To report an extragastrointestinal stromal tumor （EGIST） that occurs outside the gastrointestinal tract and shows unique clinicopathologic and immunohistochemical features. In our case, we experienced multiple soft tissue tumors that originate primarily in the greater omentum, and in immunohistochemical analysis, the tumors showed features that correspond to malignant EGIST. Two large omental masses measured 15 cm×10 cm and 5 cm×4 cm sized and several small ovoid fragments were attached to small intestine, mesentery and peritoneum. On histologic findings, the masses were separated from small bowel serosa and had high mitotic count （115/50 HPFs）. In the results of immunohistochemical stains, the tumor showed CDl17 （c-kit） positive reactivity and high Ki-67 labeling index. On mutation analysis, the c-kit gene mutation was found in the juxtamembrane domain （exon 11）and it was heterozygote. Platelet-derived growth factor receptor （PDGFR） gene mutation was also found in the juxtamemembrane （exon 12） and it was polymorphism. From above findings, we proposed that there may be several mutational pathways to malignant EGIST, so further investigations could be needed to approach this unfavorable disease entity.

Mirizzi syndrome is a rare complication of gallstone disease,and results in partial obstruction of the common bile duct or a cholecystobiliary fistula. Moreover,congenital anatomical variants of the cystic duct are common,occurring in 18%-23% of cases,but Mirizzi syndrome underlying an anomalous cystic duct is an important clinical consideration. Here,we present an unusual case of typeⅠMirizzi syndrome with an uncommon anomalous cystic duct,namely,a low lateral insertion of the cystic duct with a common sheath of cystic duct and common bile duct.

Inflammatory myofibroblastic tumor （INT） of the stomach in adults is extremely rare, with unpredictable prognosis. We present a 55-year-old woman with a gastric IMT. She experienced sudden abdominal pain 4 d previously. Physical examination showed mild abdominal tenderness in the hypogastrium, but no palpable abnormal abdominal mass. Abdominal CT showed a mass of approximately 8 cm in the gastrocolic ligament. On laparoscopic exploration, unexpected hemoperitoneum of approximately 1.5 L of blood was found, and an exophytic gastric mass of approximately 10 cm, appeared from the anterior wall of the gastric body along the greater curvature. Laparoscopy further showed that non- clotting blood in the abdominal cavity seemed to be from the gastric tumor. After conversion to open surgery for more precise evaluation of the cause of hemoperitoneum and the large friable tumor, gastric wedge resection, including the tumor, was conducted. The final diagnosis was consistent with IMT that originated from the gastric wall.

Background:Immune checkpoint inhibitors(ICIs)are effective in a subset of patients with metastatic solid ***,the patients who would benefit most from ICIs in biliary tract cancer(BTC)are still *** and methods:We molecularly characterized tissues and blood from 32 patients with metastatic BTC treated with the ICI pembrolizumab as second-line ***:All patients had microsatellite stable(MSS)type *** of the 32 patients achieved partial response(PR),with an objective response rate(ORR)of 9.4%(95%confidence interval[CI],2.0–25.2)and nine showed stable disease(SD),exhibiting a disease control rate(DCR)of 37.5%(95%CI,21.1–56.3).For the 31 patients who had access to PD-1 ligand 1(PD-L1)combined positive score(CPS)testing(cut-off value≥1%),the ORR was not different between those who had PD-L1-positive(PD-L1+;1/11,9.1%)and PDL1-(2/20,10.0%)tumors(p=1.000).The tumor mutational burden(TMB)of PD-L1+BTC was comparable to that of PD-L1-BTC(p=0.630).TMB and any exonic somatic mutations were also not predictive of pembrolizumab *** analysis of blood and tumor samples demonstrated a relatively high natural killer(NK)cell proportion in the peripheral blood before pembrolizumab treatment in patients who achieved tumor ***,the tumors of these patients presented high enrichment scores for NK cells,antitumor cytokines,and Th1 signatures,and a low enrichment score for cancer-associated ***:This study shows the molecular characteristics associated with the efficacy of pembrolizumab in BTC of the MSS type.

Al-doped ZnO(AZO) is considered as an alternative to transparent conductive oxide *** and achieving a stable surface are important challenges in the development and optimization of dry etching processes, which must be overcome for the application of AZO in various devices. Therefore, in this study, the etch rate and surface properties of an AZO thin film after plasma etching using the adaptive coupled plasma system were investigated. The fastest etch rate was achieved with a CF_(4)/Ar ratio of 50:50 sccm. Regardless of the ratio of CF_(4) to Ar,the transmittance of the film in the visible region exceeded 80%. X-ray photoelectron spectroscopy analysis of the AZO thin film confirmed that metal-F bonding persists on the surface after plasma etching. It was also shown that F eliminates O vacancies. Consequently, the work function and bandgap energy increased as the ratio of CF-4 increased. This study not only provides information on the effect of plasma on AZO thin film, but identifies the cause of changes in the device characteristics during device fabrication.

Obscure gastrointestinal bleeding(OGIB)has traditionally been defined as gastrointestinal bleeding whose source remains unidentified after bidirectional *** can present as overt bleeding or occult bleeding,and small bowel lesions are the most common *** small bowel can be evaluated using capsule endoscopy,device-assisted enteroscopy,computed tomography enterography,or magnetic resonance *** the cause of smallbowel bleeding is identified and targeted therapeutic intervention is completed,the patient can be managed with routine ***,diagnostic tests may produce negative results,and some patients with small bowel bleeding,regardless of diagnostic findings,may experience *** those at risk of rebleeding can help clinicians form individualized surveillance *** studies have identified different factors associated with rebleeding,and a limited number of studies have attempted to create prediction models for *** article describes prediction models developed so far for identifying patients with OGIB who are at greater risk of *** models may aid clinicians in forming tailored patient management and surveillance.

Evaluating the effects of granular and membrane filtrations on chlorine demand in drinking water

In this study, chlorine decay experiments were conducted for the raw water from Nakdong River that is treated by Chllseo Water Treatment Plant （CWTP） situated in Haman, Korea as well as the effluents from sand and granular activated carbon （GAC） filters of CWTP and fitted using a chlorine decay model. The model estimated the fast and slow reacting nitrogenous as well as organic/inorganic compounds that were present in the water. It was found that the chlorine demand due to fast and slow reacting （FRA and SRA） organic/inorganic substances was not reduced significantly by sand as well as GAC filters. However, the treated effluents from those filters contained FRA and SRA that are less reactive and had small reaction rate constants. For the effluents from microfiltration, ultrafiltration, and nanofiltration the chlorine demand because FRA and SRA were further reduced but the reaction rate constants were larger compared to those of sand and GAC filter effluents. This has implications in the formation of disinfection by products （DBPs）. If DBPs are assumed to form due to the interactions between chlorine and SRA, then it is possible that the DBP formation potential in the effluents from membrane filtrations could be higher than that in the effluents from granular media filters.

Recently,studies in many medical fields have reported that image analysis based on artificial intelligence(AI)can be used to analyze structures or features that are difficult to identify with human *** diagnose early gastric cancer,related efforts such as narrow-band imaging technology are ***,diagnosis is often ***,a diagnostic method based on AI for endoscopic imaging was developed and its effectiveness was confirmed in many *** gastric cancer diagnostic program based on AI showed relatively high diagnostic accuracy and could differentially diagnose non-neoplastic lesions including benign gastric ulcers and *** AI system has also been developed that helps to predict the invasion depth of gastric cancer through endoscopic images and observe the stomach during endoscopy without blind ***,if AI is used in the field of endoscopy,it is expected to aid in the diagnosis of gastric neoplasms and determine the application of endoscopic therapy by predicting the invasion depth.

In tissue engineering, microstructure and material composition of tissue scaffolds have major influences on the proliferation and differentiation of cells in the scaffolds. However, once tissue scaffolds implanted, it is extremely difficult to monitor the change of their microstructure and compositions during tissue regeneration. Here, we report how random lasing can be utilized to non-invasively monitor the structure and composition of scaffolds. We hypothesize that morphological and compositional change of silk fibroin (SF) scaffolds can be conveniently detected based on random lasing responses. Engineered SF scaffolds with hydroxyapatite (HAP) nanoparticles and controlled pore alignment were fabricated, and their random lasing responses were analyzed depending on the concentration of HAP nanoparticles and the degree of,:internal pore alignment. We also examined the real-time random lasing responses of porous SF scaffolds by applying a compressive force to' the scaffolds. Introduction of HAP nanoparticles lowered the lasing thresholds and narrowed the random lasing (RL) width dramatically, which :is likely due to the increase in heterogeneity in both refractive index and physical arrangement within the SF and HAP composites. The strong dependency of RL response on pore alignment was also measured and validated by numerical calculation with the finite element method (FEM).Finally, real-time monitoring of RL on compressed scaffolds demonstrated the possibility of using RL as a monitoring tool for structural change of SF scaffolds in vivo.
