Pleomorphic Adenoma Consisting of Multiple Cysts with Squamous Epithelial Lining: Findings on MRI, FNAC, and Histopathological Examination

The incidence of pleomorphic adenoma (PA) of the minor salivary glands is reported to be 10%, and while the histological findings in PA can be diverse, keratin-filled cysts lined by squamous epithelium are rarely reported. The condition can, however, present with cyst formation in some cases. We review a rare case of pleomorphic adenoma in the buccal mucosa that involved the formation of multiple squamous epithelium-lined cysts in a 69-year-old woman. Magnetic resonance imaging (MRI), fine needle aspiration cytology, and histopathological examination were performed. Physical examination revealed a painless, mobile, elastic hard mass in the right buccal mucosa, measuring 2.5 × 1.0 cm. The MRI revealed a well-defined lesion with different signal intensities in the medial and distal regions of the right cheek. The medial side of the lesion showed a low signal intensity on T1-weighted imaging (T1WI) and T2-weighted imaging (T2WI), while the distal side showed a low signal intensity on T1WI, and a high signal intensity on T2WI and short tau inversion recovery (STIR) imaging. Fine needle aspiration of the lesion was performed under local anesthesia and a cytological diagnosis of an epidermoid or dermoid cyst was made. The tumor was completely resected under local anesthesia combined with intravenous sedation. The histopathological examination demonstrated the proliferation of atypical tumor cells with poor atypia and the formation of glandular, alveolar, large, and small cysts. The cysts were lined by keratinized squamous epithelial cells, their cavities were filled with keratinous material, and foreign body reaction was observed after rupture. Histopathological evaluation led to the diagnosis of pleomorphic adenoma. The patient had no evidence of recurrence, 2 years and 3 months after the surgery. In conclusion, the presence of multiple cysts lined by squamous epithelium can pose a significant diagnostic challenge in patients with PA. It is important to make the correct

Salivary gland tumors represent 2% - 6.5% of all head and neck tumors. Since salivary gland tumors have various clinical features and histological types, it is often difficult to diagnose and treat them. The aim of this study was to conduct a retrospective clinical-statistical analysis of 37 minor salivary gland tumors (MSGTs) treated in the Department of Oral Surgery at Nihon University School of Dentistry at Matsudo over a 16-year period. The frequencies and distributions of sex, age, occurrence site, preoperative examination (fine needle cytology and/or biopsy), treatment, and prognosis of the tumors were analyzed and compared with previous reports. The average age at diagnosis was 58.1 years (range 22 - 91 years). The peak occurrence of tumors was in the sixties (10 cases, 27.0%), followed by the forties (8 cases, 21.6%) and the seventies (7 cases, 18.9%). The average age of patients with benign tumors was 56.8 years (range 22 - 91 years). For malignant tumors, the average age was 61.7 years (range 42 - 81 years). The male-female ratio was 1:2.1 for all minor salivary gland tumors, 1:2.4 for benign tumors, and 1.5:1 for malignant tumors. There were 27 (73.0%) benign and 10 (27.0%) malignant tumors. Pleomorphic adenoma (PA) was the most common tumor (24 (64.9%) cases). Most salivary gland tumors originated from the palate (21 cases, 56.8%);the second most common site was the buccal mucosa (14 cases, 37.8%). For most patients, an intraoral mass was the primary presentation, and the second most common symptom was swelling. Surgical treatment was performed for all cases, both benign and malignant tumors. There was one case of local recurrence of PA, as well as one of multiple lung metastases after surgery for adenoid cystic carcinoma. Diagnosis and treatment of MSGTs are often delayed because the patients have few symptoms, and it is difficult to distinguish benign from malignant tumors. High-density circulating fluidized bed gasifier for advanced IGCC/IGFC-Advantages and challenges

Coal-fired Integrated Gasification Combined Cycle （IGCC） and Integrated coal Gasification Fuel-cell Com- bined cycle （IGFC） are being developed as high-efficiency electric power generation technology. However, the highest theoretical gross thermal efficiency of the conventional IGCC]IGFC is still below 52~. In order to obtain higher power generation efficiency, an advanced IGCC （A-IGCC） or advanced IGFC （A-IGFC） sys- tem making use of the exergy recuperation concept by recycling waste heat from gas turbine or fuel cells for steam gasification of coal and biomass was proposed in our laboratory, Corresponding to this system, a novel high-density triple-bed combined circulating fluidized bed （TBCFB） gasifier, composed of a downer pyrolyzer, a bubbling fluidized bed char gasifier, and a riser combustor, was proposed to replace traditional gasifiers such as the entrained flow bed gasifier. The new system is expected to more effectively utilize the waste heat from gas turbines or fuel cells and the heat produced by the combustion of the unreacted char in the riser combustor for pyrolysis and gasification of coal and biomass. In this short review, the advantages and future challenges in the development of high-density TBCFB gasifier are presented and discussed.

Impact of COVID-19 infection rates on admissions for ambulatory care sensitive conditions:nationwide difference-in difference design in Japan

Objectives SARS-CoV-2 infection(COVID-19)has affected tertiary medical institutions and primary *** for ambulatory care sensitive conditions(ACSCs)is an important indicator of primary care ***,no nationwide study,especially in Asia,has examined the association between admissions for ACSCs and local surges in *** study aimed to examine how the number of admissions for ACSCs has changed in Japan between the areas with higher and lower rates of COVID-19 *** This was a retrospective two-stage cross-sectional *** employed a difference-in difference design to compare the number of hospital admissions for ACSCs between the areas with higher and lower rates of COVID-19 infection in *** The study used a nationwide database in *** All patients were aged 20 years and above and were admitted due to ACSCs during the study period between March and September 2019(before the pandemic)and between March and September 2020(during the pandemic).Results The total number of ACSC admissions was 464560(276530 in 2019 and 188030 in 2020).The change in the number of admissions for ACSCs per 100000 was not statistically significant between the areas with higher and lower rates of COVID-19 infection:7.50(95%CI−87.02 to 102.01).In addition,in acute,chronic and preventable ACSCs,the number of admissions per 100000 individuals did not change *** Although admissions for ACSCs decreased during the COVID-19 pandemic,there was no significant change between the areas with higher and lower rates of COVID-19 *** implies that the COVID-19 pandemic affected the areas with higher infection rates and the areas with lower rates.

§1 Introduction The Operations Research Society of Japan (ORS J) was founded by 305 members in *** then the numher of members has increased steadily, and it had 2617 regular members and342 student members along with 164 corporate memberships as of July 1st 1997, when ORSJcelebrated its 40th anniversary. Unfortunately the memberships have slightly decreased since thattime, and as of February 28, 2004, it had 2262 regular members, 265 student members as well as91 corporate memberships. This tendency may be due to the depression of Japanese economy.

Ｔｏｏｔｉｌｌ等人提出了一种渐近的随机Ｔａｕｓｗｏｒｔｈｅ序列，并给出了一个偶然发现的此种序列，但直到现在尚未有人提出一个寻求此种序列的系统性的方法，本文中我们将给出一种设计ＧＦＳＲ序算法，所产生的序列近似地满足渐近随机性，我们的算法基于反复地运用由Ｆｕｓｈｉｍｉ提出的算法，由现在的算法所设计出来的序列不有一种附加的好处，即其十中抽一的序列也是近似地渐近随机的，文中给出了一些数字例子。

Synchronous Double Primary Combined Hepatocellularcholangiocarcinoma and Cholangiolocarcinoma in a Cirrhotic Liver

Both combined hepatocellular-cholangiocarcinoma(cHCCCCA)and cholangiolocarcinoma are rare primary liver ***-CCA is believed to originate from transformed hepatocellular carcinoma or liver stem/progenitor *** is characterized by ductular reaction-like anastomosing cords and glands resembling cholangioles or canals containing hepatocellular carcinoma components and adenocarcinoma *** to the 2019 revision of the World Health Organization criteria,a subtype with stem cell features as a subclassification of cHCC-CCA was abolished for lack of conclusive evidence of the stem cell origin *** led to the classification of cholangiolocarcinoma with hepatocytic differentiation as ***,cholangiolocarcinoma without hepatocytic differentiation is classified as a subtype of small-duct cholangiocarcinoma and is assumed to originate from the bile ***,we report the first case of double primary cHCC-CCA and cholangiolocarcinoma without hepatocytic differentiation in different hepatic segments of a cirrhotic *** believe this case supports the validity of the new World Health Organization criteria because the pathological finding of cHCC-CCA in this case shows the transformation of hepatocellular carcinoma to ***,this case may demonstrate that immature ductular cell stemness and mature hepatocyte cell stemness in hepatocarcinogenesis can coexist in the same *** results provide valuable insights into the mechanisms of growth,differentiation,and regulation of liver cancers.
