A comparison of the efficacy of dutasteride on reducing lower urinary tract symptoms among patients with small versus large benign prostatic hyperplasia

A Case of Warthin’s Tumor Together with IgG4-Related Sialadenitis

Warthin’s tumor is the second most frequent neoplasm next to pleomorphic adenoma in the salivary gland. The tumor contains the epithelial oncocyte cells with the presence of rich-mitochondria and their surrounding abundant lymphocytes. A relatively new disease entity of IgG4-related disease frequently occurs in the salivary gland. However, the coexistence of Warthin’s tumor and IgG4-related disease is scarcely observed. We have recently experienced a rare case of Warthin’s tumor with IgG4-related sialadenitis. A 51-year-old man presented to our hospital, complaining of a mass with right submandibular tenderness and spontaneous pain. A computed tomography scan of the cervical region revealed a suspicion of lymph node proliferative disease, including malignant lymphoma. Elevated serum levels of IL-2R: 1843 U/ml (reference value 122 - 496 U/ml), IgG: 3430 mg/dl (reference value 861 - 1747 mg/dl), and IgG4: 3140 mg/dl (reference value 11 - 121 mg/dl) were observed. Other laboratory data showed within normal ranges. The cervical tumor was diagnosed as Warthin’s tumor by the findings of fine-needle aspiration cytology and biopsy examination. Immunohistochemistry revealed numerous IgG4- and IgG-positive cells with fibrosis surrounding the epithelial component of Warthin’s tumor, suggesting IgG4-rerated sialadenitis. Finally, we diagnosed the cervical tumor as Warthin tumor with IgG4-related sialadenitis. This is the second report describing a case of Warthin’s tumor with possible involvement of IgG4-related sialadenitis.

Interaction between hepatitis C virus and metabolic factors

Hepatitis C virus (HCV) infection disrupts the normal metabolism processes, but is also influenced by several of the host’s metabolic factors. An obvious and significantly detrimental pathophysiological feature of HCV infection is insulin resistance in hepatic and peripheral tissues. Substantial research efforts have been put forth recently to elucidate the molecular mechanism of HCV-induced insulin resistance, and several cytokines, such as tumor necrosis factor-α, have been identified as important contributors to the development of insulin resistance in the distant peripheral tissues of HCV-infected patients and animal models. The demonstrated etiologies of HCV-induced whole-body insulin resistance include oxidative stress, lipid metabolism abnormalities, hepatic steatosis and iron overload. In addition, myriad effects of this condition have been characterized, including glucose intolerance, resistance to antiviral therapy, progression of hepatic fibrosis, development of hepatocellular carcinoma, and general decrease in quality of life. Metabolic-related conditions and disorders, such as visceral obesity and diabetes mellitus, have been shown to synergistically enhance HCV-induced metabolic disturbance, and are associated with worse prognosis. Yet, the molecular interactions between HCV-induced metabolic disturbance and host-associated metabolic factors remain largely unknown. The diet and lifestyle recommendations for chronic hepatitis C are basically the same as those for obesity, diabetes, and metabolic syndrome. Specifically, patients are suggested to restrict their dietary iron intake, abstain from alcohol and tobacco, and increase their intake of green tea and coffee (to attain the beneficial effects of caffeine and polyphenols). While successful clinical management of HCV-infected patients with metabolic disorders has also been achieved with some anti-diabetic (i.e., metformin) and anti-lipid (i.e., statins) medications, it is recommended th

Importance of hepatitis C virus-associated insulin resistance:Therapeutic strategies for insulin sensitization

Sensitivity study of subgrid scale ocean mixing under sea ice using a two-column ocean grid in climate model CESM

Brine drainage from sea ice formation plays a critical role in ocean mixing and seasonal variations of halocline in polar oceans. The horizontal scale of brine drainage and its induced convection is much smaller than a climate model grid and a model tends to produce false ocean mixing when brine drainage is averaged over a grid cell. A two-column ocean grid （TCOG） scheme was implemented in the Community Earth System Model （CESM） using coupled sea ice-ocean model setting to explicitly solve the different vertical mixing in the two sub- columns of one model grid with and without brine rejection. The fraction of grid with brine rejection was tested to be equal to the lead fraction or a small constant number in a series of sensitivity model runs forced by the same atmospheric data from 1978 to 2009. The model results were compared to observations from 29 ice tethered profilers （ITP） in the Arctic Ocean Basin from 2004 to 2009. Compared with the control run using a regular ocean grid, the TCOG simulations showed consistent reduction of model errors in salinity and mixed layer depth （MLD）. The model using a small constant fraction grid for brine rejection was found to produce the best model comparison with observations, indicating that the horizontal scale of the brine drainage is very small compared to the sea ice cover and even smaller than the lead fraction. Comparable to models using brine rejection parameterization schemes, TCOG achieved more improvements in salinity but similar in MLD.

Bandgap-tunable lateral and vertical heterostructures based on monolayer Mo1-xWxS2 alloys

The fabrication of heterostructures of two-dimensional semiconductors with specific bandgaps is an important approach to realizing the full potential of these materials in electronic and optoelectronic devices. Several groups have recently reported the direct growth of lateral and vertical heterostructures based on monolayers of typical semiconducting transition metal dichalcogenides （TMDCs） such as WSe2, MoSe2, WS2, and MoS2. Here, we demonstrate the single-step direct growth of lateral and vertical heterostructures based on bandgap-tunable Mo1-xWxS2 alloy monolayers by the sulfurization of patterned thin films of WO3 and MoO3. These patterned films are capable of generating a wide variety of concentration gradients by the diffusion of transition metals during the crystal growth phase. Under high temperatures, this leads to the formation of monolayer crystals of Mo1-xWxS2 alloys with various compositions and bandgaps, depending on the positions of the crystals on the substrates. Heterostructures of these alloys are obtained through stepwise changes in the ratio of W/Mo within a single domain during low-temperature growth. The stabilization of the monolayer Mo1-xWxS2 alloys, which often degrade even under gentle conditions, was accomplished by coating the alloys with other monolayers. The present findings demonstrate an efficient means of both studying and optimizing the optical and electrical properties of TMDC-based heterostructures to allow use of the materials in future device applications.

Motor vehicle accidents: How should cirrhotic patients be managed?

Motor vehicle accidents (MVAs) are serious social issues worldwide and driver illness is an important cause of MVAs. Minimal hepatic encephalopathy (MHE) is a com- plex cognitive dysfunction with attention deficit, which frequently occurs in cirrhotic patients independent of severity of liver disease. Although MHE is known as a risk factor for MVAs, the impact of diagnosis and treatment of MHE on MVA-related societal costs is largely unknown. Recently, Bajaj et al demonstrated valuable findings that the diagnosis of MHE by rapid screening using the inhibitory control test (ICT), and subsequent treatment with lactulose could substantially reduce the societal costs by preventing MVAs. Besides the ICT and lactulose, there are various diagnostic tools and therapeutic strategies for MHE. In this commentary, we discussed a current issue of diagnostic tools for MHE, including neuropsychological tests. We also discussed the advantages of the other therapeutic strategies for MHE, such as intake of a regular breakfast and coffee, and supplementation with zinc and branched chain amino acids, on the MVA-related societal costs.

Photoluminescence and radioluminescence properties of (Gd8Ca2)(SiO4)6O2 crystals with different concentrations of Ce

(GdCa)(SiO)Oapatite crystals doped with different concentrations of Ce were synthesized by the Floating Zone(FZ) method, and we have evaluated the photoluminescence(PL) and radio luminescence(RL) properties. In PL, an emission band appeared around 400 nm under excitation at 300 nm due to the5 d-4 f transitions of Ce3+. The PL decay curves were approximated by a second-order exponential decay function, and the derived decay time constants ranged around 20-25 ns and 40-480 ns. Similar emission was observed in RL, but the decay time constants were much longer than those in PL. The RL afterglow was the lowest in the 1.0% Ce-doped sample. The 1.0% and 2.0% Ce-doped samples showed a full-energy peak in the pulse height spectrum measured underAm 5.5 MeV α-ray irradiation, and the absolute scintillation light yields were estimated to be around 55 and 36 ph/5.5 MeV-a, respectively.

Investigation of the factors affecting the limit of detection of laser-induced breakdown spectroscopy for surface inspection

Laser-induced breakdown spectroscopy(LIBS) was examined to detect a trace substance adhered onto Al alloys for the surface inspection of materials to be adhesively bonded. As an example of Si contamination, silicone oil was employed and sprayed onto substrates with a controlled surface concentration. LIBS measurements employing nanosecond UV pulses(λ?=?266 nm) and an off-axis emission collection system with different detecting heights were performed. Because surface contaminants are involved in the plasma formed by laser ablation of the substrates, the relative contribution of the surface contaminants and the substrates to the plasma emission could be changed depending on the conditions for plasma formation. The limit of detection(LOD) was evaluated under several detecting conditions for investigating the factors that affected the LOD. A significant factor was the standard deviation values of signal intensities obtained for the clean substrates. This value varied depending on the measurement *** the Al alloy(A6061), the smallest LOD obtained was 0.529 μg?·?cm^(-2). Furthermore, an improved LOD(0.299 μg?·?cm^(-2)) was obtained for the Al alloy with a lower Si content.
