Biallelic germline mutations in the mismatch repair genes, including MLH1, MSH2, MSH6 or PMS2, lead to a recessive constitutional mismatch repair-deficiency (CMMR-D) syndrome characterized by early onset malignancies in children and young adults. Because consanguinity unmasks autosomal recessive disorders, we hypothesized that the frequency of CMMR-D is inflated in the highly consanguineous population of Saudi Arabia. In this study, 371 pediatric and young adult?patient samples from Saudi Arabia that cover the tumor spectrum of CMMR-D syndrome were analyzed for biallelic germline mutations in the MLH1, MSH2, MSH6 and PMS2 with the use of direct genomic sequencing. However, none of the 371 patients involved in the study was found to have biallelic pathological mutations of MLH1, MSH2, MSH6 or PMS2. This result indicates that CMMR-D is exceptionally rare among pediatric cancer patients and adult early onset cancer patients, even in the highly consanguineous Saudi population. Our findings suggest that larger cohorts will be needed, particularly in outbred populations, to determine the frequency of CMMR-D and that routine screening for this syndrome among cancer patients is not warranted.

Homotopy analysis method (HAM) is employed to investigate amperometric biosensor at mixed enzyme kinetics and diffusion limitation. Mathematical modeling of the problem is developed utilizing non-Michaelis-Menten kinetics of the enzymatic reaction. Different results of the problem are obtained for different values of the dimensionless parameters. Accuracy of the obtained results is verified by comparing them with the available actual and simulated ones. It is concluded that the obtained solution can be considered as a promising one to investigate different aspects of the phenomena.

Nanopharmaceuticals containing quantum dot nanoparticles (Q-Dot NPs) for treating serious cancers such as breast cancer have made fantastic proposals. In this study, ZnO quantum dot NPs are formulated via ZnO@PVP nanopolymer as co-assistants coordinating with efficacious suitable wetting agents, PEG-binding compound, and W/O emulsifier for producing eco-friendly water-based nanodrug. Several characterization techniques containing SEM, TEM, FTIR, photoluminescence, zeta potential, and UV-Vis absorption were employed for ZnO Q-Dot NPs in nanodrug. This work aims to investigate the anti-tumor effects of such nanomedicine on the 4T1 breast cancer cell line in BALB/c mice, being elaborated through intraperitoneal, injection (IVP) and oral therapy. The impressive findings showed that ZnO nanodrug caused changes in blood factors, having the most effectiveness at 40 μg/ml concentration after two weeks of oral treatments. The significant increase in white blood cells (WBC) neutrophils and meaningful decreases in lymphocytes and especially cholesterol were powerful simultaneous impacts, successfully treating malignant breast cancer masses. In this significant animal model research for breast cancer, the sick mice recovered entirely and even had a safe space to mate. Histopathological results showed no evidence of breast tumor formation or metastasis in the group treated with nanodrug and their children. This nanomedicine has a therapeutic effect, and is ready to be applied for treating volunteer breast cancer patients. However, its prevention (inhibitory) effect can also be analyzed and added to current data in future studies.

Ergonomic modifications toprevent musculoskeletal injury, including placement of endoscopic monitor at eye level and thecardiac monitor in front, stopping the procedure to move patients, sitting while performingcolonoscopy, and navigating height-adjustable bed were used by 21.4%. Colorectal cancer(CRC)is a predominant life-threatening cancer,with liver and peritoneal metastases as the primary causes of *** inflammation,a known CRC risk factor,nurtures a local inflammatory environment enriched with tumor cells,endothelial cells,immune cells,cancer-associated fibroblasts,immunosuppressive cells,and secretory growth *** complex interactions of aberrantly expressed cytokines,chemokines,growth factors,and matrix-remodeling enzymes promote CRC pathogenesis and evoke systemic responses that affect disease *** evidence suggests that these cytokines and chemokines play a role in the progression of CRC through immunosuppression and modulation of the tumor microenvironment,which is partly achieved by the recruitment of immunosuppressive *** cells impart features such as cancer stem cell-like properties,drug resistance,invasion,and formation of the premetastatic niche in distant organs,promoting metastasis and aggressive CRC growth.A deeper understanding of the cytokineand chemokine-mediated signaling networks that link tumor progression and metastasis will provide insights into the mechanistic details of disease aggressiveness and facilitate the development of novel therapeutics for ***,we summarized the current knowledge of cytokine-and chemokine-mediated crosstalk in the inflammatory tumor microenvironment,which drives immunosuppression,resistance to therapeutics,and metastasis during CRC *** also outlined the potential of this crosstalk as a novel therapeutic target for *** major cytokine/chemokine pathways involved in cancer immunotherapy are also discussed in this review.

Objective： The high burden of injuries in Iran necessitates the establishment of a comprehensive trauma care system. The purpose of this paper is to describe the current status of trauma system regarding the components and function. Methods： The current status of trauma system in all components of a trauma system was described through expert panels and semi-structured interviews with trauma specialists and policy makers. Results： Currently, various organizations are involved in prevention, management and rehabilitation of injuries, but an integrative system approach to trauma is rather deficient. There has been ongoing progress in areas of public education through media, traffic regulation reinforcement, hospital care and prehospital services. Meanwhile, there are gaps regarding financing, legislations and education of high risk groups. The issues on education and training standards of the front line medical team and continuing education and evaluation are yet to be addressed. Trauma registry has been piloted in some provinces, but as it needs the well-developed infrastructure （regarding staff, maintenance, financial resources）, it is not yet established in our system of trauma care. Conclusions： It seems that one of the problems with trauma care in Iran is lack of coordination among trauma system organizations. Although the clinical management of trauma patients has improved in our country in the recent decade, decreasing the burden of injuries necessitates an organized approach to prevention and management of trauma in the context of a trauma system.

Flyrock is a significant environmental and safety concern in mining and *** arises from various geological and blast design factors,posing risks to workers,machinery,and nearby *** study examined how these factors affect the rate and distance of flyrock projections caused by *** address this issue,advanced machine learning(ML)models were used to predict flyrock distances in the Akoko Edo dolomite *** models examined included bidirectional recurrent neural networks(BRNNs),support vector regression(SVR)with different kernels(SVR-S,SVR-RBF,SVR-L,SVR-P),long short-term memory(LSTM)networks,and random forest(RF)algorithms.A case study was conducted using 258 blasting data samples to develop these *** factors influencing flyrock were identified:blast hole burden distance,maximum instantaneous charge,and rock brittleness *** these factors,a flyrock possibility assessment chart was created to enhance the safety of small-scale mining *** model’s prediction accuracy was evaluated using correlation coefficients and four performance *** LSTM model stood out,achieving the highest coefficient of correlation(R2=0.99)for both training and testing *** indicates that the LSTM model accurately predicts blast-induced flyrock *** study also revealed that the Gaussian-RBF kernel SVR has high prediction accuracy when compared to other SVR variants(SVR-S,SVR-L,and SVR-P).In conclusion,the study compared various ML models for flyrock reduction and found that the LSTM model was the most effective in estimating blast-induced flyrock distances.
