Background:The management of suspected critical congenital heart defects(CCHD)relies on timely echocardiographic *** availability of experienced echocardiographers is limited or even non-existent in many hospitals with obstetric *** study evaluates remote-mentored echocardiography performed by physicians without experience in imaging of congenital heart defects(CHD).Methods:The setup included a pediatric cardiologist in a separate room,guiding a physician without experience in echocardiographic imaging of CHD in the examination of a symptomatic *** remote-mentoring pair was blinded to the diagnosis of the newborn and presented with a simplified patient *** echocardiographic images were streamed to the laptop of the mentor,along with a webcam feed showing the probe *** task was to identify CCHD in need of immediate transfer to a pediatric cardiac surgical *** result was compared to the previously completed echocardiographic report and the clinical decision of the patient-responsible pediatric ***:During 17 months,15 newborns were *** six newborns with CCHD were correctly labeled by the remotementoring *** newborn with Tetralogy of Fallot was erroneously labeled as needing immediate *** newborns without CCHD were correctly ***:remote-mentored echocardiography performed by examiners without experience in imaging CHD identified all newborns with CCHD in need of immediate transfer for specialist *** setup shows promising results for improving the management of CCHD in hospitals without continuous pediatric cardiology service.

Paraneoplastic myopathy-related rhabdomyolysis and pancreatic cancer:A case report and review of the literature

BACKGROUND Rhabdomyolysis is a life-threatening condition,often leading to progressive renal failure and *** is caused by destruction of skeletal muscle and the release of myoglobin and other intracellular contents into the *** most frequent cause of this condition is“crush syndrome”,although several others have been described and paraneoplastic inflammatory myopathies associated with various types of cancer are repeatedly *** SUMMARY We describe a rare case of a patient with pancreatic cancer who developed rhabdomyolysis early on,possibly due to paraneoplastic myositis leading to acute renal failure and eventually to rapid death.A 78-year-old Caucasian woman was referred to our hospital for obstructive jaundice and weight loss due to a lesion in the pancreatic *** presented increasingly severe renal insufficiency with anuria,a dramatic increase in creatine phosphokinase(36000 U/L,n.v.20-180 U/L)and myoglobin(>120000μg/L,n.v.12-70μg/L).On clinical examination,the patient showed increasing pain in the lower limbs associated with muscle weakness which was severe enough to immobilize *** myopathy linked to the malignant lesion of the pancreatic head was *** patient was treated with hemodialysis and intravenous *** all the efforts to prepare the patient for surgery,her general condition rapidly deteriorated and she eventually died 30 d after hospital *** The possible causes of rhabdomyolysis in this patient with pancreatic cancer are discussed,the development of paraneoplastic myopathy being the most *** should bear in mind that these syndromes may become clinically manifest at any stage of the cancer course and their early diagnosis and treatment could improve the patient’s prognosis.

The asthmatic inflammatory process results in the generation of volatile organic compounds(VOCs),which are subsequently secreted by the *** study of these elements through gas chromatography-mass spectrometry(GC-MS),which can identify individual molecules with a discriminatory capacity of over 85%,and electronic-Nose(e-NOSE),which is able to perform a quick onboard pattern-recognition analysis of VOCs,has allowed new prospects for non-invasive analysis of the disease in an"omics"*** this review,we aim to collect and compare the progress made in VOCs analysis using the two methods and their instrumental *** have described the potential of GC-MS and e-NOSE in a multitude of relevant aspects of the disease in both children and adults,as well as differential diagnosis between asthma and other conditions such as wheezing,cystic fibrosis,COPD,allergic rhinitis and last but not least,the accuracy of these methods compared to other diagnostic tools such as lung function,FeNO and eosinophil *** to significant limitations of both methods,it is still necessary to improve and standardize ***,e-NOSE appears to be the most promising aid in clinical practice,whereas GC-MS,as the gold standard for the structural analysis of molecules,remains an essential tool in terms of research for further studies on the pathophysiologic pathways of the asthmatic inflammatory *** conclusion,the study of VOCs through GC-MS and e-NOSE appears to hold promise for the noninvasive diagnosis,assessment,and monitoring of asthma,as well as for further research studies on the disease.

Autoimmune liver diseases and SARS-CoV-2

Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),causing coronavirus disease 2019(COVID-19),can trigger autoimmunity in genetically predisposed individuals through hyperstimulation of immune response and molecular *** we summarise the current knowledge about autoimmune liver diseases(AILDs)and SARS-CoV-2,focusing on:(1)The risk of SARS-CoV-2 infection and the course of COVID-19 in patients affected by AILDs;(2)the role of SARS-CoV-2 in inducing liver damage and triggering AILDs;and(3)the ability of vaccines against SARS-CoV-2 to induce autoimmune responses in the *** derived from the literature suggest that patients with AILDs do not carry an increased risk of SARS-Cov-2 infection but may develop a more severe course of COVID-19 if on treatment with steroids or *** SARSCoV-2 infection can lead to the development of several autoimmune diseases,few reports correlate it to the appearance of de novo manifestation of immunemediated liver diseases such as autoimmune hepatitis(AIH),primary biliary cholangitis(PBC)or AIH/PBC overlap *** case series of an AIHlike syndrome with a good prognosis after SARS-CoV-2 vaccination have been *** the causal link between SARS-CoV-2 vaccines and AIH cannot be definitively established,these reports suggest that this association could be more than coincidental.

The evolution of properties of crosslinked polyethylene(XLPE)-based insulating mate-rials under radio-chemical ageing conditions is presented and *** total,seven different polymer compounds were tested,characterised by an increasing quantity of additives,for example,antioxidants(AOs)and fillers as flame *** were analysed using different testing techniques,that is,Fourier Transform Infra-red and dielectric *** allowed a broad characterisation of the polymer,bringing to good correlations between the arising of new chemical groups caused by ageing and the modifications on the complex ***-based AOs are found to be the most efficient in terms of protection against *** the contrary,flame retardants are concluded not to have any impact on the material *** data were then treated using the principal component *** data treatment successfully distinguished the material composition(with and without additives)and the ageing *** results confirm the relevance of this analysis in understanding the impact of irradiation on the chemical and functional properties of the polymer materials studied.

Propulsion tests on ultra-high-temperature ceramic matrix composites for reusable rocket nozzles

Ultra-high-temperature ceramic matrix composites(UHTCMCs)based on a ZrB_(2)/SiC matrix have been investigated for the fabrication of reusable nozzles for *** de Laval nozzle prototypes,obtained by sintering with either hot pressing(HP)or spark plasma sintering(SPS),were tested 2-3 times in a hybrid rocket motor for proving *** were extracted after oxidation tests to study the microstructural changes and oxidative and thermomechanical stresses induced by the repeated *** to a reference graphite nozzle,no measurable erosion was observed for the UHTCMC-based *** oxidation mechanism consisted in the formation of a ZrO_(2)intermediate layer,with a liquid silicon oxide(SiO_(2))layer on the surface that was displaced by the action of the gas flux towards the divergent part of the nozzle,protecting it from further *** specimens obtained by HP and SPS displayed similar performance,with very slight differences,which were attributed to small changes in *** tests demonstrated the capability of complex-shaped prototypes made of the developed UHTCMCs to survive repeated exposure to environments representative of a realistic space propulsion application,for overall operating time up to 30 s,without any failure nor measurable erosion,making a promising step towards the development of reusable rocket components.

Measurements of Non-reacting and reacting Flow Fields of a Liquid Swirl Flame Burner

The understanding of the liquid fuel spray and flow field characteristics inside a combustor is crucial for designing a fuel efficient and low emission *** of the flow field of a model gas turbine liquid swirl burner is performed by using a2-D particle imaging velocimetry（PIV）*** flow field pattern of an axial flow burner with a fixed swirl intensity is compared under confined and unconfined conditions,i.e.,with and without the combustor *** effect of temperature on the main swirling air flow is investigated under open and non-reacting *** result shows that axial and radial velocities increase as a result of decreased flow density and increased flow *** flow field of the main swirling flow with liquid fuel spray injection is compared to non-spray swirling *** of liquid fuel spray changes the swirl air flow field at the burner outlet,where the radial velocity components increase for both open and confined *** reacting condition,the enclosure generates a corner recirculation zone that intensifies the strength of radial *** reverse flow and corner recirculation zone assists in stabilizing the flame by preheating the *** flow field data can be used as validation target for swirl combustion modelling.

N-Hydroxypipecolic acid(NHP)is a signaling molecule crucial for systemic acquired resistance(SAR),a systemic immune response in plants that provides long-lasting and broad-spectrum protection against secondary pathogen *** identify negative regulators of NHP biosynthesis,we performed a forward genetic screen to search for mutants with elevated expression of the NHP biosynthesis gene FLAVIN-DEPENDENT MONOOXYGENASE 1(FMO1).Analysis of two constitutive expression of FMO1(cef)and one induced expression of FMO1(ief)mutants revealed that the AIPP3–PHD2–CPL2 protein complex,which is involved in the recognition of the histone modification H3K27me3 and transcriptional repression,contributes to the negative regulation of FMO1 expression and NHP *** study suggests that epigenetic regulation plays a crucial role in controlling FMO1 expression and NHP levels in plants.
