Epithelial-mesenchymal, mesenchymal-epithelial, and endothelial-mesenchymal transitions in malignant tumors: An update

Epithelial-to-mesenchymal transition(EMT) represents conversion of an epithelial cell in an elongated cell with mesenchymal phenotype, which can occur in physiologic and pathologic processes such as embryogenesis(type 1 EMT), wound healing and/or fibrosis(type 2 EMT) and malignant tumors(type 3 EMT). The proliferation rate, metastasizing and recurrence capacity, as also the individualized response at chemotherapics, in both epithelial and mesenchymal malignant tumors is known to be influenced by reversible switch between EMT and mesenchymal-to-epithelial transition(MET). Although much research work has already been done in these fields, the specific molecular pathways of EMT, relating to the tumor type and tumor localization, are yet to be elucidated. In this paper, based on the literature and personal experience of the authors, an update in the field of EMT vs MET in epithelial and mesenchymal tumors is presented. The authors tried to present the latest data about the particularities of these processes, and also of the so-called endothelialto-mesenchymal transition, based on tumor location. The EMT-angiogenesis link is discussed as a possible valuable parameter for clinical follow-up and targeted therapeutic oncologic management. The paper begins with presentation of the basic aspects of EMT, its classification and assessment possibilities, and concludes with prognostic and therapeutic perspectives. The particularities of EMT and MET in gastric and colorectal carcinomas, pancreatic cancer, hepatocellular and cholangiocarcinomas, and lung, breast and prostate cancers, respectively in sarcomas and gastrointestinal stromal tumors are presented in detail.

AIM:To find out if by combining 2 ultrasound based elastographic methods:acoustic radiation force impulse(ARFI)elastography and transient elastography(TE),we can improve the prediction of fibrosis in patients with chronic hepatitis ***:Our study included 197 patients with chronic hepatitis *** each patient,we performed,in the same session,liver stiffness(LS)measurements by means of TE and ARFI,respectively,and liver biopsy(LB),assessed according to the Metavir score.10 LS measurements were performed both by TE and ARFI;median values were calculated and expressed in kilopascals(kPa)and meters/second(m/s),*** TE and ARFI measurements with IQR<30%andSR≥60%were considered ***:On LB 13(6.6%)patients had F0,32(16.2%) had F1,52(26.4%)had F2,47(23.9%)had F3,and 53(26.9%)had F4.A direct,strong correlation was found between TE measurements and fibrosis(r=0.741),between ARFI and fibrosis(r=0.730)and also between TE and ARFI(r=0.675).For predicting significant fibrosis(F≥2),for a cutoff of 6.7 kPa,TE had 77.5% sensitivity(Se)and 86.5%specificity(Sp)[area under the receiver operating characteristic curve(AUROC)0.87] and for a cutoff of 1.2 m/s,ARFI had 76.9%Se and 86.7%Sp(AUROC 0.84).For predicting cirrhosis(F=4),for a cutoff of 12.2 kPa,TE had 96.2%Se and 89.6% Sp(AUROC 0.97)and for a cutoff of 1.8 m/s,ARFI had 90.4%Se and 85.6%Sp(AUROC 0.91).When both elastographic methods were taken into consideration,for predicting significant fibrosis(F≥2),(TE≥6.7 kPa and ARFI≥1.2 m/s)we obtained 60.5%Se,93.3% Sp,96.8%positive predictive value(PPV),41.4%negative predictive value(NPV)and 68%accuracy,while for predicting cirrhosis(TE≥12.2 kPa and ARFI≥1.8 m/s) we obtained 84.9%Se,94.4%Sp,84.9%PPV,94.4% NPV and 91.8%***:TE used in combination with ARFI is highly specific for predicting significant fibrosis;therefore when the two methods are concordant,liver biopsy can be avoided.

Interactions between hyaluronic acid and CoCrMo alloy surface in simulated synovial fluids

The performance of CoCrMo alloy in orthopaedic implants may be unfavourably affected by hyaluronic acid(HA)in synovial *** this study,the authors aimed to understand the interactions between HA and CoCrMo using dedicated electrochemical experiments and surface analyses.A sequence of electrochemical measurements(open-circuit potential,linear polarization resistance,potentiodynamic and potentiostatic polarizations)was run on LC-CoCrMo(ASTM F1537)in Dulbecco's phosphate-buffered saline(DPBS)solution with and without HA and in DPBS mixed with newborn calf serum(NCS)and HA,partially under simultaneous recording of surface pH using custom-made *** were analysed by optical and elec-tron *** had no significant impact on the corrosion potential of CoCrMo alloy(ECORR=−173�8,−211�16,and−254�30 V_(Ag/AgCl),in DPBS,DPBS+HA,and DPBS+NCS+HA,respectively).Average current density values at the transpassive domain were double in DPBS compared to DPBS+HA and DPBS+NCS+*** potentials above+0.6 V_(Ag/AgCl),surface pH values decreased from 7.5 to 6.5 in DPBS and from 7.5 to below 4 in DPBS+*** conclusion,the presence of HA did not compromise the corrosion resistance of CoCrMo alloy at free potential,but it enhanced acidic conditions at the near surface under anodic-applied potential in the transpassive domain.

AIM：To apply the multifractal analysis method as a quantitative approach to a comprehensive description of the microvascular network architecture of the normal human ***：Fifty volunteers were enrolled in this study in the Ophthalmological Clinic of Cluj-Napoca,Romania,between January 2012 and January 2014. A set of 100 segmented and skeletonised human retinal images,corresponding to normal states of the retina were studied. An automatic unsupervised method for retinal vessel segmentation was applied before multifractal analysis. The multifractal analysis of digital retinal images was made with computer algorithms,applying the standard boxcounting method. Statistical analyses were performed using the Graph Pad In Stat ***：The architecture of normal human retinal microvascular network was able to be described using the multifractal geometry. The average of generalized dimensions（D_q）for q=0,1,2,the width of the multifractal spectrum（Δα=α_（max）-α_（min））and the spectrum arms＇ heights difference（│Δf│）of the normal images were expressed as mean±standard deviation（SD）：for segmented versions,D_0=1.7014±0.0057; D_1=1.6507±0.0058; D_2=1.5772±0.0059; Δα=0.92441±0.0085; │Δf│= 0.1453±0.0051; for skeletonised versions,D_0=1.6303±0.0051; D_1=1.6012±0.0059; D_2=1.5531± 0.0058; Δα=0.65032±0.0162; │Δf│= 0.0238±0.0161. The average of generalized dimensions（D_q）for q=0,1,2,the width of the multifractal spectrum（Δα）and the spectrum arms＇ heights difference（│Δf│）of the segmented versions was slightly greater than the skeletonised ***：The multifractal analysis of fundus photographs may be used as a quantitative parameter for the evaluation of the complex three-dimensional structure of the retinal microvasculature as a potential marker for early detection of topological changes associated with retinal diseases.

Evaluation of the potential exposure of butterflies to genetically modified maize pollen in protected areas in Italy

Environmental impacts of genetically modified crops are mandatorily assessed during their premarket phase. One of the areas of concern is the possible impact on nontar- get organisms. Crops expressing Cry toxins might affect Lepidoptera larvae living outside cultivated fields, through pollen deposition on wild plants, which constitute their food source. While pollen toxicity varies among different events, possible exposure ofnontarget species depends on the agro-environmental conditions. This study was conducted in two protected areas in Italy, characterized by different climatic conditions, where many Lepi- doptera species thrive in proximity to maize cultivations. To estimate the possible exposure in absence of the actual stressor （e.g., Cryl-expressing maize plants）, we conducted a two-year field survey of butterflies and weeds. Indicator species were selected--Aglais （Inachis） io in the Northern site and Vanessa cardui in the Southern site--and their phe- nology was investigated. Pollen dispersal from maize fields was measured by collection in Petri dishes. Duration and frequency of exposure was defined by the overlap between pollen emission and presence of larvae on host plants. Different risk scenarios are expected in the two regions： highest exposure is foreseen forA. io in the Northern site, while minimal exposure is estimated for V. cardui in the Southern site. In the latter case, locally grown maize cultivars flower in mid-summer in coincidence with an aestivation period for several butterfly species due to hot and dry conditions. Moreover, host plants of V. cardui are at the end of their life cycle thus limiting food availability.

Cystic jejunal duplication with Heinrich's type Ⅰ ectopic pancreas, incidentally discovered in a patient with pancreatic tail neoplasm

The aim of this study was to present a case of enteric duplication cyst and criteria for a proper differential diagnosis. A 51-year-old male was hospitalized for pancreatic tail neoplasm and distal pancreatectomy with splenectomy was performed. During surgery, a jejunal cystic lesion was incidentally detected and jejunectomy was performed. Microscopically, the cyst was observed to be covered by Keratin 7/Keratin 20 positive intestinal type epithelium and the muscularis layer was shared by the cyst and adjacent jejunum, without a cleavage plane between the cyst wall and jejunal muscularis propria. In the deep muscularis propria, a Heinrich's type Ⅰ ectopic pancreas was also noted. In the pancreatic tail, a low grade intraepithelial lesion(panIN-1a) was diagnosed. This case highlights the necessity for a correct differential diagnosis of such rare lesions. Roughly 30 cases of jejunal duplication cysts have been reported to date in the PubM ed database.
