Diagnostic performance of C-reactive protein level and its role as a potential biomarker of severe dengue in adults

Objective: To determine the relationship between C-reactive protein(CRP) level and the severity of dengue and the potential use of CRP in predicting acute dengue ***: A prospective observational study was performed on dengue patients admitted to a tertiary care hospital in southern India. All patients of age above 18 years, diagnosed with dengue were included in the study. The detailed laboratory parameters pertaining to dengue were recorded. CRP levels were estimated and compared between groups i.e. severe and non-severe dengue. CRP cut-off value was detected using the receiver-operator ***: Totally 98 patients with a mean age of 40 years were included. Among them, 11.2% of the patients suffered from severe dengue, 54.1% of the cases had non-severe dengue without warning signs, and 34.7% had non-severe dengue with warning signs. The median CRP was significantly higher in patients with severe dengue compared to patients with non-severe dengue(96.2 mg/d L vs. 5.3 mg/d L). univariate logistic regression analysis showed that the odd’s ratio(OR) of CRP was 1.053(P≤0.001, 95% CI=1.029-1.078). CRP at a cutoff value of 21.6 mg/L(0.929 AuC) had excellent sensitivity(100%) and specificity(81.6%) in predicting severe dengue infection. Multivariate logistic regression analysis showed that CRP(OR=1.089, P=0.013) and ALT(OR=1.010, P=0.034) were statistically significant independent predictors of dengue ***: CRP level could be used as a potential biomarker to predict severity of dengue in adults.

Comparing acid steatocrit and faecal elastase estimations for use in M-ANNHEIM staging for pancreatitis

AIM To compare two tests for exocrine pancreatic function(EPF) for use in M-ANNHEIM staging for pancreatitis. METHODS One hundred and ninety four consecutive patients with acute pancreatitis(AP; n = 13), recurrent acute pancreatitis(RAP; n = 65) and chronic pancreatitis(CP; n = 116) were enrolled. EPF was assessed by faecal elastase-1(FE-1) estimation and stool fat excretion by the acid steatocrit method. Patients were classified as per M-ANNHEIM stages separately based on the results of the two tests for comparison. Independent Student's t-test, χ~2 test, Kruskal-Wallis test, Mann-Whitney u test and Mc Nemar's test were used as appropriate. RESuLTS Sixty-one(52.5%) patients with CP had steatorrhoea when assessed by the acid steatocrit method; 79 (68.1%) with CP had exocrine insufficiency by the FE-1 test(χ~2 test, P ults of acid steatocrit and FE-1 showed a significant negative correlation(Spearman's rho =-0.376, P use of acid steatocrit as against 13(6.7%), 85(43.8%), 75(38.6%), and 21(10.8%) respectively by FE-1 in stages 0,?Ⅰ, Ⅱ, and Ⅲ thereby altering the stage in 28(14.4%) patients(P uSION FE-1 estimation performed better than the acid steatocrit test for use in the staging of pancreatitis by the M-ANNHEIM classification since it diagnosed a higher proportion of patients with exocrine insufficiency.

Identification of SARS-CoV-2 Spike Protein Inhibitors from urtica dioica to Develop Herbal-Based Therapeutics Against COVID-19

Objective: The high transmission rate and mutations of SARS-Co V-2 have made it a global pandemic, and the shortage of any effective clinical treatment has created such a commotion. There are some synthetic antiviral drugs, such as remdesivir and lopinavir that are being repurposed to treat SARS-Co V-2, but all of these demonstrate extreme side effects in humans. Hence, promoting herbal-based drug development has become crucial as they are cost-effective and have lesser or no side effects. urtica dioica is abundant in the Himalayan region and the compounds present in it have shown significant antiviral and anti-SARS activity. Therefore, molecular docking studies were performed to identify SARS-Co V-2 spike protein inhibitors from u. dioica to combat the COVID-19 disease. Materials and Methods: Compounds from u. dioica were screened using the bioinformatic approach, and subsequently, these compounds were docked with the S1 subunit of the COVID-19 spike protein(PDB ID: 6YOR). Molecular docking was carried out using the Py Rx software(0.8 version) and further examined by employing the Discovery Studio Visualizer. Results: About all the selected compounds showed significant binding energy(e.g., beta-sitosterol:-10.3 kcal/mol) in contrast to the control chloroquine phosphate. This binding was observed with the spike protein residues that were common in the old strain and the more contagious newly modified B.1.1.7 strain of SARS-Co V-2. Conclusions: Thus, our study can be used in effective drug development against SARS-Co V-2 and its mutant strains also.

Frequency and temperature dependent dielectric dispersion of 20PbF2？20TeO2？（60？x）B2O3？xEu2O3（x=0 to 2.5, mole fraction, %） glasses prepared by the melt？quenching technique were investigated in the frequency range 1 Hz？10 MHz and temperature range 313？773 K. Dielectric relaxation dynamics was analyzed based on the electric modulus behavior. Dielectric losses （tanδ） are found to be negligibly small in the temperature range 313？523 K, proving good thermal stability of the glasses. The present Eu2O3-doped oxyfluroborate glasses showed low dielectric loss at higher frequency and lower temperature, proving their suitability for nonlinear optical materials.

Isolated hepatic non-obstructive sinusoidal dilatation, 20-year single center experience

AIM To characterize isolated non-obstructive sinusoidal dilatation(SD) by identifying associated conditions, laboratory findings, and histological patterns. METHODS Retrospectively reviewed 491 patients with SD between 1995 and 2015. Patients with obstruction at the level of the small/large hepatic veins, portal veins, or right-sided heart failure were excluded along with history of cirrhosis, hepatic malignancy, liver transplant, or absence of electrocardiogram/cardiac echocardiogram. Liver histology was reviewed for extent of SD, fibrosis, red blood cell extravasation, nodular regenerative hyperplasia, hepaticpeliosis, and hepatocellular plate atrophy(HPA). RESuLTS We identified 88 patients with non-obstructive SD. Inflammatory conditions(32%) were the most common cause. The most common pattern of liver abnormalities was cholestatic(76%). Majority(78%) had localized SD to Zone Ⅲ. Medication-related SD had higher proportion of portal hypertension(53%), ascites(58%), and median AST(113 u/L) and ALT(90 u/L) levels. Nineteen patients in our study died within one-year after diagnosis of SD, majority from complications related to underlying *** Significant proportion of SD and HPA exist without impaired hepatic venous outflow. Isolated SD on liver biopsy, in the absence of congestive hepatopathy, requires further evaluation and portal hypertension should be rule out.
