Background:recently,an innovative tool called“proficiency score”was introduced to assess the learning curve for robot-assisted radical prostatectomy(RARP).However,the initial study only focused on patients with low-risk prostate cancer forwhompelvic lymph node dissection(PLND)was not *** address this issue,we aimed to validate proficiency scores of a contemporarymulticenter cohort of patients with high-risk prostate cancer treated with RARP plus extended PLND by trainee *** andmethods:Between 2010 and 2020,4 Italian institutional prostate-cancer datasets weremerged and queried for“RARP”and“high-risk prostate cancer.”High-risk prostate cancer was defined according to the most recent European Association of Urology guidelines as follows:prostate-specific antigen>20 ng/mL,International Society ofUrological Pathology≥4,and/or clinical stage(cT)≥2c on preoperative *** selected cohort(n=144)included clinical cases performed by trainee surgeons(n=4)after completing their RARP learning curve(50 procedures for low-risk prostate cancer).The outcome of interest,the proficiency score,was defined as the coexistence of all the following criteria:a comparable operation time to the interquartile range of the mentor surgeon at each center,absence of any significant perioperative complications Clavien-Dindo Grade 3–5,no perioperative blood transfusions,and negative surgical margins.A logistic binary regression model was built to identify the predictors of 1-year trifecta achievement in the trainee *** all statistical analyses,a 2-sided pred ***:A proficiency score was achieved in 42.3%*** univariable level,proficiency score was associated with 1-year trifecta achievement(odds ratio,8.77;95%confidence interval,2.42–31.7;p=0.001).After multivariable adjustments for age,nerve-sparing,and surgical technique,the proficiency score independently predicted 1-year trifecta achievement(odds ratio,9.58;95%confidence i

The dynamics of polymers under nanoscale confinement are different from the bulk behavior. Although many studies have been devoted to polymer dynamics within Anodic aluminum oxide（AAO） nanopores,the reported results are diverse and sometimes controversial. Rainfall data is probably one of the longest-recorded climatic parameters in Lebanon. On the central coast of Lebanon, the Beirut weather station started collecting rainfall data in 1876. However, the recorded data is not available at one data provider source. Published data is found in historical documents but it reaches the early 1970s and then appears a data gap till 1990. Still the data is available, but it might be found to be saved privately. This study investigated the SPI variability on annual time scale between the years 1876 and 2021. The SPI was computed using R-Stat software to compare every year between 1876 and 2021. The majority (about 70% of the years) of the years are near normal in the precipitation rate. The Standardized Precipitation Index (SPI) demonstrated a normal distribution of years. Dry and wet years constitute about 15% of the total 146 years (1876-2021). Extremely dry years might appear in two consecutive years between 50 to 60 years count. After 1991, there were no wet years it was only near normal and few dry years. The last 30 years showed a trend of increasing drought years without any occurrence of wet years. This study demonstrated the importance of keeping records of at least rainfall data and it must be recorded on a daily basis or intensity on time. It is highly important on a managerial basis and for water security reasons to understand the drought event occurrence and investigate the changes in rainfall rates. Climate change scenarios always forecast a decrease in rainfall rates which will not appear without such studies.

This cross-sectional study examined the lower limb balance,ankle dorsiflexion,orofacial tissue pressure,and occlusal strength of rugby ***-six participants were divided into groups:rugby players(n=13)and healthy sedentary adults(n=13).Participants underwent an analysis of lower limb balance using a composite score(Y-Balance Test).Ankle dorsiflexion was measured using the Lunge *** Iowa Oral Performance Instrument was employed to measure orofacial tissue *** force was measured with a dynamometer,and T-Scan assessed occlusal contact *** were analyzed using the t-test(pre it is possible to verify that these factors did not influence the results *** differences were observed in the balance of the right(p=0.07)and left(p=0.02)lower limbs,where rugby players had lower composite *** were significant differences in the right(p=0.005)and left(p=0.004)lunges,with rugby players showing lower values,as well as lower tongue pressure(p=0.01)and higher lip pressure(p=0.03),with significant differences to sedentary *** was no significant difference in molar bite force and distribution occlusal contacts between *** seems to reduce lower limb displacement,cause ankle hypomobility,lead to changes in orofacial tissues,particularly the tongue and *** study is significant for identifying significant differences between rugby players and sedentary individuals,providing new insights into the impact of rugby on health and performance,which can benefit sports training and injury prevention.

Background Pediatric post coronavirus disease 2019(COVID-19)condition(PPCC)is a heterogeneous syndrome,which can significantly affect the daily lives of *** study aimed to identify clinically meaningful phenotypes in children with PPCC,to better characterize and treat this *** Participants were children with physician-diagnosed PPCC,referred to the academic hospital Amsterdam UMC in the Netherlands between November 2021 and March *** factors and information on post-COVID symptoms,comorbidities,and impact on daily life were *** clusters were identified using an unsupervised and unbiased approach for mixed data *** Analysis of 111 patients(aged 3–18 years)revealed three distinct clusters within *** 1(n=62,median age=15 years)predominantly consisted of girls(74.2%).These patients suffered relatively more from exercise intolerance,dyspnea,and smell *** 2(n=33,median age=13 years)contained patients with an even gender distribution(51.5%girls).They suffered from relatively more sleep problems,memory loss,gastrointestinal symptoms,and *** 3(n=16,median age=11 years)had a higher proportion of boys(75.0%),suffered relatively more from fever,had significantly fewer symptoms(median of 5 symptoms compared to 8 and 10 for clusters 1 and 2 respectively),and experienced a lower impact on daily *** This study identified three distinct clinical PPCC phenotypes,with variations in sex,age,symptom patterns,and impact on daily *** findings highlight the need for further research to understand the potentially diverse underlying mechanisms contributing to post-COVID symptoms in children.

俯冲带中的流体可能对俯冲过程产生重要影响.然而,关于大陆俯冲期间流体规模及其作用的地球物理约束仍然有限.西阿尔卑斯造山带是全球保存最完好的大陆俯冲带之一,为认识大陆俯冲过程中流体作用的影响提供了重要的参考.本文综合多种地球物理方法分析了西阿尔卑斯造山带地区的V_(P)/V_(S)比值,发现在俯冲区的上盘侧存在高V_(P)/V_(S)(>1.9)带,与部分蛇纹岩化的上覆地幔楔位置一致;而在下盘侧存在异常低的V_(P)/V_(S)(<1.7)带,异常深度浅于30km.本文认为,观测到的低V_(P)/V_(S)比值异常是由地壳内广泛分布的富硅岩脉导致,指示了在俯冲时期大陆俯冲界面之上可能存在的富硅流体流动.本文的结果暗示俯冲时期的流体流动可能降低了俯冲界面的有效应力,从而促进了大陆俯冲.

Obesity is one of the diseases with severe health consequences and rapidly increasing worldwide *** the complex network of food intake and energy balance regulation is an essential prerequisite for pharmacological intervention with obesity.G protein-coupled receptors(GPCRs)are among the main modulators of metabolism and energy ***,for instance,regulate appetite and satiety in certain hypothalamic neurons,as well as glucose and lipid metabolism and hormone secretion from *** in some GPCRs,such as the melanocortin receptor type 4(MC4R),have been associated with early-onset ***,we identified the adhesion GPCR latrophilin 1(ADGRL1/LPHN1)as a member of the regulating network governing food intake and the maintenance of energy *** of the highly conserved receptor in mice results in increased food consumption and severe obesity,accompanied by dysregulation of glucose ***,we identified a partially inactivating mutation in human ADGRL1/LPHN1 in a patient suffering from ***,we propose that LPHN1 dysfunction is a risk factor for obesity development.

The classical theory of mass-spring-damper-type dynamical systems on the ordinary flat space R^3 may be generalized to higher-dimensional Riemannian manifolds by reformulating the basic underlying physical principles through differential *** dynamical systems have been studied in the scientific literature because they arise naturally from the modeling of complex physical structures and because such dynamical systems constitute the basis for several modern applications such as the secure transmission of *** flows of nonlinear dynamical systems may evolve over time in complex,non-repeating(although deterministic) *** focus of the present paper is on formulating the general equations that describe the dynamics of a point-wise particle sliding on a Riemannian manifold in a coordinate-free *** paper shows how the equations particularize in the case of some manifolds of interest in the scientific literature,such as the Stiefel manifold and the manifold of symmetric positive-definite matrices.
