Adopting environmentally sustainable diets can substantially reduce anthropogenic greenhouse gas emissions(GHG). However, there remain concerns that consuming sustainable foods will increase the likelihood of nutritional deficiencies in periods of increased demand, including pregnancy. This study aimed to identify commonly consumed foods of pregnant women and determine the effect of their replacement with sustainable alternatives on nutrient intake and measures of environmental sustainability. Methods Dietary intake data from 171 pregnant women was assessed using a validated food frequency questionnaire. The food groups and food items that contributed most to energy and protein intake were identified. Of these foods, those that produce the highest GHG emissions(Poore et al. 2018) were matched with proposed sustainable alternatives for energy and protein content, and their impact on nutrient provision and sustainability measures were determined. Results Meats(bovine, poultry, fish, lamb and eggs), grains and dairy products were important sources of both energy and protein intake. With the highest GHG emissions, beef was selected as the reference food. Proposed food alternatives included lamb, chicken, eggs, fish, tofu, legumes and nuts. The most pronounced reductions in CO2 emission were from replacing beef with tofu, legumes and nuts. For example, replacing one serve of beef with an isocaloric serve of firm tofu per week during pregnancy could reduce GHG emissions by 372 kg CO2 eq, equivalent to that produced by an average passenger vehicle driving 1498 km. This replacement would not adversely impact iron(+1.1 mg per serve) content, whilst providing a richer source of calcium(+294.8 mg per serve), folate(+28.1μg per serve) and dietary fibre(+3.3 g per serve). Helicobacter pylori:High dose amoxicillin does not improve primary or secondary eradication rates in an Irish cohort

BACKGROUND Helicobacter pylori(H.pylori)eradication rates have fallen globally,likely in large part due to increasing antibiotic resistance to traditional therapy.In areas of high clarithromycin and metronidazole resistance such as ours,Maastricht VI guidelines suggest high dose amoxicillin dual therapy(HDADT)can be considered,subject to evidence for local efficacy.In this study we assess efficacy of HDADT therapy for H.pylori eradication in an Irish cohort.AIM To assess the efficacy of HDADT therapy for H.pylori eradication in an Irish cohort as both first line,and subsequent therapy for patients diagnosed with H.pylori.METHODS All patients testing positive for H.pylori in a tertiary centre were treated prospectively with HDADT(amoxicillin 1 g tid and esomeprazole 40 mg bid×14 d)over a period of 8 months.Eradication was confirmed with Urea Breath Test at least 4 wk after cessation of therapy.A delta-over-baseline>4%was considered positive.Patient demographics and treatment outcomes were recorded,analysed and controlled for basic demographics and prior H.pylori treatment.RESULTS One hundred and ninety-eight patients were identified with H.pylori infection,10 patients were excluded due to penicillin allergy and 38 patients refused follow up testing.In all 139 were included in the analysis,55%(n=76)were female,mean age was 46.6 years.Overall,93(67%)of patients were treatment-naïve and 46(33%)had received at least one previous course of treatment.The groups were statistically similar.Self-reported compliance with HDADT was 97%,mild side-effects occurred in 7%.There were no serious adverse drug reactions.Overall the eradication rate for our cohort was 56%(78/139).Eradication rates were worse for those with previous treatment[43%(20/46)vs 62%(58/93),P=0.0458,odds ratio=2.15].Age and Gender had no effect on eradication status.CONCLUSION Overall eradication rates with HDADT were disappointing.Despite being a simple and possibly better tolerated regime,these results do not support its routine use in a high dual resistanc

Timing of surgical repair of bile duct injuries after laparoscopic cholecystectomy: A systematic review

BACKGROUND The surgical management of bile duct injuries(BDIs)after laparoscopic cholecystectomy(LC)is challenging and the optimal timing of surgery remains unclear.The primary aim of this study was to systematically evaluate the evidence behind the timing of BDI repair after LC in the literature.AIM To assess timing of surgical repair of BDI and postoperative complications.METHODS The MEDLINE,EMBASE,and The Cochrane Library databases were systematically screened up to August 2021.Risk of bias was assessed via the Newcastle Ottawa scale.The primary outcomes of this review included the timing of BDI repair and postoperative complications.RESULTS A total of 439 abstracts were screened,and 24 studies were included with 15609 patients included in this review.Of the 5229 BDIs reported,4934(94%)were classified as major injury.Timing of bile duct repair was immediate(14%,n=705),early(28%,n=1367),delayed(28%,n=1367),or late(26%,n=1286).Standardization of definition for timing of repair was remarkably poor among studies.Definitions for immediate repair ranged from24 h to>12 wk after LC)and late repair(>6 wk after LC)showed a broad overlap.CONCLUSION The lack of standardization among studies precludes any conclusive recommendation on optimal timing of BDI repair after LC.This finding indicates an urgent need for a standardized reporting system of BDI repair.

Localization of urea transporter B in the developing bovine rumen

Urea nitrogen secreted from blood to rumen is a crucial factor shaping the symbiotic relationship between host ruminants and their microbial populations.Passage of urea across rumen epithelia is facilitated by urea transporter B(UT-B),but the long-term regulation of these proteins remains unclear.As ruminal function develops over a period of months,the developing rumen is an excellent model with which to investigate this regulation.Using rumen epithelium samples of calves from birth to 96 d of age,this study performed immunolocalization studies to localize and semi-quantify UT-B protein development.As expected,preliminary experiments confirmed that ruminal monocarboxylate transporter 1(MCT1)short chain fatty acid transporter protein abundance increased with age(P<0.01,n=4).Further investigation revealed that ruminal UT-B was present in the first few weeks of life and initially detected in the basolateral membrane of stratum basale cells.Over the next 2 months,UT-B staining spread to other epithelial layers and semi-quantification indicated that UT-B abundance significantly increased with age(P<0.01,n=4 or 6).These changes were in line with the development of rumen function after the advent of solid feed intake and weaning,exhibiting a similar pattern to both MCT1 transporters and papillae growth.This study therefore confirmed age-dependent changes of in situ ruminal UT-B protein,adding to our understanding of the long-term regulation of ruminal urea transporters.

Combined antrum and corpus biopsy protocol improves Helicobacter pylori culture success

BACKGROUND Helicobacter pylori(H.pylori)causes chronic gastritis,peptic ulcer disease,gastric adenocarcinoma and mucosa-associated lymphoid tissue lymphoma.Eradication rates have fallen,mainly due to antimicrobial resistance.Consensus guidelines recommend that first-line treatment is based on the local prevalence of antimicrobial resistance and that rescue therapies are guided by antimicrobial susceptibility testing(AST).However,H.pylori culture is challenging and culture-based AST is not routinely performed in the majority of hospitals.Optimisation of H.pylori culture from clinical specimens will enable more widespread AST to determine the most appropriate antimicrobials for H.pylori eradication.AIM To determine whether dual antrum and corpus biopsy sampling is superior to single antrum biopsy sampling for H.pylori culture.METHODS The study received ethical approval from the joint research ethics committee of Tallaght University Hospital and St.James’s Hospital.Patients referred for upper gastrointestinal endoscopy were invited to participate.Biopsies were collected in tubes containing Dent’s transport medium and patient demographics were recorded.Biopsies were used to inoculate Colombia blood agar plates.Plates were incubated under microaerobic conditions and evaluated for the presence of H.pylori.Statistical analyses were performed using Graphpad PRISM.Continuous variables were compared using the two-tailed independent t-test.Categorical variables were compared using the two-tailed Fisher exact test.In all cases,a P value less than 0.05 was considered significant.RESULTS In all,samples from 219 H.pylori-infected patients were analysed in the study.The mean age of recruited patients was 48±14.9 years and 50.7%(n=111)were male.The most common endoscopic finding was gastritis(58.9%;n=129).Gastric ulcer was diagnosed in 4.6%(n=10)of patients,while duodenal ulcer was diagnosed in 2.7%(n=6).Single antrum biopsies were collected from 73 patients,whereas combined antrum and corpus biopsies were collected from 146 pati

MicroRNAs(miRNAs)have been widely implicated in immune regulation,but evidence for the coordinated function of paralogous miRNA clusters remains scarce.Here,by using genetically modified mice with individual or combined cluster deficiencies,we found that three paralogous clusters of the miR-17-92 family of miRNAs collectively suppressed IL-12 production in macrophages.Accordingly,miR-17-92 family miRNAs deficiencies resulted in heightened production of IL-12 and thus enhanced the host defense against intracellular pathogen Listeria monocytogenes in vivo.Mechanistically,different members of the miR-17-92 family of miRNAs acted on a common target,PTEN,to inhibit IL-12 expression by modulating the PI3K-Akt-GSK3 pathway.In addition,the expression of miR-17-92 family miRNAs was collectively inhibited by the transcription factor RBP-J,and RBP-J-associated macrophage functional defects were genetically rescued by deleting three clusters of miR-17-92 family miRNAs on a RBP-J null background.Thus,our results illustrated key roles of three clusters of miR-17-92 family miRNAs in cooperatively controlling IL-12-mediated immune responses and identified miR-17-92 family miRNAs as functional targets of RBP-J in macrophages.

Quantitative analysis of ruminal methanogenic microbial populations in beef cattle divergent in phenotypic residual feed intake(RFI)offered contrasting diets

Background：Methane（CH_4） emissions in cattle are an undesirable end product of rumen methanogenic fermentative activity as they are associated not only with negative environmental impacts but also with reduced host feed efficiency.The aim of this study was to quantify total and specific rumen microbial methanogenic populations in beef cattle divergently selected for residual feed intake（RFI） while offered（i） a low energy high forage（HF） diet followed by（ii） a high energy low forage（LF） diet.Ruminal fluid was collected from 14 high（H）and 14 low（L） RFI animals across both dietary periods.Quantitative real time PCR（qRT-PCR） analysis was conducted to quantify the abundance of total and specific rumen methanogenic microbes.Spearman correlation analysis was used to investigate the association between the relative abundance of methanogens and animal performance,rumen fermentation variables and diet digestibility.Results：Abundance of methanogens,did not differ between RFI phenotypes.However,relative abundance of total and specific methanogen species was affected（P〈 0.05） by diet type,with greater abundance observed while animals were offered the LF compared to the HF diet.Conclusions：These findings suggest that differences in abundance of specific rumen methanogen species may not contribute to variation in CH_4 emissions between efficient and inefficient animals,however dietary manipulation can influence the abundance of total and specific methanogen species.

Examination of the molecular control of ruminal epithelial function in response to dietary restriction and subsequent compensatory growth in cattle

Background： The objective of this study was to investigate the effect of dietary restriction and subsequent compensatory growth on the relative expression of genes involved in volatile fatty acid transport, metabolism and cell proliferation in ruminal epithelial tissue of beef cattle. Sixty Holstein Friesian bulls（mean liveweight 370 ± 35 kg;mean age 479 ± 15 d） were assigned to one of two groups：（i） restricted feed allowance（RES; n = 30） for 125 d（Period 1） followed by ad libitum access to feed for 55 d（Period 2） or（ii） ad libitum access to feed throughout（ADLIB; n = 30）. Target growth rate for RES was 0.6 kg/d during Period 1. At the end of each dietary period, 15 animals from each treatment group were slaughtered and ruminal epithelial tissue and liquid digesta harvested from the ventral sac of the rumen. Real-time q PCR was used to quantify m RNA transcripts of 26 genes associated with ruminal epithelial function. Volatile fatty acid analysis of rumen fluid from individual animals was conducted using gas chromatography.Results： Diet × period interactions were evident for genes involved in ketogenesis（BDH2, P = 0.017）, pyruvate metabolism（LDHa, P = 0.048; PDHA1, P = 0.015） and cellular transport and structure（DSG1, P = 0.019; CACT, P = 0.027）. Ruminal concentrations of propionic acid（P = 0.018） and n-valeric acid（P = 0.029） were lower in RES animals, compared with ADLIB, throughout the experiment. There was also a strong tendency（P = 0.064）toward a diet × period interaction for n-butyric with higher concentrations in RES animals, compared with ADLIB, during Period 1.Conclusions： These data suggest that following nutrient restriction, the structural integrity of the rumen wall is compromised and there is upregulation of genes involved in the production of ketone bodies and breakdown of pyruvate for cellular energy. Rectal neuroendocrine tumours represent a rare colorectal tumour with a 10 fold increased prevalence due to incidental detection in the era of colorectal screening.Patient outcomes with early diagnosis are excellent.However endoscopic recognition of this lesion is variable and misdiagnosis can result in suboptimal endoscopic resection with subsequent uncertainty in relation to optimal long-term management.Endoscopic techniques have shown particular utility in managing this under-recognized neuroendocrine tumour.
