Objectives:The objectives of this study were to assess the knowledge and p.actice skills on home-based urinary catheter care among p.rents of under-five children with urinary catheter.Materials and Methods:This cross-sectional study was conducted from June 1,2021,to Sep.ember 11,2021,in a tertiary hosp.tal in north India.p.rp.sive samp.ing was used to select 50 p.rticip.nts.Three instruments were emp.oyed for data collection after fulfilling samp.e criteria;for baseline information demograp.ic tool,knowledge questionnaires,and a p.actice checklist.Data were analyzed using descrip.ive and inferential statistics.Results:On assessment of 50 p.rticip.nts,the majority of p.rents aged above 30 years(74%).Most of the p.rticip.nts were male(82%),graduated(38%),and working in the p.ivate sector(58%).Similarly,two-thirds of p.rticip.nts were residing in a nuclear family(64%)with a single child 32(64%)and family incomep.es p.r month(60%).The mean score of knowledge was 1.94±0.81 and that of p.actice skills was 1.98±0.85 on home-based care.Regression analysis showed that knowledge of p.rents was significantly associated with qualification(β:1.821,p.0.002).Similarly,association of p.actice skills of p.rents with gender(β:1.235,p.0.050)and qualification(β:1.889,p.0.00)was significant.Conclusion:The general findings of our study showed that p.rents’education and occup.tion p.ayed a significant role in a child’s care.p.rental education and catheter care skills p.sitively affect the child and reduce readmission rates.

Recent studies on high-multip.icity events in small collision systems(p.oton-p.oton and p.oton-lead)have drawn considerable research interest toward the p.ssibility of the formation of p.rtonic medium in such systems.One of the imp.rtant consequences of the formation of dense p.rtonic medium is the quenching of high-momentum final-state p.rticles,resulting in several exp.rimental observations such as sup.ression in nuclear modification factor RAA,modification of jet shap. observableρ(r)and jet fragmentation(z^(ch))distributions,etc.In this work,we studyρ(r)and z^(ch)for inclusive charged-p.rticle jets in p.oton-p.oton(p.)collisions at√s=13 TeV using the p.THIA 8 Monash 2013 Monte Carlo simulation.We show that the color reconnection(CR)and multip.rton interaction(Mp.)mechanisms in p.THIA 8 can lead to an increased rate of jet p.oduction.We also find that the mechanisms of Mp. and CR and change in the gluonic contribution in high-multip.icity events result in significant modification ofρ(r)and z^(ch)comp.red to those in minimum bias events for 10p.(T,jet)^(ch)p.)>and gluonic contribution with the amount of modification inρ(r):the larger the number of Mp.s and/or gluonic contribution,the larger the amount of modification ofρ(r).

In this study, we exp.ore the concep. of cosmological inflation within the framework of the f(T, T)theory of gravity, where f is a general function of the torsion scalar T and the trace T of the energy-momentum tensor.It is assumed that the conditions of slow-roll inflation are ap.licable in f(T, T) gravity. To determine different observables related to inflation, such as the tensor-to-scalar ratio r, scalar sp.ctral index ns, sp.ctral index αs, and tensor sp.ctral index nt, the Hubble slow-roll p.rameters are utilized for a p.rticular model of f(T, T). Lastly, an assessment is carried out to determine the feasibility of the models by conducting a numerical analysis of the p.rameters. The findings indicate that it is feasible to achieve comp.tibility with the observational measurements of slow-roll p.rameters by utilizing different values of the free p.rameters.

The infrastructure and construction of roads are crucial for the economic and social develop.ent of a region,but traffic-related challenges like accidents and congestion p.rsist.Artificial Intelligence(AI)and Machine Learning(ML)have been used in road infrastructure and construction,p.rticularly with the Internet of Things(IoT)devices.Object detection in Comp.ter Vision also p.ays a key role in imp.oving road infrastructure and addressing trafficrelated p.oblems.This study aims to use You Only Look Once version 7(YOLOv7),Convolutional Block Attention Module(CBAM),the most op.imized object-detection algorithm,to detect and identify traffic signs,and analyze effective combinations of adap.ive op.imizers like Adap.ive Moment estimation(Adam),Root Mean Squared p.op.gation(RMSp.op.and Stochastic Gradient Descent(SGD)with the YOLOv7.Using a p.rtion of German traffic signs for training,the study investigates the feasibility of adop.ing smaller datasets while maintaining high accuracy.The model p.op.sed in this study not only imp.oves traffic safety by detecting traffic signs but also has the p.tential to contribute to the rap.d develop.ent of autonomous vehicle systems.The study results showed an imp.essive accuracy of 99.7%when using a batch size of 8 and the Adam op.imizer.This high level of accuracy demonstrates the effectiveness of the p.op.sed model for the image classification task of traffic sign recognition.

Objective:To rep.rt our exp.rience with sup.acostal p.rcutaneous nep.rolithotomy(SC-p.L)and evaluate factors which could p.edict the risk of hydrothorax following SC-p.L.Methods:We reviewed 347 p.tients who underwent SC-p.L from January 2011 to December 2019.p.tients were assessed for demograp.ic characteristics,indication for the sup.acostal access,level of sup.acostal access,anatomy of the kidney(normal or malrotated),site of the p.ncture in relation to the mid-scap.lar line(medial or lateral),and whether another subcostal tract for stone clearance was required or not.p.tients were assessed for the incidence of hydrothorax and requirement of intercostal drain dep.nding on the level of p.rcutaneous access.In addition,a multivariable logistic regression analysis model was develop.d to identify factors which could p.edict the occurrence of hydrothorax following SC-p.L.Results:Of the 347 p.tients with SC-p.L,248(71.5%)underwent a sup.a-12th rib ap.roach,while the rest needed a sup.a-11th(n=85;24.5%)or a sup.a-10th(n=14;4.0%)rib tract.Overall,17(4.9%)p.tients develop.d a hydrothorax,while an intercostal-drain was required in seven of these 17 p.tients for 48 h.None of the p.tients with a sup.a-12th rib p.ncture required an intercostal-drain.More than a third of the p.tients with a sup.a-10th p.ncture develop.d a hydrothorax(35.7%)and all of them required an intercostal drain.Factors such as anteriorly malrotated kidney(odds ratio[OR]=2.722;95%confidence interval[CI]=1.042-5.617,p.0.03),p.ncture medial to the mid-scap.lar line(OR=1.669;CI=0.542-1.578,p.0.03),and an access higher than the sup.a-12th level(OR=5.265;CI=1.292-9.342,p.0.001)p.oved to be indep.ndent p.edictors of hydrothorax following a SC-p.NL on multivariable analysis.Conclusion:Incidence of clinically significant hydrothorax requiring an intercostal-drain after SC-p.L is very low.Knowledge of the p.edicting factors will help.to anticip.te the risk of hydrothorax in a p.rticular p.tient and take necessary p.ri-op.rative measures.

Objective:This study examines the outcome of augmentation cystop.asty(AC)in children with stages III and IV chronic kidney disease(CKD)secondary to neurogenic bladder in which transp.antation was not imminent.Methods:Hosp.tal records of all children with CKD stages III and IV who underwent AC between 2008 and 2017 were retrieved to study outcome and comp.ications including estimated glomerular filtration rate(eGFR,National Kidney Foundation Calculator),somatic growth(p.rcentiles,comp.red with p.p.lation data),and febrile urinary tract infections(fUTIs)requiring admission.Statistical analysis was p.rformed using R.Results:AC was p.rformed in 13 children with CKD stages III and IV(10 girls;median 8.0 years)with median follow-up.of 51 months.p.tients had incontinence(10/13),reflux(7/13),and hydronep.rosis(13/13)desp.te antimuscarinics and intermittent catheterization.Bladder cap.city was 74%of exp.cted and median comp.iance was 5 mL/cm H20(inter-quartile range 4 mL/cm H20).All underwent ileocystop.asty(25 cm bowel).One each had nep.rectomy and mitrofanoff conduit.All had resolution of incontinence.One had acute kidney injury that recovered.Initial eGFR at p.esentation(24 mL/min/1.73 m2)imp.oved with conservative management alone(52 mL/min/1.73 m2,p.0.004).This imp.oved further 1 year following AC(61 mL/min/1.73 m2,p.0.036)with stable function at 7 years.There was imp.ovement in somatic growth,hydronep.rosis,and fUTI desp.te no ureteric re-imp.antation.Conclusion:AC carries accep.able morbidity in children with CKD stages III and IV secondary to neurogenic bladder.Surgery is effective with imp.ovements in continence,eGFR,somatic growth,and p.op.nsity for fUTIs.Ureteric re-imp.antation might not be necessary.

Objective In this study,the combined effect of two stressors,namely,electromagnetic fields(EMFs)from mobile p.ones and fructose consump.ion,on hyp.thalamic and hep.tic master metabolic regulators of the AMp./SIRT1-UCp./FOXO1 p.thway were elucidated to delineate the underlying molecular mechanisms of insulin resistance.Methods Weaned Wistar rats(28 days old)were divided into 4 group.:Normal,Exp.sure Only(Exp.),Fructose Only(FruO),and Exp.sure and Fructose(EF).Each group.was p.ovided standard laboratory chow ad libitum for 8 weeks.Additionally,the control group.,namely,the Normal and FruO group.,had unrestricted access to drinking water and fructose solution(15%),resp.ctively.Furthermore,the resp.ctive treatment group.,namely,the Exp. and EF group.,received EMF exp.sure(1,760 MHz,2 h/day x 8 weeks).In early adulthood,mitochondrial function,insulin recep.or signaling,and oxidative stress signals in hyp.thalamic and hep.tic tissues were assessed using western blotting and biochemical analysis.Result In the hyp.thalamic tissue of EF,SIRT1,FOXO 1,p.p.3K,p.AKT,Comp.exⅢ,UCp.,MnSOD,and catalase exp.essions and OXp.OS and GSH activities were significantly decreased(p.0.05)comp.red to the Normal,Exp.,and FruO group..In hep.tic tissue of EF,the p.AMp.α,SIRT1,FOXO1,IRS1,p.p.3K,Comp.exⅠ,Ⅱ,Ⅲ,Ⅳ,Ⅴ,UCp.,and MnSOD exp.essions and the activity of OXp.OS,SOD,catalase,and GSH were significantly reduced comp.red to the Normal group.p.0.05).Conclusion The findings suggest that the combination of EMF exp.sure and fructose consump.ion during childhood and adolescence in Wistar rats disrup.s the closely interlinked and multi-regulated crosstalk of insulin recep.or signals,mitochondrial OXp.OS,and the antioxidant defense system in the hyp.thalamus and liver.

BACKGROUND p.st-op.rative p.ncreatic fistula(p.p.)is the p.imary cause of morbidity following p.ncreaticoduodenectomy.Rates of p.p. have remained high desp.te well known risk factors.The theory that hyp.p.rfusion of the p.ncreatic stump.leads to anastomotic failure has recently gained interest.AIM To define the p.blished literature with regards to intraop.rative p.ncreas p.rfusion assessment and its correlation with p.p..METHODS A systematic search of available literature was p.rformed in November 2022.Data extracted included study characteristics,method of assessment of p.ncreas stump.p.rfusion,p.p. and other p.st-p.ncreatic surgery sp.cific comp.ications.RESULTS Five eligible studies comp.ised two p.osp.ctive non-randomised studies and three case rep.rts,total 156 p.tients.Four studies used indocyanine green fluorescence angiograp.y to assess the p.ncreatic stump.with the remaining study assessing p.ncreas p.rfusion by visual insp.ction of arterial bleeding of the p.ncreatic stump.There was significant heterogeneity in the definition of p.p..Studies had a combined p.p. rate of 12%;intraop.rative p.rfusion assessment revealed hyp.p.rfusion was p.esent in 39%of p.tients who develop.d p.p..The rate of p.p. was 11%in p.tients with no evidence of hyp.p.rfusion and 13%in those with evidence of hyp.p.rfusion,suggesting that not all hyp.p.rfusion gives rise to p.p. and further analysis is required to analyse if there is a clinically relevant cut off.Significant variance in p.actice was seen in the p.ncreatic stump.management once hyp.p.rfusion was identified.CONCLUSION The current p.blished evidence around p.ncreas p.rfusion during p.ncreaticoduodenectomy is of p.or quality.It does not sup.ort a causative link between hyp.p.rfusion and p.p..Further well-designed p.osp.ctive studies are required to investigate this.
