Background:The Korea National Health and Nutrition Examination Survey nonalcoholic fatty liver disease(K-NAFLD)score was recently developed with the intent to operationally define nonalcoholic fatty liver disease(NAFLD).However,there remained an external validation that confirmed its diagnostic performance,especially in patients with alcohol consumption or hepatitis virus infection.Methods:Diagnostic accuracy of the K-NAFLD score was evaluated in a hospital-based cohort consisting of 1388 participants who received Fibroscan®.Multivariate-adjusted logistic regression models and the contrast estimation of receiver operating characteristic curves were used for validation of the K-NAFLD score,fatty liver index(FLI),and hepatic steatosis index(HSI).Results:K-NAFLD-moderate[adjusted odds ratio(aOR)=2.53,95%confidence interval(CI):1.13-5.65]and K-NAFLD-high(aOR=4.14,95%CI:1.69-10.13)groups showed higher risks of fatty liver compared to the K-NAFLD-low group after adjustments for demographic and clinical characteristics,and FLI-moderate and FLI-high groups revealed aORs of 2.05(95%CI:1.22-3.43)and 1.51(95%CI:0.78-2.90),respectively.In addition,the HSI was less predictive for Fibroscan®-defined fatty liver.Both K-NAFLD and FLI also demonstrated high accuracy in the prediction of fatty liver in patients with alcohol consumption and chronic hepatitis virus infection,and the adjusted area under curve values were comparable between K-NAFLD and FLI.Conclusions:Externally validation of the K-NAFLD and FLI showed that these scores may be a useful,noninvasive,and non-imaging modality for the identification of fatty liver.In addition,these scores also predicted fatty liver in patients with alcohol consumption and chronic hepatitis virus infection.

Tumor-associated lymphangiogenesis is thought to be an importantmetastaticstepin intrahepaticccholangiocarcinoma(ICC),while the origin of tumor-associated lymphatic vessels within tumors is little known.Cancerassociated fibroblasts(CAFs)enhance tumor progression through many aspects.2 CAFs have been discovered exhibiting cellular plasticity of phenotypic transition-such as epithelial-mesenchymal transition(EMT)which promotes increased migratory and invasive capabilities of cells.3However,mesenchymal-to-endothelial,(MEndT)transition is yet to be identified in tumor entities.

Dear Editor,Antibiotics are considered indispensable in numerous treatments against infectious diseases.However,their over-prescription and widespread use are emerging as important global issues,as they lead to resistant bacteria,reducing the efficacy of antibiotics[1].The defined daily doses per 1,000 habitants per day for antibiotics in South Korea is 28,which is much higher than the average of Organization for Economic Co-operation and Development countries[1].In animal studies,antibiotic tigecycline was found to inhibit the proliferation and induce apoptosis of hepatocellular carcinoma(HCC)cells[2].In addition,isoprenoid antibiotic was found to suppress the growth and invasion of HCC by targeting the signal transducer and activator of the transcription 3 signaling cascade[3].Despite evidence of the beneficial effects of antibiotics against HCC,evidence regarding their effects on the longterm development of HCC is limited.Introduction regarding the incidence of HCC is provided in the Supplementary file.

To the Editor: We thank Dr. Braillon for the thoughtful remarks regarding prognostic impact of interleukin (IL)-26 on patients with hepatocellular carcinoma (HCC) undergoing surgical resection [1]. This study drew a conclusion that relative high expression of IL-26 is an independent prognostic factor for HCC patients after resection.

Background: There is no data regarding prognostic impact of interleukin(IL)-26 on outcomes of patients with hepatocellular carcinoma(HCC). The present study aimed to evaluate the prognostic impact of IL-26 on HCC patients undergoing liver resection. Methods: From 2003 to 2008, 122 patients with HCC who received surgical curative resection were enrolled. Patients were stratified into IL-26-upper and-lower groups according to the median expression level from immunohistochemical staining of resected specimens. Prognostic impact of IL-26 was estimated using Kaplan–Meier curves. Univariate and multivariate analyses were performed to evaluate timedependent prognostic impact and independency of IL-26. Demographic and clinical factors that were associated with IL-26 were comprehensively identified. Results: Prognosis of the patients with high level of IL-26 revealed to be significantly unfavorable in both cumulative recurrence-free survival( P < 0.001) and overall survival( P = 0.002). Upper expression of IL-26(HR: 1.643;95% CI: 1.021 to 2.644;P = 0.041) and microvascular invasion(HR: 3.303;95% CI: 1.255 to 8.696;P = 0.016) were identified as significant independent prognostic factors for overall survival in the multivariable analysis. Conclusions: IL-26 is a novel prognostic factor for HCC after resection. Evaluation of IL-26 expression may be potentially valuable in clinical therapy when planning individualized follow-up schedule and evaluating candidates for prophylactic adjuvant treatment to prevent recurrence.

Background:Transarterial chemoembolization(TACE)and percutaneous microwave coagulation therapy(PMCT)are commonly used to treat intrahepatic recurrent liver cancers.However,there is no informa-tion regarding their effectiveness in patients with recurrent intrahepatic cholangiocarcinoma(ICC)after resection.Methods:A total of 275 patients with localized recurrent ICC who received either TACE(n=183)or PMCT(n=92)were studied.A propensity score matching analysis was performed to compare prognostic impact of TACE and PMCT.Prognostic factors for TACE and PMCT were identified respectively.Predictive nomograms for each TACE and PMCT were developed using the Cox independent prognostic factors and were validated in independent patient groups by receiver operating characteristic curves and area under curve values.Results:Both TACE and PMCT provided curativeness in partial patients(5-year overall survival:21.4%and 6.1%,respectively),but TACE provided better survival benefit in both overall patients(hazard ratio[HR]=0.71;95%confidence interval[CI]:0.50–0.97;P=0.034)and propensity score matching analysis(HR=0.69;95%CI:0.47–0.98;P=0.041).Independent prognostic factors for TACE were tumor size>5 cm,poor differentiation,and major resection,whereas poor differentiation,hepatitis B virus infection,cholelithiasis,and lymph node metastasis were identified for PMCT.Both predictive nomograms for TACE and PMCT were validated to be effective with area under curve values of 0.77 and 0.70,respectively.Conclusions:TACE provided better survival benefits compared to PMCT.However,there was a disparity in prognostic factors,suggesting evaluation of the two nomograms may be supportive in modality selection.Further prospective validation studies are required for the results to be applied in clinical medicine.

肝内胆管细胞癌(ICC)起源于二级胆管及其分支上皮细胞,是恶性程度较高和预后较差的肝脏恶性肿瘤之一。淋巴结转移是严重影响ICC患者预后的因素。近年来,许多研究发现乙型肝炎病毒(HBV)是ICC发生的危险因素之一,HBV相关ICC具有类似与肝细胞癌的临床病理表现。另外,新近研究表明HBV阳性ICC患者术后预后优于HBV阴性患者,且HBV阳性ICC患者淋巴结转移率较低。因此,进一步探索HBV在ICC的发生和预后中的作用有着重要的临床意义。

肝内胆管细胞癌是发病率逐渐上升的第二大常见原发性肝脏恶性肿瘤。目前,根治性手术切除被认为是最有效的治疗方案。然而,由于其高侵袭性和致命性等恶性特征,如早期淋巴结转移及手术切除后易肝内复发等,肝内胆管癌患者的预后仍然令人失望。因此,全面探索可改善肝内胆管癌患者预后的潜在因素具有非常重要的意义。研究对象包括106例在上海交通大学医学院附属仁济医院和1093例在东方肝胆外科医院接受手术治疗的肝内胆管癌患者。本研究对可影响预后的变量,包括病因学亚型、临床病理特征和治疗方案的选择,进行全面分析。结果显示预防性肝动脉化疗栓塞治疗显著延长生存时间。对于肝切除术后的复发肿瘤,二次切除术是最有效的治疗方法,其次为肝动脉化疗栓塞术、经皮微波组织凝固疗法及化疗,但放疗没有提供术后复发患者生存获益。然而,这些不同治疗方案在不同肿瘤的亚型及不同临床病理特征的患者中的疗效有显著差异。预防性和复发后治疗在特定人群的使用具有大幅改善肝内胆管癌患者预后的潜力。同时,在选择治疗方案时均需考虑病因学及临床病理学因素。我们的数据在临床上的应用前,需要未来前瞻性研究来进一步验证我们的结果。

目的探讨人肝内胆管癌相关成纤维细胞(iCAF)的分离、纯化和鉴定,并初步探究其对肿瘤细胞的影响作用。方法肝内胆管癌标本通过酶消化法(Ⅰ型胶原酶与RPMI 1640混合消化液)分离获得原代细胞,并通过密度梯度离心法进行纯化,iCAF的鉴定通过免疫化学、荧光染色及流式细胞检测进行。通过CCK8及Transwell实验观察iCAF条件培养基对胆管癌细胞的影响作用。结果获得的iCAF以长梭形为主,细胞生长密集相互交织形成网状或漩涡状;免疫细胞化学染色及荧光染色显示:α-SMA阳性、FSP-1阳性;流式细胞检测FSP-1阳性率为98.2%。iCAF条件培养基明显促进了肿瘤细胞增殖和迁移。结论成功分离并纯化了人iCAF,并初步证明iCAF可促进胆管癌细胞增殖和迁移能力,为进一步研究iCAF在肿瘤发生发展中的具体作用机制奠定了重要基础。
