Diagnostic accuracy of ePOS score in predicting DNr labeling after ICU admission:A prospective observational study(ePOS-DNr)

Background resuscitation can sometimes be futile and making a do-not-resuscitate(DNr)decision is in the best interest of the *** electronic poor outcome screening(ePOS)score was developed to predict 6-month poor outcomes of critically ill *** explored the diagnostic accuracy of the ePOS score in predicting DNr decisions in the intensive care unit(ICU).Methods This study was conducted at the ICU of a tertiary referral hospital in Saudi Arabia between March and May ***,we calculated ePOS scores for all eligible consecutive admissions after 48 h in the ICU and recorded the DNr *** ability of the score to predict DNr was explored using logistic ***'s ideal cut-off value was calculated using the DeLong method,and different diagnostic accuracy measures were generated with corresponding 95%confidence intervals(CIs).results We enrolled 857 patients,125 received a DNr order and 732 did *** average ePOS score of DNr and non-DNr patients was 28.2±10.7 and 15.2±9.7,*** score,as a predictor of DNr order,had an area under receiver operator characteristic(AUrOC)curve of 81.8%(95%CI:79.0 to 84.3,P17 was associated with a sensitivity of 87.2(95%CI:80.0 to 92.5,Predictive value of 29.2(95%CI:24.6 to 33.8,Predictive value of 96.7(95%CI:95.1 to 98.3,Pratio 12.1(95%CI:7.0 to 20.8,Pre performed well as a diagnostic test for patients who will be labeled as DNr during their ICU stay.A cut-off score>17 may help guide clinical decisions to withhold or commence resuscitative measures.

Fast and Efficient Security Scheme for Blockchain-Based IoT Networks

Internet of Things(IoT)has become widely used nowadays and tremendous increase in the number of users raises its security requirements as *** constraints on resources such as low computational capabilities and power requirements demand lightweight *** algorithms are not applicable in IoT network communications because of the constraints mentioned *** this work,a novel and efficient scheme for providing security in IoT applications is *** scheme proposes how security can be enhanced in a distributed IoT application by providing multilevel protection and dynamic key generation in the data uploading and transfer *** works rely on a single key for communication between sensing device and the attached gateway *** proposed scheme,this session key is updated after each session and this is done by applying principles of cellular *** proposed system provides multilevel security by using incomparable benefits of blockchain,dynamic key and random number generation based on cellular *** same was implemented and tested with the widely known security protocol verification tool called Automated Validation of Internet Security Protocols and Applications(AVISPA).results show that the scheme is secure against various *** proposed scheme has been compared with related schemes and the result analysis shows that the new scheme is fast and efficient also.

Multispectral imaging of acute wound tissue oxygenation

This paper investigates the appropriate range of values for the transcutaneous blood oxygen saturation(StO2)of granulating tissues and the surrounding tissue that can ensure timely wound *** work has used a multispectral imaging system to collect wound images at wave-lengths ranging between 520 nm and 600 nm with a resolution of 10 *** part of this research,a pilot study was conducted on three injured individuals with superfcial wounds of different wound ages at different skin *** S_(t)O_(2)value predicted for the examined wounds using the Extended Modified Lambert-Beer model revealed a mean S_(t)O_(2)of 61±10.3%compared to 41.6±6.2%at the surrounding tissues,and 50.1±1.53%for control *** preliminary results contribute to the existing knowledge on the possible range and variation of wound bed S_(t)O_(2)that are to be used as indicators of the functioning of the vasomotion system and wound *** study has concluded that a high S_(t)O_(2)of approximately 60%and a large fuctuation in this value should precede a good progression in wound healing.

An efficient permutation approach for SbPN-based symmetric block ciphers

It is challenging to devise lightweight cryptographic primitives efficient in both hardware and software that can provide an optimum level of security to diverse Internet of Things applications running on low-end constrained ***,an effcient hardware design approach that requires some specific hardware resource may not be effcient if implemented in *** bit Permutation Network based ciphers such as PrESENT and GIFT are effcient,lightweight cryptographic hardware design *** ciphers introduce confusion and diffu-sion by employing a 4×4 static substitution box and bit *** bit-wise permutation is realised by sim-ple rerouting,which is most cost-effective to implement in hardware,resulting in negligible power ***,this method is highly resource-consuming in software,particularly for large block-sized ciphers,with each single-bit permutation requiring multiple *** paper proposes a novel software-based design approach for permutation operation in Substitution bit Permutation Network based ciphers using a bit-banding *** conventional permutation using bit rotation and the proposed approach have been implemented,analysed and compared for GIFT and PrESENT ciphers on ArM Cortex-M3-based LPC1768 development platform with KEIL MDK used as an Integrated Development *** real-time performance comparison between conven-tional and the proposed approaches in terms of memory(rAM/rOM)footprint,power,energy and execution time has been carried out using ULINKpro and ULINKplus debug adapters for various code and speed optimisation *** proposed approach substantially reduces execution time,energy and power consumption for both PrE-SENT and GIFT ciphers,thus demonstrating the effciency of the proposed method for Substitution bit Permutation Network based symmetric block ciphers.

Landscape Characterization of Sariska National Park (India) and Its Surroundings

Landscape characterization gives an overall information on the status of Land Use and Land Cover （LULC）, changes in its composition and the impact of natural and human influences operating at different spatial and temporal scales. This information can be used to monitor changes in natural forest resources and protected areas, delineate potential conservation areas and can serve in effective management of ecologically fragile landscapes. In the present study, geo-spatial tools were used to characterize the landscape of Sariska National Park and its surroundings. Satellite data was used to prepare LULC maps for 1989 and 2000, change detection analysis and computation of landscape metrics. Climatic data, field records and modeling tools were used to map the potential spread of two invasive species, Prosopis juliflora and Adhatoda vasica. The results show that the forest area increased from 1989 to 2000, indicating better management practices. Landscape metrics （PAFrAC, PLADJ and AI） also support this argument. Improvements in the degraded forest can further enhance this effect. The entire reserve however is suitable for the invasion of P. juliflora and A. vasiea but is more pronounced in Boswellia serrata and Anogeissus pendula - Acacia catechu （open） forests. A detailed landscape characterization map can help forest managers to make important policy decisions concerning issues such as invasive species.
