Protective effects of glycine pretreatment on brain-death donor liver

BACKGROUND: Morphological and functional changes commonly occur in livers of brain-death donors. Preven- tion of liver injury from brain-death will benefit the results of transplantation. This study was conducted to evaluate the protection effects of glycine on the liver of brain-death donor. METHODS: Fourty-two male Wistar rats were randomly divided into brain-death donor (BDD) group (B), glycine pretreatment group with BDD (G), and strychnine pre- treatment group with BDD(S). For these groups, brain death model was established in donor rats and liver trans- plantation was performed subsequently utilizing microsur- gical techniques. After the establishment of the model and during cold rinsing of liver donors or liver reperfusion of recipients, glycine was given at a dose of 0. 6 mmol, 25 μmol and 25 μmol in the group G, and a same dose of gly- cine and strychnine (1000 :1) was prescribed for the group S, but nothing for the group B. Before cold rinsing at 2 and 6 hours after portal vein(PV) reperfusion, blood sam- ples were taken from infrahepatic vena cava (IHVC) to de- termine the levels of alanine aminotransferase (ALT), as- partate aminotransferase (AST), tumor necrosis factor al- pha (TNF-α) and hyaluronic acid (HA). At 6 hours after PV reperfusion, graft samples were fixed for morphological observation and apoptosis of hepatocytes was detected using the TUNEL method. RESULTS: Before liver cold rinsing and at 2 and 6 hours af- ter PV reperfusion, the serum levels of ALT, AST, TNF-α, HA and apoptosis index (AI) in the groups B and S were significantly higher than those in the group G (P 0.05). Electron microscopy showed that Kupffer cells were activated and hepatic cells injured more obvious- ly in the groups B and S than in the group G. CONCLUSION: Glycine pretreatment can improve the via-bility of the liver of the brain-death donor rat.

BACKGROUND:This study aimed to establish an eff ective nomogram to predict the survival of heat stroke(HS)based on risk ***:This was a retrospective,observational multicenter cohort *** analyzed patients diagnosed with HS,who were treated between May 1 and September 30,2018 at 15 tertiary hospitals from 11 cities in Northern ***:Among the 175 patients,32 patients(18.29%)died before hospital *** the univariate analysis,mechanical ventilation,initial mean arterial pressure<70 mmHg,maximum heart rate,lab results on day 1(white blood cell count,alanine aminotransferase,creatinine),and Glasgow admission prediction score were included in multivariate *** Cox regression showed that invasive ventilation,initial mean arterial pressure<70 mmHg(1 mmHg=0.133 kPa),and Glasgow admission prediction score were independent risk factors for *** nomogram was established for predicting 7-d and 14-d survival in the training *** nomogram exhibited a concordance index(C-index)of 0.880(95%confidence interval[95%CI]0.831-0.930)by bootstrapping validation(B=1,000).Furthermore,the nomogram performed better when predicting 14-d survival,compared to 7-d *** prognostic index cut-off value was set at 2.085,according to the operating characteristic curve for overall survival *** model showed good calibration ability in the internal and external validation ***:A novel nomogram,integrated with prognostic factors,was proposed;it was highly predictive of the survival in HS patients.

Background:There is no consensus on whether triplet regimen is better than doublet regimen in the first-line treatment of advanced gastric cancer(AGC).We aimed to compare the efficacy and safety of oxaliplatin plus capecitabine(XELOX)and epirubicin,oxaliplatin,plus capecitabine(EOX)regimens in treating ***:This phase III trial enrolled previously untreated patients with AGC whowere randomly assigned to receive the XELOXor *** primary endpoint was non-inferiority in progression-free survival(PFS)for XELOX as compared with EOX on an intention-to-treat ***:Between April 10,2015 andAugust 20,2020,448AGCpatientswere randomized to receive XELOX(n=222)or EOX(n=226).The median PFS(mPFS)was 5.0 months(95%confidence interval[CI]=4.5-6.0 months)in the XELOX arm and 5.5 months(95%CI=5.0-6.0 months)in the EOX arm(hazard ratio[HR]=0.989,95%CI=0.812-1.203;P_(non-inferiority)=0.003).There was no significant difference inmedian overall survival(mOS)(12.0 vs.12.0months,P=0.384)or objective response rate(37.4%vs.45.1%,P=0.291)between the two *** patients with poorly differentiated adenocarcinoma and liver metastasis,the EOX arm had a significantly longer mOS(P=0.021)and a trend of longer mPFS(P=0.073)than the XELOX *** rate of grade 3/4 adverse events(AEs)was 42.2%(90/213)in the XELOX arm and 72.5%(156/215)in the EOX arm(P=0.001).The global health-related quality of life(QoL)score was significantly higher in the XELOX arm than in the EOX arm during ***:This non-inferiority trial demonstrated that the doublet regimen was as effective as the triplet regimen and had a better safety profile and QoL as a first-line treatment for AGC ***,the triplet regimen might have a survival advantage in patients with poorly differentiated adenocarcinoma and liver metastasis.

Background:Previous studies have demonstrated the preclinical pharmacological and toxicological consistency,and clinical pharmacokinetic equivalence of bevacizumab biosimilar LY01008 with reference bevacizumab(Avastin).This randomized controlled trial aimed to compare the efficacy and safety of LY01008 with Avastin in first-line treatment of Chinese patients with advanced or recurrent non-squamous non-small cell lung cancer(NSCLC).Methods:StageⅢB-ⅣNSCLC patients with evaluable lesions,good physical status,and adequate organ functions from 67 centers across china were randomized in a ratio of 1:1 to receive LY01008 or Avastin 15 mg/kg intravenously in combination with paclitaxel/carboplatin(combined treatment)for 4-6 cycles,followed by maintenance monotherapy with LY01008 until disease progression,intolerable toxicity,or *** primary endpoint was objective response rate(ORR)in accordance with Response Evaluation Criteria in Solid Tumors(RECIST)version 1.1 confirmed by independent radiological review committees(IRRC).Secondary endpoints included disease control rate(DCR),duration of response(DoR),progression-free survival(PFS),overall survival(OS),and *** study was registered in Clinical ***(NCT03533127).Results:Between December 15^(th),2017,and May 15^(th),2019,a total of 649 patients were randomized to the LY01008(n=324)or Avastin(n=325)*** of September 25th,2019 for primary endpoint analysis,589 patients received ORR evaluation,with a median number of combined treatment cycles of 5(range 1-6)andmedian duration of treatment of 3.0(range 0.0-5.1)*** responseevaluable patients in the LY01008 and Avastin groups were 48.5% and 53.0%,*** stratified ORR ratio was 0.91(90%CI 0.80-1.04,within the prespecified equivalence margin of 0.75-1.33).Up to May 15^(th),2020,with a median follow-up of 13.6(range 0.8-28.4)months,no notable differences in DCR,median DoR,median PFS,median OS,and 1-year OS rate were observed between the LY010

静脉血栓栓塞症(venous thromboembolism, VTE)是外科术后常见并发症和医院内非预期死亡的重要危险因素,也是恶性肿瘤患者的第二大死亡原因。晚期头颈部恶性肿瘤患者通常需要接受大范围的肿瘤根治手术和同期修复重建手术,又由于高龄兼长时间手术、术后长时间卧床以及肿瘤和化疗药物导致的血液高凝状态等多种危险因素,提示头颈部恶性肿瘤患者应是VTE的极高危群体,但相关文献对头颈部恶性肿瘤患者的VTE发生情况却很少关注,有关VTE在头颈肿瘤术后发生率的数据也有限。迄今为止,国内对于头颈肿瘤围术期静脉血栓栓塞症缺乏相关治疗共识或指南,不同单位对于头颈肿瘤围术期静脉血栓栓塞症的预防与管理水平参差不齐,缺乏统一、科学的诊疗标准及客观的预防与管理评价体系。为统一和规范头颈肿瘤围术期静脉血栓栓塞症的诊治,减少医疗资源浪费,提高治疗效果,国内该研究领域的专家经反复讨论,汇集全国14家著名医学院校及附属医院专家的诊治意见,同时借鉴和参考国内外近年来对头颈肿瘤围术期静脉血栓栓塞症的研究成果与诊治经验,制订本专家共识,供相关临床医师参考。

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;P<0.001).Grade 3/4 adverse events were observed at a rate of 15.8%and 7.5%in combination and monotherapy groups,*** results suggest that TACE plus PD-(L)1 blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

目的评价虎贞清风胶囊治疗湿热蕴结证急性痛风性关节炎的临床疗效和安全性。方法采用随机、双盲、安慰剂平行对照、多中心、优效性检验临床试验设计。由全国14家医院共收集480例中医辨证为湿热蕴结证急性痛风性关节炎患者,采用分层区组随机化方法,使用SAS软件按3:1随机分为治疗组(360例)和对照组(120例)。治疗组口服虎贞清风胶囊,对照组口服虎贞清风胶囊模拟剂,两组均每次4粒、每日3次给药,共治疗3天。在治疗第3天进行疼痛视觉模拟评分(VAS),观察治疗前后关节疼痛VAS评分、中医证候积分、各单项症状消失率、血沉(ESR)、C反应蛋白(CRP)、白细胞(WBC)计数及不良事件发生率。结果480例患者中438例纳入全分析集(FAS)进行疗效分析,其中治疗组325例,脱落35例(不良事件2例,失访29例,违背方案2例,缺乏疗效退出试验2例);对照组113例,脱落7例(失访2例,违背方案5例)。治疗组第3天疼痛VAS评分及差值下降程度均优于对照组,差异均有统计学意义(t=7.241,t=6.452,P0.05)。治疗期间不良事件发生率治疗组24.4%(87/357),对照组24.8%(30/121),差异无统计学意义(χ2=0.009,P>0.05)。结论虎贞清风胶囊可有效缓解急性痛风性关节炎(湿热蕴结证)关节疼痛,起效时间短,但安全性与安慰剂相当,临床应用较安全。
