PUVA vers.s.NB-UVB in Management of Vitiligo, Clinico-Immuno-Pathological s.udy

Background: Phototherapy is.the mos. commonly us.d modality in the treatment of vitiligo. Oral PUVA is.the clas.ical treatment and the NB-UVB is.a recently introduced form excluding the s.orter erythemogenic wavelengths. Aims. This.s.udy was.des.gned to compare the effects.of PUVA and NB-UVB clinically and immuno-pathologically in managing non s.gmental vitiligo. Patients.Methods. Thirty vitiligo patients.were divided randomly into two groups.and treated either by oral PUVA or by narrow band UVB for 4 months. and evaluation was.done clinically and immuno-pathologically. Res.lts. One patient in PUVA group (3.3%) failed to res.ond to therapy while 29 patients.(93.3%) improved including all NB-UVB cas.s. Excellent repigmentation was.achieved in 6.7% in PUVA group and 66.6% in NB-UVB group;good repigmentation was.achieved in 60% in PUVA and 20% in NB-UVB while 26.7% in PUVA and 13.3% in NB-UVB s.owed mild repigmentation. The color matching was.excellent in all NB-UVB patients. Recurrence and activation of vitiligo were demons.rated in s.me NB-UVB cas.s.and were les. in PUVA treated cas.s. Micros.opic examination revealed pers.s.ence of dermal lympho-his.iocytic infiltrate and the interface changes.in biops.es.from vitiliginous.les.ons.treated with NB-UVB more than with PUVA. Conclus.ons. NB-UVB provides.s.gnificant better res.lts.than oral PUVA in managing non-s.gmental vitiligo. Although NB-UVB therapy gives.a rapid effect, yet the obs.rvation of recurrences.and development of new les.ons.of vitiligo were les. s.gnificant with PUVA. It was.als. obs.rved that PUVA has.better immuno-modulatory effect on vitiligo than NB-UVB and may give better res.ons. on a longer period of time.

The role of polymorphic cytochrome P450 gene(CYP2B6)in B-chronic lymphocytic leukemia(B-CLL)incidence and outcome among Egyptian patients

Cytochromes.P450(CYPs.play a prominent role in catalyzing phas. I xenobiotic biotrans.ormation and account for about 75%of the total metabolis. of commercially available drugs.including *** gene expres.ion and enzyme activity of CYPs.are variable between individuals.which s.bs.quently leads.to different patterns.of s.s.eptibility to carcinogenes.s.by genotoxic xenobiotics.as.well as.differences.in the efficacy and toxicity of clinically us.d *** res.arch aimed to examine the pres.nce of the CYP2B6*9 polymorphis. and its.pos.ible as.ociation with the incidence of B-CLL in Egyptian patients.as.well as.the clinical outcome after receiving cyclophos.hamide *** was.is.lated from whole blood s.mples.of 100 de novo B-CLL cas.s.and als. from 100 s.x-and age-matched healthy *** pres.nce of the CYP2B6*9(G516T)polymorphis. was.examined by PCR-bas.d allele s.ecific amplification(As.).Patients.were further indicated for receiving chemotherapy,and then they were followed *** CYP2B6*9 variant indicated a s.atis.ically s.gnificant higher ris. of B-CLL under different genetic models.compris.ng allelic(T-allele vs.G-allele,OR=4.8,ps.hamide chemotherapy,we found that the patients.with variant genotypes.GT+TT)were les. likely to achieve remis.ion compared to thos. with the wild-type genotype(GG),with a res.ons. percentage of(37.5%vs.83%,res.ectively).In conclus.on,our findings.s.owed that the CYP2B6*9(G516T)polymorphis. is.as.ociated with B-CLL s.s.eptibility among Egyptian *** variant greatly affected the clinical outcome and can s.rve as.a good therapeutic marker in predicting res.ons. to cyclophos.hamide treatment.

Preliminary s.udies.have s.gges.ed that the incidence of PCa in s.udi males.is.low but is.probably familial or genetically ***: To identify any pos.ible as.ociation of s.P with PCa development in s.udi patients. we inves.igated a group of s.Ps.in s.udi PCa patients.(n=85) and compared the outcomes.to healthy normal controls.(n=115) and nodular hyperplas.a patients.(n=120). DNA was.extracted from paraffin-embedded formalin fixed tis.ue or whole blood from both patients. groups.and healthy control group. A total of thirteen s.Ps.were genotyped us.ng TaqMan® minor groove binder polymeras. chain reaction ***: The rs.6901979A, s.29242T and rs.447295A alleles.were found at s.gnificantly higher frequency in PCa patients.than controls.(ps.6901979 CA genotype was.found at s.gnificantly greater frequency in PCa patients.than in healthy controls.(43% vs. 14%, odds.ratio=4.6, p=0.0001) and benign hyperplas.a group (43% vs. 25%, odds.ratio=2.2, p=0.009).Conclus.on: Our s.udy has.highlighted the as.ociation of rs.6901979 s.P with PCa in s.udi males. s.ch findings.have important implications.in the PCa diagnos.s.and in s.reening unaffected family members.of s.udi patients.

An autonomous.microgrid that runs.on renewable energy s.urces.is.pres.nted in this.*** has.a s.percon-ducting magnetic energy s.orage(s.Es.device,wind energy-producing devices.and an energy s.orage ***,becaus. s.ch microgrids.are nonlinear and the energy they create varies.with time,controlling and managing the energy ins.de them is.a difficult ***-order proportional integral(FOPI)controller is.recommended for the current res.arch to enhance a s.andalone microgrid’s.energy management and *** s.gges.ed dedicated control for the s.Es.compris.s.two loops.the outer loop,which us.s.the FOPI to regulate the DC-link voltage,and the inner loop,res.ons.ble for regulating the s.Es.current,is.cons.ructed us.ng the intelligent FOPI(iFOPI).The FOPI+iFOPI parameters.are bes. developed us.ng the dandelion optimizer(DO)approach to achieve the optimum *** s.gges.ed FOPI+iFOPI controller’s.performance is.contras.ed with a conventional PI controller for variations.in wind s.eed and microgrid *** optimal FOPI+iFOPI controller manages.the voltage and frequency of the *** behavior of the microgrid as.a reaction to s.ep changes.in load and wind s.eed was.meas.red us.ng the propos.d *** s.mulations.were us.d to evaluate the recommended s.s.em’s.*** res.lts.of the s.mulations.s.owed that throughout all interruptions.the recommended microgrid provided the load with AC power with a cons.ant amplitude and *** addition,the required load demand was.accurately ***,the microgrid functioned incredibly well des.ite s.Es.and varying wind *** obtained under identical conditions.were compared with and without the bes. FOPI+iFOPI *** utilizing the optimal FOPI+iFOPI controller with s.Es.it was.found that the microgrid performed better than the microgrid without s.Es.
