集束化护理预防颈椎前路术后咽喉刺激症

[目的]探讨集束化护理在预防颈椎前路术后咽喉刺激症的临床效果。[方法]回顾性分析本院收治的172例行颈椎前路手术患者的临床资料,其中2022年7月—2022年11月收治的83例患者行常规护理,2022年12月—2023年4月收治的89例患者实施集束化护理,比较两组Bazaz吞咽困难评级、吞咽生存质量(swallowing quality of life,SWAL-QOL)以及舒适状况(general comfort questionnaire,GCQ)的评分。[结果]两组均顺利手术,无严重并发症。集束组在术后第2、5 d的Bazaz评级[无/轻/中/重,(49/23/12/5)vs(41/18/14/10),P=0.004;(80/7/2/0)vs(50/18/10/5),P=0.015],SWAL-QOL评分[(56.1±7.9)vs(50.6±10.0),P=0.014;(60.1±5.9)vs(52.3±9.8),P<0.001]和GCQ评分[(93.3±7.1)vs(89.3±10.4),P=0.003;(96.1±3.4)vs(93.5±7.5),P=0.002]均显著优于常规组;此外,集束组的咽喉刺激症发生率显著低于常规组[例(%),6(6.7)vs 17(20.0),P<0.001]。[结论]在颈椎前路手术后应用集束化护理对患者进行干预,有利于预防咽喉刺激症的发生,提升患者就医体验。

The purpose of this article is to extend some spectral properties of regular Sturm- Liouville problems to the special type discontinuous boundary-value problem, which consists of a Sturm-Liouville equation together with eigenparameter-dependent boundary conditions and two supplementary transmission conditions. We construct the resolvent operator and Green＇s function and prove theorems about expansions in terms of eigenfunctions in modified Hilbert space L2[a, b].

A study is presented concerning a cold plasma technique for improving the bondability of high-strength high-modulus multi-filament polyethylene fibers to polymer matrices and the fibers impregnation with the objective to fabricate composite materials (CMs). Strong bonding between the matrixes and reinforcing fibers during the production of composites appears in the case if interaction is chemical. The value of the activation energy of the chemical interaction for very high performance polyethylene fiber was estimated. It was 1.14 eV. This allows using the cold plasma technique for producing CMs. In order to understand the effect of cold plasma treatment treated and untreated fibers were used to fabricate CMs. The strong bond between the matrix and plasma-activated fibers affects both the properties and failure mode of composite. The properties and failure modes were compared to those of CMs reinforced with untreated fibers. After plasma treatment the properties of CMs are increased. CMs are broken as a unit whole under tension. The ideas of the activating the fibers by cold plasma treatment above the activation energy of the chemical interaction may be extended over other types of the fibers and matrices to produce new types of fiber-reinforced composite materials with high physicomechanical indices.

Percutaneous Mitral Valvotomy in Children and Adolescents: Immediate and Midterm Results

Objective: To analyze immediate and midterm results of percutaneous mitral valvotomy (PMV) in patients ≤ 18 year, a single centre experience of Sohag university hospital. Methods: Between May 2010 and May 2012, 64 procedures of Inoue technique mitral valvotomy were performed on 64 patients. The mean age was 15.8 ± 2.2 years;75% were females. Results: Success was obtained in 96.8% of patients. Immediate complications were severe mitral regurgitation (3.1%), cardiac tamponade (1.6%) and embolic stroke in (1.6%). Late follow-up was obtained in 93.7% of the patients (at 6 and 24 months). Restenosis developed in 3 patients. Most of the patients had no symptoms at discharge but most of them had NYHA class I to II at 6 months to 2 years follow up. No patients died. Patients who operated early with MVA around 1.4 had best outcome. Conclusion: PMV represents a safe therapeutic option in young patients with mitral valve stenosis. In these patients, recurrence of symptoms and restenosis occurs at an earlier stage than in elderly. Earlier intervention carries better outcome and decreases restenosis rate.

The article provides a phytocenotic description of Acanthophyllum cyrtostegium, distributed in the sandy and gypsum soils of Bukhara region (Uzbekistan). During the study, 6 plant communities were studied with the participation of the field work. There were 70 pcs vascular plants in these communities. The area of this plant, which is rare in the region, is 10 - 12 hectares, and the total number of 750 - 850 pcs. The results obtained from the evaluation of populations are used in carrying out monitoring studies for long years on rare plants. At the same *** used it as a material for local “Red books”.

Kieselguhr for chromatography was loaded with Cyanex 572 extractant and characterized using X-ray diffraction(XRF),scanning electron microscopy(SEM),Fourier transform infra-red(FT-IR),and thermal *** of some heavy lanthanides together with yttrium by the prepared kieselguhr/Cy-572 sorbent was investigated by batch experiment in terms of initial metal ions concentrations,volume to solid mass ratio(V/m),sulfuric acid concentration as well as *** sorption equilibrium of Cy-572 loaded kieselguhr was satisfactorily explained by the pseudo second-order kinetic model and Langmuir isotherm ***,the values of thermodynamic parameters △G°,△H°,and △S°at25℃of the studied metal ions sh.w that the process is spontaneous and endothermic in nature.2.0 mol/L HNO3 is promising to desorb these metal ions.

A new type of chiral 1,1'-binaphthol(BINOL) derived samarium complex, prepared from (R)- or (S)-BINOL and SmCl3, serves as an enantioselective catalyst for the Meerwein-Ponndorf-Verley (MPV) reduction in the presence of Molecular Sieves 4 Angstrom(MS 4 Angstrom). Moderate enantioselectivity was obtained. Kinetic studies sh.w slight decrease of the enantiomeric excess with the conversion.
