To the Editor:We sincerely applaud the work done by Gupta et al.,in which they analyzed a single-center experience regarding gallbladder per-foration(GBP)management[1].Currently,there is a lack of cohort studies to adequately describe the approach strategies and man-agement for this pathology,with international guidelines making ambiguous recommendations[2,3].Their study stratified GBP types and provided details regarding clinical presentation,comorbidities,preoperative diagnosis,etiology,imaging techniques,and surgical approach.The study reported a high conversion rate when a la-paroscopic approach was attempted for cholecystectomy.

Melt extrusion-based additive manufacturing(ME-AM)is a promising technique to fabricate porous scaffolds for tissue engi-neering applications.However,most synthetic semicrystalline polymers do not possess the intrinsic biological activity required to control cell fate.Grafting of biomolecules on polymeric surfaces of AM scaffolds enhances the bioactivity of a construct;however,there are limited strategies available to control the surface density.Here,we report a strategy to tune the surface density of bioactive groups by blending a low molecular weight poly(ε-caprolactone)5k(PCl5k)containing orthogonally reactive azide groups with an unfunctionalized high molecular weight PCl75k at different ratios.Stable porous three-dimensional(3D)scaf-folds were then fabricated using a high weight percentage(75 wt.%)of the low molecular weight PCl 5k.As a proof-of-concept test,we prepared films of three different mass ratios of low and high molecular weight polymers with a thermopress and reacted with an alkynated fluorescent model compound on the surface,yielding a density of 201-561 pmol/cm^(2).Subsequently,a bone morphogenetic protein 2(BMP-2)-derived peptide was grafted onto the films comprising different blend compositions,and the effect of peptide surface density on the osteogenic differentiation of human mesenchymal stromal cells(hMSCs)was assessed.After two weeks of culturing in a basic medium,cells expressed higher levels of BMP receptor II(BMPRII)on films with the conjugated peptide.In addition,we found that alkaline phosphatase activity was only significantly enhanced on films contain-ing the highest peptide density(i.e.,561 pmol/cm^(2)),indicating the importance of the surface density.Taken together,these results emphasize that the density of surface peptides on cell differentiation must be considered at the cell-material interface.Moreover,we have presented a viable strategy for ME-AM community that desires to tune the bulk and surface functionality via blending of(modified)polymers.Furthermore,the use of alkyne-a

Association of Congenital Heart Defects (CHD) with Factors Related to Maternal Health and Pregnancy in newborns in Puerto Rico

Background:Given the pervasive issues of obesity and diabetes both in Puerto Rico and the broader United States,there is a compelling need to investigate the intricate interplay among body mass index(BMI),pregesta-tional,and gestational maternal diabetes,and their potential impact on the occurrence of congenital heart defects(CHD)during neonatal development.Methods:Using the comprehensive System of Vigilance and Surveillance of Congenital Defects in Puerto Rico,we conducted a focused analysis on neonates diagnosed with CHD between 2016 and 2020.Our assessment encompassed a range of variables,including maternal age,gestational age,BMI,pregestational diabetes,gestational diabetes,hypertension,history of abortion,and presence of preeclampsia.Results:A cohort of 673 patients was included in our study.The average maternal age was 26 years,within a range of 22 to 32 years.The mean gestational age measured 39 weeks,with a median span of 38 to 39 weeks.Of the 673 patients,274(41%)mothers gave birth to neonates diagnosed with CHD.Within this group,22 cases were linked to pre-gestational diabetes,while 202 were not;20 instances were associated with gestational diabetes,compared to 200 without;and 148 cases exhibited an overweight or obese BMI,whereas 126 displayed a normal BMI.Conclusion:We identiﬁed a statistically signiﬁcant correlation between pre-gestational diabetes mellitus and the occurrence of CHD.However,our analysis did not show a statistically signiﬁcant association between maternal BMI and the likelihood of CHD.These results may aid in developing effective strategies to prevent and manage CHD in neonates.

A Theoretically Grounded Question Answering Data Set for Evaluating Machine Common Sense

Achieving machine common sense has been a longstanding problem within Artificial Intelligence.Thus far,benchmark data sets that are grounded in a theory of common sense and can be used to conduct rigorous,semantic evaluations of common sense reasoning(CSR)systems have been lacking.One expectation of the AI community is that neuro-symbolic reasoners can help bridge this gap towards more dependable systems with common sense.We propose a novel benchmark,called Theoretically Grounded common sense Reasoning(TG-CSR),modeled as a set of question answering instances,with each instance grounded in a semantic category of common sense,such as space,time,and emotions.The benchmark is few-shot i.e.,only a few training and validation examples are provided in the public release to avoid the possibility of overfitting.Results from recent evaluations suggest that TG-CSR is challenging even for state-of-the-art statistical models.Due to its semantic rigor,this benchmark can be used to evaluate the common sense reasoning capabilities of neuro-symbolic systems.

Fluoride is an impurity in nickel sulfate production,which is required for electric vehicle batteries.Hydrous zirconium oxide(HZO)was evaluated for removing fl uoride from nickel sulfate solution.Maximum fluoride removal occurred at pH value 4 and optimal pH value is 4–5,considering Zr solubility.Fluoride availability decreases with pH due to hydrogen fluoride and zirconium fluoride aqueous species.Fluoride removal is initially rapid,with 50 wt.%removal in 7 min,followed by slow removal up to 68 wt.%after 72 h and follows second order rate kinetics.Fluoride removal was dominated by an ion exchange mechanism and resulting Zr–F bonds were observed using Fourier-transform infrared spectroscopy.The presence of nickel sulfate decreased loading capacity compared to a salt-free solution.HZO maintained adsorption capacity through five cycles of loading and regeneration.

BACKGROUnD The impact of social determinants of health in allogeneic transplant recipients in low-and middle-income countries is poorly described.This observational study analyzes the impact of place of residence,referring institution,and transplant cost coverage(out-of-pocket vs government-funded vs private insurance)on outcomes after allogeneic hematopoietic stem cell transplantation(alloHSCT)in two of Mexico's largest public and private institutions.AIM To evaluate the impact of social determinants of health and their relationship with outcomes among allogeneic transplant recipients in Mexico.METHODS In this retrospective cohort study,we included adolescents and adults≥16 years who received a matched sibling or haploidentical transplant from 2015-2022.Participants were selected without regard to their diagnosis and were sourced from both a private clinic and a public University Hospital in Mexico.Three payment groups were compared:Out-of-pocket(OOP),private insurance,and a federal Universal healthcare program“Seguro Popular”.Outcomes were compared between referred and institution-diagnosed patients,and between residents of nuevo leon and out-of-state.Primary outcomes included overall survival(OS),categorized by residence,referral,and payment source.Secondary outcomes encompassed early mortality,event-free-survival,graft-versus-host-relapse-free survival,and non-relapse-mortality(nRM).Statistical analyses employed appropriate tests,Kaplan-Meier method,and Cox proportional hazard regression modeling.Statistical software included SPSS and R with tidycmprsk library.RESUlTS Our primary outcome was overall survival.We included 287 patients,n=164 who lived out of state(57.1%),and n=129 referred from another institution(44.9%).The most frequent payment source was OOP(n=139,48.4%),followed by private insurance(n=75,26.1%)and universal coverage(n=73,25.4%).no differences in OS,event-free-survival,nRM,or graft-versus-host-relapse-free survival were observed for patients diagnosed locally vs in another institution,nor pat

Background The rise in suicides among children aged 10–14 years demands urgent attention globally.This study aims to assess the global burden of suicide-related deaths in this age group from 1990 to 2019,considering factors such as sex,geography,and sociodemographics,to inform prevention strategies and interventions.Methods The data from Global Burden of Disease 2019,encompassing 204 countries and territories,were analyzed to investigate deaths and years of life lost(Ylls)due to suicide among children aged 10–14 years.Statistical analyses,including mortality rates,Ylls,and the sociodemographic index(SDI),were conducted using standardized tools.Results In 2019,a total of 8327[95%uncertainty interval(UI)=7073–9685]children aged 10–14 years died globally due to suicide,with a mortality rate of 1.30(95%UI=1.10–1.51)per 100,000.The rates varied across countries/territories ranging between 0.05(95%UI=0.02–0.10)in South Africa and 7.49(95%UI=5.13–10.57)in Greenland.The contribution of suicide-related deaths to all-cause mortality ranged from 0.07%(95%UI=0.04%–0.15%)in South Africa to 33.02%(95%UI=24.36%–41.53%)in Greenland.Worldwide,there were approximately 636,196(95%UI=540,383–740,009)Ylls due to suicide,with a rate of 99.07(95%UI=84.15–115.23)per 100,000.The association between SDI and suicide-related deaths was evident,with higher contributions observed in countries with higher SDI.Conclusions This study reveals a concerning global burden of suicide-related deaths among children aged 10–14 years.Despite progress in reducing mortality rates,suicide remains a significant issue.While overall rates have declined,the percentage of deaths caused by suicide in this age group is increasing.

Gastroesophageal reflux disease(GERD)is defined by recurrent and troublesome heartburn and effortless regurgitation or by the specific complications of this condition—that is,esophagitis,esophageal peptic strictures,and Barrett esophagus.1Although GERD is caused by gastric contents'reaching the esophagus,gastric juices can also reach other anatomical locations(pharynx,mouth,larynx,and airways),leading to(or worsening)extra-esophageal conditions(notably asthma)or symptoms(e.g.,wheezing,cough).1 Together with obesity,GERD is the main risk factor for gastroesophageal malignancies.

Background:Exercise training promotes brain plasticity and is associated with protection against cognitive impairment and Alzheimer’s disease(AD).These beneficial effects may be partly mediated by blood-borne factors.Here we used an in vitro model of AD to investigate effects of blood plasma from exercise-trained donors on neuronal viability,and an in vivo rat model of AD to test whether such plasma impacts cognitive function,amyloid pathology,and neurogenesis.Methods:Mouse hippocampal neuronal cells were exposed to AD-like stress using amyloid-βand treated with plasma collected from human male donors 3 h after a single bout of high-intensity exercise.For in vivo studies,blood was collected from exercise-trained young male Wistar rats(high-intensity intervals 5 days/week for 6 weeks).Transgenic AD rats(McGill-R-Thyl-APP)were inj ected 5 times/fortnight for 6 weeks at2 months or 5 months of age with either(a)plasma from the exercise-trained rats,(b)plasma from sedentary rats,or(c)saline.Cognitive function,amyloid plaque pathology,and neurogenesis were assessed.The plasma used for the treatment was analyzed for 23 cytokines.Results:Plasma from exercised donors enhanced cell viability by 44.1%(p=0.032)and reduced atrophy by 50.0%(pn amyloid-β-treated cells.In vivo exercised plasma treatment did not alter cognitive function or amyloid plaque pathology but did increase hippocampal neurogenesis by~3 fold,regardless of pathological stage,when compared to saline-treated rats.Concentrations of 7 cytokines were significantly reduced in exercised plasma compared to sedentary plasma.Conclusion:Our proof-of-concept study demonstrates that plasma from exercise-trained donors can protect neuronal cells in culture and promote adult hippocampal neurogenesis in the AD rat brain.This effect may be partly due to reduced pro-inflammatory signaling molecules in exercised plasma.

Objective To evaluate the chemical composition and effects of Artemisia vulgaris(AV)hydroalcoholic extract(HEAV)on breast cancer cells(MCF-7 and SKBR-3),chronic myeloid leukemia(K562)and nIH/3T3 fibroblasts.Methods Phytochemical analysis of HEAV was done by high-performance liquid chromatography-mass(HPlC)spectrometry.Viability and cell death studies were performed using trypan blue and Annexin/FITC-7AAD,respectively.Ferrostatin-1(Fer-1)and necrostatin-1(nec-1)were used to assess the mode of HEAV-induced cell death and acetoxymethylester(BAPTA-AM)was used to verify the involvement of cytosolic calcium in this event.Cytosolic calcium measurements were made using Fura-2-AM.Results HEAV decreased the viability of MCF-7,SKBR-3 and K562 cells(Plity of HEAV-treated K562 cells was reduced compared to HEAV-exposed fibroblasts(Pnt of K562 cells with HEAV induced cell death primarily by late apoptosis and necrosis in assays using annexin V-FITC/7-AAD(Pnec-1 and Fer-1 increased the viability of K562 cells treated with HEAV relative to cells exposed to HEAV alone(Pnduced Ca^(2+)release mainly from lysosomes in K562 cells(Pn intracellular Ca^(2+)chelator,decreased the number of non-viable cells treated with HEAV(Pnclusions HEAV is cytotoxic and activates several modalities of cell death,which are partially dependent on lysosomal release of Ca^(2+).These effects may be related to artemisinin and caffeoylquinic acids,the main compounds identified in HEAV.
