Aim:Safety and efficacy evidence of drug-eluting-microspheres trans-arterial chemoembolization(deM-TACE)in patients with hepatocellular carcinoma(HCC)and trans-jugular intrahepatic portosystemic shunt(TIPS)is lacking.The aim of this retrospective study was to report the safety and efficacy of deM-TACE procedures performed with microspheres smaller than 300μm in patients with HCC and TIPS in a high-volume transplant center.Methods:Embolization was standardized by initiating deM-TACE with microspheres smaller than 100μm,and if stasis was not achieved,adjunctive embolization with 100-300 or 200μm microspheres was administered.With regards to efficacy,the oncological response was evaluated and categorized according to mRECIST criteria at 1,3-6,9-12,and 15-18 months.Reporting the safety profile,detailed laboratory analysis was performed before,at 36-48 h,and 30-60 days after the procedure.Adverse events(AEs)were recorded;post-embolic syndrome was defined as the onset of fever/nausea/pain after the procedure.Late onset hepatobiliary complications were evaluated by follow-up imaging with computed tomography or magnetic resonance(CT/MR).Results:From december 2007 to November 2020,17 HCC patients(25 HCC nodules)with patent TIPS underwent 20 deM-TACE.Embolization was performed only with microspheres smaller than 100μm in 3/20 deM-TACE(15%);adjunctive embolization with 100-300 or 200μm microspheres was required in 17/20 deMTACE(85%).Reported early AEs were post-embolic syndrome(9/20;45%)all of grade 1-2,late AEs were asymptomatic acute liver bile duct injury(2/20;10%),and in one case we observed hepatic abscess(1/20;5%)resulting in death due to sepsis.With regards to efficacy,the oncological response was evaluated and categorized according to mRECIST criteria.Complete response(CR)at 1,3-6,9-12,and 15-18 months was 52%,50%,50%,and 50%,respectively.Objective response(CR+partial response)at 1,3-6,9-12,and 15-18 months was 95%,71%,70%,and 50%,respectively.Conclusion:deM-TACE with drug-eluting-microspheres smaller than 300μm can

Photoacoustic dual-comb spectroscopy(DCS),converting spectral information in the optical frequency domain to the audio frequency domain via multi-heterodyne beating,enables background-free spectral measurements with high resolution and broad bandwidth.However,the detection sensitivity remains limited due to the low power of individual comb lines and the lack of broadband acoustic resonators.Here,we develop cavity-enhanced photoacoustic DCS,which overcomes these limitations by using a high-finesse optical cavity for the power amplification of dual-frequency combs and a broadband acoustic resonator with a flat-top frequency response.We demonstrate high-resolution spectroscopic measurements of trace amounts of C2H2,NH3 and CO in the entire telecommunications C-band.The method shows a minimum detection limit of 0.6 ppb C2H2 at the measurement time of 100 s,corresponding to the noise equivalent absorption coefficient of 7×10−10 cm−1.The proposed cavityenhanced photoacoustic DCS may open new avenues for ultrasensitive,high-resolution,and multi-species gas detection with widespread applications.

Statistical analysis of physico-mechanical parameters of sandstones occurring in orogenic settings

In northeastern Sicily(Italy),sandstone rock masses widely crop out as cover deposits over crystalline terrains belonging to the orogenic belt.despite being part of the same geological formation,these sandstones are characterized by highly different features in terms of texture and physico-mechanical properties.This poses a scientific question on the possibility of tracing these rocks to a single statistical model,which could be representative of their main engineering geological properties.Therefore,it is worth investigating on the possible reasons of such differences,that should be searched either in the current geographical sandstone distribution or in the rock texture.For this study,sandstone samples were collected from different sites and were analyzed at both the hand and thin section scales.Three sandstone types were recognized,characterized by a different texture.Then,the laboratory characterization allowed estimating their main physico-mechanical and ultrasonic properties,such as porosity,density,mechanical strength,deformability,and ultrasonic velocities.The rock mechanical strength proved linked to the rock compactness and to the presence of lithic fragments,while pores and a pseudo-matrix between grains represent weakening features.Rock data were also statistically analyzed by grouping the specimens according to a geographical criterion,with respect to their sampling area,but no link was found between location and rock properties.Finally,with the aim of achieving mathematical laws that could be used to predict some rock properties from others,useful for practical purposes when dealing with such a high property variability,single and multiple regression analyses were carried out.Results show that the Uniaxial Compressive Strength,porosity,and P-wave velocity are the best predictors for a quick,indirect estimation of the main physico-mechanical parameters.The methodological approach developed for this research can be taken as reference to study other worldwide cases,involving rocks characterized by a wi

Carbon fibre-reinforced ultra-high temperature ceramics(UHTCs)are considered a class of promising materials for several applications,the most appealing ones being in the aerospace sector.Reinforcement is necessary to overcome the brittleness and low thermal shock resistance of ceramics and is often provided through the addition of carbon fibres or other carbon-based phases,such as nanotubes,graphene,and graphite.The present work is focused on the toughening of UHTCs through incorporation of 30-50 vol% thin,ordered graphite layers from conventional filter paper followed by hot pressing sintering.Cellulose filter paper was selected because it undergoes thermolysis with no melting stage forming a strong carbonaceous residue that can be used as toughener.Microstructure and mechanical properties of toughened composites were compared to those of other materials reinforced with short carbon fibres and the effect of different distribution of graphite was studied.Addition of graphite allowed toughness to be increased from 3−4 MPa·m^(1/2)(for unreinforced materials)to 5.1−5.5 MPa·m^(1/2),similar to results obtained with short fibre reinforcement.The high-temperature properties,such as strength and toughness as well as oxidation resistance at 1500℃,were also examined.

Effects of early postnatal gastric and colonic microbiota transplantation on piglet gut health

Background Diarrhea is a major cause of reduced growth and mortality in piglets during the suckling and weaning periods and poses a major threat to the global pig industry.Diarrhea and gut dysbiosis may in part be prevented via improved early postnatal microbial colonization of the gut.To secure better postnatal gut colonization,we hypothesized that transplantation of colonic or gastric content from healthy donors to newborn recipients would prevent diarrhea in the recipients in the post-weaning period.Our objective was to examine the impact of transplanting colonic or gastric content on health and growth parameters and paraclinical parameters in recipient single-housed piglets exposed to a weaning transition and challenged with enterotoxigenic Escherichia coli(ETEC).Methods Seventy-two 1-day-old piglets were randomized to four groups:colonic microbiota transplantation(CMT,n=18),colonic content filtrate transplantation(CcFT,n=18),gastric microbiota transplantation(GMT,n=18),or saline(CON,n=18).Inoculations were given on d 2 and 3 of life,and all piglets were milk-fed until weaning(d 20)and shortly after challenged with ETEC(d 24).We assessed growth,diarrhea prevalence,ETEC concentration,organ weight,blood parameters,small intestinal morphology and histology,gut mucosal function,and microbiota composition and diversity.Results Episodes of diarrhea were seen in all groups during both the milk-and the solid-feeding phase,possibly due to stress associated with single housing.However,CcFT showed lower diarrhea prevalence on d 27,28,and 29 compared to CON(all P<0.05).CcFT also showed a lower ETEC prevalence on d 27(P<0.05).CMT showed a higher alpha diversity and a difference in beta diversity compared to CON(P<0.05).Growth and other paraclinical endpoints were similar across groups.Conclusion In conclusion,only CcFT reduced ETEC-related post-weaning diarrhea.However,the protective effect was marginal,suggesting that higher doses,more effective modalities of administration,longer treatment periods,and better donor quali

Stereotactic body radiotherapy in pancreatic adenocarcinoma

Background:Stereotactic body radiotherapy(SBRT)in pancreatic cancer allows high delivery of radiation doses on tumors without affecting surrounding tissue.This review aimed at the SBRT application in the treatment of pancreatic cancer.Data sources:We retrieved articles published in MEDLINE/PubMed from January 2017 to december 2022.Keywords used in the search included:“pancreatic adenocarcinoma”OR“pancreatic cancer”AND“stereotactic ablative radiotherapy(SABR)”OR“stereotactic body radiotherapy(SBRT)”OR“chemoradiotherapy(CRT)”.English language articles with information on technical characteristics,doses and fractionation,indications,recurrence patterns,local control and toxicities of SBRT in pancreatic tumors were included.All articles were assessed for validity and relevant content.Results:Optimal doses and fractionation have not yet been defined.However,SBRT could be the standard treatment in patients with pancreatic adenocarcinoma in addition to CRT.Furthermore,the combination of SBRT with chemotherapy may have additive or synergic effect on pancreatic adenocarcinoma.Conclusions:SBRT is an effective modality for patients with pancreatic cancer,supported by clinical practice guidelines as it has demonstrated good tolerance and good disease control.SBRT opens a possibility of improving outcomes for these patients,both in neoadjuvant treatment and with radical intent.

Solid pseudopapillary tumor of the pancreas:A systematic review of clinical,surgical and oncological characteristics of 1384 patients underwent pancreatic surgery

Background:Pancreatic solid pseudopapillary tumors(SPTs)are rare clinical entity,with low malignancy and still unclear pathogenesis.They account for less than 2%of exocrine pancreatic neoplasms.This study aimed to perform a systematic review of the main clinical,surgical and oncological characteristics of pancreatic SPTs.Data sources:MEDLINE/PubMed,Web of Science and Scopus databases were systematically searched for the main clinical,surgical and oncological characteristics of pancreatic SPTs up to April 2021,in accordance with the preferred reporting items for systematic reviews and meta-analyses(PRISMA)standards.Primary endpoints were to analyze treatments and oncological outcomes.Results:A total of 823 studies were recorded,86 studies underwent full-text reviews and 28 met inclusion criteria.Overall,1384 patients underwent pancreatic surgery.Mean age was 30 years and 1181 patients(85.3%)were female.The most common clinical presentation was non-specific abdominal pain(52.6%of cases).Mean overall survival was 98.1%.Mean recurrence rate was 2.8%.Mean follow-up was 4.2 years.Conclusions:Pancreatic SPTs are rare,and predominantly affect young women with unclear pathogenesis.Radical resection is the gold standard of treatment achieving good oncological impact and a favorable prognosis in a yearly life-long follow-up.
