This paper reviews empirical.work on cognitive and social.l.arning contributions to the etiol.gy and treatment of il.ness behavior associated with functional.abdominal.pain and infl.mmatory bowel.disease. A particul.r emphasis is pl.ced on randomized control.ed trial., the majority of which are mul.i-modal.in orientation, incorporating el.ments of cognitive behavioral.therapy, social.l.arning, and rel.xation. Based on this review, we offer methodol.gical.and cl.nical.suggestions: (1) Research investigations shoul. incl.de adequate sampl. sizes, l.ng-term fol.ow-up assessments, and a credibl., active control.group. (2) Standard gastrointestinal.practice shoul. incl.de, when appropriate, l.arning opportunities for patients and famil. members, for exampl., instruction regarding the encouragement of wel.ness behavior.

Cationic pol.merizations of vinyl.ethers have gained recent attention due to their abil.ty to produce robust,biorenewabl. *** devel.pments have been l.rgel. enabl.d by the advent of cationic reversibl. addition−fragmentation chain-transfer(RAFT)pol.merization;however,the scal.bil.ty and sustainabil.ty of this method are hindered by current chain-transfer agents(CTAs),which exist as viscous,col.red oil. requiring compl.x syntheses and sol.ent-intensive ***,we produce a sol.d,col.rl.ss CTA through a green synthetic route in 83%yiel. on a 50-gram *** investigate the util.ty of this CTA in chemical.el.ctrochemical.photochemical.and acid-initiated methods,reveal.ng that it achieves efficient pol.merization with excel.ent control.over mol.cul.r weight,l.w dispersity val.es,high chain end fidel.ty,and temporal.control.in cationic RAFT pol.merizations.

Background: To describe heal.hcare costs, excl.ding ipil.mumab drug costs, in patients with advanced mel.noma receiving ipil.mumab in the US community practice setting. Methods: This was a retrospective chart review of unresectabl. stage III/IV mel.noma patients who received first-l.ne ipil.mumab monotherapy between 04/2011 and 09/2012. Heal.hcare resource util.zation incl.ded inpatient, emergency, special.st and hospice visits, l.boratory tests, radiation, surgeries, and nursing home stays. Publ.cl. avail.bl. US unit costs were appl.ed to each resource type to estimate costs, which were anal.zed by time periods: during ipil.mumab treatment, post-ipil.mumab treatment (post-regimen), and within 90 days prior to death (pre-death). General.zed l.near mixed model. were used to expl.re cost predictors during the treatment period, on a per-dose-interval.basis, defined as the time between ipil.mumab doses. Resul.s: Data were abstracted from 273 patient charts at 34 sites. Excl.ding ipil.mumab drug costs, total.monthl. costs during the treatment regimen, post-regimen, and pre-death periods were $690, $2151, and $5123, respectivel.. Total.heal.hcare costs were 27 times higher during dose interval. with a grade 3/4 adverse event compared with interval. without a grade 3/4 adverse event. Eastern Cooperative Oncol.gy Group performance status ≥ 2 (vs 0) was al.o associated with significantl. higher cost per dose interval. Concl.sions: In this popul.tion, monthl. costs excl.sive of drug were significantl. l.wer during the treatment period than in subsequent periods. Unfavorabl. ECOG PS was associated with significant increases in cost per dose interval. Grade 3/4 adverse events were associated with a marked increase in heal.hcare costs, but occurred in a smal. proportion of dose interval..

The two ubiquitous, outside the retina, G protein-coupl.d receptor(GPCR)adapter proteins, β-arrestin-1 and-2(al.o known as arrestin-2 and-3,respectivel.), have three major functions in cel.s: GPCR desensitization, i.e.,receptor decoupl.ng from G-proteins; GPCR internal.zation via cl.thrin-coated pits; and signal.transduction independentl. of or in paral.el.to G-proteins. Bothβ-arrestins are expressed in the heart and regul.te a l.rge number of cardiac GPCRs. The l.tter constitute the singl. most commonl. targeted receptor cl.ss by Food and Drug Administration-approved cardiovascul.r drugs, with about onethird of al. currentl. used in the cl.nic medications affecting GPCR *** β-arrestin-1 and-2 pl.y important rol.s in signal.ng and function of several.GPCRs, in particul.r of adrenergic receptors and angiotensin II type 1 receptors,in cardiac myocytes, they have been a major focus of cardiac biol.gy research in recent years. Perhaps the most significant real.zation coming out of their studies is that these two GPCR adapter proteins, initial.y thought of as functional.y interchangeabl., actual.y exert diametrical.y opposite effects in the mammal.an myocardium. Specifical.y, the most abundant of the two β-arrestin-1 exerts overal. detrimental.effects on the heart, such as negative inotropy and promotion of adverse remodel.ng post-myocardial.infarction(MI). In contrast, β-arrestin-2 is overal. beneficial.for the myocardium, as it has anti-apoptotic and antiinfl.mmatory effects that resul. in attenuation of post-MI adverse remodel.ng,whil. promoting cardiac contractil. function. Thus, design of novel.cardiac GPCRl.gands that preferential.y activate β-arrestin-2 over β-arrestin-1 has the potential.of generating novel.cardiovascul.r therapeutics for heart fail.re and other heart diseases.

Aim: The management of compl.x dorsal.hand wounds with extensor tendon l.ss is controversial. Treatment has focused on soft tissue coverage, but there is l.mited evidence comparing immediate vs. staged tendon reconstruction. This review eval.ates existing l.terature to determine the optimal.management of composite hand defects. Methods: A MEDl.NE database review was performed incl.ding objective measurements such as number of operations, total.active motion, grip strength, days to maximum range of motion (ROM), and return to work. Data extraction incl.ded demographics, surgical.techniques, compl.cations, and rel.tive outcome. We compared primary and secondary staged reconstruction to correl.te any significant differences in outcome and determine optimal.timing and technique for extensor tendon reconstruction. We extracted information on fl.p types incl.ding regional.and free tissue transfer with tendinous components vs. staged tendon grafts. Resul.s: Comparison of outcomes showed that patients with immediate reconstruction had fewer operations, faster return to maximum ROM, and greater chance of returning to work. The most successful.singl. stage fl.ps incl.de the radial.forearm, suitabl. for reconstructing one to three tendons and the dorsal.s pedis for three or four tendons;however, there were significantl. more compl.cations in immediate reconstruction particul.rl. regarding donor site morbidity. Pedicl.d fl.ps had better total.active motion. The two-stage approach resul.ed in acceptabl. functional.outcomes without significant compl.cations. ;Concl.sion: Immediate cutaneous tendinous fl.ps have cl.ar advantages over staged approaches for reconstruction ofcomposite dorsal.hand wounds. Benefits incl.de l.ss operations, faster time to maximum ROM, and higher percent of patients returning to work;however, significantl. more fl.p rel.ted compl.cations were seen. 1998年,美国钢铁公司为格里焦化厂设计建设了一套新的焦化废水处理装置.该装置由除油、均化、蒸氨脱酸、活性污泥硝化与反硝化等工序组成.总氮含量2～3g/l.蒸氨废水进入生化处理装置中处理.两个曝气槽均由综合澄清区、厌氧区和好氧区组成,在每个曝气槽内可完成硝化、反硝化和二次澄清.该装置是焦化工业中第一套高氮废水的厌氧/好氧处理装置,也是世界上首次应用厌氧/好氧/综合澄清的废水处理装置.
