Epidemiology of Leprosy in China, 2021: An Update

Objective: Leprosy is an infectious diseases that remains a concerning public health issue. The infection ofMycobacterium leprae still exists at the county level in hard-to-access areas in southwest China. An accurate analysis of the prevalence of leprosy in various regions is necessary for the formulation of prevention and control strategies, so we conducted this study to describe the epidemiological characteristics of leprosy in China in 2021 and provide essential information for future national leprosy control and prevention strategies.Methods: We collected epidemiological data of leprosy from 31 provinces, autonomous regions, and municipalities of China (except for hong Kong, Macao, and Taiwan) provided by the Leprosy Management Information System. We performed a comparative analysis of the epidemiological features of patients with leprosy including age, sex, geographical distribution, and grade of disability.Results: In total, 374 newly detected leprosy cases were reported nationwide in China in 2021, giving an incidence rate of 0.27 per million population, which represented a decrease of 7.9% compared with 2020. Among the population with leprosy in 2021 that comprised 238 males (63.6%) and 136 females (36.4%), 1.9% (7/374) were children younger than 15 years, 92.8% (347/374) had multibacillary leprosy, and 18.2% (68/374) had grade 2 disability. More than half of the new leprosy cases (54.8%, 205/374) were distributed in southwest China. There were 33 relapsed leprosy cases reported in 2021. By the end of 2021, there were 1,897 registered leprosy cases reported nationwide, giving a prevalence rate of 1.35 per million population.Conclusion: The leprosy epidemic in China shows a downward trend in terms of the prevalence and incidence rates. However, the high prevalence of leprosy in southwest China is still a matter of concern.

A cellular mechanism underlying the restoration of thermo/photoperiod-sensitive genic male sterility

During anther development,the transformation of the microspore into mature pollen occurs under the pro-tection of first the tetrad wall and later the pollen wall.Mutations in genes involved in this wall transition often lead to microspore rupture and male sterility;some such mutants,such as the reversible male sterile(rvms)mutant,are thermo/photoperiod-sensitive genic male sterile(P/TGMS)lines.Previous studies have shown that slow development is a general mechanism of P/TGMS fertility restoration.In this study,we iden-tified restorer of rvms-2(res2),which is an allele of QUARTET3(QRT3)encoding a polygalacturonase that shows delayed degradation of the tetrad pectin wall.We found that MS188,a tapetum-specific transcrip-tion factor essential for pollen wall formation,can activate QRT3 expression for pectin wall degradation,indicating a non-cell-autonomous pathway involved in the regulation of the cell wall transition.Further as-says showed that a delay in degradation of the tetrad pectin wall is responsible for the fertility restoration of rvms and other P/TGMS lines,whereas early expression of QRT3 eliminates low temperature restoration of rvms-2 fertility.Taken together,these results suggest a likely cellular mechanism of fertility restoration in P/TGMS lines,that is,slow development during the cell wall transition of P/TGMS microspores may reduce the requirement for their wall protection and thus support their development into functional pollens,leading to restored fertility.

Perception of fundamental science to boost lithium metal anodes toward practical application

As a key material for lithium metal batteries(LMBs),lithium metal is one of the most promising anode materials to break the bottleneck of battery energy density and a commonly used active material for reference electrodes.Although lithium anodes are regarded as the holy grail of lithium batteries,decades of exploration have not led to the successful commercialization of LMBs,due mainly to the challenges related to the inherent properties of lithium metal.To pave the way for further investigation,herein,a comprehensive review focusing on the fundamental science of lithium are provided.Firstly,the natures of lithium atoms and their isotopes,lithium clusters and lithium crystals are revisited,especially their structural and energetic properties.Subsequently,the electrochemical properties of lithium metal are reviewed.Numerous important concepts and scientific questions,including the electronic structure of lithium,influence of high pressure and low temperature on the properties of lithium,factors influencing lithium deposition,generation of lithium dendrites,and electrode potential of lithium in different electrolytes,are explained and analyzed in detail.Approaches to improve the performance of lithium anodes and thoughtfulness about the electrode potential in lithium battery research are proposed.

Autophagy is a conserved lysosomal degradation pathway where cellular components are dynamically degraded and re-processed to maintain physical homeostasis.However,the physiological effect of autophagy appears to be multifaced.On the one hand,autophagy functions as a cytoprotective mechanism,protecting against multiple diseases,especially tumor,cardiovascular disorders,and neurodegenerative and infectious disease.Conversely,autophagy may also play a detrimental role via pro-survival effects on cancer cells or cell-killing effects on normal body cells.During disorder onset and progression,the expression levels of autophagy-related regulators and proteins encoded by autophagy-related genes(ATGs)are abnormally regulated,giving rise to imbalanced autophagy flux.However,the detailed mechanisms and molecular events of this process are quite complex.Epigenetic,including DNA methylation,histone modifications and miRNAs,and post-translational modifications,including ubiquitination,phosphorylation and acetylation,precisely manipulate gene expression and protein function,and are strongly correlated with the occurrence and development of multiple diseases.There is substantial evidence that autophagy-relevant regulators and machineries are subjected to epigenetic and post-translational modulation,resulting in alterations in autophagy levels,which subsequently induces disease or affects the therapeutic effectiveness to agents.In this review,we focus on the regulatory mechanisms mediated by epigenetic and post-translational modifications in disease-related autophagy to unveil potential therapeutic targets.In addition,the effect of autophagy on the therapeutic effectiveness of epigenetic drugs or drugs targeting post-translational modification have also been discussed,providing insights into the combination with autophagy activators or inhibitors in the treatment of clinical diseases.

Objective:To study the epidemiology of leprosy in China in 2019 and give suggestions to improve future leprosy control activities.Methods:We collected leprosy data from the Leprosy Management Information System in China,which includes data from 31 Chinese provinces,autonomous regions,and municipalities,except for hong Kong,Macao,and Taiwan.We then performed a descriptive analysis of these data.Results:A total of 464 new leprosy cases were reported nationwide in 2019,with a case detection rate of 0.033/100,000.The proportions of children younger than 15 years,multibacillary cases,and cases with World Health Organization grade 2 disability were 1.3%,90.3%,and 21.6%,respectively.A total of 2,219 leprosy cases were registered in China by the end of 2019,giving a prevalence rate of 0.158/100,000.Fifty-six counties had a prevalence rate of more than 1/100,000,and most of them were located in Southwest China.There were 31 reported relapse cases.Conclusion:The prevalence of leprosy has improved continuously throughout China,but the southwest region is still an area of concern.

Assessing myocardial indices and inflammatory factors to determine anxiety and depression severity in patients with chronic heart failure

BACKGROUND Patients with chronic heart failure(CHF)have a progressive disease that is associated with poor quality of life and high mortality.Many patients experience anxiety and depression(A&D)symptoms,which can further accelerate disease progression.We hypothesized that indicators of myocardial function and inflammatory stress may reflect the severity of A&D symptoms in patients with CHF.Changes in these biomarkers could potentially predict whether A&D symptoms will deteriorate further in these individuals.AIM To measure changes in cardiac and inflammatory markers in patients with CHF to determine A&D severity and predict outcomes.METHODS We retrospectively analyzed 233 patients with CHF treated at the Jingzhou Hospital,Yangtze University between 2018-2022 and grouped them according to Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)scores.We compared clinical data in the no-A&D,mild-A&D,moderate-A&D,and severe-A&D groups,the SAS and SDS scores with the New York Heart Association(NYHA)functional classification,and cardiac markers and inflammatory factors between the no/mild-A&D and moderate/severe-A&D groups.Regression analysis was performed on the markers with P0.05).In addition,we found that higher NYHA classes were associated with higher the SAS and SDS scores(P<0.05).Regression analysis showed that LVEF,NTproBNP,and IL-6 were independent risk factors for A&D severity(P<0.05).Among them,NT-proBNP had the best predictive ability as a single

BACKGROUND Diabetic kidney disease(DKD),characterized by increased urinary microalbumin levels and decreased renal function,is the primary cause of end-stage renal di-sease.Its pathological mechanisms are complicated and multifactorial;Therefore,sensitive and specific biomarkers are needed.Urinary exosome originate from diverse renal cells in nephron segments and partially mirror the pathological changes in the kidney.The microRNAs(miRNAs)in urinary exosome are remark-ably stable and highly tissue-specific for the kidney.METHODS Type 2 diabetic mellitus(T2DM)patients were recruited from the Second Hospital of Hebei Medical University and were divided into two groups:DM,diabetic pa-tients without albuminuria[urinary albumin to creatinine ratio(UACR)<30 mg/g]and DKD,diabetic patients with albuminuria(UACR≥30 mg/g).Healthy subjects were the normal control(NC)group.Urinary exosomal miR-145-5p,miR-27a-3p,and miR-29c-3p,were detected using real-time quantitative polymerase chain reaction.The correlation between exosomal miRNAs and the clinical in-dexes was evaluated.The diagnostic values of exosomal miR-145-5p and miR-27a-3p in DKD were determined using receiver operating characteristic(ROC)analysis.Biological functions of miR-145-5p were investigated by performing RESULTS Urinary exosomal expression of miR-145-5p and miR-27a-3p was more upregulated in the DKD group than in the DM group(miR-145-5p:4.54±1.45 vs 1.95±0.93,P<0.001;miR-27a-3p:2.33±0.79 vs 1.71±0.76,P<0.05)and the NC group(miR-145-5p:4.54±1.45 vs 1.55±0.83,P<0.001;miR-27a-3p:2.33±0.79 vs 1.10±0.51,P<0.001).The exosomal miR-145-5p and miR-27a-3p positively correlated with albuminuria and serum creatinine and negatively correlated with the estimated glomerular filtration rate.miR-27a-3p was also closely related to blood glucose,gly-cosylated hemoglobin A1c,and low-density lipoprotein cholesterol.ROC analysis revealed that miR-145-5p had a better area under the curve of 0.88[95%confidence interval(CI):0.784-0.985,P<0.0001]in diagnosing DKD than miR-27a-3p with

The Multiple Roles of Autophagy in Neural Function and Diseases

Autophagy involves the sequestration and delivery of cytoplasmic materials to lysosomes,where proteins,lipids,and organelles are degraded and recycled.According to the way the cytoplasmic components are engulfed,autophagy can be divided into macroautophagy,microautophagy,and chaperone-mediated autophagy.Recently,many studies have found that autophagy plays an important role in neurological diseases,including Alzheimer's disease,Parkinson's disease,Huntington's disease,neuronal excitotoxicity,and cerebral ischemia.Autophagy maintains cell homeostasis in the nervous system via degradation of misfolded proteins,elimination of damaged organelles,and regulation of apoptosis and inflammation.AMPK-mTOR,Beclin 1,TP53,endoplasmic reticulum stress,and other signal pathways are involved in the regulation of autophagy and can be used as potential therapeutic targets for neurological diseases.Here,we discuss the role,functions,and signal pathways of autophagy in neurological diseases,which will shed light on the pathogenic mechanisms of neurological diseases and suggest novel targets for therapies.

Objective:To describe the epidemiological characteristics of leprosy in China in 2020 and provide a basis for optimizing leprosy prevention and control strategies.Methods:We collected leprosy data from the Leprosy Management Information System in China,which includes data from 31 provinces,autonomous regions,and municipalities,except for hong Kong,Macao,and Taiwan,and then performed a descriptive analysis of these data.Results:A total of 406 newly detected leprosy cases were reported nationwide in China in 2020,giving an incidence rate of 0.29 per 1,000,000 population.Among the population with leprosy,the proportions of children younger than 15 years,multibacillary cases,and cases with grade 2 disability were 1.2%,93.3%,and 17.7%,respectively.Southwest China was the region with the largest proportion of new leprosy cases(54.7%).There were 31 relapse leprosy cases reported in 2020.By the end of 2020,there were 1,893 registered leprosy cases reported nationwide,giving a prevalence rate of 1.35 per 1,000,000 population.Conclusion:Leprosy is at a low endemic level in China in terms of the prevalence and incidence rates.However,the geographical distribution of leprosy in China is unbalanced;leprosy is highly endemic in southwest China,including Yunnan,Guizhou,Sichuan,Guangdong,and Guangxi.
