOS-2 cells,then the osteogenic differentiation of the entrapped cells was evaluated in vitro and compared to cells cultured under standard *** production by cells cultured in DMPG/Dex-SF hydrogels with Dex-depleted osteogenic medium was equivalent to that of cells cultured in conventional osteogenic medium containing *** extended-release of the entrapped Dex by the hydrogels was able to provide a sufficient drug amount for osteogenic *** controlled release of Dex was also advantageous for cell viability even though its dose in the hydrogels was far higher than that in osteogenic *** results confirmed the possibility of using DMPG-induced SF hydrogels to enable dual cell and drug encapsulation to fulfil the practical applications of tissue-engineered constructs.

Adulticidal, larvicidal, pupicidal and oviposition deterrent activities of essential oil from Zanthoxylum limonella Alston(Rutaceae) against Aedes aegypti(L.) and Culex quinquefasciatus(Say)

Objective: To evaluate adulticidal, larvicidal and oviposition deterrent response of the essential oil from dried Zanthoxylum limonella(Z. limonella) fruit against Aedes aegypti(Ae. aegypti) and Culex quinquefasciatus(Cx. quinquefasciatus).Methods: Z. limonella oil was tested by biological assays at 1%, 5% and 10% concentrations in ethanol. Adulticidal efficacy was tested against the 2–3 day old adult females. Larvicidal activity was tested against immature stage of mosquitoes. Oviposition deterrence of the oil was evaluated on gravid ***: The adult mortality was observed after 24 h with the LC50 of 6.0% for ***, and 5.7% for Cx. quinquefasciatus. Larvicidal bioassay was carried out with the 10% Z. limonella oil against immature stages of Ae. aegypti and Cx. quinquefasciatus,which caused 100% mortality after 12 h and 24 h. In the larvicidal experiment,Z. limonella showed effective result at 1%, 5% and 10% concentrations with the values of LT50 Ae. aegypti = 9.78, 5.61, 0.24 h for larvae and LT50= 64.08, 21.23 h for pupae;Cx. quinquefasciatus had LT50= 28.46, 20.25, 1.01 h for larvae and LT50= 67.52, 27.96,4.11 h for pupae, respectively. Oviposition deterrence of the oil was evaluated on gravid females. In the study, 10% Z. limonella showed 100% repellency for Ae. aegypti and99.53% for Cx. quinquefasciatus. Likewise, oviposition activity indexes of these oil concentrations were all negative values ranging from–0.89 to-1.00 for Ae. aegypti and–0.64 to–0.99 for Cx. quinquefasciatus. The oviposition activity indexes values revealed that Z. limonella oil has deterrent effect, and it caused a remarkable negative response resulting in very few ***: This result indicates that Z. limonella oil can be used as an effective adulticide, larvicide and oviposition deterrent against Ae. aegypti and Cx. quinquefasciatus.

Mitochondrial Diversity and Phylogeographic Patterns of Gekko gecko(Squamata: Gekkonidae) in Thailand

The Tokay Gecko, Gekko gecko(Linnaeus, 1758) is widely distributed in Asia and there have been concerns regarding locally decreasing populations due to overexploitation for traditional Chinese medicine. Previous studies of the genetic relationships of G. gecko populations included few populations from Thailand. Here we investigated the phylogeographic patterns of G. gecko from different regions in Thailand using mitochondrial cytochrome b sequences. Phylogenetic analyses revealed two lineages: one(Lineage A) comprising populations from Laos, Vietnam, and Thailand;and a second(Lineage B) comprising three genetically distinct groups within Thailand alone. Some Thai populations were found to have both lineages represented within them. Highly significant genetic differentiation(FST) showed geographic population structuring in Lineage B, indicating limited gene flow among groups in Thailand. Although G. gecko has a wide distribution and is well adapted to human habitation, the observed genetic structure could potentially be explained by geographic barriers such as mountain ranges. In Lineage A, our study provided primary phylogeographic evidence for lineage mixture that might be a result of human-mediated transport. Future research should include more extensive sampling across the geographic distribution of G. gecko and a landscape genetics approach could be applied for conservation planning.
