Enteral metallic stenting by balloon enteroscopy for obstruction of surgically reconstructed intestine

We present three cases of self-expandable metallic stent(SEMS) placement using a balloon enteroscope(BE) and its overtube(OT) for malignant obstruction of surgically reconstructed intestine. A BE is effective for the insertion of an endoscope into the deep bowel. However, SEMS placement is impossible through the working channel, because the working channel of BE is too small and too long for the stent device. Therefore, we used a technique in which the BE is inserted as far as the stenotic area; thereafter, the BE is removed, leaving only the OT, and then the stent is placed by inserting the stent device through the OT. In the present three cases, a modification of this technique resulted in the successful placement of the SEMS for obstruction of surgically reconstructed intestine, and the procedures were performed without serious complications. We consider that the present procedure is extremely effective as a palliative treatment for distal bowel stenosis, such as in the surgically reconstructed intestine.

Gastric cancer(GC)is one of the most common malignancies and remains the second leading cause of cancer-related death worldwide.There is an increasing understanding of the roles that genetic and epigenetic alterations play in GCs.Recent studies using nextgeneration sequencing(NGS)have revealed a number of potential cancer-driving genes in GC.Whole-exome sequencing of GC has identified recurrent somatic mutations in the chromatin remodeling gene ARID1A and alterations in the cell adhesion gene FAT4,a member of the cadherin gene family.Mutations in chromatin remodeling genes(ARID1A,MLL3 and MLL)have been found in 47%of GCs.Whole-genome sequencing and whole-transcriptome sequencing analyses have also discovered novel alterations in GC.Recent studies of cancer epigenetics have revealed widespread alterations in genes involved in the epigenetic machinery,such as DNA methylation,histone modifications,nucleosome positioning,noncoding RNAs and microRNAs.Recent advances in molecular research on GC have resulted in the introduction of new diagnostic and therapeutic strategies into clinical settings.The antihuman epidermal growth receptor 2(HER2)antibody trastuzumab has led to an era of personalized therapy in GC.In addition,ramucirumab,a monoclonal antibody targeting vascular endothelial growth factor receptor(VEGFR)-2,is the first biological treatment that showed survival benefits as a single-agent therapy in patients with advanced GC who progressed after firstline chemotherapy.Using NGS to systematically identify gene alterations in GC is a promising approach with remarkable potential for investigating the pathogenesis of GC and identifying novel therapeutic targets,as well as useful biomarkers.In this review,we will summarize the recent advances in the understanding of the molecular pathogenesis of GC,focusing on the potential use of these genetic and epigenetic alterations as diagnostic biomarkers and novel therapeutic targets.

Successful treatment with laparoscopic surgery and sequential multikinase inhibitor therapy for hepatocellular carcinoma:A case report

BACKGROUND Hepatocellular carcinoma(HCC)with massive portal vein tumor thrombosis(PVTT)and distant metastasis is considered unresectable.However,due to recent developments in systemic chemotherapy,successful cases of conversion therapy for unresectable diseases have been reported.Herein,we report a successful multidisciplinary approach for treatment of multi-visceral recurrence with sequential multikinase inhibitor and laparoscopic surgery.CASE SUMMARY A 63-year-old woman with chronic hepatitis B virus infection was diagnosed with HCC.Subsequently,she underwent two rounds of laparoscopic partial hepatectomy,laparoscopic left adrenalectomy,and transcatheter arterial chemoembolization plus sorafenib for recurrence.Four years after initial hepatectomy,she presented with a 43-mm mass in the spleen and tumor thrombus involving the main portal vein trunk with ascites.Her liver function was Child-Pugh B(8),and protein induced by vitamin K absence or antagonist II(PIVKA II)levels were elevated up to 46.291 mAU/mL.Since initial treatment with regorafenib for three months was unsuccessful,the patient was administered lenvatinib.Ten months post-treatment,there was no contrast enhancement of PVTT or splenic metastasis.Chemotherapy was discontinued due to severe diarrhea.Afterward,splenic metastasis became viable,and PIVKA II increased.Therefore,hand-assisted laparoscopic splenectomy was performed.She experienced no clinical recurrence 14 mo after resection.CONCLUSION Conversion surgery after successful multikinase inhibitor treatment might be considered an effective treatment option for advanced HCC.

A burnup calculation was performed to analyze the Apr`es ORIENT process, which aims to create highlyvaluable elements from fission products separated from spent nuclear fuels. The basic idea is to use nuclear transmutation induced by a neutron capture reaction followed by a β-decay, thus changing the atomic number Z of a target element in fission products by 1 unit. LWR(PWR) and FBR(MONJU) were considered as the transmutation devices. High rates of creation were obtained in some cases of platinum group metals(44Ru by FBR,46 Pd by LWR) and rare earth(64Gd by LWR,66 Dy by FBR). Therefore, systems based on LWR and FBR have their own advantages depending on target elements. Furthermore, it was found that creation rates of even Z(= Z + 1) elements from odd Z ones were higher than the opposite cases. This creation rate of an element was interpreted in terms of "average 1-group neutron capture cross section of the corresponding target element σc Z defined in this work. General trends of the creation rate of an even(odd) Z element from the corresponding odd(even) Z one were found to be proportional to the 0.78th(0.63th) power of σc Z, however with noticeable dispersion. The difference in the powers in the above analysis was explained by the difference in the number of stable isotopes caused by the even-odd effect of Z.

A resourceability on nuclear fuel cycle by transmutation of fission products in the spent fuel of nuclear reactors is discussed in this paper to investigate the feasibility of "creation and utilization" of Après ORIENT from Adv.-ORIENT cycle,in which chemical "separation and utilization" of nuclear rare metals(platinum group metals,Mo,Tc,rare earth,etc.) has been proposed since FY2006.Après ORIENT research program was newly initiated in FY2011 for nuclear transmutation of fission products into stable or short-lived highly-valuable elements.In the resourceability of rare earth metals from fission products,non-radioactive Nd and Dy can be created from Pr and Tb,respectively,by transmutation.Especially,the Dy creation has a relatively high feasibility of about 10-20 %/y in creation rate.A proper moderation of neutrons in blanket of fast reactors may be required to provide a high creation rate of La from Ba.In light platinum group metals,non-radioactive Ru can be created from Tc by transmutation,of which creation rate is about 4-5 %/y in blanket of fast reactors.Pd created from Rh is almost non-radioactively depending on the isotope fraction of 107 Pd.Rh creation from Ru is not feasible under the neutron irradiation of typical nuclear reactors.

The vacuolar (H+)-ATPases (V-ATPases) are ATP-driven proton pumps with multiple functions in many organisms. In this study, we performed structural and functional analysis of vha55 gene that encodes V-ATPase subunit B in the smokybrown cockroach Periplaneta fuliginosa (Blattodea). We observed a high homology score of the deduced amino acid sequences between 10 species in seven orders. RNAi of the vha55 gene in R fuliginosa caused nymphal/nymphal molting defects with incomplete shedding of old cuticles, growth inhibition, as well as bent and wrinkled cuticles of thoraxes and abdominal segments. Since growth inhibition caused by vha55 RNAi did not interfere in the commencement of cockroach molting, molting timing and body growth might be controlled by independent mechanism. Our study suggested V-ATPases might be a good candidate molecule for evolutionary and developmental studies of insect molting.

Obfuscation is rampant in both benign and malicious JavaScript(JS)codes.It generates an obscure and undetectable code that hinders comprehension and analysis.Therefore,accurate detection of JS codes that masquerade as innocuous scripts is vital.The existing deobfuscation methods assume that a specific tool can recover an original JS code entirely.For a multi-layer obfuscation,general tools realize a formatted JS code,but some sections remain encoded.For the detection of such codes,this study performs Deobfuscation,Unpacking,and Decoding(DUD-preprocessing)by function redefinition using a Virtual Machine(VM),a JS code editor,and a python int_to_str()function to facilitate feature learning by the FastText model.The learned feature vectors are passed to a classifier model that judges the maliciousness of a JS code.In performance evaluation,the authors use the Hynek Petrak’s dataset for obfuscated malicious JS codes and the SRILAB dataset and the Majestic Million service top 10,000 websites for obfuscated benign JS codes.They then compare the performance to other models on the detection of DUD-preprocessed obfuscated malicious JS codes.Their experimental results show that the proposed approach enhances feature learning and provides improved accuracy in the detection of obfuscated malicious JS codes.

The three-dimensional visualization of flow and screech-tone emission from an underexpanded circular jet is first investigated experimentally using high-speed cross-schlieren imaging and microphone measurements in the cross-sectional planes along the jet axis. This experimental technique allows the visualization of the shock-cell structure, directivity of sound intensity, and frequency spectrum in the cross-sectional planes of the screeching jet. The high-speed cross-schlieren observation of the screeching jet shows the occurrence of an asymmetrical shock-cell structure that is generated by the flapping mode in the screeching jet. This contributes to the generation of a screech tone propagating upstream along the jet axis and non-uniform sound intensities around the jet in circumferential direction. These observations by high-speed cross-schlieren imaging were validated by the microphone measurements.

We report a method for laser-assisted electrophoretic deposition using a Laguerre-Gaussian beam for maskless patterning of metal rings and tubes.These are structures of utmost importance,particularly in photonic devices.Metal nanoparticles,which are gathered in a colloidal solution by a laser trapping technique,are deposited on a substrate via electrophoresis.The deposition pattern is dependent on the focused spot shape during laser trapping.The intensity distribution of the Laguerre-Gaussian beam is ring-shaped.Rings with different inner diameters can be fabricated by varying the topological charge of the Laguerre-Gaussian beam.The equivalent inner diameters of the deposited rings with topological charges of 1,3,and 5 were 0.30,0.78,and 1.45μm,respectively.The inner diameter of the deposited ring with a topological charge of 1 was smaller than the wavelength of the laser beam(532 nm).A tube was also fabricated with a topological charge of 3 by vertical displacement of the deposition cell.The deposition technology developed here using Laguerre-Gaussian beam will contribute to advancements in the fabrication of photonic and nano-fluidic devices.
