The Ready-Made Garments (RMG) industry is a major contributor to the economy of Bangladesh, accounting for over 80% of the country’s total exports. In recent years, the industry has faced challenges due to changing global fashion trends and increasing competition from other manufacturing countries. One of the latest trends in the fashion industry is micro seasonal fashion, which has significant impacts on the RMG industry of Bangladesh. The purpose of this study is to examine the impact of micro seasonal fashion on the RMG industry of Bangladesh. It will examine the changes in buyer and consumer behavior and demand patterns, the implications for manufacturers and suppliers, and the strategies adopted by industry players to adapt to this trend. The emergence of micro seasonal fashion has disorderly the traditional seasonal cycle of the fashion industry. Consumers are no longer satisfied with the two-season model of spring/summer and fall/winter, but instead demand new styles and trends every few weeks. This trend has created opportunities for the RMG industry of Bangladesh to cater to the fast-changing demands of consumers. Micro Seasonal Fashion also poses significant challenges for the industry. Manufacturers and suppliers must be able to produce and deliver garments quickly and efficiently, while ensuring high quality and sustainability standards. This requires investments in technology, supply chain management, and training of workers. To address these challenges, industry players in Bangladesh have adopted several strategies, such as diversifying product offerings, investing in technology and innovation, and enhancing sustainability practices. These strategies have helped the industry to remain competitive and meet the demands of fast-changing consumer preferences. The industry must be able to adapt quickly to the changing demands of consumers and invest in technology and sustainability practices to remain competitive. Tabletting Characteristics of the Selected Malaysian Herbs

This work describes a study of tabletting of selected Malaysian herbs such as Eurycoma longifolia Jack, Ficus deltoidea and Morinda citrifolia. These herbs are famous for their therapeutic properties and are used as herbal medicine or as medicinal food. A cylindrical uniaxial die of 13 mm was used to compress the herb powders into tablets. Pressures ranging from 7.5 MPa to 75 MPa were applied using a universal testing machine. Two feed weights, 0.5 g and 1.0 g, were used to produce the tablets. A binder, the microcrystalline cellulose known as Avicel, was used to investigate its effect on tabletting. Properties associated with both the binder and the compounds were studied, and binary mixtures with compositions ranging from 10% to 60% were examined. The strength of the herb tablets was then tested using an indirect tensile strength test. Upon compression, the Morinda citrifolia powder showed the highest density compared to that of Avicel. However, due to its high elasticity, the tensile strength of the Morinda citrifolia tablet was the lowest. The experimental data were also verified using an established model for compression property development. The results indicated that this simple approach can be used to understand the tabletting characteristics of these herbs.

Background: Ageing of the population is one of the most important demographic facts that come to the foreground in the 21st century. Objective: To assess the relationship between psychological factors (depression, loneliness, using anti anxiety medication, fear from falls and internal displacement of the population) and falls among elderly people in Baghdad city, Iraq. Methods: A matched 1:1 community based case-control study involving 716 elderly respondents, recruited randomly from six Non Governmental Organization (NGO) in different areas of Baghdad. Interviews to each respondent were done accordingly. Geriatric depression scale (GDS) was used to assess the depression among the elderly people. Results: The minimum age for cases and controls was 60 years old while the maximum age for cases was 87 and for controls was 85 years old. The female was predominant than male, 53.6% to 46.4% respectively. The relation between depression, fear from fall, using anti anxiety medication, internal displacement and falls was statistically significant

Surgical management of clinoidal meningiomas: 10 cases analysis

Aim: The purpose of this article is to advocate standard skull base technique for removing the clinoidal meningioma and to delineate the technique's advantages that aid in achieving an improved extent of tumor resection and enhancing the patients' overall outcome, specially their visual outcome. Methods: A retrospective analysis was performed on 10 consecutive patients with clinoidal meningiomas who underwent surgical resection at the Bangabandhu Sheikh Mujib Medical University and other private clinics between May 2013 and July 2016. A standard pterional craniotomy technique consisting of extradural anterior clinoidectomy, coupled with optic canal unroofing and optic nerve sheath opening was used in all patients. All patients had thorough preoperative and postoperative ophthalmological evaluations. The follow-up period ranged from 6 to 42 months. Results: Total resection was achieved in 5 (50.0%) of the 10 patients in this series. The majority of the patients with preoperative visual impairment experienced significant visual improvement 7 of 10 patients;70.0%). Conclusion: In the majority of patients with clinoidal meningiomas, total resection may be achieved with minimal complications. For large tumors encasing the optic nerve and internal carotid artery, or for those tumors causing preoperative visual impairment, use of the cranial base technique delineated in this study may lead to significant improvement in the patients' visual and overall outcomes.

Antiproteinuric Effect of Aliskiren versus Losartan in Primary Glomerulonephritis

Introduction: Treatment with renin-angiotensin-aldosterone-system (RAAS) blockers plays a major role in halting chonic kidney disease (CKD) progression. Aliskiren is the first orally available direct renin inhibitor (DRI). Objective: We studied the efficacy of aliskiren compared to losartan in patients with primary GN. Design and Method: This was a prospective open-label randomized control trial in patients with primary GN. Patients were randomized to receive either aliskiren or losartan to maximum tolerated doses for 24 weeks. Blood and urine investigations were measured at baseline and at 4-weekly intervals. Adverse effects were recorded. Results: 22 patients were recruited (aliskiren-11 and losartan-11). Their baseline characteristics were comparable with the exception of a higher proteinuria (uPCI) in the aliskiren arm. There were no significant differences in proteinuria, blood pressure and other renal parameters between both groups. At end-study, only patients in the aliskiren arm showed significant reductions in both the systolic blood pressure and in proteinuria. There were no changes in the other parameters of renal function over time and no adverse events occurred. Conclusion: Aliskiren appears to be as efficacious and tolerable as losartan both as an antihypertensive and antiproteinuric agent in this study.

<i>In-Vitro</i>Regeneration of <i>Citrus sinensis</i>(L.) Osbeck from Mature Seed Derived Embryogenic Callus on Different Solid Basal Media

In-vitro callus induction and regeneration method was developed using different plant growth regulators (PGRs), and basal media (Murashige and Skoog (MS), CHU (N6) and Gamborg (B5) media) of Citrus sinensis (L.) Osbeck. Observations of the effect of PGRs were carried out using different concentrations of 2,4-dichlorophenoxyacetic acid (2,4-D),1-naphthalene acetic acid (NAA) and combinations of 2,4-D and NAA using different basal media. This study found Citrus sinensis (L.) Osbeck exhibited a high frequency of callus induction on MS medium supplemented with 3 mg/L 2,4-D and callus induction frequency was 86.7% ± 3.4% whereas N6 and B5 showed lower callus induction frequency of 83.3% ± 8.8% and 82.2% ± 1.9% respectively compared to that of MS media with supplementation of the same hormone. Among the induced calli, the morphological analysis showed only 40% - 50% was embryogenic calli. Regeneration of plantlets from calli was done using different concentrations and combinations of auxin and cytokinin. The study showed that 3 mg/L 6-benzylaminopurine (BAP) supplemented medium has the maximum potential to promote regeneration of Citrus sinensis (L.) Osbeck from embryogenic calli with the frequency of 89.3% ± 8.8% but no regeneration occurred from the non-embryogenic calli. The regenerated plantlets were rooted on MS medium with supplementation of 5 mg/l NAA. These observations in Citrus sinensis (L.) Osbeck regeneration will be helpful for genetic improvement with desired traits.

Antiproteinuric Effect of Sulodexide versus Losartan in Primary Glomerulonephritis

Introduction: Limited data are available for the use of sulodexide in primary glomerulonephritis (GN). Objective: We studied the efficacy of sulodexide compared to losartan in patients with primary GN. Design and Method: This was a prospective, open labelled, randomized control trial in patients with stable primary GN. Patients were randomized to receive either sulodexide or losartan to maximum tolerated doses for 12 weeks. Blood and urine investigations were measured at baseline and at 4-weekly intervals. Adverse effects were recorded. Results: 18 patients were recruited (10-sulodexide and 8-losartan). Their baseline characteristics were comparable. At end study, patients in both groups showed no significant reduction in proteinuria and there were no differences between groups at each visit. Nonetheless, there was a trend towards lower protein uria in the losartan but not in sulodexide group. There were no changes in the other parameters of renal function or of coagulation over time. No adverse events in particular clinical bleeding occurred. Conclusion: Sulodexide and losartan did not demonstrate any significant anti-proteinuric effect in primary GN. Nevertheless, there was a trend of better proteinuria reduction in losartan group. Furthermore, other renal parameters were not significantly affected by both drugs.

The Efficacy and Safety of Saxagliptin in Haemodialysis Patients

Introduction: Good glycaemic control without causing excessive hypoglycaemia reduced the risk of macrovascular and microvascular complications in type 2 DM patients on regular haemodialysis (HD). The objectives of this study were to assess the efficacy and safety of add-on saxagliptin to insulin therapy in blood sugar control compared to insulin therapy alone in diabetic patients undergoing HD. Design and Methods: In this prospective open-labelled randomized controlled trial, HD patients with type 2 DM and on stable insulin therapy with HbA1c 7% - 13% were randomized to receive add-on saxagliptin 2.5 mg once daily to insulin therapy or insulin therapy only for 12 weeks. Results: 24 patients were randomized into each arm equally. Baseline and week-12 serum HbA1c, fructosamine, fasting blood glucose (FBS) and mean self monitoring blood glucose (SMBG) were comparable in the groups. Reduction of HbA1c and mean SMBG were significant in both groups. There was a significant drop in fructosamine levels (p = 0.004) and trend of lower FBS (p = 0.097) in add-on saxagliptin group but not in insulin alone group. The incidence of hypoglycaemia was the same in both groups. Conclusion: Add-on saxagliptin to insulin is comparable to insulin therapy alone in blood sugar control in regular HD patients and is safe and generally well tolerated. Add-on saxagliptin group may have more persistent and less fluctuation of glucose control compared to insulin only group.

Background: Depression is a common and serious medical illness around the world. It occurs more frequently in patients with chronic illness than in the general population. It is a common psychiatric problem in patients with chronic kidney disease (CKD). Objective: To assess the level of depression in pre-dialytic CKD patients attending at a tertiary care hospital in Bangladesh. Methodology: This cross sectional study was conducted at Department of Nephrology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from January 2020 to June 2020. A total of 100 pre-dialytic CKD patients were selected by convenience sampling technique and their level of depression was assessed by Zung Self-Rating Depression Scale (ZSDS). Data were collected by existing questionnaire with face to face interview and analyzed by statistical test. Results: The mean age of the participants was 49.70 ± 11.80 years. Among them 64% were male, 95% were muslim, 98% were married, 65% were completed secondary/above higher secondary level education, 65% were unemployed and 55% were lived in urban area. It was observed that 65% participants were economically dependent to their family, only 6% were engaged in regular exercise, 14% had family history of depression, 8% were smoker and 12% were alcohol/drug abuser. According to the Zung Self-Rating Depression Scale (ZSDS) we found 29% participants were mildly depressed, 39% participants were moderately depressed and 18% participants were severely depressed. Prevalence of depression was 86% in pre-dialytic CKD patients. Of them, 7% was in CKD stage-I, 17.4% was in CKD stage-II, 19.8% was in CKD stage-III, 22.1% was in CKD stage-IV and 33.7% was in CKD stage-V. Conclusion: Depression is highly prevalent in pre-dialytic CKD patients and more frequent in the advanced stages of CKD.
