Histopathological differences utilizing the nonalcoholic fatty liver disease activity score criteria in diabetic(type 2 diabetes mellitus) and non-diabetic patients with nonalcoholic fatty liver disease

AIM: To study clinical and histopathological features of nonalcoholic fatty liver disease(NAFLD) in patients with and without type 2 diabetes mellitus(T2DM) using updated nonalcoholic steatohepatitis clinical research network(NASH-CRN) grading ***: We retrospectively analyzed data of 235 patients with biopsy proven NAFLD with and without T2 *** database was utilized in the previously published study comparing ethnicity outcomes in NAFLD by the same corresponding *** pathology database from University of Chicago was utilized for enrolling consecutive patients who met the criteria for NAFLD and their detailed clinical and histopathology findings were obtained for *** relevant clinical profile of patients was collected from the Electronic Medical Records around the time of liver biopsy and the histology was read by a single well-trained *** updated criteria for type 2 diabetes have been utilized for *** data of patients with NASH and NAFLD has been *** mean differences were compared using χ2 and t-test along with regression analysis to evaluate the predictors of NASH and advanced ***: Patients with NAFLD and T2 DM were significantly older(49.9 vs 43.0,P < 0.01),predominantly female(71.4 vs 56.3,P < 0.02),had higher rate of metabolic syndrome(88.7 vs 36.4,P < 0.01),had significantly higher aspartate transaminase(AST)/alanine transaminase(ALT) ratio(0.94 vs 0.78,P < 0.01) and Fib-4 index(1.65 vs 1.06,P < 0.01) as markers of NASH,showed higher mean NAFLD activity score(3.5 vs 3.0,P = 0.03) and higher mean fibrosis score(1.2 vs 0.52,P < 0.01) compared to patients with NAFLD without T2 ***,advanced fibrosis(32.5 vs 12.0,P < 0.01) and ballooning(27.3 vs 13.3,P < 0.01) was significantly higher among patients with NAFLD and T2 DM compared to patients with NAFLD without T2 *** multivariate analysis,T2 DM was independently associated with NASH(OR = 3.27,95%CI: 1.43-7.50,P < 0.01)

Immune monitoring post liver transplant

Many of the causes of short and late morbidity following liver transplantation are associated with immunosuppression or immunosuppressive medications. Current care often involves close monitoring of liver biochemistry as well as therapeutic drug levels. However, the postoperative course following liver transplantation can often be associated with significant complications including infection and rejection, suggesting an inadequacy in current immune function monitoring. Many assays have been tested in the research setting to identify possible biomarkers that may be used to predict clinical events such as acute cellular rejection, and therefore allow modification of a patient's immunosuppressive regimen prior to a clinical event. However, these generally require significant laboratory processing and have had difficulty becoming established in common clinical use outside the research setting. One assay, Cylex ImmuK now has been food and drug administration approved but has had variable results. In this review we discuss the assays that have been used to assess monitoring of immune function after liver transplantation and consider possible future directions.

PIK3CA基因激活突变所导致的一系列发育障碍表型谱被统称为PIK3CA基因相关过度生长综合征群(PIK3CA-related overgrowth spectrum,PROS)。临床表现分型众多,各型分类见表1A。由于基因变异发生在受精卵形成后,临床表型为嵌合体,因此PIK3CA基因变异在外周血中的阳性检出率很低,通常需要非血液标本或通过组织活检来发现超低频嵌合体。PIK3CA基因激活变异的作用为促生长,变异导致的表型取决于基因变异发生的时间、组织细胞类型、变异细胞的分布以及变异的激活强度。

In addition to cholesterol reduction,statins,currently the most commonly prescribed drug in the world,have been shown to have anti-neoplastic and immunomodulatory *** observational studies and meta-analyses have shown reduction in risk of multiple *** recently there has been an increasing interest in the potential role of statins as adjuvant therapy after cancer diagnosis and in modifying cancer *** post-hoc analyses of randomized controlled trials of statins for cardiovascular outcomes have not shown reduction in the risk of cancer mortality with statin use,these studies lack sufficient power to detect a significant difference in cancer ***,in a Danish nationwide population-based cohort study,Nielsen et al showed a 15%reduction in all-cause and cancer-specific mortality in statin users as compared to *** survival with statin exposure was seen in 13/27 cancer subtypes,including the 4 most common cancers-lung,prostate,colorectal and *** this commentary,we examine this important study,review its implications and limitations,and briefly discuss impact of other drugs like metformin and aspirin that also exhibit anti-neoplastic effects.

Molecular pathogenesis of glioblastoma multiforme: Nuances, obstacles, and implications for treatment

Glioblastoma multiforme(GBM), the literal apogee on the hierarchy of malignant brain tumors, remains one of the greatest therapeutic challenges in oncology andmedicine. Historically this may be contextualized in the fact that the medical and scientific communities have had a very elementary understanding of its intricate and complex pathophysiology. The last 10-15 years have yielded a number of studies that have elucidated much of the molecular and genetic complexities of GBM that underlie its pathogenesis. Excitingly, some of these discovered genetic mutations and molecular profiles in GBM have demonstrated value in prognostication and utility in predicting response to treatment. Despite this, however, treatment options for patients have remained somewhat limited. These treatment options are expected to expand with the availability of new data and with the transition of novel treatment modalities from animal to human studies. This paper will have a threefold objective: provide an overview of the traditional paradigm in understanding and treating GBM, describe recent discoveries in the molecular pathogenesis of GBM against this historical backdrop, and acquaint the reader with new treatment modalities that hold significant therapeutic potential for patients.

AIM To identify gaps in the existing knowledge onsingle, dual and triple infections of human immunodeficiencyvirus （HIV）, hepatitis B virus （HBV） andhepatitis C virus （HCV） in the Middle East and NorthAfrica （MENA） region among men who have sex withmen （MSMs）, female sex workers （FSWs）, injectingdrug users （IDUs） and ***： We performed an extensive literaturesearch on articles published on the topic in the 25countries of the MENA region. PubMed database wasused as the main search engine. Case reports, caseseries, qualitative studies, editorials, commentaries,authors＇ replies and animal studies were *** articles and reviews dealing with the prevalenceof HIV, HBV and HCV and their co-infection wereincluded. Data on population type, sample size, age andmarkers of infections were extracted from the ***： HIV, HBV and HCV are blood-borne viruseswith similar modes of transmission. The categories ofpeople at high risk of acquiring HIV-1, HBV and HCVcommonly include： MSMs, FSW and IDUs. It is wellestablished that HIV-positive individuals co-infectedwith HBV or HCV suffer from liver pathology associatedwith morbidity and mortality. Moreover, HIV-infectedindividuals do not respond well to treatment for HBV orHCV and hence are at increased risk of hepatic ***, co-infection of HIV-positive individualswith HBV and/or HCV is a global health problem of significant magnitude. Our review reveals the paucityof epidemiological data for key populations in manycountries of the region. Limited number of studiesexists in the MENA region on the status of HIV, HBVand HCV and their co-infections among prisoners, MSMsand FSWs. Evidence support the continued increaseof the HIV epidemic among MSMs. In addition to thelack of studies on MSMs and FSWs in the MENA region,our review highlights the lack of data on the practices,characteristics, or the status of HIV infection and viralhepatitis among male sex workers selling or exchanging

BACKGROUND Management of single small hepatocellular carcinoma(HCC)is straightforward with curative outcomes achieved by locoregional therapy or *** transplantation is often considered for multiple small or single large *** of two small HCC whether presenting synchronously or sequentially is less *** To define the outcomes of patients presenting with two small *** Retrospective review of HCC databases from multiple institutions of patients with either two synchronous or sequential HCC≤3 cm between January 2000 and March *** outcomes were overall survival(OS)and transplant-free survival(TFS).RESULTS 104 patients were identified(male n=89).Median age was 63 years(interquartile range 58-67.75)and the most common aetiology of liver disease was hepatitis C(40.4%).59(56.7%)had synchronous HCC and 45(43.3%)had sequential.36 patients died(34.6%)and 25 were transplanted(24.0%).1,3 and 5-year OS was 93.0%,66.1% and 62.3% and 5-year post-transplant survival was 95.8%.1,3 and 5-year TFS was 82.1%,45.85% and 37.8%.When synchronous and sequential groups were compared,OS(1,3 and 5 year synchronous 91.3%,63.8%,61.1%,sequential 95.3%,69.5%,64.6%,P=0.41)was similar but TFS was higher in the sequential group(1,3 and 5 year synchronous 68.5%,37.3% and 29.7%,sequential 93.2%,56.6%,48.5%,P=0.02)though this difference did not remain during multivariate *** TFS in patients presenting with two HCC≤3 cm is poor regardless of the timing of the second *** patients presenting with two small HCC should be considered for transplantation.
