Osteoarticular manifestations of human brucellosis:A review

Brucellosis is a common global zoonotic disease, which is responsible for a range of clinical manifestations. Fever, sweating and musculoskeletal pains are observed in most patients. The most frequent complication of brucellosis is osteoarticular involvement, with 10% to 85% of patients affected. The sacroiliac(up to 80%) and spinal joints(up to 54%) are the most common affected *** and spondylodiscitis are the most frequent complications of brucellar spinal involvement. Peripheral arthritis, osteomyelitis, discitis, bursitis and tenosynovitis are other osteoarticular manifestations, but with a lower prevalence. Spinal brucellosis has two forms: focal and diffuse. Epidural abscess is a rare complication of spinal brucellosis but can lead to permanent neurological deficits or even death if not treated promptly. Spondylodiscitis is the most severe form of osteoarticular involvement by brucellosis, and can have single-or multifocal involvement. Early and appropriate diagnosis and treatment of the disease is important in order to have a successful management of the patients with osteoarticular brucellosis. Brucellosis should be considered as a differential diagnosis for sciatic and back pain, especially in endemic regions. Patients with septic arthritis living in endemic areas also need to be evaluated in terms of brucellosis. Physical examination, laboratory tests and imaging techniques are needed to diagnose the disease. Radiography, computed tomography, magnetic resonance imaging(MRI) and bone scintigraphy are imaging techniques for the diagnosis of osteoarticular brucellosis. MRI is helpful to differentiate between pyogenic spondylitis and brucellar spondylitis. Drug medications(antibiotics)and surgery are the only two options for the treatment and cure of osteoarticular brucellosis.

Single-and double-dose of platelet-rich plasma versus hyaluronic acid for treatment of knee osteoarthritis: A randomized controlled trial

BACKGROUND Platelet-rich plasma(PRP)and hyaluronic acid have been shown to be useful in the treatment of knee ***,investigations comparing the efficacy of these two drugs together are *** To compare the outcomes of PRP vs hyaluronic acid injections in three groups of patients with bilateral knee *** This randomized controlled trial study involved 95 ***-one subjects received a single injection of PRP(group PRP-1),33 subjects received two injections of PRP at an interval of 3 wk(group PRP-2)and 31 subjects received three injections of hyaluronic acid at 1-wk intervals(group hyaluronic acid).The patients were investigated prospectively at the enrollment and at 4-,8-and 12-wk follow-up with the Western Ontario and McMaster Universities Arthritis Index(WOMAC)and Visual Analogue Scale *** Percentages of patients experiencing at least a 30%decrease in the total score for the WOMAC pain subscale from baseline to wk 12 of the intervention were 86%,100%and 0%in the groups PRP-1,PRP-2 and hyaluronic acid,respectively(P<0.001).The mean total WOMAC scores for groups PRP-1,PRP-2 and hyaluronic acid at baseline were 63.71,61.57 and 63.11,*** WOMAC scores were significantly improved at final follow-up to 42.5,35.32 and 57.26,*** highest efficacy of PRP was observed in both groups at wk 4 with about 50%decrease in the symptoms compared with about 25%decrease for hyaluronic *** PRP-2 had higher efficacy than group *** major adverse effects were found during the *** PRP is a safe and efficient therapeutic option for treatment of knee *** was demonstrated to be significantly better than hyaluronic *** also found that the efficacy of PRP increases after multiple injections.

Human chorionic gonadotropin in vitro: Effects on rat sperm motility and fertilization outcome

Objective:To investigate the effect of human chorionic gonadotropin (hCG) on the motility of rat sperm and the fertilization rate following in vitro fertilization. Methods: hCG concentrations of 25, 50, 75 and 100 ng/mL with incubation time of 1 h and 2 h were used in medium to study sperm motility. Then, 25 and 100 ng/mL of hCG with incubation time of 2 h were applied for in vitrofertilization. After 6 h, the number of two pronuclei was counted. Obtained data was subjected to one way ANOVA and Bonferroni-Dun hoc-post ***:Total motility and progressive motility of sperms were increased significantly (P<0.05) in hCG experimental group with concentration of 25 ng/L and incubation time of 2 h compared to control group, however, total motility and progressive motility of sperms were decreased significantly (P<0.05) with increasing dose of hCG. Fertilization rate was decreased significantly (P<0.05) in hCG experimental group with dose of 100 ng/mL compared to 25 ng/mL and control group. Conclusions:The effect of hCG on the rat sperm motility and the rate of their fertility is dose-dependent so that hCG with dose of 25 ng/mL leads to an increase in rat sperm motility and to some extent fertilization rate.

Objective： The high burden of injuries in Iran necessitates the establishment of a comprehensive trauma care system. The purpose of this paper is to describe the current status of trauma system regarding the components and function. Methods： The current status of trauma system in all components of a trauma system was described through expert panels and semi-structured interviews with trauma specialists and policy makers. Results： Currently, various organizations are involved in prevention, management and rehabilitation of injuries, but an integrative system approach to trauma is rather deficient. There has been ongoing progress in areas of public education through media, traffic regulation reinforcement, hospital care and prehospital services. Meanwhile, there are gaps regarding financing, legislations and education of high risk groups. The issues on education and training standards of the front line medical team and continuing education and evaluation are yet to be addressed. Trauma registry has been piloted in some provinces, but as it needs the well-developed infrastructure （regarding staff, maintenance, financial resources）, it is not yet established in our system of trauma care. Conclusions： It seems that one of the problems with trauma care in Iran is lack of coordination among trauma system organizations. Although the clinical management of trauma patients has improved in our country in the recent decade, decreasing the burden of injuries necessitates an organized approach to prevention and management of trauma in the context of a trauma system.

Incidence of Enterobius vermicularis in acute appendicitis: A systematic review and meta-analysis

Objective: To evaluate the incidence of Enterobius vermicularis (E. vermicularis) in appendectomies in Iran from 1993 to 2018 . Methods: Data were systematically collected on the electronic databases including PubMed, GoogleScholar, ScienceDirect, Scopus, Web of Science, Magiran, Irandoc (Bareket), IranMedex, and Scientific Information Database in English or Persian. Six articles including 29694 cases of appendicitis with 258 positive cases of infection withE. vermicularis were eligible to enter into this systematic review and meta-analysis. Results: The overall prevalence of E. vermicularis in appendicitis in Iran was 1% (95%CI=0.00-0.02) with the highest rate of 3% (95%CI=0.02-0.03) and lowest rate of 0% (95%CI=0.00-0.01). Conclusions: Prevalence of E. vermicularis infectionin appendicitis in Iran is low and has no statistically significant correlation with age and gender.

Analytical investigation of temperature of a single micron sized iron particle during combustion

The present study deals with analytical investigation of temperature of a single burning iron *** mathematical methods including AGM(Akbari-ganji’s method),CM(Collocation method)and GM(Galerkin Method)are applied to solving non-linear differential governing equation and effectiveness of these methods is examined as *** further investigation,forth order Runge-Kutta approach,a numerical method,is used to validate the obtained analytical *** the present study,the developed mathematical model takes into account the effects of thermal radiation,convective heat transfer and particle density variations during combustion *** to particles’small size and high thermal conductivity,the system is assumed to be lumped in which the particle temperature does not change within the body and all of its regions are at the same *** temperature distributions obtained by analytical methods have satisfactory agreement with numerical ***,the results indicate that AGM is a more appropriate method than GM and CM due to its lower mean relative error and less run time.
