Uhwang Chungsim Won Decreases Blood Oxygen Level-Dependent fMRI signal Response to A Motor Stimulation Task

Objectives： TO evaluate the effect of Uhwang Chungsim Won [UC, Niuhuang Qingxin Pill （牛黄清心丸] on the blood oxygen level-dependent （BOLD） functional magnetic resonance imaging （fMRI） signal response to a motor stimulation task using a hand grasping paradigm. Methods： Ten healthy right-handed male volunteers were recruited. Their BOLD fMRI signal characteristics during hand grasping were determined before and 1 h after consuming UC. Results： With the administration of UC, BOLD responses to the right hand motor task showed a decrease in both hemispheres including the right cerebellar culmen, right and left medial frontal gyrus, right parahippocampal gyrus, right and left sublobar insula, right and left posterior cingulate, left precuneus, and right precentral gyrus. Decreased BOLD responses to the left hand motor task were shown in the right lingual gyrus, right cerebellar culmen, left superior temporal gyrus, right and left middle frontal gyrus, and right precentral gyrus after UC administration. Increased BOLD responses to the left hand motor task were shown in the left and right posterior cingulate, while there was no increased response to the right hand motor task after UC administration. Conclusions： UC administration significantly decreased fMRI BOLD signal responses to hand grasping in most of the brain areas including the primary motor cortex, but it is not clear whether these decreased responses were caused by a decrease in neuronal activities or by an increase in cerebral blood flow due to the cerebral vasodilatory effects of UC. Further studies using positron emission tomography or single photon emission computed tomography are needed to fully elucidate these findings.

Brain Activity Associated with Memory and Cognitive Function During Jaw-Tapping Movement in Healthy Subjects Using Functional Magnetic Resonance Imaging*

Objective： TO determine whether jaw-tapping movement, a classically described as an indication of personal well-being and mental health, stimulates the memory and the cognitive regions of the brain and is associated with improved brain performance. Methods： Twelve healthy right-handed female subjects completed the study. Each patient performed a jaw-tapping task and an n-back task during functional magnetic resonance imaging （fMRI）. The subjects were trained to carry out the jaw-tapping movement at home twice a day for 4 weeks. The fMRI was repeated when they returned. Results： During the first and second jaw-tapping session, both sides of precentral gyrus and the right middle frontal gyrus （BA 6） were activated. And during the second session of the jaw-tapping task, parts of frontal lobe and temporal lobe related to memory function were more activated. In addition, the total percent task accuracy in n-beck task significantly increased after 4 weeks of jaw- tapping movement. After jaw-tapping training for 4 weeks, brain areas related to memory showed significantly increased blood oxygen level dependent signals. Conclusion： Jaw-tapping movement might be a useful exercise for stimulating the memory and cognitive regions of the brain.

Comparison of efficacy and safety between standard-dose and modified-dose FOLFIRINOX as a first-line treatment of pancreatic cancer

AIM To directly compare the efficacy and toxicity of standarddose FOLFIRINOX(sFOLFIRINOX) and modified-dose FOLFIRINOX(mFOLFIRINOX, 75% of standard-dose) for pancreatic *** One hundred and thirty pancreatic cancer patients who received sFOLFIRINOX(n = 88) or mFOLFIRINOX(n = 42) as their first-line chemotherapy from January 2013 to July 2017 were retrospectively reviewed. For efficacy analysis, the objective response rate(ORR),disease control rate(DCR), progression-free survival(PFS), and overall survival(OS) were evaluated and compared using Pearson's chi-square test, Kaplan-Meier plot and log-rank test. The adverse events(AEs) were evaluated, and severe(≥ grade 3) AEs rates of the two groups were compared for toxicity *** The mFOLFIRINOX group included more female patients(30.7% vs 57.1%; P = 0.004) and older patients [age(median), 57 vs 63.5; P = 0.018] than the sFOLFIRINOX group. In the efficacy analysis, the ORR and DCR were not significantly different between the two groups(ORR: 39.8% vs 35.7%; P = 0.656; DCR: 80.7% vs 83.3%; P = 0.716). The median PFS and OS were also not different between the groups(PFS: 8.7 mo vs 8.1 mo, P = 0.272; OS: 13.9 mo vs 13.7 mo, P = 0.476). In the safety analysis with severe AEs, the rates of neutropenia(83.0% vs 66.7%; P = 0.044), anorexia(48.9% vs 28.6%; P = 0.029) and diarrhea(13.6% vs 0.0%; P = 0.009) were markedly lower in the mFOLFIRINOX *** m FOLFIRINOX showed comparable efficacy but better safety compared to sFOLFIRINOX. If clinically necessary, initiating FOLFIRINOX with 75% of the standard-dose can alleviate toxicity concerns without compromising efficacy.

Paeonol attenuates inflammation-mediated neurotoxicity and microglial activation

Chronic activation of microglial cells endangers neuronal survival through the release of various proinflammatory and neurotoxic factors. The root of Paeonia lactiflora Pall has been considered useful for the treatment of various disorders in traditional oriental medicine. Paeonol, found in the root of Paeonia lactiflora Pall, has a wide range of pharmacological functions, including anti-oxidative, anti-inflammatory and neuroprotective activities. The objective of this study was to examine the efficacy of paeonol in the repression of inflammation-induced neurotoxicity and microglial cell activation. Organotypic hippocampal slice cultures and primary microglial cells from rat brain were stimulated with bacterial lipopolysaccharide. Paeonol pretreatment was performed for 30 minutes prior to lipopolysaccharide addition. Cell viability and nitrite （the production of nitric oxide）, tumor necrosis factor-alpha and interleukin-lbeta products were measured after lipopolysaccharide treatment. In organotypic hippocampal slice cultures, paeonol blocked lipopolysaccharide-related hippocampal cell death and inhibited the release of nitrite and interleukin-lbeta. Paeonol was effective in inhibiting nitric oxide release from primary microglial cells. It also reduced the lipopolysaccharide-stimulated release of tumor necrosis factor-alpha and intefleukin-1β from microglial cells. Paeonol possesses neuroprotective activity in a model of inflammation-induced neurotoxicity and reduces the release of neurotoxic and proinflammatory factors in activated microglial cells.

A Comparison of Brain Activity between Healthy Subjects and Stroke Patients on fMRI by Acupuncture Stimulation

Objective： To investigate brain activity patterns during acupuncture in stroke patients, and to compare the result with normal subjects using functional magnetic resonance imaging （fMRI）. Methods： A total of 11 stroke patients with motor weakness and 10 healthy subjects were studied, fMRI was performed during acupuncture on the left side at points Quchi （Llll） and Zusanli （ST36）. Data were analyzed using statistical parametric maps of brain activation induced by acupuncture stimulation. Results： The results showed that stimulation of both LI11 and ST36 produced significantly different brain activation patterns between the two groups. The normal group showed a greater overall activation than the stroke group. In the normal group, parts of the frontal lobe, parietal lobe, sub-lobar, cerebellum and midbrain regions were activated by acupuncture at the left LI11. On the other hand, only the right side of the inferior parietal Iobule region was activated in the stroke patients. When the left ST36 was stimulated in the normal group, both sides of the frontal lobe, parietal lobe, temporal lobe, and sub-lobar, and the left side of occipital lobe, and the right side of cerebellum and midbrain regions were activated. For the same stimulation in the stroke group, only both sides of the inferior parietal Iobule and cerebellum regions were activated （P〈0.05, cluster level）. Deactivation pattern was not noted during any acupuncture stimulation in both groups. Conclusion： Brain signal activations during the same acupuncture were different between the healthy and the stroke patients, and the effects showed a correlation of different acupuncture points.

Role of oxygen free radicals in patients with acute pancreatitis

AIM:The generation of oxygen free radicals has been implicated in the pathogenesis of experimental pancreatitis. The aim of this study was to determine the role of oxygen free radicals in patients with acute pancreatitis. METHODS:The plasma levels of C-reactive protein(CRP), lipid peroxide(LPO),myeloperoxidase(MPO)and superoxide dismutase(SOD)were measured in 13 patients with acute pancreatitis and 14 healthy volunteers. RESULTS:Among the patients with acute pancreatitis,there were higher plasma levels of LPO and MPO and lower SOD activity in patients with severe pancreatitis than in those with mild ***,there was no significant difference in the serum marker of oxidative stress no matter what the etiology *** LPO level was especially correlated with the concentration of serum CRP and CT severity index. CONCLUsiON:The oxygen free radicals may be closely associated with inflammatory process and the severity of acute ***,the concentration of plasma LPO is a meaningful index for determining the severity of the disease.

Lithium-sulfur batteries(LSBs)have emerged as promising power sources for high-performance devices such as electric ***,the poor energy density of LSBs owing to polysulfide shuttling and passivation has limited their further market *** mitigate this challenge,two-dimensional(2D)siloxene(2Dsi),a si-based analog of graphene,is utilized as an additive for sulfur *** 2Dsi is fabricated on a large scale by simple solvent extraction of calcium disilicide to form a thin-layered structure of si planes functionalized with vertically aligned hydroxyl groups in the *** stoichiometric reaction of 2Dsi with polysulfides generates a thiosulfate redox mediator,secures the intercalation pathway,and reveals Lewis acidic sites within the siloxene *** 2Dsi utilizes the corresponding in-situ-formed electrocatalyst,the 2D confinement effect of the layered structure,and the surface affinity based on Lewis acid-base interaction to improve the energy density of 2Dsi-based LSB *** with the commercial carbon-based current collector,2Dsi-based LSB cells achieve a volumetric energy density of 612 Wh Lcell^(−1) at 1 mA cm^(−2) with minor degradation of 0.17%per cycle,which rivals those of state-of-the-art *** study presents a method for the industrial production of high-energy-dense LSBs.

Severe thrombocytopenia before liver transplantation is associated with delayed recovery of thrombocytopenia regardless of donor type

AIM： To compare the recovery of thrombocytopenia and splenomegaly during long-term follow-up after liver transplantation in patients receiving a living donor transplant or a cadaveric donor transplant. METHODS： This was a retrospective cohort study of 216 consecutive liver transplant patients who survived for 〉 6 mo after transplantation; 169 received a liver transplant from a living donor and 47 from a cadaveric donor. The platelet counts or spleen volumes were examined before transplant, i, 6, and 12 mo after transplant, and then annually until 5 years after transplant. RESULTS： The mean follow-up period was 49 mo （range, 21-66）. Platelet counts increased continuously for 5 years after orthotopic liver transplant. The restoration of platelet counts after transplant was significantly slower in patients with severe pretransplant thrombocytopenia （〈 50000/μL） until 4 years after transplant （P = 0.005）. Donor type did not significantlyaffect the recovery of platelet count and spleen volume in either patient group. In multivariate analysis, pretransplant severe thrombocytopenia （〈 50000/μL） was an independent factor associated with sustained thrombocytopenia （P 〈 0.001, odds ratio 6.314; confidence interval, 2.828-14.095）. Thrombocytopenia reappeared after transplant in seven patients with portal flow disturbance near the anastomosis site. CONCLUsiON： Our study suggests that severe thrombocytopenia before transplant is closely associated with delayed recovery of platelet count after transplant and donor type did not affect the recovery of thrombocytopenia. The reappearance of thrombocytopenia after transplant should be considered a possible indicator of flow disturbance in the portal vein.

Comparison of litterfall production in three forest types in Jeju Island, South Korea

Litterfall, which is influenced by physical and biological factors, is a major pathway for carbon and nutrient cycling in forest ecosystems. The purpose of this study was to investigate monthly litterfall production in three forests in Jeju Island differentiated by forest composition and precipitation: Cheongsu(Quercus glauca as the dominant species;low precipitation), Seonheulb(Q. glauca as the dominant species;high precipitation), and Seonheulm(Q. glauca and Pinus thunbergii as the dominant species;high precipitation). Litterfall was collected monthly from April to December 2015 and divided into leaf litter, twig, bark, seeds, and unidentified *** patterns of litterfall production varied across stands according to their species composition. However,the amount of leaf litterfall and total litterfall were comparable among stands, ranging from 362 to 375 g m-2 for leaf litter and 524 g m-2 to 580 g m-2 for total *** leaf litter in May was the highest in all stands, while needle litter was the highest in December in *** twig litterfall in July may be attributable to high rainfall with strong winds and storms during the rainy season. Although forest type and climate factor had no influence on litterfall amounts in this study, the pattern of litterfall production was species dependent, suggesting diverse effects on carbon and nutrient cycling in these forests.
