Robotic approach with neoadjuvant chemotherapy in adult Wilms’ tumor: A feasibility study report and a systematic review of the literature

Objective:The incidence of Wilms’tumor(WT)among adult individuals accounts for less than 1%of kidney cancer cases,with a prognosis usually less favorable when compared to younger individuals and an overall survival rate of 70%for the adult patients versus 90%for the pediatric cases.The diagnosis and treatment of WT are complex in the preoperative setting;neoadjuvant chemotherapy(NAC)or robotic surgery has rarely been described.This study aimed to review the literature of robotic surgery in WT and report the first adult WT management using both NAC and robotic strategy.Methods:We reported a case of WT managed in a multidisciplinary setting.Furthermore,according to Preferred Reporting Items for Systematic reviews and Meta-Analyses recommendations,a systematic review of the literature until August 2020 of WT treated with a robotic approach was carried out.Results:A 33-year-old female had a diagnosis of WT.She was scheduled to NAC,and according to the clinical and radiological response to a robotic radical nephrectomy with aortic lymph nodes dissection,she was managed with no intraoperative rupture,a favorable surgical outcome,and a follow-up of 25 months,which did not show any recurrence.The systematic review identified a total number of 230 cases of minimally invasive surgery reported in the literature for WT.Of these,approximately 15 patients were carried out using robotic surgery in adolescents while none in adults.Moreover,NAC has not been administered before minimally invasive surgery in adults up until now.Conclusion:WT is a rare condition in adults with only a few cases treated with either NAC or minimally invasive approach so far.The advantage of NAC followed by the robotic approach could lead to favorable outcomes in this complex scenario.Notwithstanding,additional cases of adult WT need to be identified and investigated to improve the oncological outcome.

Alzheimer's disease(AD) is a progressive neurodegenerative disease histologically characterized by the presence of extraneuronal plaques,mainly formed by the 42-aminoacid isoform of amyloid-(Aβ_(1-42)),and by intraneuronal neurofibrillary to ngles,mainly formed by the tau protein and its hyperphosphorylated isoforms(p-tau).AD is the most common cause of dementia,with an estimated lifetime risk of about 1 in 10 for men and 1 in 5 for women.

Photoacoustic dual-comb spectroscopy(DCS),converting spectral information in the optical frequency domain to the audio frequency domain via multi-heterodyne beating,enables background-free spectral measurements with high resolution and broad bandwidth.However,the detection sensitivity remains limited due to the low power of individual comb lines and the lack of broadband acoustic resonators.Here,we develop cavity-enhanced photoacoustic DCS,which overcomes these limitations by using a high-finesse optical cavity for the power amplification of dual-frequency combs and a broadband acoustic resonator with a flat-top frequency response.We demonstrate high-resolution spectroscopic measurements of trace amounts of C2H2,NH3 and CO in the entire telecommunications C-band.The method shows a minimum detection limit of 0.6 ppb C2H2 at the measurement time of 100 s,corresponding to the noise equivalent absorption coefficient of 7×10−10 cm−1.The proposed cavityenhanced photoacoustic DCS may open new avenues for ultrasensitive,high-resolution,and multi-species gas detection with widespread applications.

Statistical analysis of physico-mechanical parameters of sandstones occurring in orogenic settings

In northeastern Sicily(Italy),sandstone rock masses widely crop out as cover deposits over crystalline terrains belonging to the orogenic belt.Despite being part of the same geological formation,these sandstones are characterized by highly different features in terms of texture and physico-mechanical properties.This poses a scientific question on the possibility of tracing these rocks to a single statistical model,which could be representative of their main engineering geological properties.Therefore,it is worth investigating on the possible reasons of such differences,that should be searched either in the current geographical sandstone distribution or in the rock texture.For this study,sandstone samples were collected from different sites and were analyzed at both the hand and thin section scales.Three sandstone types were recognized,characterized by a different texture.Then,the laboratory characterization allowed estimating their main physico-mechanical and ultrasonic properties,such as porosity,density,mechanical strength,deformability,and ultrasonic velocities.The rock mechanical strength proved linked to the rock compactness and to the presence of lithic fragments,while pores and a pseudo-matrix between grains represent weakening features.Rock data were also statistically analyzed by grouping the specimens according to a geographical criterion,with respect to their sampling area,but no link was found between location and rock properties.Finally,with the aim of achieving mathematical laws that could be used to predict some rock properties from others,useful for practical purposes when dealing with such a high property variability,single and multiple regression analyses were carried out.Results show that the Uniaxial Compressive Strength,porosity,and P-wave velocity are the best predictors for a quick,indirect estimation of the main physico-mechanical parameters.The methodological approach developed for this research can be taken as reference to study other worldwide cases,involving rocks characterized by a wi

ZrB_(2)-SiC ceramics toughened with oriented paper-derived graphite for a sustainable approach

Carbon fibre-reinforced ultra-high temperature ceramics(UHTCs)are considered a class of promising materials for several applications,the most appealing ones being in the aerospace sector.Reinforcement is necessary to overcome the brittleness and low thermal shock resistance of ceramics and is often provided through the addition of carbon fibres or other carbon-based phases,such as nanotubes,graphene,and graphite.The present work is focused on the toughening of UHTCs through incorporation of 30-50 vol% thin,ordered graphite layers from conventional filter paper followed by hot pressing sintering.Cellulose filter paper was selected because it undergoes thermolysis with no melting stage forming a strong carbonaceous residue that can be used as toughener.Microstructure and mechanical properties of toughened composites were compared to those of other materials reinforced with short carbon fibres and the effect of different distribution of graphite was studied.Addition of graphite allowed toughness to be increased from 3−4 MPa·m^(1/2)(for unreinforced materials)to 5.1−5.5 MPa·m^(1/2),similar to results obtained with short fibre reinforcement.The high-temperature properties,such as strength and toughness as well as oxidation resistance at 1500℃,were also examined.

Effects of early postnatal gastric and colonic microbiota transplantation on piglet gut health

Background Diarrhea is a major cause of reduced growth and mortality in piglets during the suckling and weaning periods and poses a major threat to the global pig industry.Diarrhea and gut dysbiosis may in part be prevented via improved early postnatal microbial colonization of the gut.To secure better postnatal gut colonization,we hypothesized that transplantation of colonic or gastric content from healthy donors to newborn recipients would prevent diarrhea in the recipients in the post-weaning period.Our objective was to examine the impact of transplanting colonic or gastric content on health and growth parameters and paraclinical parameters in recipient single-housed piglets exposed to a weaning transition and challenged with enterotoxigenic Escherichia coli(ETEC).Methods Seventy-two 1-day-old piglets were randomized to four groups:colonic microbiota transplantation(CMT,n=18),colonic content filtrate transplantation(CcFT,n=18),gastric microbiota transplantation(GMT,n=18),or saline(CON,n=18).Inoculations were given on d 2 and 3 of life,and all piglets were milk-fed until weaning(d 20)and shortly after challenged with ETEC(d 24).We assessed growth,diarrhea prevalence,ETEC concentration,organ weight,blood parameters,small intestinal morphology and histology,gut mucosal function,and microbiota composition and diversity.Results Episodes of diarrhea were seen in all groups during both the milk-and the solid-feeding phase,possibly due to stress associated with single housing.However,CcFT showed lower diarrhea prevalence on d 27,28,and 29 compared to CON(all P<0.05).CcFT also showed a lower ETEC prevalence on d 27(P<0.05).CMT showed a higher alpha diversity and a difference in beta diversity compared to CON(P<0.05).Growth and other paraclinical endpoints were similar across groups.Conclusion In conclusion,only CcFT reduced ETEC-related post-weaning diarrhea.However,the protective effect was marginal,suggesting that higher doses,more effective modalities of administration,longer treatment periods,and better donor quali

Background:Pancreatic solid pseudopapillary tumors(SPTs)are rare clinical entity,with low malignancy and still unclear pathogenesis.They account for less than 2%of exocrine pancreatic neoplasms.This study aimed to perform a systematic review of the main clinical,surgical and oncological characteristics of pancreatic SPTs.Data sources:MEDLINE/PubMed,Web of Science and Scopus databases were systematically searched for the main clinical,surgical and oncological characteristics of pancreatic SPTs up to April 2021,in accordance with the preferred reporting items for systematic reviews and meta-analyses(PRISMA)standards.Primary endpoints were to analyze treatments and oncological outcomes.Results:A total of 823 studies were recorded,86 studies underwent full-text reviews and 28 met inclusion criteria.Overall,1384 patients underwent pancreatic surgery.Mean age was 30 years and 1181 patients(85.3%)were female.The most common clinical presentation was non-specific abdominal pain(52.6%of cases).Mean overall survival was 98.1%.Mean recurrence rate was 2.8%.Mean follow-up was 4.2 years.Conclusions:Pancreatic SPTs are rare,and predominantly affect young women with unclear pathogenesis.Radical resection is the gold standard of treatment achieving good oncological impact and a favorable prognosis in a yearly life-long follow-up.

Intraoperative ultrasonography(IOUS)in hepatobiliary surgery is well known as an essential tool for radical and safe hepatectomies,allowing to perform parenchymal sparing surgery and,as consequence,to expand the surgical indications for patients otherwise considered unresectable.Nevertheless,since many years,despite its effectiveness in the study of vascular anatomy of the liver,the gold standard for the assessment of biliary anatomy during surgery is intraoperative cholangiography(IOC),which is used for the validation of preoperative imaging as well as for guiding reconstruction in case of bile duct injury or resection.

The initial stages of Li_(2)O_(2) formation during oxygen reduction reaction in Li-O_(2) batteries:The significance of Li_(2)O_(2) in charge-transfer reactions within devices

Lithium-oxygen batteries are a promising technology because they can greatly surpass the energy density of lithium-ion batteries.However,this theoretical characteristic has not yet been converted into a real device with high cyclability.Problems with air contamination,metallic lithium reactivity,and complex discharge and charge reactions are the main issues for this technology.A fast and reversible oxygen reduction reaction(ORR)is crucial for good performance of secondary batteries',but the partial knowledge of its mechanisms,especially when devices are concerned,hinders further development.From this perspective,the present work uses operando Raman experiments and electrochemical impedance spectroscopy(EIS)to assess the first stages of the discharge processes in porous carbon electrodes,following their changes cycle by cycle at initial operation.A growth kinetic formation of the discharge product signal(Li_(2)O_(2))was observed with operando Raman,indicating a first-order reaction and enabling an analysis by a microkinetic model.The solution mechanism in the evaluated system was ascribed for an equivalent circuit with three time constants.While the time constant for the anode interface reveals to remain relatively constant after the first discharge,its surface seemed to be more non-uniform.The model indicated that the reaction occurs at the Li_(2)O_(2) surface,decreasing the associated resistance during the initial discharge phase.Furthermore,the growth of Li_(2)O_(2) forms a hetero-phase between Li_(2)O_(2)/electrolyte,while creating a more compact and homogeneous on the Li_(2)O_(2)/cathode surface.The methodology here described thus offers a way of directly probing changes in surface chemistry evolution during cycling from a device through EIS analysis.
