Comprehensive molecular characterization to predict immunotherapy response in advanced biliary tract cancer: a phase II trial of pembrolizumab

Background:Immune checkpoint inhibitors (ICIs) are effective in a subset of patients with metastatic solid tumors. However, the patients who would benefit most from ICIs in biliary tract cancer (BTC) are still controversial.Materials and methods:We molecularly characterized tissues and blood from 32 patients with metastatic BTC treated with the ICI pembrolizumab as second-line therapy.Results:All patients had microsatellite stable (MSS) type tumors. Three of the 32 patients achieved partial response (PR), with an objective response rate (ORR) of 9.4% (95% confidence interval [CI], 2.0-25.2) and nine showed stable disease (SD), exhibiting a disease control rate (DCR) of 37.5% (95% CI, 21.1-56.3). For the 31 patients who had access to PD-1 ligand 1 (PD-L1) combined positive score (CPS) testing (cut-off value ≥1%), the ORR was not different between those who had PD-L1-positive (PD-L1+; 1/11, 9.1%) and PDL1-(2/20, 10.0%) tumors ( = 1.000). The tumor mutational burden (TMB) of PD-L1+ BTC was comparable to that of PD-L1-BTC ( = 0.630). TMB and any exonic somatic mutations were also not predictive of pembrolizumab response. Molecular analysis of blood and tumor samples demonstrated a relatively high natural killer (NK) cell proportion in the peripheral blood before pembrolizumab treatment in patients who achieved tumor response. Moreover, the tumors of these patients presented high enrichment scores for NK cells, antitumor cytokines, and Th1 signatures, and a low enrichment score for cancer-associated fibroblasts.Conclusions:This study shows the molecular characteristics associated with the efficacy of pembrolizumab in BTC of the MSS type.

Self-lubricating A1203-15wt% ZrO2 composites with sulfides, such as molybdenum disulfide （MoS2） and tungsten disulfide （WSz） serving as solid lubricants, were fabricated by using the pulse electric current sintering （PECS） technique. The coefficient of friction （COF） of the A1203-15wt% ZrO2 composite without/with sulfides was in the range of 0.37-0.48 and 0.27-0.49, respectively. As the amoant of sul- fides increased, the COF and the wear rate decreased. The reduction in COF and wear rate of the sulfide-containing composite is caused by a reduction in shear stresses between the specimen and the tribological medium due to the formation of a lubricating film resulting from the lamellar structure of sulfides located on the worn surface.

Several approaches for fast generation of digital holograms of a three-dimensional （3D） object have been discussed. Among them, the novel look-up table （N-LUT） method is analyzed to dramatically reduce the number of pre-calculated fringe patterns required for computation of digital holograms of a 3D object by employing a new concept of principal fringe patterns, so that problems of computational complexity and huge memory size of the conventional ray-tracing and look-up table methods have been considerably alleviated. Meanwhile, as the 3D video images have a lot of temporally or spatially redundant data in their inter- and intra-frames, computation time of the 3D video holograms could be also reduced just by removing these redundant data. Thus, a couple of computational methods for generation of 3D video holograms by combined use of the N-LUT method and data compression algorithms are also presented and discussed. Some experimental results finally reveal that by using this approach a great reduction of computation time of 3D video holograms could be achieved.

In this study, numerical analysis is performed to adopt the equivalence ratio on the high velocity oxygen fuel （HVOF） thermal spray coating systems equipped with a minimum length nozzle. The analysis is applied to investigate the axisymmetric, steady-state, turbulent, and chemically combusting flow both within the torch and in a free jet region between the torch and the substrate to be coated. The combustion is modeled using a single-step and eddy-dissipation model which assumes that the reaction rate is limited by the turbulent mixing rate of the fuel and oxidant. As the diameter of the nozzle throat is increased, the location of the Mach shock disc moves backward from the nozzle exit. As the throat diameter and the divergent portion are 6 mm and 8 mm, respectively, the pressure in the HVOF system is the lowest at the chamber and the expanding gas is steadily maintained with both high velocity and high temperature for different equivalence ratios. Thus, relatively minor amendments of the equivalence ratio and the geometry of HVOF can lead to improved control over coating characteristics.

The design of notch and barrier was optimized in order to improve the characteristics of constant torque while minimizing the cogging torque that occurs as a result of teeth and slot structure. The barrier was installed in order to minimize the cogging torque and torque ripple by finite element method (FEM) with a reduced barrier width toward the center of magnetic pole. The position and width of notch, which can offset cogging torque, can be calculated with energy distribution of air-gap using Fourier series. The optimized model demonstrates a 60% decrease in the cogging torque, a 75.3% decrease in the torque ripple and a 3% increase in the operating torque when compared with the basic model.

Sheet metal formed of lightweight materials such as aluminum sheeting has received great attention related to the reduction of vehicle emissions. This paper evaluates the anisotropic yield locus using Kuwabara＇s biaxial tensile tester and stretches formability using Hecker＇s hemispheri- cal punch stretching test for aluminum 6016-T4 sheet material. The anisotropic yield locus of the A16016-T4 sheet measured is fitted well by the modified Drucker yield func- tion. Moreover the best fitting to the experimental stress- strain curve from the tensile test was obtained by taking an appropriate hardening model. Analytical study to predict the stretch formability by using Hora＇s Modified Maximum Force Criterion （MMFC） was performed. The predicted forming limit curves （FLC） based on various yield functions were compared with the experiments and discussed.

AIM:To evaluate the effect of proton pump inhibitors(PPIs) on the development of gastrointestinal ***:All patients who were more than 20 years old and who had received a prescription for PPIs among those who visited Seoul National University Hospital from January 1,2005 to December 31,2009 were *** to the low sensitivity of the microbiologic test and the nonspecific pathologic findings,the diagnosis of gastrointestinal tuberculosis was confirmed through the presence of active ulcerations and the responses to anti-tuberculosis *** patients were divided into two groups according to treatment duration(group 1:≤ 3 mo;group 2:> 3 mo) and were followed up from the time they took the first prescription of PPIs until their last *** regression analysis was used to calculate the relative risks(RR) and 95%CI,adjusting for ***:Among the 61 834 patients exposed to PPIs(50 534 in group 1;11 300 in group 2),21 patients were diagnosed with PPI-associated gastrointestinal tuberculosis during 124 274 person-years of *** 21 patients,the 12 who revealed only scar changes in the colonoscopy were excluded from the statistical *** those who remained,2 were excluded because they underwent gastrointestinal endoscopy within 4 wk of the first prescription for *** exposure to PPI was associated with a higher mean age(55.0 ± 14.5 in group 1 vs 58.2 ± 13.3 in group 2,P < 0.001) and a higher Charlson co-morbidity index(0.50 ± 0.93 in group 1 vs 0.77 ± 1.14 in group 2,P < 0.001).The true incidence of active gastrointestinal tuberculosis was 0.65 per 1000 person-years in group 1 and 0.03 per 1 000 person-years in group *** the less-than-three-month PPI treatment period in group 1,the over-three-month PPI therapy period in group 2 was not associated with increased risk of acquiring gastrointestinal tuberculosis,after adjusting for age and co-morbidities,whereas the Charlson co-morbidity index was associated
