Dysfunction of synaptic endocytic trafficking in Parkinson's disease

Analysis of Critical Incidents during Anesthesia in a Tertiary Hospital

Introduction: Critical incident monitoring is important in quality improvement as it identifies potential risks to patients by analyzing adverse events or near-misses. Methods: This study analyses the reported incidents in a tertiary hospital over a 4-year period. Results: A total of 441 incidents were reported out of 98,502 anesthetics performed during the study period. Of these incidents, 67 resulted in no harm caused, 116 with unanticipated ICU admissions and 20 mortalities. The odds of having a critical incident increased with ASA status: from an odds ratio of 2.08 (95% CI: 1.58 to 2.74) for ASA 2 patients compared to ASA1, to OR of 13.70 (5.91 to 31.74) in ASA 5 compared to ASA 1. Critical incidents also have higher odds occurring out of hours (OR 1.7 (1.45 to 2.23) compared to daytime hours (08:00-17:00). They occurred most commonly in maintenance phase (142, 32.7%), followed by induction (120, 27.6%). The most common types of incidents include airway and respiratory (110, 24.9%) followed by drug related incidents (67, 15.2%). Human error was attributed as a significant contributing factor in 276 incidents (61.5%) followed by patient factors in 112 incidents (25.4%). Mitigating factors such as vigilance by staff involved were significant in 136 incidents (30.3%). Conclusion: Higher ASA status appears to be the most important factor associated with actual or potential patient harm in our study. Also significant, was time of incident, with incidents more likely out of hours. Critical incident reporting is a valuable part of quality assurance. We should continue to invest in incident reporting, incident analysis and improvement plans.

NADPH oxidase 4(NOX4)as a biomarker and therapeutic target in neurodegenerative diseases

Diseases like Alzheimer’s and Parkinson’s diseases are defined by inflammation and the damage neurons undergo due to oxidative stress. A primary reactive oxygen species contributor in the central nervous system, NADPH oxidase 4, is viewed as a potential therapeutic touchstone and indicative marker for these ailments. This in-depth review brings to light distinct features of NADPH oxidase 4, responsible for generating superoxide and hydrogen peroxide, emphasizing its pivotal role in activating glial cells, inciting inflammation, and disturbing neuronal functions. Significantly, malfunctioning astrocytes, forming the majority in the central nervous system, play a part in advancing neurodegenerative diseases, due to their reactive oxygen species and inflammatory factor secretion. Our study reveals that aiming at NADPH oxidase 4 within astrocytes could be a viable treatment pathway to reduce oxidative damage and halt neurodegenerative processes. Adjusting NADPH oxidase 4 activity might influence the neuroinflammatory cytokine levels, including myeloperoxidase and osteopontin, offering better prospects for conditions like Alzheimer’s disease and Parkinson’s disease. This review sheds light on the role of NADPH oxidase 4 in neural degeneration, emphasizing its drug target potential, and paving the path for novel treatment approaches to combat these severe conditions.

In this big data era, the use of large dataset in conjunction with machine learning (ML) has been increasingly popular in both industry and academia. In recent times, the field of materials science is also undergoing a big data revolution, with large database and repositories appearing everywhere. Traditionally, materials science is a trial-and-error field, in both the computational and experimental departments. With the advent of machine learning-based techniques, there has been a paradigm shift: materials can now be screened quickly using ML models and even generated based on materials with similar properties;ML has also quietly infiltrated many sub-disciplinary under materials science. However, ML remains relatively new to the field and is expanding its wing quickly. There are a plethora of readily-available big data architectures and abundance of ML models and software;The call to integrate all these elements in a comprehensive research procedure is becoming an important direction of material science research. In this review, we attempt to provide an introduction and reference of ML to materials scientists, covering as much as possible the commonly used methods and applications, and discussing the future possibilities.

To critically appraise short-term outcomes in patients treated in a new Pelvic Exenteration (PE) Unit. METHODSThis retrospective observational study was conducted by analysing prospectively collected data for the first 25 patients (16 males, 9 females) who underwent PE for advanced pelvic tumours in our PE Unit between January 2012 and October 2016. Data evaluated included age, co-morbidities, American Society of Anesthesiologists (ASA) score, Eastern Cooperative Oncology Group (ECOG) status, preoperative adjuvant treatment, intra-operative blood loss, procedural duration, perioperative adverse event, lengths of intensive care unit (ICU) stay and hospital stay, and oncological outcome. Quantitative data were summarized as percentage or median and range, and statistically assessed by the χ2 test or Fisher’s exact test, as applicable. RESULTSAll 25 patients received comprehensive preoperative assessment via our dedicated multidisciplinary team approach. Long-course neoadjuvant chemoradiotherapy was provided, if indicated. The median age of the patients was 61.9-year-old. The median ASA and ECOG scores were 2 and 0, respectively. The indications for PE were locally invasive rectal adenocarcinoma (n = 13), advanced colonic adenocarcinoma (n = 5), recurrent cervical carcinoma (n = 3) and malignant sacral chordoma (n = 3). The procedures comprised 10 total PEs, 4 anterior PEs, 7 posterior PEs and 4 isolated lateral PEs. The median follow-up period was 17.6 mo. The median operative time was 11.5 h. The median volume of blood loss was 3306 mL, and the median volume of red cell transfusion was 1475 mL. The median lengths of ICU stay and of hospital stay were 1 d and 21 d, respectively. There was no case of mortality related to surgery. There were a total of 20 surgical morbidities, which occurred in 12 patients. The majority of the complications were grade 2 Clavien-Dindo. Rechargeable magnesium metal batteries need an electrolyte that forms a stable and ionically conductive solid electrolyte interphase(SEI)on the ***,we used molecular dynamic simulation,density functional theory calculation,and X-ray photoelectron spectroscopy analysis to investigate the solvation structures and SEI compositions in electrolytes consisting of dual-salts,magnesium bis(trifluoromethanesulfonyl)imide(MgTFSI_(2)),and MgCl_(2),with different additives in 1,2-dimethoxyethane(DME)*** found that the formed[Mg_(3)(μ-Cl)_(4)(DME)mTFSI_(2)](m=3,5)inner-shell solvation clusters in MgTFSI_(2)-MgCl_(2)/DME electrolyte could easily decompose and form a MgO-and MgF_(2)-rich *** electron-rich inorganic species in the SEI,especially MgF_(2),turned out to be detrimental for Mg plating/*** reduce the MgF_(2)and MgO contents in SEI,we introduce an electron-deficient tri(2,2,2-trifluoroethyl)borate(TFEB)additive in the ***//Mg cells using the MgTFSI_(2)-MgCl_(2)/DME-TFEB electrolyte could cycle stably for over 400 h with a small polarization voltage of~150 *** with the presence of 800 ppm H_(2)O,the electrolyte with TFEB additive could still preserve its good electrochemical *** optimized electrolyte also enabled stable cycling and high-rate capability for Mg//Mo6S8 and Mg//CuS full cells,showing great potential for future applications.

Hydrogen(H_(2))is a promising renewable energy which finds wide applications as the world gears toward low-carbon ***,current H_(2) production via steam methane reforming of natural gas or gasification of coal are laden with high CO_(2) ***,methane(CH_(4))pyrolysis has emerged as a potential technology to generate low-carbon H_(2) and solid *** this review,the current state-of-art and recent progress of H_(2) production from CH_(4) pyrolysis are reviewed in *** such as funda-mental mechanism and chemistry involved,effect of process parameters on the conversion efficiency and reaction kinetics for various reaction media and catalysts are elucidated and critically ***-ature,among other factors,plays the most critical influence on the methane pyrolysis *** metal/salt could lower the operating temperature of methane pyrolysis to1000℃.Based on the reaction kinetics,metal-based cata-lysts were more efficient in lowering the activation energy of the reaction to 29.5-88 kJ/mol from that of uncatalyzed reaction(147-420.7 kJ/mol).Besides,the current techno-economic performance of the pro-cess reveals that the levelized cost of H_(2) is directly influenced by the sales price of carbon(by-product)generated,which could offset the overall ***,the main challenges of reactor design for efficient product separation and retrieval,as well as catalyst deactivation/poisoning need to be debottlenecked.
