It is well-known that insulin can activate both PDK/Akt pathway,which is responsible for glucose uptake,and MAPK pathway,which is crucial for insulin resistance *** it is unclear exactly how the two pathways coordinate to regulate insulin sensitivity upon hyperinsulinism stress of type 2 diabetes mellitus(T2DM).Here we show that an early response transcription factor Egr-1 could tilt the signaling balance by blocking PI3K/Akt signaling through PTEN and augmenting Erk/MAPK signaling through GGPPs,resulting in insulin resistance in ***-1,PTEN and GGPPs are upregulated in the fat tissue of T2DM patients and db/db ***-1 overexpression in epididymal fat induced systematic insulin resistance in wild-type mice,and loss of Egr-1 function improved whole-body insulin sensitivity in diabetic mice,which is mediated by Egr-1 controlled PDK/Akt and Erk/MAPK signaling balance. Therefore,we have revealed,for the first time,the mechanism by which Egr-1 induces insulin resistance under hyperinsulinism stress,which provides an ideal pharmacological target since inhibiting Egr-1 can simultaneously block MAPK and augment PDK/Akt activation during insulin stimulation.

Metabolomics of gastric cancer metastasis detected by gas chromatography and mass spectrometry

AIM:To elucidate the underlying mechanisms of metastasis and to identify the metabolomic markers of gastric cancer ***:Gastric tumors from metastatic and nonmetastatic groups were used in this *** and different metabolic patterns were analyzed by gas chromatography,mass spectrometry and principal components analysis (PCA),*** performance was validated by the area under the curve (AUC) of receiver operating characteristic ***:Twenty-nine metabolites were differentially expressed in animal models of human gastric *** the 29 metabolites,20 were up-regulated and 9 were down-regulated in metastasis group compared to non-metastasis *** models from the metabolite profiles could differentiate the metastatic from the nonmetastatic specimens with an AUC value of *** metabolites were mainly involved in several metabolic pathways,including glycolysis (lactic acid,alaline),serine metabolism (serine,phosphoserine),proline metabolism (proline),glutamic acid metabolism,tricarboxylic acid cycle (succinate,malic acid),nucleotide metabolism (pyrimidine),fatty acid metabolism (docosanoic acid,and octadecanoic acid),and methylation(glycine).The serine and proline metabolisms were highlighted during the progression of ***:Proline and serine metabolisms play an important role in *** metabolic profiling of tumor tissue can provide new biomarkers for the treatment of gastric cancer metastasis.

Obesity is a medical condition with excess body fat accumulation to the extent which leads to serious health consequences. Abdominal obesity, also known as central obesity, refers to the presence of excess fat in the abdominal area. Obesity, especially abdominal obesity, contributes to many metabolic disorders including metabolic syndrome （Mets）, type 2 diabetes （T2DM） and cardiovascular diseases （CVD）. The incidence of obesity has increased dramaticaUy in recent yeats worldwide. In china, more than one-third of adults are overweight or obese and 10%-20% of all adults are affected by Mets. The pathogenesis underlying the abdominal obesity remains unclear. The ultimate health outcome of obesity and its related metabolic disorders have prompted physicians to take aggressive treatments （lifestyle changes, pharmacological interventions and surgical therapies） before a serious consequence becomes clinically apparent. In this review, we discuss the prevalence, pathogenesis and clinic features of obesity in china.

Promoting white adipose tissue(WAT)browning and enhancing brown adipose tissue(BAT)activity are attractive therapeutic strategies for obesity and its metabolic *** sympathetic innervation in WAT and BAT represents a promising therapeutic ***,there are few reports on extracellular microenvironment remodeling,especially changes in nerve terminal *** the key molecules mediating the neuro-adipose synaptic junctions is a key *** this study,we used bioinformatics methods to identify the differentially expressed predicted secreted genes(DEPsGs)during WAT browning and BAT *** DEPsGs largely reflect changes of cytokines,extracellular matrix remodeling,vascularization,and adipocyteneuronal *** then performed functional enrichment and cellular distribution specificity *** upregulated and downregulated DEPDGs during WAT browning displayed a distinctive biological pattern and cellular *** listed a cluster of adipocyte-enriched DEPsGs,which might participate in the cross-talk between mature adipocytes and other cells;then identified a synaptogenic adhesion molecule,Clstn3,as the top gene expressed enriched in both mature white and brown *** Q-PCR and immunohistochemistry,we found significantly increased Clstn3 expression level during WAT browning and BAT activation in mice subjected to cold exposure(4℃).We further demonstrated that treatment with isoproterenol significantly increased Clstn3 and UCP1 expression in differentiated white and beige adipocytes in *** conclusion,our study demonstrates that the secretion pattern was somewhat different between WAT browning and BAT *** reveal that Clstn3 may be a key gene mediating the neuro-adipose junction formation or remodeling in WAT browning and BAT activation process.

AIM:To investigate the endoscopy and histology of short-segment Barrett's esophagus (ssBE) and cardia intestinal metaplasia (CIM),and their correlation with Helicobacter pylori (H. pylori) gastritis and gastroesophageal reflux disease (GERD). METHODs:Biopsy specimens were taken from 32 ssBE patients and 41 CIM patients with normal appearance of the esophagogastric junction. Eight biopsy specimens from the lower esophagus,cardia,and gastric antrum were stained with hematoxylin/eosin,Alcian blue/periodic acid-schiff,Alcian blue/high iron diamine and Gimenez dye. Results were graded independently by one pathologist. REsULTs:The ssBE patients were younger than the CIM patients (P splasia and incomplete intestinal metaplasia subtype was higher in ssBE patients than in CIM patients (P s correlated with antral intestinal metaplasia (P symptomatic,endoscopic,or histological markers of GERD in CIM patients. ssBE was correlated with reflux symptomatic and endoscopic esophagitis (P stinal metaplasia. CONCLUsION:Dysplasia risk is significantly greater in ssBE patients than in CIM patients. CIM is a manifestation of H. pylori-associated and multifocal atrophic gastritis,whereas ssBE may result from GERD.

Infantile hemangioma(IH) is one of the most common benign tumors in infants characterized by occurrence within a few weeks after birth,rapid growth during the first year and spontaneous involution over a period of several *** the high incidence rate of 5%-10% in infants of mixed European descent,detailed pathogenesis of IH remains *** studies have indicated multipotential stem cells derived from hemangioma tissue(HemsCs) could recapitulate human infantile hemangioma in immunodeficient *** the effect of progesterone on regulation of cytokines and growth factors in endometrium as well as the inhibition of immune response,using progesterone during pregnancy might help the HemsCs escape from the immune response and reside in the tissue of embryo by the aid of increased MMPs and decreased TIMPs,then proliferation was stimulated by increased growth factors like VEGF and ***,IH is potentially produced.

阻塞性睡眠呼吸暂停(obstructive sleep apnea,OsA)是一种常见的慢性睡眠呼吸障碍疾病,可引起全身系统性损伤,导致沉重的社会经济负担。在OsA诊断(包括筛查)领域尚存在诸多未明确的问题,包括各种筛查诊断工具在OsA早发现及早诊断中的作用,对部分特殊人群进行早期OsA筛查及规范诊断的必要性,以及合适筛查工具的确定等。随着新的筛查、诊断技术的不断涌现和国际新标准的相继出台,目前的OsA诊断指南需要进一步的完善和更新。外科手术是OsA重要治疗方法,而这方面国内鲜有可用于指导临床实践的高质量指南共识。因此,结合国内外的循证证据构建规范化OsA诊断和外科治疗指南对于更好地指导当前OsA的临床实践至关重要。指南制订工作组将参考《世界卫生组织指南制订手册》,注册并撰写《成人阻塞性睡眠呼吸暂停诊断和外科治疗指南(2024)》计划书。本计划书主要介绍该指南制订的背景、意义、目的、人员组成,目标人群、干预措施、对比措施和结局指标(population、intervention、control、outcome,PICO)问题的确定,证据的获取及评价,以及推荐意见的形成等指南制订过程和具体方法。本计划书旨在为科学和循证制订本指南提供纲领性及指导性的实施方案。

Rupture of ovarian abscess following ultrasound-guided transvaginal oocyte retrieval for in-vitro fertilization in a patient with ovarian endometriomata:a case report

Ultrasound-guided transvaginal oocyte retrieval （TVOR） has become the gold standard for couples undergoing in-vitro fertilization （IVF）. Despite a relatively low complication rate following the procedure, here we report a rare case of a ruptured ovarian abscess presenting late after ultrasound-guided TVOR in a 32-year-old woman with ovarian endometriomata. Prompt intervention and proper choices of treatment led to rapid patient recovery with no long-term sequelae. Rupture of ovarian abscess needs to be included in the differential diagnosis of a patient presenting with abdominal pain following TVOR.

Costunolide alleviates HKsA-induced acute lung injury via inhibition of macrophage activation

staphylococcus aureus(***)infection leads to a severe inflammatory response and causes acute lung injury(ALI),eventually threatening human ***,it is of importance to find an agent to inhibit inflammation and reduce ***,we found that costunolide,a sesquiterpene lactone,displays anti-inflammatory effects and ameliorates heat-killed ***(HKsA)-induced *** treatment attenuated HKsA-induced murine ALI in which pulmonary neutrophil infiltration was inhibited,lung edema was decreased,and the production of pro-inflammatory cytokines was significantly *** addition,costunolide dose-dependently inhibited the generation of IL-6,TNF-α,IL-1β,and keratinocyte-derived cytokine(KC),as well as the expression of iNOs,in HKsA-induced ***,costunolide attenuated the phosphorylation of p38 MAPK and cAMP response element-binding protein(CREB).Collectively,our findings suggested that costunolide is a promising agent for alleviating bacterialinduced ALI via the inhibition of the MAPK signaling pathways.

Purpose:The aim of this study was to evaluate the feasibility of three-dimensional printing technology for the preparation and fabricate titanium/hydroxyapatite(Ti/HA)composite and functionally graded material(FGM)by three-dimensional printing,which providing the foundation theories and experimental bases of functionally graded material from designing,modeling,sintering and the performance evaluation. Methods:The CAD model of the Green Ti/HA composite was designed to be the cylinder(diameter 25mm,height 20mm)and set each layer with 100%bond area. The Ti/HA functionally graded material sample was divided into two layers,the upper layer with 100%Ti and the lower layer with 80%Ti/20%HA,which also design to be a CAD model of the cylinder with 25mm in diameter and 10 mm in *** three dimensional printing technology was used to process five specimens of 80%Ti/20%HA and three FGM *** process was comprised by debind sintering and the final *** specimens were first debinded under vacuum with 500℃.Then they were sintered under the protection of the argon gas with 1200℃*** sintered 80%Ti/20%HA composites were observed their microstructure,analyzed the diffraction element and tested in mechanical properties such as compressive *** the microstructure of sintered FGM specimens was also observed. Resu I ts:The sintered 80%Ti/20%HA specimens had the aperture size from 50 to 150μ*** XRD analysis of Ti/HA composite showed the characteristic peak contains Ca(PO),CaTiO,TiO and CaO features,which pointed that there occurred a chemical reaction between Ti and *** compressive strength was 184.3±*** Ti/HA functionally graded material,with its upper part for 80%Ti/20%HA layer and lower layer of 100%Ti,had graded structures with a visible decomposition of a clearer line between the two layers under the optical microscope.
