Botulinum toxin type A for treating chronic low back pain:A double blinded randomized control study

BACKGROUND Low back pain(LBP)is a prevalent issue that orthopedic surgeons frequently address in the outpatient *** can arise from various causes,with stiffness in the paraspinal muscles being a notable *** administration of Botulinum toxin type A(BoNT-A)has been found to alleviate back pain by relaxing these stiff *** BoNT-A is approved for use in numerous conditions,a limited number of randomized clinical trials(RCTs)validate its efficacy specifically for treating *** To study the safety and the efficacy of BoNT-A in minimizing pain and improving functional outcomes in patients of chronic LBP(CLBP).METHODS In this RCT,adults aged 18-60 years with mechanical LBP persisting for at least six months were *** were allocated to either the Drug group,receiving 200 Ipsen Units(2 mL)of BoNT-A,or the Control group,which received a 2 mL *** a 2-month follow-up period,both groups were assessed using the Visual Analog Scale(VAS)for pain intensity and the Oswestry Disability Index(ODI)for disability at the start and conclusion of the study.A decrease in pain by 50%was deemed clinically *** The study followed 40 patients for two months,with 20 in each group.A clinically significant reduction in pain was observed in 36 *** was a statistically significant decrease in both VAS and ODI scores in the groups at the end of two ***,when comparing the mean score changes,only the reduction in ODI scores(15 in the placebo group vs 16.5 in the drug group,clinically insignificant)was statistically significant(P=0.012),whereas the change in mean VAS scores was not significant(P=0.45).CONCLUSION The study concludes that BoNT-A does not offer a short-term advantage over placebo in reducing pain or improving LBP scores in CLBP patients.

Hybrid IPOM: A Novel Technique for the Management of Incisional Hernia

Background and Aim: The incidence of incisional hernias has been reported to be around 15%. In the present scenario, a wide array of surgical procedures are available for their better management. In this study, we intend to share our experience with one novel technique, “Hybrid IPOM (Intraperitoneal onlay meshplasty)” as a management option for a selected cohort of patients. Methods: This prospective study was undertaken during January 2019 to July 2023 at King Abdullah medical city, Makkah. A total of 51 cases were selected for Hybrid IPOM repair as per inclusion criteria;medium sized (4 - 10 cm) hernia defects;uncomplicated hernias;age more than 18 years. The follow-up period of the patients varied from 6 months to 4 years. The operation commenced with open hernia dissection, mesh deployment into abdomen, defect closure and then conversion to laparoscopy for the posterior mesh placement. Results: A total of 51 cases were repaired successfully with this technique. 48 out of 51 cases were incisional hernias secondary to some primary procedure done either for hernias itself or some other intra-abdominal pathology. The three cases were primary hernias falling in medium to large category with unaesthetic overlying skin. The age range was 19 to 72 years. The mean (range) operative time was 135 (90 - 240) min, and the average blood loss was 70 ml. The mean (range) hospital stay was 3 (2 - 11) days. All patients returned to routine work within 2 - 3 weeks of surgery. The median follow-up was 15 (6 - 48) months. Of the 51 cases, 3 patients developed seroma (managed conservatively), 1 patient developed a large hematoma (needed evacuation), and 1 patient developed superficial wound infection (managed with antibiotics). Two patients had recurrences;one patient had previously failed multiple repairs, and the other developed a postoperative hematoma. None of our patients had an iatrogenic bowel injury. Citrus family especially Grapefruit,has attained considerable attention due to the presence of a number of essential components that have cardiovascular and anti-hypertensive *** juice and essential oil extracted from eight recently released grapefruit cultivars were used to study physicochemical and antioxidant *** total soluble solids(TSS),titratable acidity(TA) and pH of juice samples extracted from various grapefruit varieties differed *** refractive index,specific gravity and optical rotation values for the oil varied from 1.473 to 1.396,0.863 to 0.847 and +93 to +86,*** percent 2,2-diphenyl-1-picrylhydrazyl radical activity(% DPPH activity) and ferric reducing antioxidant power(FRAP) values for grapefruit juice and peel oil varied from 24.06 to 18.79,2.91 to 1.44 mmol g^–1 and 84.87 to 74.73,7.76 to 5.73 mmol g^-1,*** were significant differences in physicochemical,antioxidant properties and volatile profiles of extracted juice and *** oil exhibited higher DPPH and FRAP values than the *** different components identified which accounted for over 99% of the volatile fraction,limonene,myrcene,and benzopyran were major components in all oil samples.

Lifestyle behaviours of immigrant and Australian children:Evidence from a nationally representative sample

Input-parallel output-parallel configuration of boost half bridge DC-DC converter with control strategy

The concept of connecting two boost half bridge DC-DC converter modules in input-paral- lel output-parallel configuration is presented. The input-parallel-output-parallel （IPOP） converter consists of multiple boost half bridge （BHB） DC-DC converter modules which are connected in par- allel at the input and output side. This kind of converter is an attractive solution for high power ap- plications. The correlation between input current sharing （ICS） and output current sharing （OCS） of the IPOP converter basic modules is described. Two loop control strategies, consisting of input cur- rent loop and output voltage loop, have been developed to achieve equal ICS and OCS in this present work. The control strategy for the IPOP configuration of boost haft bridge DC-DC converter has been verified for different load conditions （half load and full load）, The IPOP system proposed here is comprising of two modules but it can be extended to three or more. The performance of the pro- posed system along with the control strategy is verified by simulation in MATLAB using Simpower tool. Finally the satisfactory simulation results are obtained.

Simulation of multiphase boost DC-DC converter with the stable control strategy

The multiphase boost DC-DC converter with stable control strategy is presented. Multi- phase boost DC-DC converter is designed for high voltage and high power applications, and could be achieved by the adjustment of voltage doubler rectifiers on the secondary side of high frequency transformers. The stable control strategy for three phase boost DC-DC converter has been utilized during simulation in this study and this strategy can be extend to N-number of phases. The stable control strategy consists of only three voltage loops, which are sufficient for appropriate and efficient operation of three phase boost DC-DC converter. With the stable control strategy, the equal power balance sharing can be obtained between input and output. The stability of control strategy has been evaluated by simulating the multiphase boost DC-DC converter for the same and mismatch turn ratios of high frequency transformers. The simulation result is good and the objective of the strategy is a- chieved.
