AIm. To investigate ASCA production over tim. in CD and m.rine colitis in order to further our understanding of their etiology. m.TERIALS AND m.THODS： Sixty-six CD patients were com.ared to ulcerative colitis （UC） and irritable bowel syndrom. patients with respect to ASCA production as m.asured by ELISA. ASCA IgG or IgA positivity as well as change in titers over a period of up to 3 years （△tgG/A） was correlated with clinical param.ters such as CD activity index （CDm. and C-reactive protein levels （CRP）. m.reover, two m.rine m.dels of colitis （DSS and IL-10 knock out） were com.ared to control anim.ls with respect to ASCA titers after oral yeast exposure. RESULTS： ASCA IgG and IgA titers are stable over tim. in CD and non-CD patients. Fistular disease was associated with a higher rate of ASCA IgA positivity （P = 0.014）. Ileal disease was found to have a significant influence on the △tgG of ASCA （P = 0.032）. There was no correlation found between ASCA positivity or △tgG/A and clinical param.ters of CD： CDAI and CRP. In m.ce, neither healthy anim.ls nor anim.ls with DSS-induced or spontaneous colitis exhibited a m.rked increase in ASCA titers after high-dose yeast exposure. On the other hand, m.ce im.unized intraperitoneally with m.nnan plus adjuvant showed a m.rked and significant increase in ASCA titers com.ared to adjuvant-only im.unized controls （P = 0.014）. CONCLUSION： The propensity to produce ASCA in a subgroup of CD patients is largely genetically predeterm.ned as evidenced by their stability and lack of correlation with clinical disease activity param.ters. Furtherm.re, in anim.l m.dels of colitis, m.re oral exposure of m.ce to yeast does not lead to the induction of m.rked ASCA titers irrespective of concom.tant colonic inflam.ation. Hence, environm.nt m.y play only a m.nor role in inducing ASCA.

AIm. To test whether hum.ral im.une reaction against m.cobacteria m.y play a role in anti- Saccharom.ces cerevisiae antibodies （ASCA） generation in Crohn＇s disease （CD） and/or whether it correlates with clinical subtypes. m.THODS： The dom.nant ASCA epitope was detected by Galanthus nivalis lectin （GNL）-binding assay. ASCA and IgG against m.cobacterial lysates （m.avium. m.sm.gm.tis, m.chelonae, m.bovis BCG m.avium.ssp. paratuberculosis （m.P）] or purified lipoarabinom.nnans （LAm. were detected by ELISA. ASCA and anti-m.cobacterial antibodies were affinity purified to assess cross-reactivities. Anti-m.cobacterial IgG were induced by BCG-infection of m.ce. RESULTS： GNL bound to different extents to m.cobacterial lysates, abundantly to purified m.nnosecapped （m.n） LAm.from.m.tuberculosis, but not to uncapped LAm.from.m.srnegrnatis. Fifteen to 45% of CD patients but only 0%-6% of controls were seropositive against different m.cobacterial antigens. Anti-m.cobacterial IgG correlated with ASCA （r = 0.37-0.64; P = 0.003-P 〈 0.001）. ASCA-positivity and deficiency for m.nnan-binding lectin synergistically associated with anti-m.cobacterial IgG. In som. patients, anti-m.cobacterial antibodies represent crossreactive ASCA. Vice-versa, the predom.nant fraction of ASCA did not cross-react with m.cobacteria. Finally, fistulizing disease associated with antibodies against m.avium. m.sm.gm.tis and m.P （P = 0.024, 0.004 and 0.045, respectively）. CONCLUSION： Sim.lar to ASCA, seroreactivity against m.cobacteria m.y define CD patients with com.licated disease and a predisposition for im.une responses against ubiquitous antigens. While in som. patients anti-m.cobacterial antibodies strongly cross-react with yeast m.nnan; these cross-reactive antibodies only represent a m.nor fraction of total ASCA. Thus, m.cobacterial infection unlikely plays a role in ASCA induction.

AIm.To quantify intraoperative and postoperative com.lications in com.lex phacoem.lsification cataract extraction(phacoem.lsification)with iris m.nipulation com.ared to non-com.lex and com.lex phacoem.lsification without iris ***:All phacoem.lsification cases at the University of Colorado between January 1,2014,and June 30,2017 were *** criteria for the prim.ry outcom. of intraoperative com.lications were planned com.ination surgery and eyes with less than 28 d *** criteria for the secondary outcom.s of postoperative com.lications were unplanned additional surgery,and chronic steroid eye drop use prior to *** including sex,race/ethnicity,surgery length,visual acuity,intraoperative and postoperative com.lications,and intraocular pressures(IOP)were collected and analyzed utilizing general linear and Logistic regression ***:The m.dical records of 5772 eyes were reviewed(500 com.lex without iris m.nipulation,367 with iris m.nipulation).The num.er of any intraoperative com.lication in the com.lex with iris m.nipulation and com.lex without iris m.nipulation groups was 15(4.1%)and 26(5.2%),respectively,com.ared to 41(0.8%)in the non-com.lex *** inflam.ation was found in 135(2.8%)non-com.lex cases,20(4.1%)com.lex cases without iris m.nipulation,and 20(5.6%)com.lex cases with iris *** adjusted odds ratio of postoperative inflam.ation in phacoem.lsification with iris m.nipulation com.ared to non-com.lex was 2.3(95%CI:1.3-4.0,P=0.005).The rate of IOP spikes>10 m. Hg was significantly greater in cases with iris m.nipulation(P=0.001).CONCLUSION:Com.lex cases have m.re intraoperative ***,only com.lex cases with iris m.nipulation led to increase rates of postoperative inflam.ation and IOP spikes>10 m. Hg.
