One-year survival in recipients older than 50 bridged to heart transplant with Impella 5.5 via axillary approach

BACKGROUND Optimizing patients with advanced heart failure before orthotopic heart transplantation(OHT), especially in patients greater than 50 years old, is imperative to achieving successful post-transplant outcomes. Complications are well-described for patients bridged to transplant(BTT) with durable left ventricular assist device(LVAD) support. Given the lack of data available in older recipients after the recent increase in mechanical support use, we felt it crucial to report our center’s one-year outcomes in older recipients after heart transplantation with percutaneously placed Impella 5.5 as a *** Forty-nine OHT patients were supported with the Impella 5.5 intended as a bridge between December 2019 and October 2022 at Mayo Clinic in Florida. Data were extracted from the electronic health record at baseline and during their transplant episode of care after Institutional Review Boards approval as exempt for retrospective data *** Thirty-eight patients aged 50 or older were supported with Impella 5.5 as BTT. Ten patients underwent heart and kidney transplantation within this cohort. The median age at OHT was 63(58–68) years, with 32 male(84%) and six female patients(16%). Etiology was divided into ischemic(63%) and non-ischemic cardiomyopathy(37%). The baseline median ejection fraction was 19%(15–24). Most patients were in blood group O(60%), and 50% were diabetic. The average duration of support was 27 days(range 6–94). The median duration of follow-up is 488 days(185–693). For patients that have reached the 1-year follow-up timeframe(22 of 38, 58%), the 1-year post-transplant survival is 95%.CONCLUSION Our single-center data provides awareness for using the Impella 5.5 percutaneously placed axillary support device in older heart failure patients in cardiogenic shock as a bridge to transplantation. One-year survival outcomes after heart transplantation are excellent despite the older recipient’s age and prolonged pre-transplant support.

Seedling potential of trees species along the elevational gradient in temperate hill forest of central Nepal

Regeneration status of tree species along elevation gradient in temperate hill forest was not understood *** research examined the tree diversity and its regeneration patterns along an elevation gradient in temperate hill forest,central *** were collected from 300 sample plots within vertical elevation bands of 10,ranging from 1365 to 2450 m asl.A random sampling method was used for data collection in three seasons,winter,pre-monsoon and post monsoon *** at breast height(DBH)was used to broadly categorize the plant individual into trees,saplings and *** tree species richness ranged from 12 to 25 with density of 350 to 1200 individuals per *** richness of tree and sapling showed statistically significant unimodal pattern,which peaked at *** showed a strong and positive linear correlation with the seedling density(Deviance=0.99,pseedling(10.6%)≤mature(10.9%).Among tested environmental variables,elevation and annual mean rainfall were the most influential factors in the regeneration of tree species.

A dispersion model for the estimation of crosswind integrated concentrations in the surface-based inversion is *** generalized forms of eddy diffusivity with spatial dependence in both horizontal and vertical directions and vertical height-dependent wind speed are *** view of the computational limitation associated with numerical models for Dirac-delta function,the source term is expressed as a limiting case of normal *** accuracy of the employed numerical scheme to solve the resulting partial differential equation with appropriate physically relevant boundary conditions is checked with those obtained from the respective analytical solutions available in literature for the particular forms of eddy diffusivity and wind *** computed from the proposed model are found close to those obtained from analytical *** concentrations obtained from the proposed model are evaluated for the generalized functional forms of eddy diffusivity(Degrazia and Moraes,1992;Degrazia et al.,2001)and diabatic logarithmic profile as well as power-law profile of wind speed with the observations from Hanford(Doran et al.,1984)and Copenhagen(Gryning and Lyck,1984)diffusion experiments in stable and unstable conditions,*** of the cases i.e.,64%and 96%are predicted in factor of two to observations in both stable and unstable conditions,respectively.

Comparative Multivariate Analysis of Codon and Amino Acid Usage in Three Leishmania Genomes

Multivariate analysis of codon and amino acid usage was performed for three Leishmania species, including L. donovani, L. infantum and L. major. It was revealed that all three species are under mutational bias and translational selection. Lower GC12 and higher GC3s in all three parasites suggests that the ancestral highly expressed genes （HEGs）, compared to lowly expressed genes （LEGs）, might have been rich in AT-content. This also suggests that there must have been a faster rate of evolution under GC-bias in LEGs. It was observed from the esti- mation of synonymous/non-synonymous substitutions in HEGs that the HEG dataset of L. donovani is much closer to L. major evolutionarily. This is also supported by the higher ds value as compared to ds between L. donovani and L. major, suggesting the conservation of synonymous codon positions between these two species and the role of translational selection in shaping the composition of protein-coding genes.
