Asian variant intravascular large B-cell lymphoma with highly suspected central nervous system involvement:A case report

BACKGROUND Intravascular large B-cell lymphoma(IVLBCL)is a rare subtype of extranodal *** particular,the Asian variant of IVLBCL is characterized by hemophagocytic lymphohistiocytosis along with bone marrow ***,central nervous system(CNS)involvement is uncommon in this variant compared to the Western ***,we report a case of typical Asian variant IVLBCL with highly suspected CNS involvement and discuss the nature of the disease and its genetic *** SUMMARY A 67-year-old female patient complained of gradually worsening cognitive *** hospitalized,she developed a high fever and showed marked *** imaging revealed a suspected leptomeningeal *** no malignant cells were found in the cerebrospinal fluid(CSF),the protein and lactate dehydrogenase levels in CSF were *** marrow examination revealed an increased number of hemophagocytic histiocytes,and 18F-fluorodeoxyglucose(FDG)positron emission tomography with computerized tomography scan revealed increased FDG uptake in both adrenal glands,the liver,and the right ethmoid sinus.A tissue biopsy showed atypical large lymphoid cells with prominent nucleoli in the vessels,and the tumor cells were positive for CD20,BCL2,BCL6,and IRF4/*** addition,targeted sequencing identified MYD88,TET2,and PIM1 ***,we diagnosed the patient with the Asian variant of IVLBCL with highly suspected CNS *** Suspicion of IVLBCL and immediate diagnosis lead to timely ***,careful CNS examination at diagnosis is recommended.

Sequential spinal and intracranial dural metastases in gastric adenocarcinoma: A case report

Dural metastasis from primary gastric adenocarcinoma has been rarely reported, and its prognosis is very poor because it frequently leads to acute subdural hematoma. Here, we describe a case with sequential spinal and cranial dural metastases from gastric adenocarcinoma without subdural hematoma. A 43-yearold woman with gastric adenocarcinoma and wellcontrolled peritoneal carcinomatosis presented with back pain, right radiating leg pain, left facial palsy, and hearing loss. Magnetic resonance imaging of the spine and brain revealed dural masses at the lumbosacral junction with invasion to the L5 and S1 nerve roots and at the skull base with invasion to the internal auditory canal. She was treated with local radiotherapy, and her pain and neurologic symptoms improved after palliative radiotherapy. This is the first reported case of dural metastases of gastric adenocarcinoma of the spine and skull base but with a relatively indolent course and without subdural hematoma.

Long-term survival after gastrectomy and metastasectomy for gastric cancer with synchronous bone metastasis

Bone metastasis is a rare event in patients with gastric cancer, but pathologic fracture, paralysis, pain and hematological disorders associated with the bone metastasis may influence the quality of life. We report herein the case of a 53-year-old man who presented with primary remnant gastric cancer with bone metastasis. The patient requested further investigations after detection of a metastatic lesion in the 2 nd lumbar vertebra during evaluation for back pain that had persisted for 3 mo. No other metastatic lesions were detected. He underwent total gastrectomy and palliative metastasectomy to aid in reduction of symptoms, and he received combination chemotherapy with tegafur(S-1) and cisplatin. The patient survived for about 60 mo after surgery. Currently, there is no treatment guideline for gastric cancer with bone metastasis, and we believe that gastrectomy plus metastasectomy may be an effective therapeutic option for improving qualityof life and survival in patients with resectable primary gastric cancer and bone metastasis.

Novel monoclonal antibody against beta 1 integrin enhances cisplatin efficacy in human lung adenocarcinoma cells

The use of anti-beta 1 integrin monoclonal antibody in lung cancer treatment has proven beneficial. Here, we developed a novel monoclonal antibody （mAb）, called P5, by immunizing mice with human peripheral blood mononuclear cells （PBMC）. Its anti-tumor effect is now being tested, in a clinical phase Ⅲ trial, in combinato- rial treatments with various chemical drugs. To confirm that P5 indeed binds to beta 1 integrin, cell lysates were immunoprecipitated with commercial anti-beta 1 integrin mAb （TS2/16） and immunoblotted against P5 to reveal a 140 kDa molecular weight band, as expected. Immunoprecipitation with P5 followed by LC/MS protein sequence analysis further verified P5 antigen to be beta 1 integrin. Cisplatin treatment upregulated cell surface expression of beta 1 integrin in A549 cells, while causing inhibition of cell growth. When cells were co-treated with different concentrations of P5 mAb, the cisplatin-mediated inhibitory effect was enhanced in a dose-dependent manner. Our findings show that a combinatorial treatment of P5 mAb and cisplatin in A549 cells resulted in a 30% increase in apoptosis, compared to baseline, and significantly more when compared to either the cisplatin or P5 alone group. The entire peptide sequences in CDR from variable region of Ig heavy and light chain gene for P5 mAb are also disclosed. Together, these results provide evidence of the beneficial effect of P5 mAb in combinatorial treatment of human lung adenocarcinoma.

Lasing threshold in the conventional lasers is the minimum input power required to initiate laser *** has been widely accepted that the conventional laser threshold occurring around a unity intracavity photon number can be eliminated in the input-output curve by making the so-calledβparameter approach *** recent experiments,however,have revealed that even in this case the photon statistics still undergo a transition from coherent to thermal statistics when the intracavity mean photon number is decreased below *** the coherent output is only available above the diminished threshold,the long-sought promise of thresholdless lasers to produce always coherent light has become ***,we present an always-coherent thresholdless laser based on superradiance by two-level atoms in a quantum superposition state with the same phase traversing a high-Q *** lasing was observed without the conventional lasing threshold around the unity photon number and the photon statistics remained near coherent even below *** coherence was improved by reducing the coupling constant as well as the excited-state amplitude in the superposition *** results pave a way toward always-coherent thresholdless lasers with more practical media such as quantum dots,nitrogen-vacancy centers and doped ions in crystals.

Mg has received much attention as a next-generation implantable material owing to its biocompatibility,bone-like mechanical properties,and biodegradability in physiological *** application of various polymer coatings has been conducted in the past to reduce the rapid formation of hydrogen gas and the local change in pH during the initial phase of the chemical reaction with the body ***,we propose femtosecond(fs)laser-mediated Mg surface patterning for significant enhancement of the binding strength of the coating material,which eventually reduces the corrosion *** of the structural,physical,crystallographic,and chemical properties of the Mg surface have been conducted in order to understand the mechanism by which the surface adhesion increases between Mg and the polymer coating *** on the fs laser conditions,the surface structure becomes rough owing to the presence of several microscaled pits and grooves of nanoporous MgO,resulting in a tightly bonded poly(lactic-co-glycolic acid)(PLGA)*** corrosion rate of the PLGA-coated,fs laser-treated Mg is considerably slow compared with the non-treated Mg;the treated Mg is also more biocompatible compared with the non-treated *** fs laser-based surface modification technique offers a simple and quick method for introducing a rough coating on Mg;further,it does not require any chemical treatment,thereby overcoming a potential obstacle for its clinical use.

Horizontal distribution of Changjiang Diluted Water in summer inferred from total suspended sediment in the Yellow Sea and East China Sea

During the summer in the Yellow Sea and East China Sea, the resuspension of the bottom sediment is obstructed by strong stratification and, as a result, the concentration of total suspended sediment（TSS） can be used as an excellent tracer for Changjiang Diluted Water（CDW）. To analyze the spatial and temporal variations of the CDW distribution, the monthly mean TSS from Moderate Resolution Imaging Spectroradiometer（MODIS） ocean color data are constructed and are converted to salinity using the relationship between salinity observed from AQUARIUS and TSS. The process produces the detailed horizontal distribution of salinity with very high resolution（1 km）. From monthly mean salinity map from 2002 to 2012 in July and August, the expansion patterns of CDW are analyzed. The southerly wind in July and southeasterly wind in August transport the CDW eastward and northeastward, respectively. It is found that the yearly variation of the expansion of CDW toward the southern sea area of Korea is mostly due to the variation of southerly wind and the fluctuations of fresh water discharge into the Changjiang estuary show relatively little impact on the eastward extend of CDW. When 11-year mean（from2002 to 2012） salinity map in August is compared with World Ocean Atlas 2013, it is revealed that wind in August strengthened six times from 1994 and it causes the expansion of CDW extended 150 km further eastward.

Effects of intravenous administration of polysaccharide purified from fermented barley on tumor metastasis inhibition via immunostimulating activities

The present study was aimed that determine the effects of polysaccharide isolated from fermented barley about tumor metastasis inhibition via immune related factors such as complementary system,macrophages,natural killer(NK)cells,and cytotoxic T lymphocyte(CTL).To characterize the tumor inhibition effects through immunostimulation of polysaccharides purified from fermented barley,the active crude polysaccharide(BF-CP)was isolated from fermented barley and further fractionated into three purified polysaccharides(BF-I,-II,and-III)with different molecular weights through gel permeation ***-I was mainly composed of four monosaccharides,including xylose(32.90%),arabinose(26.71%),and glucose(33.43%).BF-I augmented the secretion of cytokines,such as interleukin(IL)-6,IL-12,and tumor necrosis factor-α,through peritoneal macrophages and potently activated the complement *** the experiment for NK cells and CTL activities,intravenous(i.v.)administration of BF-I significantly augmented the cytotoxicity of NK cells and CTL-mediated cytotoxicity against tumor *** addition,*** of BF-I potently inhibited tumor metastasis in a mouse model of lung cancer and tumor metastasis in mice with suppressed NK cell function through anti-asialo ***,BF-I shows potent tumor inhibition effects,and these effects are related to various immune factors such as macrophages,the complement system,NK cells,and ***,BF-I,the polysaccharide purified from fermented barley possess the new possibility for developing functional foods and medical ingredients.

Structural identification of active moiety in anti-tumor metastatic polysaccharide purified from fermented barley by sequential enzymatic hydrolysis

The potent anti-cancer metastatic effects induced by the immunostimulatory activities of intravenously administered BF–I,an active polysaccharide purified from fermented barley,were previously *** study aimed to evaluate the anti-cancer metastatic effect of orally administered BF–I and elucidated the microstructure and active moiety of BF–I using sequential enzymatic *** experiments using Colon26-M3.1 carcinoma,prophylactic oral administration of BF–I potently suppressed lung cancer ***,gas chromatography-mass spectrometry analysis of partially methylated alditol acetate showed that BF–I comprised 15 different glycosyl linkages,including 4-linked Xylp(16.1%),terminal Araf(12.4%),4-linked Glcp(32.8%),3-linked Glcp(13.5%),3,6-linked Glcp(1.5%),and 3,6-linked Galp(5.3%).These glycosyl linkage combinations are typical for arabinoxylan,yeast-derivedβ-glucan,barley-derivedβ-glucan,and type II *** clarify the microstructure,BF–I was sequentially degraded via linkage-specific enzymatic hydrolysis using five ***,the sugar composition,glycosyl linkage,and reactivity with theβ-glucosyl Yariv reagent were analyzed,along with sequencing based on tandem mass spectra of partially degraded fragment *** results confirmed that BF–I comprises a mixture of four different types of polysaccharides(arabinoxylan,yeast-derivedβ-glucan,barley-derivedβ-glucan,and type II arabinogalactan)with similar molecular ***,different polysaccharides contribute significantly to cytokine(interleukin-6,-12,and tumor necrosis factor-α)secretion by peritoneal ***,the potent anti-cancer metastatic activity of BF–I is proposed to be related to the immunostimulatory activities of the four different types of polysaccharides.
